ABSTRACT

Authors present a case of a female patient surgically treated for chronic otitis media (radical middle ear surgery) in which basing on postoperative pathology report a specific inflammatory process (tuberculosis) has been diagnosed. It is pointed out that in this patient there is a history of malignant neoplastic process (rhabdomyosarcoma embryonal) treated previously at the contralateral middle ear.

ABSTRACT

Labyrinthine fistulas (l.f.) are reported to be diagnosed in 3-12.9% of the chronic otitis media patients. Their most frequent location is the lateral semicircular canal. The other locations include posterior semicircular canal, superior s.c. and vestibular window. Surgical removal of the cholesteatoma matrix brings a high risk of the complete hearing loss in the operated ear. Preoperative diagnosis of the l.f. is not always possible. Authors present the analysis of the group of 18 patients with l.f. treated at the Otosurgery Department, Medical University of Lodz. In all patients the radical modified surgery has been performed. Postoperative results are strictly correlated to the l.f location and size. As a conclusion authors present an algorithm of the l.f. surgical treatment based on their experience.

ABSTRACT

The aim of this paper was evaluation of vertigo frequency in otosclerotic patients (group I N = 64) in comparison with control group (group II N = 20) and in people after stapedectomy (group III N = 64). Furthermore electronystagmographic recording spontaneous nystagmus, positional nystagmus were made. Computer analyses of ENG-recording and automatic calculation of directional preponderance and canal paresis were used. Vertigo was in 12% otosclerotic patients while in equal of age control group only in 5% subjects. Spontaneous nystagmus in 20% and positional nystagmus in 27% were recorded in otosclerotic patients while that took place in 10% and 15% control group respectively. Asymmetric reaction in-group I was in 21% cases and in 10% subject's in-group II. Real directional preponderance was in 18.8% otosclerotic patient's in-group I and it was absent in-group II. Obtained results revealed existence vestibule disturbances in otosclerotic patients. More frequent labyrinth objective symptoms were registered post stapedectomy. Vertigo in 21%, spontaneous nystagmus in 58%, positional nystagmus in 61%, asymmetric reaction in 48% was noted. Marks of vestibule and organ injury in otosclerosis and post stapedectomy were met. Vestibule disturbances were irritate and inconstant character. It is appears to be needed examination of balance system before stapedectomy in the planning of stapes operation and after operation on stapes. Presence of vestibular symptoms after stapes surgery is not cause less post stapedectomy improvement of hearing but sometimes postoperative improvement of hearing is better for lower frequencies in patients group with vestibular disturbances than in-patients without these symptoms.

ABSTRACT

The clinical analysis of the patients after tympanic membrane reconstruction with the temporal muscle fascia has been conducted. The total number of 145 patients, operated at the Otosurgery Department Medical University of Lodz, was divided into 2 Groups. Group 1 (n = 75) patients with chronic otitis media (central perforation and mobile ossicular chain). The preoperative mean conductive hearing deficit of 20 dB was observed in all evaluated patients. After surgery a satisfactory hearing improvement was observed in 90% of the patients. Group 2 (n = 70) patients with the reconstructed postero-superior part of the tympanic membrance and if necessary external auditory canal wall reconstruction. The mean conductive hearing deficit of up to 30 dB was observed before the surgery. Postoperatively in 75% of patients the hearing improvement was observed as well as the satisfactory healing of fascia implant. The postoperative evaluation indicates a better hearing improvement after the myringoplasties with the prefascial lamina than in the myringoplasties performed in the traditional ways.

ABSTRACT

Authors present new method of wind-pipe plastic operation post laryngectomy. In this method perpendicular scores of the skin are performed and special external sutures are made which stretch tracheostomy foramen to order to obtain wide inside measurement of wind-pipe. 78% operated patients may be without tracheotomy tube.

ABSTRACT

The clinical analysis and preliminary results of treatment in a group of 19 patients with malignant neoplasms of the external and middle ear, treated in ENT Clinic, Medical University in Lódz in the years 1978-1993, was reported. They were born mostly from auricle (in 9 patients-47.4%). In 15 cases (79%) squamous carcinoma was diagnosed, in 3 cases carcinoma of ceruminal gland and in 1 case basal cell carcinoma. Sixteen patients were treated surgically or with radiotherapy. The best preliminary results were obtained in glandular tumors of the external canal, the worst (100% mortality) in cancer of middle ear.

ABSTRACT

According to Ravin's method an oxidative activity of ceruloplasmin was determined in the group of patients with carcinoma of the larynx before and after a surgical treatment, in the group of patients with inflammatory lesions in the upper respiratory tract and in the control group of healthy individuals. As a result of this assay the significantly increased of oxidative activity ceruloplasmin was obtained in the individuals with the carcinoma of the larynx (before surgical treatment) in comparison to the group of patients suffering due to inflammatory diseases of the upper respiratory tract and to the group of healthy subjects. Simultaneously a significant decrease of the oxidative activity of serum ceruloplasmin in the individuals with carcinoma of the larynx after a previous surgical treatment was observed.

ABSTRACT

The threshold force inducing the stapedial movements was investigated in laboratory and clinic. The measurement was performed by use of the own made dynamometer. The threshold force was between 0.058-0.059 G. The application of this tool during the stapedial surgery may be practical and useful.

ABSTRACT

The collagenous protein level in the inflammatory status of middle ear mucosa taken during the surgery in 16 patients were analysed. The more accentuated changes were in longer duration of disease. The collagenous protein level play probably an important role in the healing processes after tympanoplasty.

ABSTRACT

The most often used skin-flap techniques in one-step and two-steps closed tympanoplasties are discussed. The correct plastic surgery of the meatal skin gave good results. In the ENT Clinic of the Medical Academy in Lódz techniques most often used are two-flaps tympanoplasties with anterior and posterior pedunculated flaps or tympanoplasties with a single pedunculated flap.

There were a lot of discussions about the role of endolymphatic sac in the regulatory mechanism of endolymphatic pressure during the decade 1960-70. Nowadays we know that the balance of metabolism in the inner ear depends on stabilization of fluids in the endolymphatic space. The histologic and anatomic examinations of petrous bone and endolymphatic sac were discussed.

ABSTRACT

The morphometric examinations of the endolymphatic saccule epithelium in Ménière's disease were performed. The squamous epithelium prevailed in lower and cuboid and cylindrical in upper part of the saccule. In Ménière's disease the cellular nuclei are more elongated. The mean height of the epithelial cells are lower of 2,236 microns than the cells in normal hearing person. The morphometric method of examination permits to follow the supposition that in Ménière's disease there are the degeneration of the epithelial cells and subsequently the disturbances in endolymphatic resorption.
