ABSTRACT

There was analyzed the experience of diagnosis and surgical treatment of 61 patients, aged from 2 to 69 years old, including 43 (70.5%) men and 18 (29.5%) women, in whom after a tracheal intubation with durable pulmonary ventilation, performed for an acute respiratory insufficiency, a tracheal stenosis have had occurred. A tracheal stenosis was manifested by a stridorous kind of respiration, the diagnosis was confirmed by the endoscopic investigation and a computer tomography data. Conservative and endoscopic treatment was effective in 10 (16.4%) patients, operative intervention--circular resection with a primary tracheal anastomosis formation and a staged treatment with tracheostomy and a T-kind silicone tube insertion--was performed in 51 (83.6%) patients. Good and fair results were achieved in 70% of the patients.

ABSTRACT

The combined hydatid disease was surgically treated in 101 patients. Among them three groups were distinguished: hydatid disease of the right lung and liver (n=51); bilateral lung and liver affection (n=33); hydatid disease of the left lung and liver (n=17). The method of choice in treatment of the left lung and liver diaphragmatic surface hydatid disease is the one-stage closed echinococcectomy by Delbe. Two- or three-stage operation is indicated in cases of bilateral echinococcosis. The surgical excision should be started from the mostly affected organ or from the site were complications are mostly expected. Patients with the combined hydatid disease of the left lung and liver are to experience the two-stage echinococcectomy. Operations should be performed in the range from 2 to 4 months. The overall lethality of the analyzed cohort was 10,2%.

ABSTRACT

Results of esophagoplasty (small intestine--5, stomach--35, colon--40) were studied in 80 patients with scarry stricture of the esophagus. The development of cancer in the burned esophagus was established in 3 out of 5 patients in 35-45 years after operation. Good and satisfactory results were obtained in 97.8% of the patients within the period from 5 to 17 years after gastro- and coloesophagoplasty. Unsatisfactory results were found in 3.2%. Total lethality after esophagoplasty was 12.5%.

ABSTRACT

The experience with treatment of 104 patients with burn cicatricial strictures of the esophagus and stomach is analyzed. Postoperative complications were seen in 28 (26.9%) patients. Hospital lethality was 12.5%. It is concluded that method of esophagoplasty in these patients should be selected individually.

The visual picture of the ulcerous defect was studied in 222 patients by endoscopic and intraoperative methods. The prognostic assessment of the development of complications of ulcer disease of the stomach and duodenum is given on the basis of the characteristics of the ulcer process (diameter, depth, character of the ulcer fundus and margins).

ABSTRACT

Time course of the healing process in the tracheal wall during silicon-lavsan tracheal grafting was investigated in experiments on 108 dogs with a new model of tracheal prosthesis. The formation of granulation tissue was noted as a result of postoperative inflammatory reaction in the prosthetic area. The tracheal epithelial layer adjacent to the anastomoses was hypertrophic and acquired the features of squamous epithelium. Later, after the implantation the prosthesis was surrounded by a thick fibrous capsula. In anastomoses, at tracheal tissue--implant border there was a poorly expressed inflammatory reaction, which had a local character and was compatible with 3 and more years of life in most experimental animals.

ABSTRACT

The authors analyze the treatment of 74 patients with non-specific spontaneous pneumothorax. Relapses were observed in 6.7% of cases. The authors have shown that a partial pneumothorax should be treated actively and conservatively. By this method in 61 of 74 patients (82,6%) the lungs were expanded. Operations were performed in 13 patients. The surgical treatment is indicated in complete pneumothorax complicated by stable collapse of the lung, continuous intrapleural hemorrhage and suppuration of the pleural cavity.
