ABSTRACT

On the basis of experimental and clinical investigations the role of vascular microcirculation disorders of gastric wall in pathogenesis of recurrent and peptic ulcers and, in particular, giant ulcers of the stomach after selective proximal vagotomy was established. Giant ulcers of the stomach are considered as trophic or ischemic ulcers, and are shown to be resistant to therapy. Early surgical treatment is recommended. The method of choice is gastric resection of ladder-type.

ABSTRACT

The dynamics of immune status in 124 patients operated on for duodenal ulcer are analysed. The patients were divided into two groups: in the study group (60 patients) regional lymphoimmune stimulation with T-activin was used, in patients of the control group T-activin was not used. The depression of immune system in short term period after surgical intervention was detected. The injection of T-activin into the round ligamentum of the liver influences favourably the course of the disease and promotes correction of immune disorders, thus preventing short-term postoperative complications.

ABSTRACT

The authors discuss the results of surgical treatment of 577 patients with duodenal ulcer and its complications by operations on the vagus nerve of an isolated character or together with operations on the stomach. The findings of dynamic endoscopic examination of healing of the ulcer in 250 patients are reported. The application of regional lymphostimulation in the prevention and treatment of postvagotomy gastroplegia and gastrostasis in more than 200 patients is described.

ABSTRACT

The effect of dalargin and some other ligands of the opioid receptors on drainage function of the pancreatic lymphatic system was studied in rats and dogs. In rats, dalargin (30-1000 micrograms/kg, subcutaneously) accelerated the elimination of Evans blue from beneath the pancreatic capsule in a dose-related manner. The effect of dalargin was attenuated by naloxone. Specific agonists of mu-, delta- and sigma-opioid receptors had no dalargin-like activity. In dogs, dalargin (60-80 micrograms/kg, subcutaneously) after the administration of Evans blue into the pancreas increased its concentration in the truncus lymphaticus and slowed down its penetration into the blood. Thus, dalargin accelerates the elimination of Evans blue from the pancreas due to the intensification of lymphatic drainage. The effect of dalargin was mediated by subpopulation of opioid receptors with which their certain selective ligands have but slight interaction.
