ABSTRACT

PURPOSE: to assess parameters of myocardial deformation and rotation in untreated hypertensive men depending on presence and degree of left ventricular hypertrophy (LVH). MATERIAL AND METHODS: We examined 98 hypertensive men aged 51 (46-58) years. Examination included clinical evaluation, conventional transthoracic echocardiography, ambulatory blood pressure monitoring, routine laboratory screening. LV deformation and rotation was studied by two-dimensional speckle tracking echocardiography. RESULTS: In patients without LVH we observed significant decrease of longitudinal strain without changes of rotation. Higher degree of LVH was associated with more pronounced lowering of longitudinal deformation. Severe LVH was characterized by decreased longitudinal, circumferential and radial deformation with compensatory increase of apical LV rotation. CONCLUSION: Changes of LV deformation are observed in patients with hypertension before LVH formation. This opens new perspectives of early assessment of LV remodeling. Changes of LV deformation are related to degree of LVH.

ABSTRACT

The article is dedicated to a pressing problem of arterial hypertension in the postmenopausal period. Our objective in this study was identifying of special features of the course of arterial hypertension by data secured from a 24-hr monitoring of AH in those women in the post-menopausal period of both surgical and natural genesis. Those women with a surgical menopause had a higher level of AH in the diastole, with that in the systole being the same as in the control group in spite of the fact that the former women were at about the same age, as evidenced by results of the study made. The load indices were also higher, which fact suggested to us, first, a gravier course of AH in those women with a natural menopause and second, that removal of ovaries at the child-bearing age is a prognostically unfavourable factor.

ABSTRACT

AIM: Assessment of efficiency and safety of enalapril (enap) and its combination with hydrochlorotiaside (enap-N). MATERIALS AND METHODS: 127 patients with mild and moderate blood hypertension entered an open non-comparative multicenter trial. 60 of them received enap (group 1), 67--enap N (group 2). Group 1 patients were given enap for 2 weeks in a dose 10 mg/day. If this dose was not adequate to normalize blood pressure, it was raised to 20-40 mg/day. Patients of group 2 received enap-N one tablet a day for 3 weeks. If the pressure persisted higher than 140/90 mm Hg, the treatment was continued for 3 weeks more in a dose of 2 tablets a day. RESULTS: Blood pressure lowered under 140/90 mm Hg in 40 patients of group 1 (66.7%). Systolic pressure dropped by 10 mm Hg minimum and diastolic by 5 mm minimum in 18 group 1 patients (30%). Enap-N reduced blood pressure under 140-90 mm Hg in 44 of 67 patients (65.7%). Systolic and diastolic pressure dropped, respectively, in 23(34.3%) patients. CONCLUSION: Enap and enap-N tablets were found highly effective and well tolerated. Side effects were caused by lowering of blood pressure.

ABSTRACT

Atrial fibrillation is a commonly seen form of disordered cardiac rhythm. Catheter-radio frequency ablation of the atrioventricular junction was found out to be an effective approach to control heart rate in continued drug-refractory cardiac fibrillation permitting the quality of the patients' life to be improved. The best results are achievable in patients presenting with chronic cardiac fibrillation.

ABSTRACT

A study of rheumatic patients after operations on the heart indicates that working capacity prognosis is determined not only by the severity of the somatic state but also by social and psychological aspects. The authors revealed sociopsychological and clinical prerequisites of high and low working adaptation that allow to make a concrete working prognosis and establish rehabilitation programs for the individual rheumatic patients undergoing heart surgery.

Life quality indices related to work trends, level of work adaptation, social psychological personality features, severity of somatic condition were evaluated in 216 patients with rheumatism operated on for heart disease. The revealed causes and mechanisms of life quality decline are important for evaluation of working prognosis and development of rehabilitation programs for patients with rheumatism following operations on the heart.

ABSTRACT

Altogether 345 patients with rheumatic fever operated on for heart disease were examined for clinicopsychological correlations and their importance for patients' adaptation to work. Cardiovascular function was estimated with the aid of clinical and instrumental methods (ECG, echocardiography, bicycle ergometry). The psychological personality traits were examined by Quetelet's 16-factorial questionnaire, the type of the attitude towards disease by a personality questionnaire developed by the V. M. Bechterew's Leningrad Research Psychoneurological Institute, work purpose by a specially made up questionnaire. Correlations between the level of adaptation to work and the sociopsychological and clinical signs were studied. The psychological characteristics, work purpose of the personality were found to be of paramount importance for restoration of work fitness, which is of significance in the design of labour prognosis of patients and in expert medical evaluation of the working capacity.

ABSTRACT

The authors studied the psychological peculiarities of personality, attitude towards the disease and orientation for work in 124 patients operated on in connection with rheumatic heart disease. The psychological peculiarities of personality were examined by means of a 16-factor questionnaire suggested by Kettell and the type of attitude towards the disease by a questionnaire prepared by the V. M. Bechterew's Institute, orientation for work--by means of a specially compiled questionnaire. It has been established that the type of attitude towards the disease is determined by psychological peculiarities of personality, it exerts influence on orientation for work. This should be taken into consideration when conducting rehabilitation measures in patients with rheumatic heart disease after the operation on the heart.

ABSTRACT

Spiroergometric examinations were carried out in 148 students with neurocirculatory dystonia. The adaptation reaction of the cardiorespiratory system to different load strength was studied. The authors used a complex approach with evaluation of the functional parameters of the heart, indices of central hemodynamics and analysis of respiratory gases. As a result of the investigation signs of deadaptation to physical load in neurocirculatory dystonia were revealed.
