ABSTRACT

Resection of large tumors involving mandible is the most common reason for mandibulectomy in patients. Adjunct therapies complicate the clinical situation and advocates innovations in conventional design concepts and treatment protocols. Xerostomia is one of the most common complications arising post-radiotherapy. Incorporation of a salivary reservoir in the design has been frequently used in xerostomia patients using acrylic dentures. The purpose of the article is to highlight a technique and steps in rehabilitation of a large acquired mandibular defect using a two-piece, magnet-retained acrylic removable partial denture with salivary reservoir.

ABSTRACT

BACKGROUND: Implant restorations are considered an ideal treatment option for replacement of missing teeth in partially edentulous patients. Abutment screw loosening is one of the frequently observed technical complications of implant-supported prosthesis. This study aimed to determine the prevalence and factors associated with the abutment screw loosening in cement-retained single-implant crowns. METHODS: Enrolment criteria included partially edentulous patients who have been rehabilitated with one or more cement-retained single-implant crowns with minimum postcementation period of 1 year. They were recalled and evaluated for the presence or absence of screw loosening both clinically and radiographically. They were further evaluated for the presence or absence of factors associated with screw loosening such as parafunctional habits, wider occlusal table, steep cuspal inclines, non-axial loading and cantilevering of the pontic. RESULTS: Twenty-six cement-retained single-implant crowns out of 280 showed screw loosening, making the overall prevalence rate of 10.77%. Among the factors evaluated, parafunctional habits were associated with three cases, wider occlusal table in four, steep cuspal inclines in three, non-axial loading in nine and cantilevering of the pontic in three cases. Exact reasons could not be ascertained in four cases. CONCLUSION: Abutment screw loosening seems to be a significant prosthetic complication of cement-retained single-implant crowns. Factors evaluated significantly affect the functional durability of the prosthesis. These factors should be considered while restoring to enhance the longevity of such restorations.

ABSTRACT

INTRODUCTION: Intraoral chair side porcelain repair system is a quick, painless and highly patient acceptable procedure, without removal of restoration or fabrication of new restoration. There are very limited studies conducted to evaluate the shear bond strength of repair systems after different surface treatment. OBJECTIVES OF RESEARCH: The objective of research was to evaluate the shear bond strength of two intraoral porcelain repair systems Clearfil repair system (Kuraray) and Ceramic repair system (Ivoclar) to repair metal-ceramic restoration after three different surface treatment. MATERIALS AND METHODS: Totally, 120 discs of base metal alloy were fabricated. The opaque, dentine and enamel of ceramic were applied to achieve the uniform thickness. Defect was created, and repair was done using two repair systems after different surface treatment. Shear bond strength was measured. RESULTS: Analysis of variance was utilized. Ceramic repair system after 40% phosphoric acid surface treatment showed the highest mean value and Clearfil repair system after surface treatment with 37% phosphoric acid showed the lowest. The statistical difference was found to be significant between the groups. CONCLUSION: The shear bond strength of Ceramic repair system with 40% phosphoric acid etching showed highest shear bond strength as compared to other system and surface treatment used in the study.

ABSTRACT

The subject of sleep medicine, for dental profession in general and prosthodontists in particular, continue to offer great challenges and opportunities in terms of diagnosis, treatment planning and treatment based on qualitative evidence. Although the role played by the prosthodontists is still in its infancy, there is much to learn and understand in the rapidly evolving field of sleep medicine as the recognition of co-managing patients with sleep disorders by the prosthodontists is fast becoming a reality. This article discusses at length the prosthodontic perspectives of the research in the field of sleep medicine, particularly on obstructive sleep apnea.

ABSTRACT

Calcineurin inhibitor induced thrombotic microangiopathy is a rare but well recognized complication of a renal transplantation that occurs in 1% of the patients who are on tacrolimus immunosuppression. Among the other aetiological factors of the "de-novo" Thrombotic Microangiopathy (TMA), the condition especially has to be differentiated from an antibody mediated rejection, as both have different pathogenesis, therapeutic connotations and outcomes.We report a case of a middle aged female renal transplant recipient treated with tacrolimus, who developed localised thrombotic microangiopathy in the early post transplantation period. Despite the normal trough levels of tacrolimus, a diagnosis of "Tacrolimus induced TMA" was rendered after excluding other causes of the "de-novo" TMA, which included an antibody mediated rejection, a meticulous clinico-pathological correlation and serological studies. The treatment included the substitution of tacrolimus by rapamycin, with the subsequent normalization of the renal function.
