ABSTRACT

Based on an unusual case of bilateral mandibular condyle fracture the diagnostic problems are described. If conventional X-ray, panorama-tomograms and bilateral tomograms of both temporomandibular joints do not exactly show the fracture dislocation and the relation of the fragments CT scans should be taken.

ABSTRACT

There is a rising need for preprosthetic surgery due to a general increase in life expectancy and the desire for optimal oral rehabilitation. A method of reconstructing residual ridges in an ideal form with a hydroxylapatite-fibrin paste is presented. Patients suffering from mild to moderately severe age-related neurological disorders with dementia and psychomotor slowing benefit from the procedure, whereas psychiatric illnesses such as hypochondria or psychotic disturbances need to be excluded from ridge reconstruction. The same is true for neurological deficits resulting in facial and oral hyperkinesia.

ABSTRACT

We registered 101 patients from the ambulatory endocrinological dispensary with an endocrine orbitopathy stage II to VI. The peak of age at the time of the disease was in the fifth decennium of age. Females are four times more, frequently affected than males. Only one third of the patients could be registered and treated at the early stage of the endocrine orbitopathy within the first six months after appearance of the symptoms. The metabolic condition of the thyroid gland was hyperthyroid in 70.3% of the cases. 82.2% of all patients showed a struma. By a thyrostatic treatment of hyperthyroid persons in 14.1% a decrease of Hertel's values and in 31.0% a reduction of the subjective eye complaints was obtained. The parabulbar dexamethasone therapy in each case in circa half the patients resulted in an improvement of Hertel's values as well as of the subjective symptoms. A comparison of the oral dexamethasone and prednisolone showed a more favourable effect, when Hertel's values were measured. It is to be stated that there is not therapeutic remedy of choice of the endocrine orbitopathy, but the combination of several forms of treatment is necessary.

ABSTRACT

In 88 patients with adenomas of the pituitary gland, among them 24 hormone-inactive tumours, 27 somatotropic adenomas and 37 prolactinomas, anamnestic data, clinical, endocrinologic and localisation-diagnostic findings as well as therapeutic measures were evaluated. Main symptoms were headache and visual disturbances. 78 of the 88 patients with adenomas of the pituitary gland were able to work at the time, when the diagnosis was made. In about 32% of the patients partial defects of the visual field were present and in 35% in the cranial computed tomogram a suprasellar and parasellar, respectively, expansion of the tumour and thus an advanced stage of the disease was established. The number of the residual findings and recidivations, respectively was great in all the three forms of the adenomas of the pituitary gland. In the prolactinomas and the somatotropic adenomas of the pituitary gland a medicamentous secondary therapy with dopamine agonists is possible. The therapy with bromocriptine reveals good results particularly in prolactinomas.

ABSTRACT

It is reported on a patient with an adenoma of the hypophysis under the picture of acromegaly and simultaneously existing primary hyperthyroidism. In this very infrequent picture of a disease at first the removal of the hyperthyroid metabolic position should take place and then the therapy of an adenoma of the hypophysis should be introduced. Since our patient refused the operative treatment of the tumour of the hypophysis, we treated with bromocriptin.

ABSTRACT

Anamnestic, clinical, biochemical and bioptic examinations were performed in 127 patients with fatty infiltration of the liver of different degrees of severity. Anamnesis, clinical practice and laboratory findings can only throw the suspicion on the fatty infiltration of the liver. For the verification of the diagnosis and for the gradation first of all the liver biopsy is necessary. Alcohol clearly plays the most important part as a cause of the development of the fatty infiltration of the liver, followed by adiposity, diabetes and hyperlipoidaemia. Because of the great medical and social-economic importance fatty infiltration of the liver cannot longer be taken for a harmless disurbance.
