ABSTRACT

Relaxin's role in differentiated thyroid cancer (DTC) has been suggested but its characterization in a large clinical sample remains limited. We performed immunohistochemistry for relaxin-2 (RLN2), CD68 (total macrophages), CD163 (M2 macrophages) on tissue microarrays from 181 subjects with non-distant metastatic DTC, and 185 subjects with benign thyroid tissue. Mean pixels/area for each marker was compared between tumor and adjacent tissue via paired-t test and between DTC and benign subjects via t-test assuming unequal variances. RNA qPCR was performed for expression of RLN2, RLN1, and RXFP1 in cell lines. Amongst 181 cases, the mean age was 46 years, 75 % were females. Tumoral tissue amongst the DTC cases demonstrated higher mean expression of RLN2 (53.04 vs. 9.79; p < 0.0001) compared to tumor-adjacent tissue. DTC tissue also demonstrated higher mean expression of CD68 (14.46 vs. 4.79; p < 0.0001), and CD163 (23.13 vs. -0.73; p < 0.0001) than benign thyroid. These markers did not differ between tumor-adjacent and benign thyroid tissue groups; and amongst cases, did not differ by demographic or clinicopathologic features. RLN1 and RXFP1 expression was detected in a minority of the cell lines, while RLN2 was expressed by 6/7 cell lines. In conclusion, widespread RLN2 expression in DTC tissue and most cell lines demonstrates that RLN2 acts in a paracrine manner, and that RLN1 and RXFP1 are probably not involved in thyroid cancer cell signaling. RLN2 is a biomarker for thyroid carcinogenesis, being associated with but not secreted by immunosuppressive macrophages. These findings will guide further investigations for therapeutic avenues against thyroid cancer.

ABSTRACT

Purpose: Sleep disturbance is suspected to increase during the COVID-19 pandemic, and people with type 2 DM are known to have a higher risk of sleep disturbance. We aimed to determine the prevalence and determinants of sleep disturbance through Pittsburgh Sleep Quality Index (PSQI) during the COVID-19 pandemic. Patients and Methods: We randomly selected two hundred eighty-eight people with T2DM from the outpatient clinics of the National Diabetes Centre in Qatar. We used Chi-square, Mann-Whitney, Spearman, and Point Biserial correlation tests to examine the association between sleep quality and the independent variables. Finally, we conducted multiple logistics regression to identify the predictors of poor sleep quality and set the alpha level at 0.05. Results: In our sample, the mean age (±SD) was 51.4 (± 9.5) years, and 64.3% of the study participants were males. The median (IQR) duration of diabetes was 10 (11) years. Additionally, 6.3% of the participants were on insulin. The median HbA1c was 7.6% (2.4). Three in ten patients reported poor sleep quality (PSQI>5); (n=103; 35.8%). Poor sleep quality was statistically associated with young age, previous history of sleep disturbance, prior diagnosis of sleep disorders, high depression score, and high perceived stress score. After adjusting for confounders, only high depressive symptoms score and previous history of sleep disorder were significant predictors of poor sleep quality (p < 0.001), with adjusted odd ratios of (aOR = 1.421; 95% CI: 1.242-1.625) and (aOR = 3.208; 95% CI: 1.574-6.537), respectively. Conclusion: The prevalence of poor sleep quality among people with T2DM during the COVID-19 pandemic is high. Physicians must screen for depression, stress, and previous history of sleep disorder to tackle poor sleep among T2DM.

ABSTRACT

Chest X-ray (CXR) imaging is a standard and crucial examination method used for suspected cases of coronavirus disease (COVID-19). In profoundly affected or limited resource areas, CXR imaging is preferable owing to its availability, low cost, and rapid results. However, given the rapidly spreading nature of COVID-19, such tests could limit the efficiency of pandemic control and prevention. In response to this issue, artificial intelligence methods such as deep learning are promising options for automatic diagnosis because they have achieved state-of-the-art performance in the analysis of visual information and a wide range of medical images. This paper reviews and critically assesses the preprint and published reports between March and May 2020 for the diagnosis of COVID-19 via CXR images using convolutional neural networks and other deep learning architectures. Despite the encouraging results, there is an urgent need for public, comprehensive, and diverse datasets. Further investigations in terms of explainable and justifiable decisions are also required for more robust, transparent, and accurate predictions.

ABSTRACT

Medulloblastoma (MB) is a malignant pediatric brain tumor with a poor prognosis. Post-surgical radiation and cisplatin-based chemotherapy have been a mainstay of treatment, which often leads to substantial neurocognitive impairments and morbidity, highlighting the need for a novel therapeutic target to enhance the sensitivity of MB tumors to cytotoxic therapies. We performed a comprehensive study using a cohort of 71 MB patients' samples and pediatric MB cell lines and found that MB tumors have elevated levels of nucleosome remodeling FACT (FAcilitates Chromatin Transcription) complex and DNA repair enzyme AP-endonuclease1 (APE1). FACT interacts with APE1 and facilitates recruitment and acetylation of APE1 to promote repair of radiation and cisplatin-induced DNA damage. Further, levels of FACT and acetylated APE1 both are correlate strongly with MB patients' survival. Targeting FACT complex with CBL0137 inhibits DNA repair and alters expression of a subset of genes, and significantly improves the potency of cisplatin and radiation in vitro and in MB xenograft. Notably, combination of CBL0137 and cisplatin significantly suppressed MB tumor growth in an intracranial orthotopic xenograft model. We conclude that FACT complex promotes chemo-radiation resistance in MB, and FACT inhibitor CBL0137 can be used as a chemo-radiation sensitizer to augment treatment efficacy and reduce therapy-related toxicity in high-risk pediatric patients.

ABSTRACT

This case report demonstrates the benefit of using the shortened dental arch (SDA) design for a comprehensive implant-supported restoration and the avoidance of the complexities of implant placement near the maxillary sinus. The SDA concept as a treatment option is encouraging in terms of function, patient satisfaction, and cost-effectiveness.

ABSTRACT

Eruption disorders are numerous varying from delayed to complete failure of eruption. Primary failure of eruption (PFE) is a rare condition that involves arrested eruption of teeth with the absence of local or general contributory factors. Another rare and clinically challenging phenomenon is canine transmigration which is the intra-osseous movement of impacted canines across the midline. This report presents the first case of combined failure of eruption of multiple teeth with bilateral mandibular canine transmigration, transposition of upper canine and the first premolar, torus palatinus, and class III incisor relationship in a 33-year-old asymptomatic and nonsyndromic female patient.

ABSTRACT

Pentalogy of Cantrell is a syndrome that consists of five anomalies: a midline, upper abdominal wall abnormality; lower sternal defect; anterior diaphragmatic defect; diaphragmatic pericardial defect, and congenital abnormalities of the heart. The pathogenesis of this condition is not fully known yet, associations are common with this condition and treatment is extremely challenging. Prognosis and outcome depends on the type of the pentalogy of Cantrell and associated cardiac anomalies. It can be diagnosed reliably by antenatal ultrasound during the first trimester. We describe a Sudanese baby of undetermined sex who had the typical features of pentalogy of Cantrell and died shortly after birth.

ABSTRACT

OBJECTIVE: To determine secreted antischistosoma antibodies in urine and to discern the epidemiological situation of schistosomiasis in the agricultural field labourers'camps city in the Gezira State-central Sudan. METHODS: Total of 66 urine and 66 serum paired samples were collected from those who confirmed parasitologically positive and negative with schistosomiasis from the two camps. Samples were tested using ELISA technique to measure and compare the immunoglobulin G (IgG) levels in serum and urine samples of schistosomiasis patients. RESULTS: The overall prevalence of S. mansoni and S. haematobium was 53.8% and 15.4%, while the intensity were (2.04 GMEC) and (0.9 GMEC) respectively. The relative percentage of positive IgG individulas in urine was 92.40% where as 96.97% in serum. Statistically no significant difference between the IgG levels in serum and urine samples was observed. CONCLUSIONS: This study shows that the detection of secreted IgG antibodies in urine can substitute serum for diagnosis of schistosomiasis.
