ABSTRACT

Light stabilizers are commonly used as additives in mulching films and have environmental persistence, bioaccumulation and ecotoxicity. However, their occurrence and distribution in mulching films and accumulation in mulched soils are seldom reported. This study firstly presents a comprehensive screening of 19 light stabilizers in 65 mulching films and 30 farmland soils collected in China, of which five and eight light stabilizers were 100% detected, respectively. The light stabilizer concentration in biodegradable mulching films was significantly higher than that in polyethylene ones, with median concentrations of 1.75 × 106 µg/kg and 4.86 × 103 µg/kg, respectively. Furthermore, a positive correlation was observed between the light stabilizer concentration in mulching films and in soils. This indicates that mulching films play a critical role in the accumulation of light stabilizers in farmland soils, and biodegradable mulching films significantly increase benzotriazole light stabilizers in soils. Although the light stabilizer concentration in farmland soil is relatively low, the sustainable quantities of mulching film input and the long-term accumulation will still pose a threat to the ecological environment and organism health. Consequently, our work reveals the occurrence and environmental risk of light stabilizers in mulching films and farmland soils and brings attention to light stabilizers in the soil environment.

ABSTRACT

Rarely, disorders of lipid metabolism cause nephrotic syndrome with progressive kidney disease. Tangier disease is a rare condition belonging to this family of lipid disorders; however, it is not associated with kidney disease. We report a patient presenting with nephrotic syndrome, leading to the unmasking of Tangier disease. A 34-year-old man presented with ankle oedema, nephrotic-range proteinuria and hypoalbuminaemia. Kidney biopsy demonstrated membranous nephropathy with features of immunoperoxidase staining, suggesting a secondary aetiology. Acute serology was negative. Imaging showed lymphadenopathy with splenomegaly suggestive of lymphoproliferative disorder. Bone marrow biopsy revealed foamy macrophages with widespread lipid deposition. Genomic sequencing revealed a pathological homozygous variant for ATP-binding cassette subfamily A member 1 (ABCA1) c.1510-1G > A, consistent with Tangier disease. Review of the ultrastructural kidney biopsy features demonstrated, in addition to membranous subepithelial and intramembranous usual-type electron-dense deposits, intramembranous osmiophilic lipid deposits similar to those in LCAT deficiency. The patient's renal function gradually declined (serum creatinine 133 µmol/L); therefore, he was started on rituximab. Metabolic disorders causing nephrotic syndrome are rare and even more so their association with membranous nephropathy. These should be considered in cases with unexplained persistent nephrotic syndrome with progressive kidney disease and lipid deposits on renal biopsy.

ABSTRACT

Acute kidney injury (AKI) is a common presentation which can result from a number of different underlying pathological processes. Haematological malignancies, particularly multiple myeloma (MM), are known to frequently present with AKI. Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a rare condition which can cause crescentic glomerulonephritis (GN), resulting in AKI. We present the case of a 60-year-old man who presented with clinical features suggestive of AAV in the context of blood tests which demonstrated AKI and positive perinuclear ANCA (p-ANCA) and anti-myeloperoxidase (anti-MPO) titres. Further investigations demonstrated an underlying diagnosis of MM. A renal biopsy was ultimately required to determine the cause of AKI, a cast nephropathy. This case is the first to our knowledge which demonstrates a rare situation in which myeloma kidney is associated with positive p-ANCA and anti-MPO titres, without any evidence of a crescentic GN. It highlights the importance of following up on all investigations sent in the context of AKI, even when a potential diagnosis seems evident. Furthermore, it demonstrates the role of renal biopsy in confirming a diagnosis in the context of AKI with multiple differential diagnoses.

BACKGROUND: Shared decision making is considered an important aspect of chronic disease management. We explored the feasibility of routinely measuring kidney patients' involvement in making decisions about renal replacement therapy (RRT) in National Health Service settings. METHODS: We disseminated a 17-item paper questionnaire on involvement in decision-making among adult patients with established kidney failure who made a decision about RRT in the previous 90 days (Phase 1) and patients who had been receiving RRT for 90-180 days (Phase 2). Recruitment rates were calculated as the ratio between the number of included and expected eligible patients (I : E ratio). We assessed our sample's representativeness by comparing demographics between participants and incident patients in the UK Renal Registry. RESULTS: Three hundred and five (Phase 1) and 187 (Phase 2) patients were included. For Phase 1, the I : E ratio was 0.44 (range, 0.08-2.80) compared with 0.27 (range, 0.04-1.05) in Phase 2. Study participants were more likely to be white compared with incident RRT patients (88 versus 77%; P < 0.0001). We found no difference in age, gender or social deprivation. In Phases 1 and 2, the majority reported a collaborative decision-making style (73 and 69%), and had no decisional conflict (85 and 76%); the median score for shared decision-making experience was 12.5 (Phase 1) and 10 (Phase 2) out of 20. CONCLUSION: Our study shows the importance of assessing the feasibility of data collection in a chronic disease context prior to implementation in routine practice. Routine measurement of patient involvement in established kidney disease treatment decisions is feasible, but there are challenges in selecting the measure needed to capture experience of involvement, reducing variation in response rate by service and identifying when to capture experience in a service managing people's chronic disease over time.

ABSTRACT

A Symposium on Lead Remediation Effectiveness, sponsored by the US Environmental Protection Agency, was held at Coeur d'Alene, Idaho, USA from 22-25 May, 2000. International participants from various levels of government, educational institutions, industry, and community representatives presented papers and posters on themes that ranged from engineering practices through community participation in the remediation processes. The papers in this volume represent a global distribution of sites, especially those outside the USA. In providing an overview of the symposium and the theme of Lead Remediation Effectiveness we have drawn on information from some presentations at the symposium, besides those described in this volume.
