ABSTRACT

Objective:  First trimester thyroid-stimulating hormone (TSH) level is an important determiner of neonatal outcome. In this study we investigated the relationship between first-trimester TSH level and fetal birthweight. Materials-Methods: First-trimester serum TSH, age, gravidity, parity, body mass index (BMI), gestational age, and birth weight were analyzed. Patients were divided two ways. The first division- group 1 with TSH < 2.5 mU/l, group 2 with TSH > 2.5 mU/l). The second division- group 3 with TSH < 4 mU/l and group 4 with TSH > 4 mU/l). Results: The study included 302 patients. High TSH levels are associated with an increased risk of macrosomic and post-term babies. A significant association was found for both thresholds of 2.5 and 4.0 mU/l. However, this relation was not significant after binary logistic regression. Conclusion: High maternal first trimester TSH levels are not associated with birth weight after separating out macrosomia-related factors.

ABSTRACT

Objective The number of vaginal rejuvenation procedures for improvement of sexual function is dramatically increasing worldwide. The objective of this study was to present our experience with women who presented to our clinic with the complaint of sexual dysfunction or desire to enhance sexual function or orgasm. Methods Demographic and descriptive data of the patients were evaluated. In addition, sexual dysfunction of the patients who underwent vaginoplasty in our center were evaluated before and after vaginoplasty procedure using Golombok Rust Inventory of Sexual Satisfaction (GRISS) scale and the scores were compared before and after the procedure, which is used in the evaluation of sexual dysfunction by relationship counsellors and clinics. Results A total of 250 women who described a sensation of a wide or floppy vagina with lost or decreased ability to achieve orgasm were included in the study. The mean age of the patients was 38.51±9.126 years. Of all women, 85.2% were college graduates. A history of normal vaginal delivery was found in 77.8% of the participants. The mean GRISS scores of "Infrequency", "Non-communication", "Dissatisfaction", "Non-sensuality", "Avoidance", "Anorgasmia" and "overall GRISS" scores were statistically significantly decreased, while the mean vaginismus score was significantly increased (p<0.01). Conclusion Highly satisfying outcomes regarding patient satisfaction were obtained from vaginoplasty procedures that we have performed.

ABSTRACT

Objective This study aims to determine the need for sexual health education, whose importance is also emphasized by the World Health Organization, so that the youths can develop a responsible, healthy, and satisfactory sexual life and evaluates Turkish women's thoughts about their first sexual experience. Methods A total of 100 persons who were willing to participate in the research, selected through random sampling from among the women above 18 years of age who have had their first sexual experiences, and applied to the Private Diva Gynecological Diseases and Women's Health Clinic in Cankaya district of Ankara province for gynecological examination, hymen examination, or pregnancy diagnosis from 01/06/2020 to 01/09/2020. Results Of the participants, 63% were undergraduate students and 47% had stated that their first sexual experience is a private experience, while 42% stated that they were not careful about protection during their first sexual experience. It is worth noting that those who were careful about protection were the participants in the age range of 18-25 (45.5%). It was seen that 80% of those who live in the Black Sea Region have not had any sexual activity before their first sexual experience and that 46.7% of participants who live in this region again were not careful about protection. Conclusion In the link from healthy individuals to a healthy society, sexual education must be started before their first sexual experience and continued throughout life.

ABSTRACT

OBJECTIVES: The purpose of our study is to evaluate the information available on YouTube regarding FGCS procedures in terms of content, accuracy, reliability and quality. METHODS: Videos were listed on YouTube on 01.10.2020 by searching the terms "plastic surgery for female genital", "vaginoplasty", "labiaplasty" and selecting their relevance from the filtering options. Among these listed videos, the 50 most watched videos were listed and recorded excluding commercials, repeating videos, those shorter than 60s and non-English ones. The videos were scored by two researchers using DISCERN and JAMA scales. The DISCERN scale varies between 0-75 points and according to the scores obtained from the scale, videos are classified as 63-75: (excellent); as 51-62: (good); as 39-50: (moderate) as 28- 38: poor and as <28 very poor. JAMA scale is a scale that evaluates the accuracy, usefulness and reliability of video sources between 0-4. Insufficient data on 1 point video source obtained from the scale, refers to. 2-3 (partially sufficient data) and 4: (completely sufficient data). RESULTS: The evaluation on DISCERN scores given to the videos revealed that the average DISCERN score of the first researcher was 45.65±13.271 and the average of the second researcher's DISCERN score was 44.65±14.235. The average of JAMA scores was 2.67±0.826 for the first researcher and 2.49±0.845 for the second researcher. Accordingly, a statistically significant linear relationship was found between DISCERN (r=0.812) scores and JAMA (r=0.622) scores by the researchers. CONCLUSION: We found that YouTube has the potential to provide patients with easy access to a large amount of information on FGCS, although almost all of the videos we included in the study were uploaded by health professionals, the overall quality of the content on the subject was moderate. We strongly suggest that any videos on health should be uploaded by experts and these videos should definitely be reviewed by a referee.

ABSTRACT

OBJECTIVES: Recent studies have shown a more frequent occurrence of sexual dysfunction in patients with headache. The aim of this study was to assess the effects of demographic and clinical characteristics and psychiatric symptoms on sexual dysfunction in Turkish female patients with migraine. METHODS: In all, 18 sexually active patients with episodic migraine (EM), 12 patients with chronic migraine (CM), and 22 healthy controls of similar age were enrolled in the study. A numeric rating scale was administered to assess pain intensity. The psychiatric symptoms and sexual function of all of the participants were evaluated using the Beck depression and anxiety scales and the Golombok-Rust Inventory of Sexual Satisfaction (GRISS). RESULTS: The mean GRISS subscale scores did not differ significantly between the migraine groups and the control group (all p values <0.05). A positive correlation was found between the duration of headache and GRISS subscales of noncommunication, dissatisfaction, vaginismus, and anorgasmia in EM patients. In addition, there was a negative correlation with the infrequency and avoidance subscales. No correlation was detected between the GRISS subscale scores and the demographic and clinical characteristics of the patients with CM, with the exception of the level of education. Higher pain intensity scores and the presence of anxiety or depression among the EM and CM patients significantly affected all of the subscale scores of the sexual function inventory. CONCLUSION: Although there was no relationship between migraine chronicity and sexual dysfunction, our data indicated that patient demographic characteristics, greater pain severity, and comorbidities of depression or anxiety were associated with greater sexual dysfunction among patients with EM and CM.

ABSTRACT

Our aim was to evaluate the optimal treatment strategy addressing cardiovascular risk in obese and nonobese patients with polycystic ovary syndrome (PCOS). We planned a prospective randomized clinical study. Normoandrogenemic and oligoamenorrheic women with PCOS and impaired glucose tolerance (n = 96) were enrolled in the study. Six months of treatment with metformin HCL or oral contraceptive pills (OCPs) were given to the patients. Group 1 were obese and receiving metformin. Group 2 were obese and receiving OCPs. Group 3 were nonobese and receiving metformin, and Group 4 were nonobese receiving OCPs. ADMA, homocysteine, high sensitive C-reactive protein (hs-CRP) and homeostasis model assessment estimate of insulin resistance (HOMA-IR) were investigated. ADMA, homocysteine, hs-CRP and HOMA-IR were similar in obese and nonobese groups before the treatment. After 6 months of treatment, a significant decrease was observed in ADMA, homocysteine and HOMA-IR levels in Groups 1 and 3. An increase in ADMA and hs-CRP levels was observed in Groups 2 and 4. In this study, metformin treatment leads to improvement in hormonal and metabolic parameters and decreases ADMA and homocysteine levels possibly independent of BMI. However, the use of oral contraceptives in obese and nonobese patients with PCOS with impaired glucose tolerance increases ADMA and hs-CRP levels and creates an increase in the metabolic risk.
