ABSTRACT

BACKGROUND: Despite the highest (88%) Prevention of Mother-To-Child Transmission (PMTCT) of HIV coverage in Eastern Africa, 50% of new HIV infections in children aged 0-14 years occur in the region. OBJECTIVE: The aim of this study was to assess the feeding modalities, the rate of HIV transmission and its predictors among HIV exposed infants (HIV-EIs) visited Gamo and Gofa Zones public health facilities, Southern Ethiopia from January 2013 to February 2019. METHOD AND MATERIALS: Institution-based retrospective follow up study was employed among 450 HIV-EIs having DNA/PCR test results. All infant-mother pair records in selected health facilities were reviewed using a standard data extraction tool from March to July 2019. HIV transmission probabilities were assessed by Kaplan-Meier time-to-event analysis method and log-rank tests were used to compare the risk among different groups. The Cox-proportional hazards model, adjusted on infant feeding modalities and other co-variants was used to identify predictors of HIV transmission, and statistical significance was declared at a p-value of < 0.05. RESULTS: In total, 383 complete records were analyzed. In the study, 85.6% (95%CI: 81.6%, 89.1%) of HIV-EIs were exclusively breastfed in the first six months. The 18 months probability of infant HIV transmission was 64 (16.7%) (95%CI: 13.1%-20.8%). The risk of HIV-transmission was higher among infants who were delivered at the hospital than health centers/health posts (AHR = 3.07; 95%CI: 1.19, 7.95); discontinued Cotrimoxazole prophylaxis in at least one visit (AHR = 6.32; 95%CI: 3.35, 11.94); did not exclusively breastfeed (AHR = 3.07; 95%CI: 1.72, 5.47) and came from urban areas (AHR = 5.90; 95%CI: 1.40, 24.85). CONCLUSIONS: The study showed that HIV-EIs had a greater rate of 18 months HIV transmission than the national pooled prevalence. The risk of transmission is higher among infants who do not breastfeed exclusively for the first 6 months, and the risk increases with the number of months spent by breastfeeding. Therefore, strengthening counselling on safer feeding options and Cotrimoxazole prophylaxis use; provision of quality PMTCT service with special focus in hospitals and urban residents were recommended.

ABSTRACT

Crop type observation is crucial for various environmental and agricultural remote sensing applications including land use and land cover mapping, crop growth monitoring, crop modelling, yield forecasting, disease surveillance, and climate modelling. Quality-controlled georeferenced crop type information is essential for calibrating and validating machine learning algorithms. However, publicly available field data is scarce, particularly in the highly dynamic smallholder farming systems of sub-Saharan Africa. For the 2020/21 main cropping season (Meher), the Ethiopian Crop Type 2020 (EthCT2020) dataset compiled from multiple sources provides 2,793 harmonized, quality-controlled, and georeferenced in-situ samples on annual crop types (7 crop groups; 22 crop classes) at smallholder field level across the complex and highly fragmented agricultural landscape of Ethiopia. The focus was on rainfed, wheat-based farming systems. A nationwide ground data collection campaign (GDCC; Source 1) was designed using a stratification approach based on wheat crop calendar information, and 1,263 in-situ data samples were collected in selected sampling regions. This in-situ data pool was enriched with 1,530 wheat samples extracted from a) the Wheat Rust Toolbox (WRTB; Source 2; 734 samples), a database for wheat disease surveillance data [1] and b) an inhouse farm household survey database (FHSD; Source 3; 796 samples). Obtained field data was labelled according to the Joint Experiment for Crop Assessment and Monitoring (JECAM) guidelines for cropland and crop type definition and field data collection [2] and the FAO Indicative Crop Classification [3]. The EthCT2020 dataset underwent extensive processing including data harmonization, mixed pixel assessment through visual interpretation using 5 m Planet satellite image composites, and quality-control using Sentinel-2 NDVI homogeneity analysis. The EthCT2020 dataset is unique in terms of crop diversity, pixel purity, and spatial accuracy while targeting a countrywide distribution. It is representative of Ethiopia's complex and highly fragmented agricultural landscape and can be useful for developing new machine learning algorithms for land use land cover mapping, crop type mapping, agricultural monitoring, and yield forecasting in smallholder cropping systems. The dataset can also serve as a baseline input parameter for crop models, climate models, and crop disease and pest forecasting systems.

ABSTRACT

Ethiopian schools were closed for nearly 40 weeks as a measure to control the COVID-19 pandemic. The objective of the study was to evaluate the role of COVID-19 pandemic's restrictive measures on cognition and behavioral difficulties of schoolchildren in Arba Minch Health and Demographic Surveillance Site. Two cross-sectional surveys were conducted in November 2019 (pre-COVID-19-lockdown) and November 2020 (post-COVID-19 lockdown). Data were collected both at the school and homes of the children. Cognitive development of the children was assessed using digit span, Raven's coloured progressive matrices (RCPM) and Visual search using cancellation task. Behavioral difficulties score of the children was assessed using Strengths and Difficulties Questionnaire. Analysis of covariance (ANCOVA) was used to compare between the cognition outcomes and behavioral difficulties score pre- and post-COVID-19-lockdown. In a sub-group of children who provided data in both surveys, the difference in cognitive and behavioral outcomes was tested using a mixed effect model. Compared to the pre-COVID-19-lockdown, schoolchildren post-COVID-19-lockdown scored lower in the standardized performance index for the visual search task, which measures attention (0.27 SD, 95% confidence intervals (95%CI): -0.40, -0.13). However, they scored higher by 0.26 SD (95%CI: 0.13, 0.40) and 0.36 SD (95%CI: 0.22, 0.49) in digit span and RCPM, respectively, measuring working memory and non-verbal intelligence. There was no significant difference in total difficulties score between pre- and post-COVID-19-lockdown (0.01 SD, 95%CI: -0.12, 0.15). The subgroup analysis showed a significant increase in digit span among children post-COVID-19-lockdown while the other domains did not show a significant change. Factors contributing to the improvement of children's cognitive domains while away from the school environment should be explored and utilized to enhance child development. This study was registered at clinicaltrials.gov as NCT04137354 on October 14, 2019.

ABSTRACT

AIMS: The prevalence of metabolic syndrome (MetS) is on the rise in an increasingly urbanized world. The study aimed to review the association between urbanization and MetS in low- and middle-income countries (LMICs). DATA SYNTHESIS: A comprehensive search of five databases (MEDLINE, Web of Science, Scopus, EMBASE, and CENTRAL) was performed in January 2022 and updated in October 2022. Peer-reviewed studies that met the eligibility selection criteria were included. Search terms were used for the main concepts which are MetS, dietary patterns, and urbanization in LMICs. Study selection was done in two stages and in duplicate. Random effects models were used to calculate the overall pooled prevalence and main study-level characteristics. Out of 9,773 identified studies, nineteen were included in the systematic review and meta-analysis. The studies were done on 313,644 participants (149,616 urban and 164,028 rural). The pooled risk ratio (RR, 95% confidence interval) of MetS between urban and rural dwellers was RR = 1.24; 95%CI [1.15, 1.34] (I2 = 96.0%, P < 0.0001). A relatively higher prevalence of MetS among urban than rural residents has been observed, especially with the International Diabetes Federation criteria (RR = 1.54; 95%CI [1.21, 1.96]; I2 = 65.0%), and in the population in India (RR = 2.19; 95%CI = 1.24, 3.88, I2 = 85%). Overall, the role of dietary patterns in the development of MetS was inconsistent, and few studies showed a lower risk of MetS with adherence to recommended healthy dietary patterns. CONCLUSIONS: There was an association between urbanization and the high prevalence of MetS. Interventions and policies to reduce the risk of MetS are needed.

Introduction: Overweight/obesity is an emerging public health problem in low income countries. Currently sub-Saharan African countries are facing a double burden of malnutrition. Evidence has shown that overweight/obesity is becoming a problem for people living with HIV. But little is known in our setting. Thus, this study is designed to assess the association between overweight/obesity and ART drug regimen among adult HIV patients in Gamo zone public health facilities southern Ethiopia. Objective: To assess the association between overweight/obesity and ART drug regimen among adult HIV patients in Gamo zone public health facilities southern Ethiopia. Methods: Institution-based cross-sectional study was conducted from April 10 to May 10, 2022, among systematically selected adult HIV patients. The data were collected by using a structured interviewer-administered questionnaire, patient record review, and physical measurements. Multivariate logistic regression model was used to assess the association between dependent and independent variables. A p-value <0.05 with its 95% CI was considered as a statistically significant and interpreted accordingly. Results: The magnitude of overweight/obesity were 13.5% (95% CI:(10.4-17.2)). Being male (2.484(1.308, 4.716)), duration on ART (took for ≥5 years) ((2.249(1.218, 4.152)), and ART drug regime (3.789(1.965, 7.304)) were significantly associated with overweight/obesity. Conclusion: Overweight/obesity is significantly associated to ART drug regimen type among adult HIV patients. Furthermore, sex and duration on ART drug were found significantly associated with overweight/obesity of adult HIV patients.

BACKGROUND: In Ethiopia, child malnutrition is a significant public health problem. To address the problem, Nutrition-Sensitive Agriculture (NSA) program was introduced. However, there is a paucity of evidence about the prevalence of child undernutrition in NSA-implemented districts. Therefore, this study aimed to assess the prevalence of undernutrition among children aged 6-59 months in NSA-implemented districts. METHOD: A community-based cross-sectional study was conducted by enrolling 422 children aged 6-59 months paired with their mothers. A systematic sampling technique was used to select respondents. Data were collected by Open Data Kit (ODK) data collection platform, and Stata version 16 was used for analysis. The multivariable logistic analysis model was fitted to assess the association between variables, and 95% CI was estimated to measure the strength of the association. The level of statistical significance was declared at a p-value of less than 0.05 in the multivariable model. RESULT: Overall, 406 respondents participated in the study, and a response rate of 96.2% was obtained. The prevalence of stunting, wasting, and underweight was 24.1% (95% CI: 19.9-28.4), 8.87% (95% CI: 6.3-12.1) and 19.95% (95% CI: 16.2-24.2), respectively. Household food insecurity was significantly associated with being underweight (AOR: 3.31, 95% CI (1.7-6.3). Child dietary diversity (AOR: 0.06, 95% CI: 0.01-0.48) and being a beneficiary of the NSA (AOR: 0.12, 95% CI: 0.02-0.96) program were associated with wasting. Lack of ANC visits and diarrhea in the past two weeks was associated with stunting and wasting, respectively. CONCLUSION: The prevalence of malnutrition was a moderate public health problem. Wasting was more prevalent than the recent national and Amhara region averages. However, the prevalence of stunting and underweight was lower than the national average and other studies conducted in Ethiopia. Healthcare providers should work to increase dietary diversity, ANC visits, and reduce diarrheal disease.

ABSTRACT

INTRODUCTION: A poor oral hygiene is associated with dental caries, gingivitis, periodontal diseases, bad breath, respiratory and cardiovascular diseases, and chronic kidney diseases. Moreover, a poor oral health has psychosocial impacts that diminish a quality of life and restrict activities in school, at work, and home. African regions carry a major burden of oral health problems. However, very few studies highlighted about oral hygiene practices and there is also paucity of information in Ethiopia. This study was, therefore, designed to identify an oral hygiene practice on patients/clients visiting dental clinics in Hawassa City, Southern Ethiopia. OBJECTIVE: To assess oral hygiene practices and associated factors among patients/clients visiting private dental clinics, Hawassa City, Southern Ethiopia. METHODS: Institution-based cross-sectional study was employed among patients/clients attending private clinics in Hawassa City from January 27 to February 8, 2018. Systematic random sampling technique was used to select 403 study participants. Data were entered into EpiData 3.1, cleaned, and analyzed by SPSS 20. A multivariable logistic regression analysis was performed to assess the association between independent and outcome variables. Crude and adjusted OR with 95% confidence level was estimated, and variables having P value ≤0.05 in multivariable analysis were considered as significant. RESULTS: 393 study participants participated making a response rate of 97.52%. A median age of respondents was 27 ± 10.9. About 153 (39.9%) of the study participants had poor oral hygienic practice. Male (AOR: 1.63, 95% CI: (1.053, 2.523)), rural residence (AOR: 3.79, 95% CI: (1.724, 8.317)), and poor knowledge about oral hygiene (AOR: 2.38, 95% CI: (1.402, 4.024)) were independently associated to poor oral hygienic practice. CONCLUSION: More than one-third of the study participants had poor oral hygienic practice. Providing health information regarding oral hygiene for the patients/clients in the facilities with a special focus from rural areas is recommended.

BACKGROUND: Tobacco use is one of the world-leading preventable killers. There was a varied prevalence of tobacco use and cigarette smoking across different areas. The aim of the study was to assess the prevalence and factors associated with current tobacco use among adults residing in Arba Minch health and demographic surveillance site (HDSS). METHODS: A community-based cross-sectional study was conducted among adults residing in Arba Minch HDSS in 2017. The estimated sample size was 3368 individuals which were selected by simple random sampling techniques using Arba Minch HDSS dataset. Data collection tools were obtained from the WHO STEPwise. Current use of tobacco, which defined as the current use of smoked and/or smokeless tobacco, was considered as the dependent variable. A binary logistic regression model was used to identify candidate variables for the multivariable logistic regression model. An adjusted odds ratio (AOR) at a p-value of less than 0.05 was used to determine a statistically significant association between independent and dependent variables. RESULT: The prevalence of tobacco use among adults was 20.2% (95% CI: 18.9-21.6%). The current use of smoked and smokeless tobacco were 17.1% (95%CI: 15.8-18.4%) and 9.7% (95%CI: 8.8-10.8%), respectively. The current use of tobacco was significantly associated with sex (female [AOR 0.54; 95%CI: 0.42-0.68] compared to men), age group (35-44 [AOR 1.57; 95%CI: 1.14-2.17], 45-54 [AOR 1.99; 95%CI: 1.45-2.74], and 55-64 [AOR 3.26; 95%CI: 2.37-4.48] years old compared to 25-35 years old), physical activity (moderate physical activity level [AOR 0.65; 95%CI: 0.44-0.96] compared with low) and residency (highland [AOR 4.39; 95% CI: 3.21-6.01] compared with at lowlander). Also, heavy alcohol consumption (AOR 3.97; 95% CI: 3.07-5.12), and Khat chewing (AOR 3.07(95%CI: 1.64-5.77) were also associated with the use of tobacco among the study participants. CONCLUSION: Nearly one in five adults used tobacco currently in the study area, which is more than the national reports. Interventions for the reduction of tobacco use need to give due attention to men, older adults, uneducated, poor, and highlanders.

ABSTRACT

BACKGROUND: Anemia, defined as a low blood hemoglobin concentration, has been shown to be a major public health concern in low-income countries like Ethiopia. School-age children are the most vulnerable population groups for anemia. The aim of this study was to assess the prevalence of anemia, with consideration of altitudinal variations, and to identify factors associated with anemia among school-age children. METHODS: A community-based cross-sectional study was conducted from April to May 2017 among randomly selected 391 school-age children (6 to 14 years) in Arba Minch Health and Demographic Surveillance Site, Southern Ethiopia. Hemoglobin concentration was measured on the spot using portable hemoglobinometer (HemoCue Hb 201). The hemoglobin cut off values, adjusted for child age and altitude, were used to define anemia. Stool microscopic examination was done for investigation of intestinal parasites. A binary logistic regression model was used to assess the possible association of independent and outcome variables. RESULTS: The overall prevalence of anemia was 37.3% (146); (95% CI: 32.5, 42.2). Among those who were anemic, 110 (28.1%) and 35 (9%) had mild (Hb 11-11.4 g/dl for children age from 6 to 11 years and 11-11.9 g/dl for children age from 12 to 14 years) and moderate (Hb 8-10.9 g/dl) anemia respectively. A single case of severe (Hb < 8 g/dl) anemia was identified. Fifty-seven (46.3%) of children living in an altitude ≥ 2500 m above sea level were anemic. Anemia was higher among children who were positive for intestinal parasitic infections (AOR = 3.30, 95% CI: 2.04, 5.35) and children not-enrolled to schools (AOR = 2.05, 95%CI: 1.26, 3.32). Anemia was less common among children who had no habit of eating vegetables in the last week prior to the survey (AOR = 0.35, 95%CI: 0.14, 0.84). CONCLUSIONS: More than one-third of school-age children were suffering from anemia. Intestinal parasitic infections and school non-enrollment were among the major factors associated with anemia among school-age children in the study area. Interventions, focusing on identified contributing factors need to be implemented by integrating with other school or community-based health programs.

INTRODUCTION: Despite consistent economic growth in the country, malnutrition remains one of the major public health problems in Ethiopia. The prevalence of malnutrition and its associated factors are well studied among under-five children. However, there is a paucity of evidence among older children in developing countries including Ethiopia. The aim of the study was to assess the prevalence of stunting and thinness and their associated factors among school-age children. METHODS: A community-based cross-sectional study was conducted among randomly selected 389 school-age children in Arba Minch Health and Demographic Surveillance Site, Southern Ethiopia, during April and May 2017. Height for age and body mass index for age z scores were calculated using WHO Anthro Plus software as indicators of stunting and thinness respectively. A binary logistic regression model was used to assess the association between independent and outcome variables. RESULTS: The prevalence of stunting and thinness were 41.9% (95% CI: 37-47) and 8.0% (95% CI: 5.4-10.8) respectively. The likelihood of stunting was significantly higher among children within the age group of 12-14 years old (AOR = 2.97, 95% CI: 1.78-4.95); children who were male (AOR = 1.94, 95% CI: 1.21-3.10); children living in households with medium wealth terciles (AOR = 2.90, 95%CI: 1.39-6.04); and children who were non-enrolled in schools (AOR = 2.25, 95% CI: (1.37-3.70). Moreover, thinness was 63% less common among children who had a dietary diversity score of <4 food groups (AOR = 0.37, 95%CI: 0.16-0.89). CONCLUSION: The prevalence of thinness is low when compared to the prevalence reported by a single national school health and nutrition survey in Ethiopia. Stunting is a major public health concern. Therefore, this finding warrants the need to implement school health and nutrition programs to improve the nutritional status of school-age children in the study area. The interventions should focus towards both gender and special emphasis should also be given to increase the enrollment of children in schools. In addition, effort should be taken by stakeholders in different hierarchies to improve the family wealth status.

BACKGROUND: Self-care practices in diabetes patients are crucial to keep the illness under control and prevent complications. Effective management of diabetes will be a difficult task without adequate understanding of the existing level of practice related to diabetes self-care. This study is, therefore, aimed at assessing the self-care practice and its associated factors among patients with type 2 diabetes in Tikur Anbessa Specialized Hospital, Ethiopia. MATERIALS AND METHODS: A health facility-based cross-sectional study was conducted among 419 type 2 diabetes patients from March 29, 2013, to May 16, 2013. The data were collected by face-to-face interview using structured and pretested questionnaire. Binary logistic regression was performed to assess the association between determinant factors and adherence to self-monitoring of blood glucose. Adjusted odds ratios (AOR) with its 95% confidence interval (CI) were estimated to identify factors associated with the outcome variables in the multivariable analysis. RESULT: In this study, 318 (75.9%) diabetes patients did not adhere to the recommended diet management, 350 (83.5%) did not adhere to self-monitoring of blood glucose level, while 18 (4.3%) of the respondents did not adhere to the prescribed medications. Diabetic patients who were unemployed were 2.4 times more likely to practice blood glucose monitoring than merchants (AOR [95% CI] =2.4 [1.3-5.9]). Those who attended primary education were 70% less likely to adhere to blood glucose self-monitoring than those educated to a tertiary educational level (AOR [95% CI] =0.3 [0.1-0.9]). Respondents within the age group of 40-49 years were 11 times more likely to adhere to their medication than those aged 60-76 years (AOR [95% CI] =11 [1.03-13.6]). CONCLUSION: The study showed that the extent to which individuals adhere to the recommended management of type 2 diabetes is substantially low. Improving awareness of patients and the community at large is imperative especially on medication adherence, glycemic control and diet management.
