ABSTRACT

Group B Streptococcus (GBS) is the leading cause of bacterial neonatal sepsis. This study aimed to confirm the effect of Ligilactobacillus salivarius V4II-90 on GBS colonisation during pregnancy. A randomised, multicentre, double-blind, placebo-controlled, parallel-group study was conducted in seven hospitals in Madrid, Spain. The sample was broken down into two groups with 20 participants each (n = 40) in order to show reduced GBS colonisation frequency in the probiotic versus the placebo group. Pregnant participants positive for vaginal-rectal colonisation before or during the 13th week of gestation were randomly assigned to either the placebo or the probiotic group. The probiotic, L. salivarius V4II-90 at 1 × 109 cfu/day was administered for 12 weeks, starting at week 21-23 of gestation. The primary outcome was the percentage of participants with vaginal and/or rectal GBS colonisation at the end of the intervention period (35 weeks of gestation). Secondary outcomes were changes in the microbial composition of vaginal and rectal exudates; premature delivery; premature rupture of membranes; intrapartum antibiotics; new-borns with early or late-onset GBS sepsis; adverse events (AEs); and GBS test results performed at the hospital at week 35 of gestation. Of the 481 participants included, 44 were vaginal-rectal colonised with GBS and randomised. 43 completed the study (20 in the probiotic group and 23 in the placebo group). After intervention, GBS was eradicated in six participants (27%) from the placebo group and in twelve participants (63%) from the probiotic group ( P = 0.030). None of the 185 AEs reported were identified as possibly, probably, or definitely related to the investigational product. In conclusion, oral administration of L. salivarius V4II-90 is a safe and successful strategy to significantly decrease the rates of GBS colonisation at the end of pregnancy and, therefore, to reduce the exposure of subjects and their infants to intrapartum antibiotic prophylaxis. Trial registered at ClinicalTrials.gov: number NCT03669094.

ABSTRACT

From 1885 to 1887, it circulated in Havana one of the most prestigious Cuban periodicals among all the ones devoted to Medicine and related disciplines that were published in the colonial period. This publication, entitled La Enciclopedia, which includes in its pages numerous works of high scientific and historic value generated by well-known personalities of that time, is a collection that constitutes nowadays a genuine bibliographical relic. This article contains an analytic index of 516 works published in La Enciclopedia during the three exact years that it circulated to spread the results of the Cuban intellectual work on chemistry, physics, pharmacy, medicine and natural sciences, in general. In this analytic index, a chronologic relation of all those works, grouped in a section entitled Bibliographical Section, is offered in the first place. Secondly, it appears the Authors Section, where the names of those who signed the works can be found in alphabetical order and, next, the Thematic Section in which the subjects or specific topics that were dealt with in each work are registered in an alphabetical order, too.

ABSTRACT

The first texts that registered information related to eye illnesses in the ancient world are cited, and the content of the works that paved the way of the ophthalmologic bibliography at the international level, from the invention of the printing press to those published in the second half of the XIX century, is revealed. The first references to the bibliographic production on this discipline in Cuba are mentioned, as well as the first articles of a true scientific character related to it, found in the pages of the first medical journal edited in the island. A brief analysis is made of the first book written in Spanish exclusively dedicated to eye affections, which was written, edited and published in two volumes, in Havana. Reference is made to other works of Cuban authors related to eye illnesses that due to their importance have gone beyond the time. The titles of the national magazines specialized in this topic that were published during the XX century are also mentioned.

ABSTRACT

ABSTRACT

OBJECTIVE: To analyze the effect of human immunodeficiency virus (HIV) status and other variables on the outcome of tuberculosis treatment in Spain. PATIENTS AND METHODS: Multicenter retrospective cohort study in 6 autonomous communities of Spain (from May 1996 to April 1997). Data on treatment outcome were collected for new cases of tuberculosis in accordance with European guidelines. Follow up of patients continued for 3 months after scheduled end of treatment. RESULTS: Of the 4899 patients included, 3417 (69.7%) had a satisfactory outcome, 438 (8.9%) died before or during treatment, and 1044 (21.4%) had a potentially unsatisfactory outcome. On stratification by HIV status, satisfactory outcome, mortality, and potentially unsatisfactory outcome were reported for 43.4%, 21.5%, and 35.1%, respectively, of HIV-positive patients; 71%, 6.2%, and 22.8%, respectively, of HIV-negative patients; and 74.3%, 7.5%, and 18.2%, respectively, of patients with no HIV status available. HIV modified the effect of several variables on the outcome of treatment, and so separate logistic regression models for each HIV category were constructed. Among HIV-positive patients, mortality increased in patients with neoplastic disease and in users of drugs by nonintravenous routes of administration, whereas potentially unsatisfactory outcomes increased in intravenous drug users and in women. CONCLUSIONS: In Spain, the outcome of tuberculosis treatment is much worse in HIV-positive patients. Drug use and presence of neoplastic disease substantially affect mortality.
