ABSTRACT

The National Spinal Cord Injury Statistical Center estimates 294,000 people in the US live with a spinal cord injury (SCI), with approximately 17,810 new cases each year. Although the physical outcomes associated with SCI have been widely studied, the psychological consequences of sustaining a SCI remain largely unexplored. Scant research has focused on posttraumatic stress disorder (PTSD) in this population, despite prevalence estimates suggesting that up to 60% of individuals with SCI experience PTSD post-injury, compared to only 7% of the general US population. Fortunately, prolonged exposure therapy (PE) is a well-researched and highly effective treatment for PTSD. However, no trauma focused exposure-based therapy for PTSD (e.g. PE) has not yet been tested in a SCI population. Thus, we aim to conduct the first test of an evidence-based intervention for PTSD among patients with SCI. Adults with SCI and PTSD (N = 60) will be randomly assigned to either: (1) 12-sessions of PE (2-3 sessions per week) or (2) a treatment as usual (TAU) control group who will receive the standard inpatient rehabilitation care for SCI patients. Primary outcomes will be assessed at 0, 6, 10, and 32 weeks.

ABSTRACT

Pelvic injuries often result from high-energy trauma and lead to significant functional impairment. While the physical outcomes of these injuries have been widely studied, the psychological consequences remain largely unexplored. The purpose of this study was to examine psychosocial and functional outcomes of patients with pelvic trauma in the year after injury. The sample (N = 32) consisted of adult patients with traumatic pelvic injures, as defined by ICD-9 codes, who were admitted to a Level I Trauma Center for at least 24 h. Participants were primarily female (53%) with a mean age of 48.7 years (SD = 17.9). Demographic, injury-related, and psychosocial data (e.g., posttraumatic stress disorder (PTSD), depression, alcohol use, quality of life, pain, return to work) were gathered at the time of hospitalization as well as at 3-, 6-, and 12 month follow-ups. Mixed regression models were used to examine the outcome variables over time. There were significant decreases in pain and alcohol use at each follow-up compared to baseline. However, despite the decrease, the levels of pain and alcohol use remained high. Physical and mental health also decreased significantly, indicating worsened functioning and lowered quality of life. Neither PTSD nor depression changed significantly over time, indicating that participants' symptoms were not likely to improve. These data suggest that sustaining a traumatic pelvic injury increases the risk of diminished quality of life, both mentally and physically. Even one-year post-injury, participants experienced moderate physical pain and higher levels of PTSD, depression, and problematic alcohol use than would be expected in the general population. These findings highlight the need for an interdisciplinary approach to treating patients with pelvic injuries, including psychological screening and intervention in acute care and throughout recovery.

ABSTRACT

This is the first study to compare both physical and psychological outcomes in geriatric and non-geriatric patients (n = 268) at baseline and 6 months post-trauma. Demographic, clinical, and psychological data, including screens for alcohol use, depressive symptoms, and post-traumatic stress symptoms (PTSS) were collected from 67 geriatric patients (70.7 ± 8.0 years) and 201 non-geriatric patients (40.2 ± 12.8 years) admitted to a Level I trauma center for ≥ 24 h. Geriatric patients were significantly less likely to screen positive for alcohol use at baseline, and depression, PTSS, and alcohol use at follow-up. When not controlling for discharge to rehabilitation or nursing facility, geriatric patients had significantly lower odds of alcohol use at follow-up. There was no significant difference in injury severity, resilience, or pre-trauma psychological status between the two groups. Results indicate that geriatric trauma patients fare better than their younger counterparts at 6 months post-trauma on measures of alcohol use, depression, and PTSS. Screenings and interventions for both age groups could improve psychological health post-trauma, but younger patients may require additional attention.

Las manifestaciones cutáneas, son comunes y diversas en los pacientes con insuficiencia renal crónica (IRC), sobre todo, en aquellos en hemodiálisis (HD). Estas pueden ser anteriores al inicio de la diálisis o podrían ser precipitadas por ella. Materiales y Métodos: estudio observacional, descriptivo, de serie de casos, prospectivo a través del examen de piel realizado por dermatólogos, a 100 pacientes con IRC en HD del Servicio de Nefrología del Hospital Provincial del Centenario de Rosario, Argentina durante el período comprendido entre el 1 de Junio de 2013 hasta el 1 de Junio de 2014. Resultados: en este estudio se incluyeron 100 pacientes con IRC en HD, de los cuales 91 presentaron manifestaciones cutáneas. Discusión: la frecuencia de patologías cutáneas en pacientes con IRC en HD, varía del 50 al 100% y generalmente, se observan más de una manifestación cutánea. Esto coincide con lo hallado en nuestro trabajo, donde de los 100 pacientes: 91 presentaban lesiones y de éstos, la mayoría presentaba 2 o más. Respecto de la edad, el rango es amplio y en nuestro trabajo se observó un predominio en el sexo masculino, al igual que en otros estudios.


Skin manifestations are common and diverse in patients with chronic renal failure (CRF), especially those on hemodialysis (HD). These may be prior to initiation of dialysis or could be precipitated by it. Materials and Methods: observational, descriptive, case series, prospective through the skin examination by dermatologists of 100 patients with CKD on HD in the Nephrology Service of Hospital Provincial del Centenario of Rosario, Argentina during the period June 1, 2013 to June 1, 2014. Results: in this study of 100 patients with CRF in HD, of which 91 had cutaneous manifestations were included. Discussion: the frequency of skin diseases in patients with CRF in HD, varies from 50 to 100% and it is usually seen more than one cutaneous manifestation. This coincides with the findings in our study where from 100 patients, 91 had injuries and of these the majority had 2 or more. Regarding age, the range is wide and our study found a predominance in males, as in other studies.

Esto se encuentra principalmente en frutas, verduras, legumbres, cereales y hortalizas. Estos alimentos bloquean el efecto perjudicial de los radicales libres.

Verrucous epidermal nevus is a hamartroma disorder of the ectodermal development. We report a 9 years old male that developed a verrucous plaque on the internal part of the right ankle. Histopathological study reveled verrucous epidermal nevus. A surgical removal was performed without complications or recurrence. There are multiple clinical variations and treatments options, depending on the extension of the lesions.(AU)

El signo de Darier fue positivo. La histopatología mostró edema intersticial y en la tinción con Giemsa, se observaron mastocitos. Se realizó el diagnóstico de un mastocitoma solitario. El mastocitoma solitario es una lesión tumoral benigna, que representa aproximadamente el 15% de la mastocitosis cutánea.(AU)


We report a 16 month years old female, with a 14 months history of a single erythematous, L shaped well-delimited plaque located on her upper back. Darier sign was positive. Histopathology revealed interstitial edema. The Giemsa staining showed mast cells. The diagnosis of a solitary mastocytoma was performed. Solitary mastocytoma is a benign tumoral lesion, that represents approximately 15% of cutaneous mastocytosis.(AU)

El fibroma digital celular consiste en una neoplasia benigna, generalmente solitaria, localizada en un dedo del pie o de la mano, que suele comprometer la región ungular y regresar espontáneamente. Presentamos un caso de sexo masculino de 27años, con una lesión nodular en el segundo dedo del pie derecho. La histopatología mostró una proliferación fuso celular dérmica acral, con presencia de fibroblastos reactivos y mastocitos, entre haces de colágeno engrosados y capilares dilatados. Inmunomarcación: vimentina (+), CD68 (+) en células aisladas, proteína s100 (-), actina muscular lisa (-), CD34 (+) focal.(AU)


Digital fibromas are common benign acral tumors typically reported as angiofibromas (AFs) or acquired digital fibrokeratomas (ADFs). Cellular variants are not well recognized. Nodules develop mainly in fingers and toes. There is high recurrence but spontaneous regression without treatment has also been reported. We report a man of 27 years-old, showing a one cm nodule in the second finger of the right foot. Histological examination revealed spindled tumor cells containing characteristic intracytoplasmic inclusion bodies that stained with vimentin, CD68 positive, protein S100 negative, muscle specific actin negative and CD34 positive. These findings suggest that a subset of digital fibromas is characterized by a dense cellular proliferation of CD34 positive spindle cells. Awareness of this variant of digital fibroma and its staining pattern is critical in preventing misdiagnosis as dermatofibrosarcoma protuberans, particularly in superficial biopsies.(AU)

Los Nevos Melanocíticos Congénitos (NMC) son lesiones pigmentarias, que pueden aparecer en el nacimiento, aunque hay casos comunicados hasta el primer año de vida (Nevo Melanocítico Congénito Tardío). Presentan ciertas características clínicas y evolutivas de gran importancia: el compromiso estético de las formas gigantes, la asociación con melanocitosis meníngea y su transformación maligna de la lesión cutánea o meníngea. Presentamos tres casos de nevos melanocíticos congénitos gigantes. Cabe destacar, que el objetivo de este artículo es realizar una revisión del tema, orientar al seguimiento y control de los mismos y destacar la dificultad para su tratamiento debido a su gran tamaño.(AU)

Certain features and evolution clinics that are important: the great compromise of aesthetic forms giant, meningeal melanocitosis association and its malignant transformation. We present three cases of Giants Congenital Melanocytic Nevus. ABSTRACT

La Neurofibromatosis fue descrita por Von Recklinghausen en 1882. Es una genodermatosis autosómica dominante con alta penetrancia, expresividad variable y curso progresivo; se calcula que en el 50% de los casos se deben a mutaciones espontáneas (de novo), con una incidencia de 1/3000 recién nacidos. Clínicamente se manifiesta por el compromiso de los tejidos derivados del neuroectodermo y mesodermo. Muestra alteraciones oculares, neurológicas, de partes blandas, cutáneas, óseas, endocrinológicas y lesiones viscerales. En la actualidad se admiten dos formas clínicas principales: la Neurofibromatosis clásica o de tipo I que representa el 85-90 % de los casos y la Neurofibromatosis tipo II (Neurofibromatosis del acústico bilateral) muy poco frecuente.


The Neurofibromatosis was described by von Recklinghausen in 1882. It is a dominant autosomic genodermatosis with a high penetration, variable clinic and progressive course; it is calculate, that in 50% of the cases the origin is a spontaneous mutations (novo). The incidence is 1/3000 newly born. The clinical manifestations are: the compromise of tissues derived from neuroectoderm and mesoderm. It also has ocular symptoms, visceral injuries, neurological alterations and endocrinological pathologies. Soft pars, cutaneous tissue and bones are affected. At the present time two main clinical forms are admitted: the Classic Neurofibromatosis or type I, that represents 85-90% of the cases and the Neurofibromatosis type II (Neurofibromatosis of the bilateral hearing aid) that it ´s not so frequent.

ABSTRACT

El síndrome de hipersensibilidad a drogas o síndrome DRESS es una reacción rara y potencialmente fatal; se caracteriza por presentar la tríada de fiebre, rash y compromiso de órganos internos. Fenitoína, fenobarbital y carbamazepina son los anticonvulsivantes aromáticos que con más frecuencia causan esta reacción. Dicho síndrome ocurre entre la 1ra y 8va semanas después de la exposición al fármaco. La eliminación inmediata de la droga es esencial para el manejo de estos pacientes. Presentamos una paciente de sexo femenino, de 62 años, medicada con carbamazepina durante un mes, que consulta por fiebre, mal estado general y rash máculopapuloso que se generalizó hasta abarcar el 90% de la superficie cutánea.


Anticonvulsivant hypersensitivity syndrome (AHS) is a rare and potentially fatal reaction characterized by the appearance of fever, skin rash and internal organ involvement. Phenytoin, phenobarbital and carbamazepine are the most frequent aromatic anticonvulsivant causing the reaction. This syndrome occurs 1-8 weeks after the initial drug exposure.

El embarazo es una condición que altera y provoca una serie de cambios metabólicos, endócrinos e inmunológicos que pueden incidir sobre la piel y sus anexos. Muchas de las influencias que el embarazo ejerce en la piel han de ser consideradas como fisiológicas; las enfermedades preexistentes pueden cambiar su comportamiento y además vamos a encontrar dermatosis propias de la gestación. El objetivo de esta revisión es actualizar los conceptos sobre estas entidades, su patogenia y manejo.


The pregnancy is a condition that alters and causes a series of metabolic changes, endocrines and immunological that can affect the skin and its attached ones. Many of influences that the pregnancy exerts in the skin have to be considered like physiological; the preexisting diseases can change behaviour and in addition we are going to find specific dermatoses of pregnancy. The objective of this revision is to update the concepts on these organizations, his pathogenic and handling.

Se presenta un caso de dermatosis acantolítica trnsitoria o enfermedad de Grover, descripta por Grover en 1970; la misma se trata de una dermatosis acantolítica papulosa benigna, de inicio en la edad adulta y de regresión espontánea. Se comentan los distintos mecanismos desencadenantes, las características clínicas, los diagnósticos diferenciales, los patrones histológicos y su terapéutica.
