ABSTRACT

Introducción: En el proceso de una intervención quirúrgica actúa un equipo multidisciplinar, cada uno de estos miembros realiza sus funciones correspondientes y son interdependientes, pero se complementan entre sí. La seguridad del paciente no consiste en crear un dispositivo complejo y muy costoso o en un profesional excelente, sino en aplicar a nuestro trabajo diario y en nuestra práctica habitual el mejor conocimiento posible. La gestión de riesgos clínicos se basa en la identificación, el análisis y la corrección de las causas que originan o pudieran originar daño secundario al paciente tras el procedimiento o la administración de cuidados, y la iatrogenia es uno de los factores de análisis y control. Caso clínico: Paciente de 15 años, que tras ser intervenido de torsión testicular derecha presentó quemadura por bisturí eléctrico en cara interna del muslo, producida de forma iatrogénica durante el procedimiento quirúrgico. La causa de la quemadura fue el descontrol de la placa de seguridad del bisturí electrónico usado en la intervención. Plan de actuación: Se realizó cura en ambiente húmedo con apósito de alginato de Ag, que se fijó con apósito de espuma de silicona con reborde. El retroceso y estancamiento de la evolución precisó de antibioterapia sistémica tras cultivo y antibiograma, que propició la resolución del caso hasta la cicatrización total de la herida. Discusión y conclusiones: La cura en ambiente húmedo se convierte en una alternativa efectiva ante este tipo de lesiones. El abordaje integral y multidisciplinar del paciente y la posterior planificación de actuaciones se han mostrado eficaces a la hora de solucionar los problemas detectados en la valoración inicial, conllevando determinadas ventajas, así como el análisis y evaluación del proceso de seguridad del pacient (AU)


Introduction: In the process of a surgical intervention, a multidisciplinary team acts, each of these members performs their corresponding functions, and they are interdependent, but they complement each other. Patient safety does not consist in creating a complex and very expensive device or in an excellent professional, but in applying the best possible knowledge to our daily work and habitual practice. Clinical risk management is based on the identification, analysis and correction of the causes that cause or could cause secondary damage to the patient after the procedure or the administration of care, with iatrogenesis being one of the analysis and control factors. Clinical case: A 15-year-old patient who, after undergoing surgery for right testicular torsion, presented an electric scalpel burn on the inner side of the thigh, produced iatrogenically during the surgical procedure. The cause of the burn was the lack of control of the security plate of the electronic scalpel used in the intervention. Action plan: A humid environment cure was performed with an Ag alginate dressing fixed with a silicone foam dressing with a border. The setback and stagnation of the evolution required systemic antibiotic therapy, after culture and antibiogram, which led to the resolution of the case until the total healing of the wound. Discussion and conclusions: Humid environment cure becomes an effective alternative to this type of injury. The comprehensive and multidisciplinary approach to the patient and the subsequent planning of actions have been shown to be effective in solving the problems detected in the initial assessment, entailing certain advantages. as well as the analysis and evaluation of the patient safety process (AU)

ABSTRACT

We propose a multilogistic regression model based on the combination of linear and product-unit models, where the product-unit nonlinear functions are constructed with the product of the inputs raised to arbitrary powers. The estimation of the coefficients of the model is carried out in two phases. First, the number of product-unit basis functions and the exponents' vector are determined by means of an evolutionary neural network algorithm. Afterwards, a standard maximum likelihood optimization method determines the rest of the coefficients in the new space given by the initial variables and the product-unit basis functions previously estimated. We compare the performance of our approach with the logistic regression built on the initial variables and several learning classification techniques. The statistical test carried out on twelve benchmark datasets shows that the proposed model is competitive in terms of the accuracy of the classifier.

ABSTRACT

The existence of a vesical diverticulum in the context of a congenital connective tissue disorder such as Ehlers-Danlos syndrome led us to consider the possibility of a relationship. Four types of diverticula can be found in the literature: congenital, acquired, iatrogenic and syndrome-associated. Within the later, Ehlers-Danlos syndromes type IV and IX, even type V, are associated to the existence of vesical diverticula. The potential spontaneous rupture of the diverticulum is a typical feature, as well as post-surgery relapse. The attitude towards such diverticula should be one of watchful waiting, and simple, plasty-free diverticulectomy on the bladder's neck is indicated when performing a surgical procedure.

ABSTRACT

Cystic-glandular cystitis is considered as part of the urothelial pre-neoplastic proliferative abnormalities. This group includes atypical hyperplasia. Von Brunn's nidus, and cystitis cystica. They are a consequence of the changes experienced at the urothelium level in response to inflammation, irritation or carcinogens. Diagnosis is mainly based in the pathoanatomical study of the biopsy obtained following endoscopic resection. The signs and symptoms it presents are varied and show a clear relationship to distribution and extension of cysts. Treatment is based in the removal of irritative factors. Cystectomy with urinary by-pass may be necessary if required by clinical evolution.

ABSTRACT

La existencia de un divertículo vesical en el contexto de una alteración congénita del tejido conectivo como es el Síndrome de Ehlers-Danlos, nos hizo plantearnos su posible relación. En la literatura se describen en la actualidad cuatro tipos de divertículos: congénitos, adquiridos, yatrogénicos y asociados a síndromes. Dentro de este último, el síndrome de Ehlers-Danlos en sus tipos IV y IX y ocasionalmente en el tipo V se relaciona con la existencia de divertículos vesicales. Es característica la posible ruptura espontánea del divertículo, así como su recidiva tras cirugía. La actitud ante estos divertículos debe a poder ser expectante y sólo, la diverticulectomía simple sin plastia sobre el cuello vesical está indicada, si fuera necesaria realizar alguna técnica quirúrgica. ABSTRACT

La cistitis quístico-glandular se engloba dentro de las anormalidades proliferativas preneoplásicas uroteliales. En este grupo se engloban la hiperplasia atípica, los nidos de Von Brunn y la cistitis quística.Se caracterizan por ser consecuencia de los cambios experimentados a nivel del urotelio en respuesta a la inflamación, la irritación o los carcinógenos. Su diagnóstico se basa fundamentalmente en el estudio anatomopatológico de la biopsia obtenida tras la resección endoscópica. Su clínica es muy variada, estando en clara relación con la distribución y extensión de los quistes. Su tratamiento se basa en la eliminación de los factores irritativos, pudiendo llegar a requerir la realización de una cistectomía con derivación urinaria si su evolución clínica asi lo requiriese (AU)

ABSTRACT

The present report is a contribution of our experience on the use of Fabian's prothesis. All patients had a urethral obstructive process which resulted from a hyperplastic or carcinomatous prostatic disease for which open- or endoscopic-surgery was totally contraindicated due to the patients' general status as a result of major cardiorespiratory disorders. A literature review and an evaluation of both indications and contraindications for using such prothesis was made.
