ABSTRACT

Relatamos o caso de mulher, 32 anos, branca, previamente hígida, atendida com corrimento vaginal e metrorragia há 3 meses. Exame colpocitológico descreveu esfregaço intensamente hemorrágico e lesão intraepitelial de alto grau com displasia acentuada (NIC III). Ultrassonografia transvaginal e ressonância magnética da pelve caracterizaram aumento dimensional do colo uterino, exibindo aspecto em "barril", por processo expansivo difuso de aspecto infiltrativo. Estudo anatomopatológico e imunohistoquímico de biopsia do colo uterino revelou um linfoma não Hodgkin difuso de grandes células B. (AU)


We report the case of a woman, 32 years old, white, previously healthy, presenting vaginal discharge and metrorrhagia for three months. Pap smear described intensely hemorrhagic smear and high-grade squamous intraepithelial lesions with severe dysplasia (CIN III). Transvaginal ultrasound and MRI of the pelvis characterized an increase in the dimensions of the cervix, displaying aspect "barrel" due process of expansive diffuse infiltrative appearance. The histopathological and immunohistochemistry of the cervical biopsy showed a diffuse non-Hodgkin's lymphoma large B cell. (AU)

ABSTRACT

Angiogenic T/natural killer (NK)-cell lymphoma is a non-Hodgkin lymphoma characterized by necrosis and vascular destruction that is strongly associated with Epstein-Barr virus and AIDS. Early diagnosis is essential to improve the chances of patient survival, but severe local inflammatory infiltrate impairs histologic diagnosis by obscuring neoplastic cells. The most common markers are CD2, CD56, cytoplasmic CD3, and CD43 EBV. We describe 3 cases of angiogenic T/NK-cell lymphoma that show the diverse presentation of the same disease. Patient 1 was HIV positive and had nasal obstruction, facial edema, and ulceration of the nasal mucosa. Patient 2 had fever, a sore throat, and weight loss. Patient 3 had facial edema, fever, proptosis, and rapid development of neurologic alterations. Several biopsies were needed for histologic confirmation in these patients, despite positivity for the CD3 and CD56 markers.

ABSTRACT

Casos de hantavirose foram notificados em diferentes regioes do Estado de Sao Paulo (SP), Brasil, durante o primeiro semestre de 1998. Dois casos fatais de sindrome pulmonar ocorreram em maio de 1998 na Cidade de Guariba, localizada na Regiao Nordeste de SP. Ambos os pacientes trabalhavam no mesmo local, estocando milho em um paiol infestado de roedores. Este pacientes, apos 2 ou 3 dias de doenca febril aguda inespecifica, desenvolveram uma grave pneumonia intersticial, que espalhou-se difusamente por ambos os pulmoes causando insuficiencia respiratoria e obito. A autopsia, ambos os casos apresentavam edema pulmonar intersticial com infiltrado de celulas mononucleares (imunoblastos) sugestivo de etiologia viral...

ABSTRACT

The case of a six-day-old neonate admitted in an emergency situation because of dyspnea and increasing cyanosis is reported. Despite abnormal opacification on the chest X-ray and left ventricular overload on the electrocardiogram and echocardiogram, features compatible with the disease, the diagnosis of massive pulmonary arteriovenous fistula affecting the whole left superior lobe, was made possible only after necroscopic examination


É relatado o caso de um neonato com seis dias de vida, encaminhado em caráter de urgência devido à cianose e dispnéia. Apesar da opacificação pulmonar na radiografia simples do tórax e da sobrecarga de câmaras esquerdas no eletrocardiograma e ecocardiograma, dados sugestivos da lesão, o diagnóstico de fístula arteriovenosa pulmonar maciça, envolvendo todo o lobo superior esquerdo, foi estabelecido, somente, através do estudo necroscópico
