ABSTRACT

Large proportion of deaths from SARS-CoV-2 infections occurred worldwide, especially in elderly patients. The aim of this research is to investigate the potential risk factors for mortality in the elderly patients with COVID-19. 65 years old or older patients with COVID-19 admitted to the Hospital «Alto Vicentino¼ of Santorso, Vicenza, from 23th February to 25th May 2020, were enrolled in this retrospective cohort study. Data of demographics, clinical features, comorbidities and blood tests were collected and compared for different outcomes. Charlson Comorbidity Index (CCI), Barthel Index (BI) and Modified Early Warning Score (MEWS) were evaluated. Univariate and multivariate logistic regression analyses were performed to explore risk factors for death. 243 patients with mean age 81,3±8,4 years were enrolled, of which 121 (49,8%) were females. This cohort included 11 (4,5%) mild, 131 (53,9%) moderate, 94 (38,7%) severe, 7 (2,9%) critical cases. CCI and BI were 2,8±2,1 points and 31,3±34,9 points respectively. Lethality rate was 28,4% (69 cases). Univariate logistic regression showed a significant increase in mortality risk with increasing age, CCI, polypharmacy, MEWS, Severity Index and reduced BI. Among blood tests thrombocytopenia, high CRP and elevated LDH showed a significant correlation with mortality. In the multivariate logistic regression high CCI, low BI and thrombocytopenia remained to be predictors of death. Tools, which evaluate functional dependence like BI and multiple morbidity like CCI, can be useful in identifying the elderly patients with COVID-19 at greater risk.

ABSTRACT

BACKGROUND: COVID-19 is still a global challenge in regard for management and therapy. Pulmonary embolism (PE) seems to have a higher prevalence in COVID-19 instead of non-COVID patients. Clinical and laboratory parameters related with PE are still unknown. METHODS: We conducted a retrospective unicentre study in Alto Vicentino Hospital between March 1st, 2020, and January 31st, 2021 in patients admitted for COVID-19 tested with a RT-PCR nasal swab. Data about patients studied with computed tomography pulmonary angiogram (CTPA) because of PE suspicion were collected, as their clinical and laboratory parameters too. RESULTS: 2621 patients were admitted for COVID-19 in Alto Vicentino Hospital between March 1st, 2020, and January 31st, 2021 and in 267 of them a CTPA was performed finding 50 PE (18.7%). Only non-Caucasian race (OR = 5.44; 95% CI 1.22-24.35; p = 0.027) and previous VTE (OR = 5.3; 95% CI 1.09-26.17; p = 0.039) were found to be independently associated with PE. CONCLUSION: PE is a frequent complication of COVID-19 and clinician need high degree of suspicion because clinical and laboratoristic parameters cannot drive diagnosis.

ABSTRACT

O objetivo deste artigo é discorrer sobre a formação que os cursos de Psicologia do Estado do Paranáoferecem para que os graduandos compreendam e avaliem alunos encaminhados com dificuldadesescolares, tomando-se como referência a Psicologia Histórico-Cultural. Foram entrevistados 14supervisores de estágio na área escolar de sete instituições de Ensino Superior do Paraná. Comoresultados da pesquisa, constatamos que, na formação dos psicólogos, a compreensão das queixasescolares é vinculada a questões sociais, saindo de uma visão que culpabiliza os indivíduos peloinsucesso escolar. Entendemos que a formação deve levar os alunos a entenderem os determinanteshistórico-sociais que influenciam na apropriação do conhecimento e contribuírem para umaintervenção que promova o desenvolvimento máximo dos indivíduos(AU)

ABSTRACT

A survey of fluoroscopic units in the state of Victoria has been carried out. For fixed fluoroscopy equipment, routine quality assurance tests were carried out on the x-ray tube used for conventional radiographs, and factors relating to image quality and patient dose were measured for the screening tube. For the mobile image intensifiers, quality assurance tests were carried out and factors relating to image quality and patient dose were measured. There was a ratio of about 20 in the dose rates between the units with the highest and lowest screening dose rates with the same size phantom. Guidance levels based on the third quartile values of the entrance dose rates and spot film doses are recommended. The recommended guidance doses are: mobile image intensifier: dose rate: 30 mGy/min; fixed screening unit: dose rate: 42 mGy/min and spot film: 5 mGy.

ABSTRACT

The case is described of a 39 year old man referred for clinical treatment of widespread oedema in the lower half of the body, especially the lower extremities. He was suffering from a gastroenteric protein malabsorption syndrome caused by syphilitic hypertrophic gastritis (tertiary syphilis). The syphilitic origin of the gastropathy was confirmed ex iuvantibus by penicillin treatment which cured the condition completely. The various forms of gastric lues, gastrointestinal malabsorption syndromes and ex iuvantibus criteria are discussed.
