ABSTRACT

BACKGROUND: Sensitization to acrylates is a concern in the occupational/environmental dermatology field. OBJECTIVE: To describe an occupational allergic contact dermatitis (ACD) outbreak from a smartphone screen protector glue. METHODS: Thirteen affected workers of a chain store selling phone screen protectors were investigated in five Spanish dermatology departments. The glue datasheet and label were assessed. A chemical analysis of the glue was performed. Based on this, some patients underwent additional testing. RESULTS: All patients (all female, mean age: 25) had severe fingertip dermatitis. The datasheet/label indicated that the glue contained isobornyl acrylate (IBOA), a "photoinitiator" and polyurethane oligomer. The company informed us that the ingredients were polyurethane acrylate, "methacrylate" (unspecified), acrylic acid, hydroxyethyl methacrylate, propylmethoxy siloxane, and photoinitiator 184. Isobornyl acrylate (or IBOA) and N,N-dimethylacrylamide (DMAA) were patch tested in eight and two cases, respectively, with negative results. A chemical analysis revealed 4-acryloylmorpholine (ACMO); isobornyl methacrylate (IBMA), and lauryl acrylate in one glue sample. Seven patients were patch tested with dilutions of the identified substances and six of seven were positive for ACMO 0.5% pet. CONCLUSION: An outbreak of occupational ACD, likely from ACMO in a glue is described. Further investigations are needed to corroborate the role played by each compound identified in the chemical analyses.

Se presenta el caso clínico de un paciente con calcinosis cutis distrófica en glúteo, una pa- tología infrecuente e infra-diagnosticada por la falta de sospecha clínica. Es de vital importancia que a quienes presenten calcificaciones cutáneas se les realice una detallada anamnesis y un estudio analítico con función renal, metabolismo del calcio y fósforo y autoinmunidad para descartar la existencia de una patología subyacente como las enfermedades autoinmunes o renales..(AU)


We present the clinical case of a patient with calcinosis dystrophic skin on the buttock, an infre- quent and infradiagnosed pathology due to the lack of clinical suspicion. It is very important that those patients with skin calcifications have a detalied anamnesis and analytical study with renal function, calcium and phosphrus metabolism and autoinmmunity to rule out the existence of un- derlying pathology such an autoimmune or renal diseases..(AU)

El diagnóstico diferencial de las lesiones dermatológicas supone un gran reto en las consultas diarias de Atención Primaria. Un ejemplo de ello son las lesiones digitales, cuya etiología más frecuente es la verruga vulgar, denominadas como tal aquellas que son causadas por el virus del papiloma humano (VPH). A pesar de ello, hay otras causas que deben ser tenidas en cuenta como el fibroqueratoma acral adquirido, debido a su relativa frecuencia. Se trata de un tumor fibroso y benigno cuyo diagnóstico se basa en la histología y cuyo tratamiento consiste en la exéresis completa


The differential diagnosis of dermatological lesions is a great challenge in primary care daily consultations. An example of this is digital lesions, whose most frequent etiology is verruca vulgaris, which are those caused by the human papillomavirus (HPV). However, there are other causes, like acquired digital fibrokeratoma, that must be taken into account due to its relative frequency. This is a fibrous and benign tumor whose diagnosis is based on its histology and whose treatment consists only in complete excision

ABSTRACT

El carcinoma basocelular es el tipo más común de cáncer cutáneo y la neoplasia más frecuente en el ser humano. Este tumor maligno es localmente infiltrante, agresivo y destructivo, pero su capacidad para metastatizar es muy limitada. Existen diferentes tipos: nodular, ulcerativo, pigmentado, esclerosante y superficial. Presentamos el caso de un paciente con carcinoma basocelular pigmentado en el que fue necesario hacer un diagnóstico diferencial con otras patologías como el melanoma maligno, el nevus azul y la queratosis seborreica, en las cuales el manejo y pronóstico del paciente son diferentes (AU)


Differential diagnosis of pigmented basal cell carcinoma Basal cell carcinoma (BCC) is the most common cutaneous cancer in humans. BCC is locally invasive, aggressive, and destructive but slow in growing, and there is very limited (literally no) tendency to metastasize. Clinically, there are different types: nodular, ulcerating, pigmented, sclerosing, and superficial. We report a pigmented basal cell carcinoma case, which required differential diagnosis from other pathologies such as superficial melanoma, seborrheic keratosis and blue nevus, in which management and prognosis are different (AU)

ABSTRACT

El carcinoma basocelular es la neoplasia maligna más frecuente en el ser humano. Se caracteriza por ser localmente invasivo y de crecimiento lento. Presentamos el caso de un paciente con un carcinoma basocelular avanzado, que le causó una marcada destrucción de la región nasal y que por ello no fue susceptible de tratamiento quirúrgico o radioterapia. Para este tipo de pacientes se propone una nueva terapia innovadora, el Vismodegib, una molécula que inhibe la proliferación tumoral y que ha supuesto un gran avance en el manejo de estos pacientes(AU)


The basal-cell carcinoma is the most frequent cancer in human beings. It is characterized by being locally invasive and slow-growing. We present the case of a patient with basal cell carcinoma which caused a marked destruction of the nasal region, for which reason he could not be treated surgically or with radiotherapy. For this type of patient, a new innovative therapy is proposed: Vismodegib, a molecule which inhibits the spreading of the tumour and which has signified a great advance in the management of these patients (AU)

ABSTRACT

La psoriasis es una enfermedad cutánea inflamatoria crónica que cursa en brotes con expresiones clínicas muy variadas, siendo la presentación más habitual en pápulas o placas eritematodescamativas recidivantes. Hay formas de presentación menos frecuentes en las que será necesario hacer diagnóstico diferencial con otras entidades, ya que modificaría nuestra actitud y tratamiento. Este es el caso de la psoriasis anular, variedad poco frecuente, en la que deberemos someter al paciente a un exhaustivo examen físico en busca de otras lesiones cutáneas que nos puedan orientar el diagnóstico y realizar un cultivo fúngico y una biopsia cutánea para confirmar nuestra sospecha diagnóstica (AU)


Psoriasis is a chronic inflammatory skin disease which occurs in outbreaks with very different clinical expressions, being the most frequent in recurrent scaly erythematous papules or plaques. There are other less frequent presentation forms which require differential diagnosis from other entities as they may require a different clinical approach and therapy. This is the case of annular psoriasis, an uncommon variant in which the physician must carry out an exhaustive physical examination of the patient to find other skin lesions that can orientate diagnosis. A fungal culture and a cutaneous biopsy must be performed in order to confirm the suspected diagnosis (AU)

ABSTRACT

El Carcinoma de Células de Merkel (CCM) es un tumor inusual de rápido crecimiento y potencial metastático, que presenta una mortalidad elevada (30%) y cuya incidencia está en aumento. Principalmente afecta a las personas de avanzada edad, inmunodeprimidos e historia de fotoexposición prolongada. En el 2008 se estableció su asociación con el Poliomavirus de las células de Merkel (MCPyV) lo que ha supuesto un nuevo campo de investigación. Su manejo está en continua discusión ya que la mayoría de las guías se basan en estudios restrospectivos. Presentamos un caso de CCM en mejilla derecha, en una paciente de 85 años. Esta revisión pretende actualizar los conocimientos de este tumor poco frecuente pero muy agresivo (AU)


The Merkel cell carcinoma (MCC) is an unusual, rapidly growing tumour and potentially metastatic, presenting a high mortality rate (30%), and whose incidence is increasing. It mainly affects people of advanced age, immunodepressed, and with a history of prolonged exposure to the sun. In 2008, an association with Merkel cell polyomavirus (MCPyV) was established, which has led to a new field of research. Its management is under continuous discussion, as most guidelines are based on retrospective studies. We report a case of MCC on the right cheek, in an 85 year old patient. The aim of this report is to update knowledge of this uncommon but very aggressive tumour (AU)

ABSTRACT

El herpes zoster es el resultado de la reactivación del virus varicela-zoster dentro del ganglio sensitivo, en el cual ha permanecido latente desde la primoinfección, produciendo una erupción característica. La complicación aguda más frecuente es la neuralgia postherpética y la más grave es la diseminación del herpes zoster, con compromiso sistémico. El objetivo del tratamiento antivírico precoz es evitar la aparición de complicaciones (AU)


Herpes zoster infection produces a characteristic eruption, and is the result of reactivation of herpes varicella-zoster virus within the sensory ganglia, where the virus can remain latent since the initial infection. The most common acute complication is postherpetic neuralgia, but the most serious complication is dissemination of the infection which can compromise overall health and organ function. The goal of early anti-viral treatment is to prevent these complications (AU)

ABSTRACT

Progressive macular hypomelanosis (PMH) is a condition of unknown etiology characterized by asymptomatic, hypopigmented macules located predominantly on the trunk. We recorded 12 adolescents with PMH over a 6-month period. Ten were female, and the mean age was 16.6 years. The average time from the patients first noticing pigment change to diagnosis was 15 months. PMH is probably an underdiagnosed condition.

ABSTRACT

La mononucleosis infecciosa debe sospecharse en pacientes que cursan un cuadro pseudogripal que evoluciona a una faringoamigdalitis inespecífica y aparición de lesiones cutáneas, que frecuentemente se confunde con una infección bacteriana. Su expresión en la piel da lugar a dos tipos de exantemas. El primero, de aparición temprana, polimorfo y que resuelve en pocos días y otro, llamado sinérgico, relacionado con la administración de antibióticos. Presentamos el caso de una paciente con mononucleosis infecciosa que desarrolló un exantema de tipo sinérgico.


Infectious mononucleosis should be suspected in patients who have flu-like symptoms which progress to nonspecific pharyngitis and skin lesions. It is usually confused with a bacterial infection. Expression in the skin gives rise to two types of exanthemas. The first one, early-onset, polymorphous and early-resolution. The other one, also called synergic, related to antibiotics. We report a case of a patient with an infectious mononucleosis and a synergic exanthema.
