ABSTRACT

BACKGROUND: Cannabidiol (CBD) is the principal non-hallucinogenic compound of Cannabis plants with high clinical interest because CBD has been described as having anti-inflammatory, analgesic and anticonvulsant properties. CBD is considered a multitarget compound as it can interact with a wide range of targets, explaining their multiplicity of effects. Some clinical studies have indicated certain side effects of CBD, including somnolence, anemia and diarrhea, while the elevation of transaminases is considered as an exclusion criterion from the trial. Since the red blood cells (RBCs) are a source of transaminase, we assayed in vitro effect on RBCs stability. METHODS: We performed in vitro experiments with RBCs obtained from human peripheral blood with normal hematological parameters exposed to CBD in the range of therapeutic uses. We evaluated RBCs morphological changes, membrane fragility and hemoglobin release as a reflection of hemolysis. RESULTS: CBD induced an increase in the hemoglobin release (3.27 µg/106 RBC), without altered RBC osmotic fragility. When RBCs suspensions were incubated with CBD the initial number of elements (RBCs + vesicles) was increased up to 65% after 20 min and returned to basal level after 40 min of incubation. In the first 20 min, the accounts of elements were enriched in the smaller vesicles that disappeared after the remaining 20 minutes. CONCLUSION: These results suggest that CBD affects the indemnity of erythrocytes in vitro, inducing the formation of hemolytic vesicles that can provide the basis for the development of anemia, transaminase elevation and underlying tissular iron overload in patients chronically treated with CBD.

ABSTRACT

The genus Hepatozoon Miller (1908) contains a wide range of obligate parasitic organisms with complex life cycles involving vertebrates and hematophagous invertebrates. Despite over 300 species being described, only a small percentage has been characterized in snakes using morphological and molecular techniques. The prevalence of these parasites in snakes is significant, highlighting the need for molecular descriptions in such elusive hosts. Thus, the objective of this study was to determine molecularly the presence of Hepatozoon species in snakes from the Northeastern region of Argentina. Thirty-two specimens of eight snake species (Bothrops alternatus, Dryophylax hypoconia, Erythrolamprus jaegeri coralliventris, Erythrolamprus poecilogyrus, Erythrolamprus semiaureus, Philodryas olfersii latirostris, Pseudablabes (ex Philodryas) patagoniensis and Palusophis (ex Mastigodryas) bifossatus were collected and examined. PCR analysis of the 18S rRNA locus detected four samples (12% prevalence) positive for the presence of Hepatozoon DNA. Phylogenetic analysis positioned the 18S rRNA Hepatozoon sequences obtained in three different clades, one with Hepatozoon musa, another with sequences of Hepatozoon cuestensis, while the third was placed as a sister taxon to a clade including Hepatozoon cevapii and Hepatozoon massardi. This study presents the first documentation of Hepatozoon infecting snakes in Argentina, thereby expanding their distribution within southern South America. Additionally, B. alternatus and Pa. bifossatus are reported as new hosts of Hepatozoon.

ABSTRACT

Dioctophyme renale (Goeze 1782) has not previously been reported in the pampas fox (Lycalopex gymnocercus) (Fisher 1814), the most abundant canid of southern South America. A wild adult pampas fox female was found dead due to unknown causes in Santa Fe province, Argentina. Post-mortem examination revealed three red worms measuring 10, 11 and 15 cm long, each with an approximate width of 5 mm. All of them were found free in the abdominal cavity. The worms were all male and were identified through morphological examination and molecular analysis as D. renale. No worm was found in the kidneys. This study reports the first case of dioctophymatosis in the pampas fox in Argentina, increasing the range of wild aberrant host species infected by the giant kidney worm in the Neotropical region.

ABSTRACT

INTRODUCTION: Special care dentistry (SCD) is still developing in XX. This study aimed to clarify whether primary care dentists are treating patients with special health care needs (SHCN), to know if they have had previous education on SCD (on an undergraduate or postgraduate level), whether their training level impacts their confidence when treating patients with SHCN, and to assess their opinion on SCD as a relevant topic in undergraduate education. METHODS: A survey was answered by 149 primary care dentists working for the National Health Service of the XX region in XX, including information on their daily clinical practice, undergraduate, and postgraduate training in SCD, and their opinions on them. RESULTS: Most interviewees would like to complement their training and believed that SCD should be formally incorporated into undergraduate programs. There was a significant association between confidence in treating patients with SHCN and the rating of their undergraduate training, and between confidence and the number of hours of continuous development courses. CONCLUSION: Most primary care dentists treat patients with SHCN regularly. Therefore, including training in the undergraduate curriculum and afterward becomes necessary to increase their confidence when facing this challenging group of patients.

ABSTRACT

OBJECTIVE: The present study primarily aims to study and model the impedance dynamics following cochlear implant (CI) surgery in humans. Secondarily, to observe how the modulation of a single-dose topic dexamethasone alters this response. DESIGN: CI impedance and impedance subcomponents were measured in a day-by-day basis between CI surgery and its activation ( 1 month). Impedance dynamics were mathematically modeled. Furthermore, we conducted a randomized double-blind placebo-controlled clinical trial to evaluate the effect of dexamethasone application. RESULTS: The model showed good fitting for all impedance's measures and both modulations over time. The clinical impedance and impedance subcomponents showed different patterns between groups during the first week postimplantation. Single-dose topical dexamethasone had a transient effect on Impedances, postponing (for 3 days) but not preventing the rise in impedance and its application was mainly effective at the base of the cochlea. CONCLUSIONS: The proposed mathematical fitting properly resembles the impedance dynamics in humans and their modulation due to the dexamethasone. The impedance subcomponents' dynamic pattern showed a time-course consistent with the biological processes of the foreign body reaction. Single-dose topic dexamethasone has a temporal reduction effect in impedance values on the basal turn of the cochlea but does not prevent the rise on the long-term.

BACKGROUND: There are limited antiviral options for the treatment of patients with COVID-19. Ivermectin (IVM), a macrocyclic lactone with a wide anti-parasitary spectrum, has shown potent activity against SARS-CoV-2 in vitro. This study aimed at assessing the antiviral effect of IVM on viral load of respiratory secretions and its relationship with drug concentrations in plasma. METHODS: Proof-of-concept, pilot, randomized, controlled, outcome-assessor blinded trial to evaluate antiviral activity of high-dose IVM in 45 COVID-19 hospitalized patients randomized in a 2:1 ratio to standard of care plus oral IVM at 0·6 mg/kg/day for 5 days versus standard of care in 4 hospitals in Argentina. Eligible patients were adults with RT-PCR confirmed SARS-CoV-2 infection within 5 days of symptoms onset. The primary endpoint was the difference in viral load in respiratory secretions between baseline and day-5, by quantitative RT-PCR. Concentrations of IVM in plasma were measured. Study registered at ClinicalTrials.gov: NCT04381884. FINDINGS: 45 participants were recruited (30 to IVM and 15 controls) between May 18 and September 9, 2020. There was no difference in viral load reduction between groups but a significant difference was found in patients with higher median plasma IVM levels (72% IQR 59-77) versus untreated controls (42% IQR 31-73) (p = 0·004). Mean ivermectin plasma concentration levels correlated with viral decay rate (r: 0·47, p = 0·02). Adverse events were similar between groups. No differences in clinical evolution at day-7 and day-30 between groups were observed. INTERPRETATION: A concentration dependent antiviral activity of oral high-dose IVM was identified at a dosing regimen that was well tolerated. Large trials with clinical endpoints are necessary to determine the clinical utility of IVM in COVID-19. RESUMEN Antecedentes: la neoplasia sólida pseudopapilar del páncreas es una entidad rara, que típicamente se presenta en mujeres jóvenes. Suele presentar síntomas abdominales inespecíficos. Es un tumor maligno de bajo grado de malignidad. Objetivos : el objetivo del siguiente informe de serie de casos es presentar 9 casos tratados en un cen tro y realizar una revisión bibliográfica del tema. Material y métodos: estudio retrospectivo descriptivo de los casos con diagnóstico anatomopatoló gico de neoplasia sólida pseudopapilar en el Servicio de Cirugía General, desde febrero de 2013 hasta septiembre de 2019. Se contemplaron como variables: edad, sexo, localización del tumor, tratamiento quirúrgico realizado, tiempo operatorio, complicaciones, estancia hospitalaria y seguimiento alejado. Resultados: fueron 9 casos, todos de sexo femenino con media de edad de 30 años (rango 20 a 70 años). La localización más frecuente fue en cola de páncreas en 4 casos (45%). Todas las pacientes fueron sometidas a cirugía, con abordaje laparoscópico en el 60% de los casos (n = 5); la resección pancreática distal con preservación esplénica fue la conducta más utilizada (n = 6). Se constataron tres complicaciones, de las cuales dos fueron colecciones abdominales como consecuencia de una fístula pancreática que se abordaron por vía percutánea, y la restante fue un retardo del vaciamiento gástrico por lo cual la paciente requirió internación prolongada. Conclusión: la neoplasia sólida pseudopapilar pancreática es una enfermedad poco frecuente, ma ligna pero con bajo riesgo de malignidad. Presenta buena sobrevida cuando se somete a cirugía de carácter curativo; la laparoscopia es la vía de abordaje de elección en centros con experiencia.


ABSTRACT Background: Solid pseudopapillary tumor of the pancreas is a rare condition that affects young women. The most common symptom is unspecific abdominal pain. It is a malignant tumor of low malignant potential. Objective: The aim of this study is to report a case series of patients treated in a single center and perform a bibliographic review. Material and methods: We conducted a retrospective study of the cases with pathological diagnosis of solid pseudopapillary tumor of the pancreas treated in the Department of General Surgery between February 2013 and September 2019. The following variables were analyzed; age, sex, tumor location, surgical treatment, operative time, complications, length of hospital stay and long-term follow-up. Results: Nine patients were included; all of them were women with mean age of 30 years (range: 20 - 70 years). The most common location of the tumor was the tail of the pancreas (n = 4; 45%). Surgery was performed in all the cases; five cases underwent video-assisted laparoscopy and spleen-preserving distal pancreatectomy was the technique more commonly used (n = 6). Three complications were recorded: two abdominal collections due to biliary leaks were trated by percutaneous approach and the other patient presented delayed gastric emptying and required prolonged hospitalization. Conclusion: Solid pseudopapillary tumor pf the pancreas is a rare low-grade malignant neoplasm. The prognosis is favorable after surgery and laparoscopy is the preferred approach in centers with experience.

ABSTRACT

As in any biophysical electrode-tissue environment, impedance measurement shows a complex relationship which reflects the electrical characteristics of the medium. In cochlear implants (CIs), which is mostly a stimulation-oriented device, the actual clinical approach only considers one arbitrary time-measure of the impedance. However, to determine the main electrical properties of the cochlear medium, the overall impedance and its subcomponents (i.e., access resistance and polarization impedance) should be described. We here characterized, validated and discussed a novel method to calculate impedance subcomponents based on CI measurement capabilities. With an electronic circuit of the cochlear electrode-tissue interface and its computational simulation, the access resistance and polarization impedance were modeled. Values of each electrical component were estimated through a custom-made pulse delivery routine and the acquisition of multiple data points. Using CI hardware, results fell within the electronic components nominal errors (± 10%). Considering the method's accuracy and reliability, it is readily available to be applied in research-clinical use. In the man-machine nature of the CI, this represents the basis to optimize the communication between a CI electrode and the spiral ganglion cells.

Introduction: Cochlear implant (CI) impedance reflects the status of the electro neural interface, potentially acting as a biomarker for inner ear injury. Most impedance shifts are diagnosed retrospectively because they are only measured in clinical appointments, with unknown behavior between visits. Here we study the application and discuss the benefits of daily and remote impedance measures with software specifically designed for this purpose. Methods: We designed software to perform CI impedance measurements without the intervention of health personnel. Ten patients were recruited to self-measure impedance for 30 days at home, between CI surgery and activation. Data were transferred to a secured online server allowing remote monitoring. Results: Most subjects successfully performed measurements at home without supervision. Only a subset of measurements was missed due to lack of patient engagement. Data were successfully and securely transferred to the online server. No adverse events, pain, or discomfort was reported by participants. Discussion: This work overviews a flexible and highly configurable platform for self-measurement CI impedance. This novel approach simplifies the CI standard of care by reducing the number of clinical visits and by proving useful and constant information to CI clinicians.

ABSTRACT

La Rehabilitación Respiratoria (RR), dirigida a las personas con enfermedad respiratoria crónica, reduce las exacerbaciones y hospitalizaciones, mejora los síntomas, la tolerancia al ejercicio y calidad de vida. Miembros de la Sección de RR de la Asociación Argentina de Medicina Respiratoria, se reunieron con el objetivo de preguntarse cuáles son los temas principales de controversia para dar respuesta desde la medicina basada en evidencia (sistema GRADE). Se eligieron diez preguntas. Resultados: La RR se debe indicar en pacientes con EPOC estadio GOLD 1 (1C). La adherencia es fundamental y se relaciona a sus beneficios (1C). La actividad física debe ser evaluada y estimulada (1B). Los beneficios de la RR dentro del mes posterior a una exacerbación de EPOC, reduce los síntomas y exacerbaciones y mejora la calidad de vida (1B). La RR está indicada en algunas enfermedades respiratorias crónicas no-EPOC (1B). Se deben indicar estrategias de entrenamiento especiales en desnutridos (3B) y obesos (3C). Los pacientes deben ser evaluados con prueba de ejercicio, escalas de disnea y calidad de vida (1A). El rol del entrenador de salud es novedoso ya que disminuye hospitalizaciones y mejora la calidad de vida (2C). Las comorbilidades afectan el resultado de la RR (3B). La RR reduce la mortalidad (1B) y la ansiedad-depresión (2B). La RR domiciliaria es efectiva (1A). La RR es fundamental en el tratamiento de enfermedades respiratorias crónicas. Se han actualizado las recomendaciones en RR en base a la evidencia científica con el objetivo que la RR pueda aplicarse en nuestro país.


Respiratory Rehabilitation (RR) is indicated in patients with chronic pulmonary diseases to reduce exacerbations and hospitalizations, and to improve symptoms, exercise tolerance and quality of life. Members of RR Section of the Asociación Argentina de Medicina Respiratoria, joined to review critical issues and controversies by an evidence-based system (GRADE). Ten questions were elected.213 Results: RR must be indicated in GOLD 1 COPD (1C). Adherence in RR is critical and related to benefits (1C). Physical activity must be evaluated and stimulated (1B). RR implemented after one month of COPD exacerbation hospitalization, reduces symptoms and exacerbations and improves quality of life(1B). RR is indicated in some no-COPD chronic respiratory diseases (1B). Special training strategies must be used in undernourished (3B) and obeses patients (3C). Exercise tests, dyspnea and quality of life questionnaires must be used to evaluate patients (1A). Health trainer is an original strategy to reduce hospitalizations and improve quality of life (2C). Comorbilities impacts on RR outcomes (3B). RR reduces mortality (1B) and ansiety-depression (2B). Domiciliary RR is efective (1A). RR is a key component of chronic pulmonary diseases treatment. Based in new scientific evidence, local recomendations have been done regarding to improve access in our country.

OBJECTIVES: To propose a remote, real-time, safe, and easy systematic method to determine electrode electric impedance components: access resistance, polarization capacitance, and polarization resistance. PATIENTS: Patients who received a cochlear implant and had normal cochlear anatomy and complete array insertion were recruited. A total of four adult patients were included and separated in two groups according to implantation time. INTERVENTION: Cochlear implant electrical impedance and its components were measured in all patients by using a novel diagnostic tool: a custom made software running in the patient's computer. Data is transmitted in real time to the investigator. Various stimulation and measuring strategies were used to obtain specific information in each cochlear region. MAIN OUTCOME MEASURES: Access resistance, polarization capacitance, and resistance of each patient were measured. Measurement success rate and required time for the patient were recorded. RESULTS: Access resistance, polarization capacitance, and resistance were obtained in different modes, thus in every specific region of the cochlea. All measurements were successful. Each measurement took approximately 7 minutes and was transmitted in real time to the investigators. CONCLUSION: Routine use of this tool may allow constant assessment of cochlear health and could be eventually used to monitor the effect of drugs in the inner ear. This methodology provides an in vivo "electrical view" of the inside of the implanted cochlea.

INTRODUCCIÓN: Los implantes cocleares son dispositivos protésicos neuronales utilizados para el tratamiento de las hipoacusias neurosensoriales severas a profundas. La activación de electrodos estimula las células del ganglio espiral y las vías nerviosas. Una estimulación más discreta de subpoblaciones neuronales se puede obtener cuando los electrodos están más cerca de las células ganglionares en la pared del modiolo. La distancia del electrodo al modiolo podría ser una de las múltiples variables que influyen en el resultado del paciente con un implante coclear. Actualmente no hay un protocolo establecido para medir esta distancia. Mediante diversas técnicas de imágenes se puede determinar la posición de la guía de electrodos dentro de la cóclea. La tomografía computada cone beam ha sido validada como una herramienta útil para evaluar la posición de los electrodos luego de la implantación. Tykocinski et al. describieron un modelo de la interfase electrodo-electrolito del implante coclear como una resistencia y un condensador en paralelo, y la resistencia del tejido circundante como una resistencia en serie. Realizando un registro detallado de la forma de la curva de voltaje, los datos pueden usarse para calcular la impedancia clínica o total, la resistencia de acceso, la impedancia de polarización y sus subcomponentes: Resistencia de polarización y capacitancia de polarización. INTRODUCTION: Cochlear implants are neuronal prosthetic devices used for the treatment of severe to profound sensorineural hearing loss. The activation of electrodes stimulates spiral ganglion cells and nerve pathways. A more discrete stimulation of neuronal subpopulations can be obtained when the electrodes are closer to the ganglion cells in the modiolus wall. The distance from the electrode to the modiolus could be one of the multiple variables that influence the outcome of the patient with a cochlear implant. Currently there is no established protocol to measure this distance. By means of various imaging techniques, the position of the electrode guide inside the cochlea can be determined. Cone beam computed tomography has been validated as a useful tool to evaluate the position of the electrodes after implantation. Tykocinski et al., described a model of the electrode-electrolyte interface of the cochlear implant as a resistance and a capacitor in parallel, and the resistance of the surrounding tissue as a series resistance. By making a detailed record of the shape of the voltage curve, the data can be used to calculate the clinical or total impedance, the access resistance, the polarization impedance and its subcomponents: polarization resistance and polarization capacitance. INTRODUCÃO: Os implantes cocleares são dispositivos protéticos neuronais utilizados no tratamento da perda auditiva neurossensorial severa a profunda. A ativação de eletrodos estimula as células ganglionares espirais e as vias nervosas. Uma estimulação mais discreta de subpopulações neuronais pode ser obtida quando os eletrodos estão mais próximos das células ganglionares na parede do modíolo. A distância do eletrodo ao modíolo pode ser uma das múltiplas variáveis que influenciam o resultado do paciente com implante coclear. Atualmente não há protocolo estabelecido para medir essa distância. Por meio de várias técnicas de imagem, a posição do guia do eletrodo dentro da cóclea pode ser determinada. ABSTRACT

El síndrome de Q-T prolongado es una afección que se caracteriza por interrupción del ritmo cardíaco normal. Esta enfermedad es causada por mutación en genes que codifican los canales de voltaje de potasio o sodio, interrumpiendo de esta forma el flujo de dichos iones en el músculo cardíaco. En algunos casos este flujo iónico también se encuentra alterado a nivel del oído interno, por lo cual puede encontrarse asociado a hipoacusia neurosensorial profunda. El diagnóstico se basa en la electrocardiografía y el cuadro clínico, caracterizado por ataques sincopales recurrentes, crisis convulsivas o muerte súbita como primera manifestación. En los casos asociados a hipoacusia neurosensorial profunda, el implante coclear como tratamiento de la sordera presenta riesgos adicionales debido a la posibilidad de arritmias cardíacas y muerte súbita; por lo cual existen consideraciones perioperatorias especiales.


Prolonged Q-T syndrome is a condition characterized by disruption of normal heart rhythm, presented as a prolonged QT interval. This disease is caused by mutation in genes encoding the potassium or sodium voltage channels, thus disrupting the flow of such ions into the cardiac muscle. In some cases this inonic flow is also altered at the level of the inner ear, which may be associated with deep neurosensorial hearing loss. The diagnosis is based on electrocardiography and the clinical picture, characterized by recurrent syncopal attacks, seizures or sudden death as the first manifestation. In cases associated with deep neurosensory hearing loss, the cochlear implant as a treatment for deafness presents additional risks due to the possibility of cardiac arrhythmias and sudden death; for which there are special peri-operative considerations.


Síndrome Q-T prolongada é uma condição caracterizada por uma ruptura do ritmo cardíaco normal, apresentado como um intervalo prolongado de QT. Esta doença é causada por mutação em genes que codificam os canais de tensão de potássio ou de sódio, interrompendo assim o fluxo de tais íons para o músculo cardíaco. Em alguns casos, este fluxo inônico também é alterado no nível da orelha interna, o que pode estar associado à perda auditiva neurosensorial profunda. O diagnóstico é baseado em eletrocardiografia e no quadro clínico, caracterizado por ataques de síncopes recorrentes, convulsões ou morte súbita como a primeira manifestação. Introducción: el edema de Reinke es la acumulación de fluidos en la capa externa de la lámina propia de las cuerdas vocales. Produce disfonía y raras veces obstrucción respiratoria. Las etiologías más frecuentes son el tabaquismo, el reflujo gastroesofágico y el mal uso y abuso vocal. Objetivos: determinar, mediante tratamiento quirúrgico, la tasa de resolución de la disnea inspiratoria severa provocada por edema de Reinke bilateral de cuerdas vocales. Diseño: estudio descriptivo y retrospectivo. Material y métodos: revisión de las historias clínicas electrónicas de todos los pacientes que consultaron y fueron tratados por disnea inspiratoria severa provocada por edema de Reinke bilateral de las cuerdas vocales, en el servicio de Otorrinolaringología del Hospital Italiano de Buenos Aires, entre febrero de 2007 y abril de 2015. Resultados: fueron tratados 4 pacientes de sexo femenino que consultaron por disnea inspiratoria severa. Fumaban más de 30 cigarrillos por día. La técnica quirúrgica consistió en resecar todo el edema polipoideo en forma bilateral, preservando el borde libre de las cuerdas vocales. Conclusiones: el edema de Reinke obstructivo es una patología infrecuente. La tasa de resolución de la disnea inspiratoria severa en las cuatro enfermas tratadas fue del100%. La resección total del edema y de la mucosa excedente, preservando un pequeño sector para que recubra el borde libre de la cuerda vocal (cordectomía vs. cordotomía), fue la técnica quirúrgica preferida. Introduction: the Reinke edema is an accumulation of fluid in the outer layer of the lamina propria of the vocal cords. Causes dysphonia and rarely produces respiratory obstruction. Objectives: to determine the rate of resolution of the severe inspiratory dyspnea caused by bilateral Reinke edema of vocal cords with surgical treatment. Design: descriptive and retrospective study. Material and methods: review of the electronic medical records of all patients who consulted and were treated for severe inspiratory dyspnea caused by bilateral Reinke edema of the vocal chords in the Hospital Italiano de Buenos Aires between February 2007 and April 2015. Results: four women were treated, consulted for severe inspiratory dyspnea. Smoked more than 30 cigarettes per day. The surgical technique consisted in to resect all the bilateral polypoid edema, while preserving the free edge of the vocal cords. Conclusions: the obstructive Reinke edema is an infrequent pathology. The rate of resolution of the severe inspiratory dyspnea in the four patients treated was 100%. The total resection of the edema and mucosa excess, preserving a small sector to cover the free edge of the vocal cord (cordectomy vs cordotomy) was the preferred surgical technique. (AU)

ABSTRACT

Objetivos: Determinar la tasa de resección quirúrgica completa en tumores seleccionados del Espacio parafaríngeo tratados por vía transoral o transnasal o por ambas. Diseño: descriptivo, retrospectivo. Materiales y métodos: Se incluyeron en este estudio los pacientes que tuvieron neoplasias localizadas en el espacio parafaríngeo y que fueron tratados con cirugía por vía transoral o transnasal. El abordaje transoral consistió en realizar una incisión con cauterio en la zona de mayor protrusión del tumor (pilar amigdalino y paladar), disección de la mucosa y del tumor de los planos profundos, traccionándolo hacia la cavidad oral. Para disecar el límite superior (rinofaringe) y lateral se utilizaron endoscopios que fueron introducidos por la incisión y por la cavidad nasal. El abordaje transnasal consistió en realizar una incisión en la pared lateral de la rinofaringe y disecar el tumor del plano profundo traccionándolo hacia el cavum. Resultados: Fueron tratados 3 pacientes por vía transoral y uno por vía endonasal por padecer tumores del espacio parafaríngeo. Tres tumores se originaron en glándulas salivales menores localizadas una en el espacio. Preestiloideo (1/3), otra en el espacio masticador (1/3) y otra en el sector superior del espacio preestiloideo (1/3). La histología dio como resultado dos adenoma pleomorfo y un carcinoma mucoepidermoide de bajo grado (1/3). (Está bien así? Qué es 1/3?). Una neoplasia se originó en el lóbulo profundo de la parótida y se extendió al espacio preestiloideo, su histología fue carcinoma mucoepidermoide de grado moderado. Conclusiones: La tasa de resección completa en pacientes con tumores del espacio parafaríngeo seleccionados tratados por vía transoral y endonasal fue del 100%. (AU)


Objectives: To determine the rate of complete surgical resection in parapharyngeal space selected tumors treated with transoral and / or transnasal approach. Design: Descriptive, retrospective. Materials and methods: Patients who had tumors localized in parapharyngealspace and who were treated with transoral or transnasal surgery. were included in this study. The transoral approach consisted in performing an incision with cautery in the area of greates tumor protrusion (tonsillar pillar and palate), dissection of the mucosa and tumor of the deep planes, pulling it into the oral cavity. To dissect the upper limit (nasopharynx) and lateral we used endoscopes that were inserted by the incision and the nasal cavity. The transnasal approach consisted in making an incision in the side wall of the nasopharynx and dissect the tumor of the deep plane pulling it towards the cavum. Results: Three patients were treated with transoral and one byendonasalapproacheswho had parapharyngeal space tumors. Three tumors originated in minor salivary glands located in prestyloidspace (1/3), masticator space (1/3) and upper sector of pree-styloid space (1/3). Histology was in two pleomorphic adenoma, and another onelow degree mucoepidermoid carcinoma (1/3). A neoplasm was originated in the deep lobe of the parotid gland and was extended to the prestyloid space, was a moderate degree of mucoepidermoid carcinoma. Conclusions: Complete resection rate in patients with selected parapharyngeal space tumors, treated by transorally and endonasal approach was 100%. (AU)

ABSTRACT

Both raw and composted poultry manure is applied as soil amendment. The aims of this study were: (1) to develop phytotoxicity indexes for organic wastes and composts, and (2) to assess the correlation among phytotoxicity indexes, ecotoxicological endpoints and stability and physicochemical parameters during passive composting of poultry manure. Six 2-m(3) composting piles were constructed and four parameter groups (physicochemical and microbiological parameters, ecotoxicological endpoints, and biological activity) were determined at four sampling times during 92days. Extracts were used to carry out acute toxicity tests on Daphnia magna, Lactuca sativa and Raphanus sativus. Composting decreased average toxicity 22.8% for the 3 species and D. magna was the most sensitive species. The static respiration index decreased from 1.12 to 0.46mgO2gOMh(-1) whilst organic matter reduced by 64.1% at the end of the process. Escherichia coli colonies remained higher than values recommended by international guidelines. The D. magna immobilization test allowed the assessment of possible leachate or run-off toxicity. The new phytotoxicity indexes (RGIC0.8 and GIC80%), proposed in this study, as well as salinity, proved to be good maturity indicators. Hence, these phytotoxicity indexes could be implemented in monitoring strategies as useful ecotoxicological tools. Multivariate analyses demonstrated positive correlations between ecotoxicological endpoints (low toxicity) and biological activity (stability). These two parameter groups were associated at the final sampling time and showed negative correlations with several physicochemical parameters (organic and inorganic contents). The final poultry manure compost was rendered stable, but immature and, thus, unsuitable for soil amending.

ABSTRACT

El síndrome de Kasabach-Merritt está caracterizado por una lesión angiomatosa grande o de rápido crecimiento, trombocitopenia y anemia hemolítica microangiopática por coagulopatía de consumo, que tiene una mortalidad entre un 20 y 40%. Está asociado a diversas lesiones angiomatosas tales como hemangioendotelioma kaposiforme, angioma en penacho y hemangioma juvenil.La paciente que presentamos es una niña de un año con angioma en penacho localizado en la pared abdominal que mostró modificación en su tamaño, consistencia y coloración. Esto se acompañó de anemia, plaquetopenia, y alteración de la coagulación.Recibió glucocorticoides de altas dosis, interferón alfa y vincristina, obteniéndose mejoría clínica y de parámetros de laboratorio luego de este ultimo tratamiento.
