ABSTRACT

Endometriosis is a common gynecologic disease in which endometrial tissue is deposited outside the normal confines of the uterine cavity. Rarely endometriosis involves urinary tract. The bladder is the most frequent organ affected into this tract. A delay in detection can result in months or years of morbidity.

BACKGROUND: Farmers are exposed to a wide variety of sensitizers. Since occupational asthma (OA) can lead to permanent disability, exposure discontinuation is the preferred treatment. When this is not possible, the identification of the causative allergen may allow an alternative therapy. METHODS: We present three farmers diagnosed with OA as a consequence of handling fodder. We carried out skin tests with common and occupational allergens and with oilseed rape (OSR) extract. Total and specific serum IgE levels were measured. The patients underwent the OSR-bronchial provocation test (OSR-BPT). The day before and 24 h after the OSR-BPT, the methacholine (M)-BPT and induced sputum were performed. Eosinophil percentages and ECP levels were measured in the sputum samples. RESULTS: OSR sensitization (skin tests and specific serum IgE) was detected in all the patients. The OSR-BPT elicited early responses in two subjects. Methacholine sensitivity, sputum eosinophils, and sputum ECP levels increased 24 h after the OSR-BPT in all the patients. CONCLUSIONS: We have demonstrated that inhalation of OSR flour causes bronchoconstriction, induces an eosinophilic inflammatory bronchial response, and increases bronchial hyperresponsiveness in sensitized asthmatics. OSR flour contained in animal fodder should be considered another potential cause of OA among farmers.

ABSTRACT

To induce anemia experimentally in rats to provide a hematocrit of 30 to 35% and provide data about the physiologic response to anemia during induction and after it is established, two methods were selected: the application of repetitive doses of phenylhydrazine and bleeding. Blood sample collection was carried out at various times, and hematologic profiles and osmotic resistance were evaluated. The morphologic features of cells and distinct organs also were examined. Results indicated similar decrease of hematocrit and hemoglobin concentration for the two experimental groups, although the reticulocyte response was higher in rats treated with phenylhydrazine, where the presence of young erythrocyte populations was linked to increases in osmotic resistance and glucose utilization. 2,3-Diphosphoglycerate was induced only during the recovery phase of the study and corresponded to a gradual response to hypoxia. Histologically, marked erythroid hyperplasia was found in the bone marrow, and extramedullary hematopoiesis was seen in the spleen and liver.

ABSTRACT

OBJECTIVES: We reviewed our series of epidermoid penile cancer to determine the predictive index of lymph node invasion and the incidence of phimosis in these patients. METHODS: We reviewed the records of 11 patients with epidermoid cancer of the penis. Six patients had been evaluated according to a protocol that included preoperative biopsy of the lesion, penile and abdominal ultrasound evaluation, CT and diagnostic lymphadenectomy for those cases suspected as having lymph node invasion. The predictive index (PI) was determined in relation to the grading of cell differentiation (G) and tumor stage (S) [PI = G+T] and the incidence of phimosis was sought during the interview. RESULTS: Lymph node invasion was confirmed in 2 patients with PI = 6 and in 1 patient with PI = 5. Diagnostic lymphadenectomy showed no evidence of lymphatic spread in 1 patient with PI = 3 and in another patient with PI = 4. The remaining 6 patients with PI < 2 did not undergo lymphadenectomy. Phimosis was associated in 73% of the cases. CONCLUSIONS: PI is a reliable index for lymph node invasion. Patients with PI < 4 do not require therapeutic lymphadenectomy. Phimosis should be operated early in childhood.

ABSTRACT

Six cases of eosinophilic cystitis are described, one of which had no previous surgery or manipulation of the bladder. The literature is reviewed and the clinical and anatomopathological features are described. We discuss the diagnostic criteria, treatment, prognosis and the hypothesis of an immediate-type of anaphylactic reaction as a pathogenic mechanism.

ABSTRACT

Hemangioendothelioma or histiocytoid hemangioma of bone is a low grade tumor, which may be a quite indolent and controlable lesion, even in the rare case it develops distant metastasis. Our case presents features of such lesion by light microscopy and with a peroxidase-antiperoxidase technique for Factor VIII related antigen, but it exhibits an unusual extensive tissular necrosis. Necrosis does not seems to make worse the usual course, after six years and ten months of conservative surgical treatment and radiation therapy.

ABSTRACT

Infestation with Enterobius (Oxyuris) vermicularis is very common but seldom produces lesions and even more rarely causes granulomas. Two cases of oxyuris granulomas/nodules), one in the serosa of ovaries and left fallopian tube in a female of 32-years and the other in the submucosa of the appendix in a 10-year old girl are identified by the presence of true necrotizing granulomas containing the parasite. The pathogenesis and differential diagnosis of abdominal granulomas is discussed.

ABSTRACT

Congenital diaphragmatic hernia is the object of this informative review, directed to the general pediatrician. The needs for urgent diagnosis and immediate surgical treatment are both stressed. Aside its embriological, clinical, and diagnostic aspects other important areas are dealt with, such as associated malformations and surgical treatment. Special points are made regarding pre and postoperative care, including pitfalls in surgical treatment due to crowding of the abdomen under severe pressure.
