ABSTRACT

Freshwater bivalves are widely used as accumulation indicators and monitoring tools for assessing contaminant effects on different levels of biological integration. This pilot study aimed to explore the phylogenetic diversity of Escherichia coli isolated from freshwater mussels (Margaritifera margaritifera and Potomida littoralis) and characterize their phenotypes and antibiotic resistance profiles. Samples were collected in the Rabaçal and Tua Rivers, in the Douro basin, Portugal-two sites representing different levels of anthropogenic contamination. Antimicrobial susceptibility testing was performed via the disk diffusion method with 21 antibiotics. Results showed that 31% of isolates were multidrug-resistant (MDR). Thus, freshwater mussels provide an effective and time-integrated approach for identifying/quantifying fecal indicators, including MDR bacteria. PCR-based assays were designed for assessing phylogenetic E. coli groups. Among the E. coli isolates, the highest prevalence (44%) was observed in group D or E, followed by group E or Clade I (25%), group A (19%), and group B1 (13%). E. coli isolated from M. margaritifera predominantly exhibited a higher prevalence of phylogroups D or E, whereas E. coli from P. littoralis showed associations with phylogroups E or clade I, B1, A, and D or E. Our results provide new insights into the phylogenetic diversity of E. coli in freshwater bivalves. Additionally, the findings highlight the possible linkage of phylogroups with the host species, the geographical location in the water stream, and human activity. Using E. coli as a bioindicator isolated from freshwater mussels helps us grasp how human activities affect the environment. This study has important implications for those interested in safeguarding water resources, especially in tackling antibiotic resistance in aquatic ecosystems.

ABSTRACT

Consumer demand for natural and healthier products has led to an increasing interest in the bioactive and therapeutic properties of plant extracts. In this study, we evaluated the phenolic compounds profile, bioactivities, and toxicities of plant extracts from eight European flora species, including Calendula officinalis L., Calluna vulgaris (L.) Hull, Hippophae rhamnoides L., Juglans regia L., Mentha cervina L., Rubus idaeus L., Sambucus nigra L., and Vitis vinifera L. The aim was to identify potential preservatives of natural origin. Phenolic compounds were identified by HPLC-DAD-ESI-MS. Caffeic acid derivatives, ellagitannins, flavonols, and flavones were the major phenolic compounds identified. The total phenolic content varied from 16.0 ± 0.2 (V. vinifera) to 123 ± 2 mg/g (H. rhamnoides) of dry extract. All extracts showed antioxidant potential and exhibited activity against some of the microorganisms tested. S. nigra showed the highest activity in the inhibition of oxidative hemolysis (OxHLIA) assay and H. rhamnoides, notably, had the lowest IC50 values in TBARS and DPPH assays, as well as the lowest minimum inhibitory concentration (MIC) values. Regarding in vitro cytotoxicity, in tumor and non-tumor cell lines, although some extracts revealed toxicity against normal cells, it was found that the samples C. vulgaris, V. vinifera and R. idaeus might be used against tumor cells since the active concentration is much lower than the one causing toxicity. In vivo acute toxicity tests using Artemia franciscana suggest low toxicity for most extracts, with LC50 > 400 mg/L. These results showed the potential of the studied extracts as natural preservatives, given their richness in compounds with bioactive properties, highlight their potential value to the production chain.

OBJECTIVE: To investigate the factors associated with the early diagnosis of autism and other types of pervasive developmental disorder (PDD) in children treated at the Psychosocial Care Center for Children and Adolescents of the Unified Health System, from 2013 to 2019,in Brazil. METHODS: An exploratory cross-sectional study, based on data from the Record of Outpatient Health Actions (RAAS) of the first appointment of children aged 1 to 12 years. The gross (RRg) and adjusted (RRa) relative risks and respective 95% confidence intervals (95%CI) were estimated using the Poisson regression model with robust variance estimation. RESULTS: Of the 22,483 children included in the study, the majority were male (81.9%), lived in the same municipality where they were diagnosed (96.8%) and in the Southeast region (57.7%). Early diagnosis was higher for childhood autism (RRg = 1.48; 95%CI 1.27-1.71) , PDD without subtype designation (RRg = 1.55; 95%CI 1.34-1.80), other PDD (RRg = 1.48; 95%CI 1.21-1.81) and PDD not otherwise specified (RRg = 1.44; 95%CI 1.22-1.69) than for atypical autism. Children residing in the same municipality where the diagnosis was made had a higher rate of early diagnosis (RRg = 1.31; 95%CI 1.10-1.55) than the others; as well as those referred by primary care (RRg = 1.51; 95%CI 1.37-1.68) and by spontaneous demand (RRg = 1.45; 95%CI 1.31-1.61) than those from other types of referral. Early diagnosis was higher from 2014 and lower in the North region than in the other regions. In the multiple analysis, the magnitude of RRa was similar to that of RRg. CONCLUSIONS: Early identification of autism and other PDD has improved in Brazil, but it still represents about 30% of the diagnoses made. The variables included in the model were significant, but still explain little of the early diagnosis of children with autism and other PDD.

ABSTRACT Objective: Describe reliabilities evidence of the Phone Screening Interview (PSI), a telephone screening interview for autism spectrum disorder (ASD) symptoms, capable of investigating mild to moderate ASD symptoms. Moreover, the PSI also works for verbal and non-verbal children and is consistent with the DSM-5 diagnostic criteria. Methods: An interview was performed with sixty-eight parents of children between 2 and 15 years old attended by the Psychiatry Ambulatory of Santa Casa de Misericórdia do Rio de Janeiro through the PSI in person and by telephone. Results: No significant differences in comparison between averages of the total score of the face-to-face and telephone applications were observed. The agreement analysis between the items indicated three items with lower values, leading to the modification of some questions, culminating in a new interview version for further studies. Given the disagreement in the values found, the order of application of the interviews seems to not impact the results, demonstrating strong correlations between both interviews, even with a different order of application. Aiming to facilitate the use of the scale by different examiners, the interobserver reliability was investigated, which did not show significant differences in the means. Conclusion: The study suggests that the telephone interview can be used similarly to the face-to-face interview, by different evaluators, with no impact on its efficiency in detecting ASD symptoms.


RESUMO Objetivo: Descrever evidências de confiabilidade da Phone Screening Interview (PSI), uma entrevista para rastreio telefônico de sintomas do Transtorno do Espectro Autista (TEA) de fácil aplicação, capaz de investigar sintomas de TEA leve a moderado, aplicável a crianças verbais e não verbais e consistente com os critérios diagnósticos do DSM-5. Métodos: Sessenta e oito pais de crianças com idade entre 2 e 15 anos atendidas pelo Ambulatório de Psiquiatria da Santa Casa de Misericórdia do Rio de Janeiro foram entrevistados por meio da PSI, tanto de maneira presencial quanto telefônica. Resultados: As médias da pontuação total da aplicação presencial e telefônica foram comparadas, não sendo obtidas diferenças significativas. A análise de concordância entre os itens apontou três itens com valores muito baixos, levando à modificação de algumas perguntas, culminando em uma nova versão, para estudos posteriores. Diante da discordância de valores encontrada, foi verificado que a ordem de aplicação das entrevistas não impactaria os resultados, demonstrando fortes correlações entre as entrevistas, mesmo com ordem de aplicação diferente. Para viabilizar o uso da escala por diferentes examinadores, investigou-se a confiabilidade interobservadores, que não mostrou diferenças significativas nas médias. Conclusão: O estudo sugere que a entrevista telefônica pode ser utilizada de forma semelhante à presencial, por diferentes avaliadores, sem impacto em sua eficiência na detecção de sintomas de TEA.

Resumo Este artigo descreve a incidência acumulada e as características das crianças de até 12 anos com transtorno global do desenvolvimento (TGD) atendidas no CAPSi. Realizou-se estudo transversal descritivo, utilizando o Registro de Ações Ambulatoriais de Saúde. Avaliou-se a incidência acumulada de TGD no período 2013-2019 e tendência temporal das características das crianças e do atendimento pelo método de Prais-Winsten. Identificaram-se 23.657 registros de primeiro atendimento, mas 10,8% sem seguimento. A incidência acumulada foi de 13,2/10.000, com ampla variação entre municípios e grandes regiões, embora vários sem registro de casos. Observou-se queda no percentual de primeiro atendimento, com incremento médio anual de 17,8%. A maioria das crianças foi atendida no município de residência (96,8%) e recebeu diagnóstico inicial de autismo infantil (30,6%), embora 54,0% sem TGD especificado no primeiro atendimento. Apenas 30,2% foram diagnosticadas até três anos de idade. A garantia do cuidado na atenção especializada e o diagnóstico precoce permanecem como desafios.


Abstract This article describes the accumulated incidence and characteristics of children until 12 years old with global developmental disorder (TGD) treated at CAPSi. A descriptive cross-sectional study was carried out using Register of Outpatient Health Actions. Cumulative incidence of TGD in the period 2013-2019 and the temporal trend of characteristics of children and care were evaluated using the Prais-Winsten method. It were identificated 23,657 records with first care, but 10.8% without follow-up. The cumulative incidence was 13.2/10,000, with wide variation between municipalities and large regions although several without record of cases. There was decrease in the percentage of first appointment, with an average annual increase of -17.8%. Most children were attended to in the municipality of residence (96.8%) and initially diagnosed as autism (30.6%), although 54.0% hadn't TGD specified in the first appointment. Only 30.2% were diagnosed until three years old. Ensuring care in specialized care and early diagnosis remain challenges.


Resumen Este artículo describe la incidencia acumulada y características de los niños hasta 12 años con trastorno del desarrollo global (TGD) tratados en CAPSi. Se realizó un estudio descriptivo transversal, utilizando Registro de Acciones Ambulatorias de Salud. Se evaluó la incidencia acumulada de TGD en período 2013-2019 y tendencia temporal de características de niños y cuidado, utilizando método Prais-Winsten. Se identificaron 23.657 registros con primera asistencia, pero 10,8% sin seguimiento. La tasa acumulada fue 13,2/10.000, con amplia variación entre municipios y grandes regiones, aunque varias sin registro de casos. No hubo descenso en porcentaje de primeras visitas, con incremento medio anual del -17,8%. La mayoría de niños fueron atendidos en municipio de residencia (96,8%) y diagnosticados de autismo (30,6%), aunque el 54,0% no tenía TGD especificado en primera visita. Solo 30,2% fueron diagnosticados hasta los tres años de edad. Asegurar la atención en cuidados especiales y diagnóstico precoz temprano como desafíos.

RESUMO OBJETIVO Investigar os fatores associados ao diagnóstico precoce do autismo e de outros tipos de transtorno global do desenvolvimento (TGD) de crianças atendidas no Centro de Atenção Psicossocial Infantojuvenil do Sistema Único de Saúde, no período de 2013 a 2019, no Brasil. MÉTODOS Estudo transversal exploratório, com base nos dados do Registro das Ações Ambulatoriais de Saúde (RAAS) do primeiro atendimento de crianças de 1 a 12 anos. Foram estimados o risco relativo bruto (RRb) e ajustado (RRa), e respectivo intervalo de confiança de 95% (IC95%), utilizando o modelo de regressão de Poisson com estimativa de variância robusta. RESULTADOS Das 22.483 crianças incluídas no estudo, a maioria era do sexo masculino (81,9%), residia no mesmo município em que foi diagnosticada (96,8%) e na região Sudeste (57,7%). O diagnóstico precoce foi maior para autismo infantil (RRb= 1,48; IC95% 1,27-1,71), TGD sem designação de subtipo (RRb= 1,55; IC95% 1,34-1,80), outros TGD (RRb= 1,48; IC95% 1,21-1,81) e TGD não especificado (RRb= 1,44; IC95% 1,22-1,69) do que para autismo atípico. As crianças que residiam no mesmo município onde foi realizado o diagnóstico tiveram maior índice de diagnóstico precoce (RRb= 1,31; IC95% 1,10-1,55) do que as demais; bem como aquelas encaminhadas pela atenção básica (RRb= 1,51; IC95% 1,37-1,68) e por demanda espontânea (RRb= 1,45; IC95% 1,31-1,61) do que as oriundas de outros tipos de encaminhamento. O diagnóstico precoce foi maior a partir de 2014 e menor na região Norte quando comparada às demais. Na análise múltipla, a magnitude do RRafoi similar ao do RRb. CONCLUSÕES A identificação precoce de autismo e outros TGD tem melhorado no país, mas ainda representa cerca de 30% dos diagnósticos realizados. As variáveis incluídas no modelo foram significativas, mas ainda explicam pouco do diagnóstico precoce de crianças com autismo e outros TGD.

ABSTRACT

Freshwater mussels are among the most endangered groups of fauna anywhere in world. The indiscriminate use of antibiotics has led to the emergence of resistant strains. These antibiotic-resistant bacteria play a key role in increasing the risk allied with the use of surface water and in spread of resistance genes. Two endangered freshwater mussel species, Margaritifera margaritifera and Potomida littoralis, were sampled at 4 sampling sites along a 50 km stretch of River Tua. Water samples were taken at same sites. Of the total of 135 isolates, 64.44% (39.26% from water and 25.19% from mussels) were coliform bacteria. Site T1, with the lowest concentration of coliform bacteria, and site T2 were the only ones where M. margaritifera was found. No E. coli isolates were found in this species and the pattern between water and mussels was similar. P. littoralis, which was present at T3/T4 sites, is the one that faces the highest concentration of bacterial toxins, which are found in treated wastewater effluents and around population centers. Sites T3/T4 have the isolates (water and mussels) with the highest resistance pattern, mainly to ß-lactams. Water and P. littoralis isolates (T3/T4) showed resistance to penicillins and their combination with clavulanic acid, and to cephalosporins, precisely to a fourth generation of cephalosporin antibiotics. The analysis provides important information on the risk to water systems, as well as the need to investigate possible management measures. It is suggested that future studies on the health status of freshwater bivalves should incorporate measures to indicate bacteriological water quality.

ABSTRACT

O-GlcNAcylation is a dynamic post-translational modification mediated by O-linked ß-N-acetylglucosamine transferase (OGT) and O-GlcNAc hydrolase (OGA), that adds or removes a single ß-N-acetylglucosamine (GlcNAc) moiety to or from serine/threonine residues of nucleocytosolic and mitochondrial proteins, respectively. The perturbed homeostasis of O-GlcNAc cycling results in several pathological conditions. Human OGA is a promising therapeutic target in diseases where aberrantly low levels of O-GlcNAc are experienced, such as tauopathy in Alzheimer's disease. A new class of potent OGA inhibitors, 2-acetamido-2-deoxy-d-glucono-1,5-lactone (thio)semicarbazones, have been identified. Eight inhibitors were designed and synthesized in five steps starting from d-glucosamine and with 15-55% overall yields. A heterologous OGA expression protocol with strain selection and isolation has been optimized that resulted in stable, active and full length human OGA (hOGA) isomorph. Thermal denaturation kinetics of hOGA revealed environmental factors affecting hOGA stability. From kinetics experiments, the synthesized compounds proved to be efficient competitive inhibitors of hOGA with Ki-s in the range of ∼30-250 nM and moderate selectivity with respect to lysosomal ß-hexosaminidases. In silico studies consisting of Prime protein-ligand refinements, QM/MM optimizations and QM/MM-PBSA binding free energy calculations revealed the factors governing the observed potencies, and led to design of the most potent analogue 2-acetamido-2-deoxy-d-glucono-1,5-lactone 4-(2-naphthyl)-semicarbazone 6g (Ki = 36 nM). The protocol employed has applications in future structure based inhibitor design targeting OGA.

Abstract The study describes the profile of children and adolescents with pervasive developmental disorders (PDD) attended at the Psychosocial Care Centers (CAPS) and professionals from these establishments in Brazil and regions. It uses data from the Outpatient Information System of the Unified Health System (SIA/SUS) and the National System of Health Establishments System (SCNES) in 2014-2017. The SIA/SUS was deterministically linked with the SCNES, based on the establishment number. PDD cases were individualized using a variable coded SUS card; 18,852 diagnoses of PDD were recorded in CAPS, most of them by spontaneous demand, 73.2% performed in CAPSi, 50.3% in the age group of 1-6 years old, 80% male and highest proportions of females over 13 years (p <0.001). PDD was not specified in 54.3% of the diagnoses; autistic disorder was the most common PDD (27.2%). Professional teams vary by CAPS type; Procedures of communicative practices and psychosocial rehabilitation were not expressive (10.3%). It was concluded that studies are necessary to clarify a high spontaneous demand, to understand treatment outside CAPSi and to define parameters to evaluate if the procedures used are appropriated for the therapeutic project and possibilities monitoring and evaluate the care of people with PDD.


Resumo O estudo descreve o perfil de crianças e adolescentes com transtorno do desenvolvimento global (TGD) atendidas nos Centros de Atenção Psicossocial (CAPS) e profissionais destes estabelecimentos no Brasil e regiões. Utiliza dados do Sistema de Informações Ambulatoriais do Sistema Único de Saúde (SIA/SUS) e do Sistema Cadastro Nacional de Estabelecimentos de Saúde (SCNES) de 2014-2017. Relacionou-se deterministicamente as bases SIA/SUS e SCNES, usando o número do estabelecimento, os casos de TGD foram individualizados utilizando a variável cartão SUS codificada. Foram registrados 18.852 diagnósticos de TGD nos CAPS, a maioria por demanda espontânea, 73,2% realizadas em CAPSi, 50,3% na faixa etária de 1-6 anos, 80% do sexo masculino e maiores proporções de casos femininos a partir de 13 anos (p<0,001). Em 54,3% dos diagnósticos não foi especificado o tipo de TGD, sendo o autismo o mais frequente (27,2%) dentre os especificados. As equipes profissionais variaram segundo tipo de CAPS. Procedimentos de práticas comunicativas e reabilitação psicossocial foram pouco expressivas (10,3%). Conclui-se pela necessidade de estudos para esclarecer a alta demanda espontânea, os atendimentos fora do CAPSi e de se estabelecer parâmetros para avaliar se os procedimentos estão adequados ao projeto terapêutico permitindo monitorar e avaliar o atendimento às pessoas com TGD.

ABSTRACT

The prevalence of Shiga toxin (Stx)-producing Escherichia coli (STEC) was determined by evaluating its presence in faecal samples from 155 heifers, and 254 dairy cows in 21 farms at North of Portugal sampled between December 2017 and June 2019. The prevalence of STEC in heifers (45%) was significantly higher than in lactating cows (16%) (p<0.05, Fisher exact test statistic value is <0.00001). A total of 133 STEC were isolated, 24 (13.8%) carried Shiga-toxin 1 (stx1) genes, 69 (39.7%) carried Shiga-toxin 2 (stx2) genes, and 40 (23%) carried both stx1 and stx2. Intimin (eae) virulence gene was detected in 29 (21.8%) of the isolates. STEC isolates belonged to 72 different O:H serotypes, comprising 40 O serogroups and 23 H types. The most frequent serotypes were O29:H12 (15%) and O113:H21 (5.2%), found in a large number of farms. Two isolates belonged to the highly virulent serotypes associated with human disease O157:H7 and O26:H11. Many other bovine STEC serotypes founded in this work belonged to serotypes previously described as pathogenic to humans. Thus, this study highlights the need for control strategies that can reduce STEC prevalence at the farm level and, thus, prevent food and environmental contamination.

ABSTRACT

The research describes the development of the Phone Screening Interview (PSI) for screening for Autistic Spectrum Disorder (ASD) symptoms. Thirty-seven parents of ASD children (2 to 12 years old) from the Applied Psychology Service of PUC-Rio were interviewed. The children were evaluated by psychiatrists and the CARS scale was applied to determine the severity of the disorder. Multivariate descriptive and inferential statistical analyses (ANOVA and hierarchical clusters) were performed. The results indicate that performance in PSI is invariant regarding age, education, severity, and linguistic and motor development. Cluster analysis for variables uncovered a grouping of social communication symptoms and another of stereotyped pattern ones. Cluster analysis of parents' responses in PSI found two groups that differ in the degree of difficulty in communication and social skills. In conclusion, PSI is shown as: (1) easy to apply; (2) brief; (3) encompassing a broad age range (2 to 12 years); (4) applicable to mild to moderate ASD symptoms; (5) including verbal and nonverbal children; (6) consistent with DSM-5 diagnostic criteria. The study discusses the accuracy of the caregiver's perception regarding signs of ASD, as well as the heterogeneity of the ASD phenotype.


A pesquisa descreve o desenvolvimento da Phone Screening Interview (PSI) para detecção telefônica de sintomas do Transtorno do Espectro Autista (TEA). Trinta e sete pais de crianças (2 a 12 anos) com TEA do Serviço de Psicologia Aplicada da PUC-Rio foram entrevistados. As crianças foram avaliadas por psiquiatras e a escala CARS foi aplicada para determinar a gravidade do transtorno. Análises estatísticas descritivas e inferenciais multivariadas (ANOVA e conglomerados hierárquicos) foram realizadas. Os resultados apontam que o desempenho na PSI não varia em função da idade, escolaridade, gravidade e desenvolvimento linguístico ou motor. A análise de conglomerado para variáveis mostrou um agrupamento de sintomas de comunicação social e outro de padrão estereotipado. A análise de conglomerado das respostas dos pais na PSI mostrou dois grupos que diferem no grau de dificuldade nas habilidades comunicativas e sociais. Concluindo, a PSI mostrou-se: (1) de fácil aplicação; (2) breve; (3) abrangendo faixa etária ampla (2 a 12 anos); (4) aplicável a sintomas de TEA leves a moderados; (5) incluindo crianças verbais e não verbais; (6) consistente com os critérios diagnósticos do DSM-5. O estudo discute a precisão da percepção do cuidador quanto aos sintomas de alerta centrais, bem como a heterogeneidade do fenótipo de TEA.


La investigación describe el desarrollo de la Phone Screening Interview (PSI) para la detección telefónica de los síntomas de los Trastornos del Espectro Autista (TEA). Se entrevistó a 37 padres de niños (de 2 a 12 años) con TEA del Servicio de Psicología Aplicada de la PUC-Rio. Los niños fueron evaluados por psiquiatras y se aplicó la escala CARS para definir la gravedad del trastorno. Se realizaron análisis estadísticos descriptivos e inferenciales multivariados (ANOVA y conglomerados jerárquicos). Los resultados muestran que el rendimiento en la PSI no varía según la edad, la educación, la gravedad y el desarrollo lingüístico o motor. El análisis de conglomerado para las variables mostró una agrupación de los síntomas de los medios de comunicación y otra de los patrones estereotipados. El análisis de conglomerado de las respuestas de los padres en la PSI mostró dos grupos que difieren en el grado de dificultad en las habilidades sociales y de comunicación. En conclusión, la PSI resultó: (1) fácil de aplicar; (2) breve; (3) con amplio rango de edad (2 a 12 años); (4) aplicable a síntomas de TEA de leves a moderados; (5) incluyendo niños verbales y no verbales; (6) consistente con los criterios de diagnóstico del DSM-5. El estudio discute la exactitud de la percepción del cuidador respecto a los síntomas centrales de alerta, además de la heterogeneidad del fenotipo de TEA.

Resumo Os critérios que subsidiaram o diagnóstico do autismo passaram por diversas mudanças ao longo dos anos, acompanhando os pressupostos conceituais hegemônicos no período. O objetivo deste estudo é analisar a evolução do diagnóstico do autismo no século XXI, a partir dos domínios e subdomínios em que se baseiam as categorizações nosológicas. Adicionalmente, são sinalizados instrumentos auxiliares utilizados e algumas tecnologias diagnósticas em desenvolvimento, além de modelos conceituais que tratam do perfil neuropsicológico. Trata-se de pesquisa documental, tendo como referência os manuais diagnósticos existentes no período. Os domínios de interação social, comunicação e padrão restrito e repetitivo de comportamento foram mantidos nos diferentes manuais diagnósticos abordados. Os subdomínios de interação social e comunicação foram reduzidos, enquanto o padrão restrito e repetitivo teve consolidação no DSM-5. Compreender a evolução dos critérios diagnósticos tende a promover o desenvolvimento da clínica, potencializando a antecipação do diagnóstico e as intervenções necessárias para um melhor prognóstico.


Abstract The criteria that subsidized the diagnosis of autism have undergone several changes over the years, following the hegemonic conceptual assumptions of the period. This study analyzed the evolution of the diagnosis of autism during the 21st century with use of the domains and subdomains that served as basis for nosological categorizations. Additionally, the auxiliary instruments used, some diagnostic technologies under development, and conceptual models that address neuropsychological profiles are presented. This documentary research utilized diagnostic manuals of the period. The domains of social interaction, communication, and restricted and repetitive behavior patterns were maintained in the different diagnostic manuals addressed. The subdomains of social interaction and communication were reduced, while restricted and repetitive pattern was consolidated in DSM-5. Understanding the evolution of diagnostic criteria tends to promote clinical development, potentiating the anticipation of the diagnosis and the interventions necessary for a better prognosis.


Résumé Les critères qui subsidient le diagnostic de l'autisme ont subi plusieurs changements, suivant les hypothèses conceptuelles hégémoniques de la période. Cette étude vise à analyser l'évolution du diagnostic de l'autisme au 21e siècle, à partir des domaines et sous-domaines sur lesquelles reposent les catégorisations nosologiques. En outre, les instruments auxiliaires utilisés et certaines technologies de diagnostic en cours de développement sont signalés, en plus des modèles conceptuels sur le profil neuropsychologique. Il s'agit d'une recherche documentaire, basée sur les manuels de diagnostic existant à l'époque. Les domaines de l'interaction sociale, de la communication et des comportements restreints et répétitifs ont été maintenus dans les manuels de diagnostic abordés. Les sous-domaines de l''interaction sociale et de la communication ont été réduits, tandis que le norme restreint et répétitif a été consolidé dans le DSM-5. Comprendre l'évolution des critères diagnostiques tend à favoriser le développement clinique, l'anticipation du diagnostic, et les interventions nécessaires à un meilleur pronostic.


Resumen Los criterios que ayudaron el diagnóstico de autismo han sufrido varios cambios, siguiendo los supuestos conceptuales hegemónicos en el período. El objetivo de este estudio es analizar la evolución del diagnóstico de autismo en el siglo XXI, en función de los dominios y subdominios que se basan las categorizaciones nosológicas. Además, se señalan los instrumentos auxiliares utilizados, algunas tecnologías de diagnóstico en desarrollo y modelos conceptuales que abordan el perfil neuropsicológico. Esta es una investigación documental, basada en los manuales de diagnóstico existentes en el período. Los dominios de interacción social, comunicación y patrones de conducta restringidos y repetitivos se mantuvieron en los manuales de diagnóstico abordados. Los subdominios de interacción social y comunicación se redujeron, mientras que el patrón restringido y repetitivo se consolidó en el DSM-5. Comprender la evolución de los criterios de diagnóstico tiende a promover el desarrollo clínico, fomentando la anticipación diagnóstica e intervenciones necesarias para un mejor pronóstico.

ABSTRACT

Este trabalho apresenta um olhar etnográfico sobre a experiência das pessoas que dependem da hemodiálise para continuar vivendo. Neste artigo, o foco é o paciente, apresentamos suas percepções e experiências, tomando como base seu convívio com a doença e suas estratégias de enfrentamento, pois as características bastante peculiares desse tratamento geram uma relação complexa, acompanhada de contradições e ambiguidades. Os entrevistados, com idades entre 18 e setenta anos, estavam há mais de um ano nesse tratamento e revelaram vidas marcadas pela experiência da doença, além de depoimentos que nos levam a analisar a hemodiálise como espaço de liminaridade. Interpretamos, aqui, à luz da teoria antropológica e a partir de alguns depoimentos dos 117 entrevistados, que todo paciente com insuficiência renal crônica, dependente da hemodiálise, vive em um espaço liminar que pode durar meses ou anos.(AU)


This study presents an ethnographic view about the experience of people who depend on hemodialysis to survive. The investigation focused on patients, their perceptions and experiences, based on their relationship with the disease and coping strategies, given that the specificities of this type of treatment originate a complex process, followed by contradictions and ambiguities. The interviewees, who were aged between 18 and 70 years and had been on this treatment over a year, revealed having lives marked by the disease experience and statements which make hemodialysis to be construed as a liminality space. The study, grounded on the anthropological theory, and some of the reports by the 117 interviewees developed the interpretation that every patient with chronic kidney disease who depends on hemodialysis lives in a liminal space that can last months or years.(AU)


Este trabajo presenta una mirada etnográfica sobre la experiencia de las personas que dependen de la hemodiálisis para continuar viviendo. En este artículo el enfoque es el paciente y presentamos sus percepciones y experiencias, tomando como base su convivencia con la enfermedad y sus estrategias de enfrentamiento, puesto que las características bastante peculiares de este tratamiento generan una relación compleja, acompañada de contradicciones y ambigüedades. Los entrevistados con edades entre 18 y 70 años estaban desde hacía más de un año en ese tratamiento y revelaron vidas marcadas por la experiencia de la enfermedad, además de declaraciones que nos llevan a analizar la hemodiálisis como espacio de condición preliminar. Interpretamos aquí a la luz de la teoría antropológica y a partir de algunas declaraciones de los 117 entrevistados que todos los pacientes con insuficiencia renal crónica, dependientes de hemodiálisis viven en un espacio preliminar que puede durar meses o años.(AU)

ABSTRACT The diagnosis of early signs of Alzheimer’s disease (AD) is a major challenge in a heterogeneous population. Objective To investigate the use of the Brief Cognitive Screening Battery (BCSB) for the diagnosis of mild AD in a geriatric outpatient unit of a public hospital in the city of Rio de Janeiro. Method BCSB was administered to 51 elderly adults with a clinical diagnosis of probable AD and 123 older adults without dementia (non-AD). Results AD patients performed worse than non-AD group in all BCSB tests, except Clock Drawing (p = 0.10). The ROC curves and Logistic Regression analysis indicated that delayed recall in the figure memory test was the best predictor, screening mild AD patients with sensibility and specificity superior to 80%. Conclusion The BCSB was accurate in identifying people with AD in a geriatric outpatient clinic at a public hospital, including elderly people with chronic diseases, physical frailty and cognitive impairment.


RESUMO O diagnóstico de sinais precoces da doença de Alzheimer (DA) é um grande desafio. Objetivo Investigar o uso da Bateria Breve de Rastreio Cognitivo (BCSB) para o diagnóstico de DA leve, em ambulatório de geriatria de hospital público do Rio de Janeiro. Método A BCSB foi administrada a 51 idosos com DA e 123 idosos sem demência (não-DA). Pacientes DA tiveram pior desempenho do que o grupo não-DA na BCSB, exceto desenho do Relógio (p = 0,10). Resultados Os resultados se mativeram após o controle de escolaridade e idade. As curvas ROC e análise de regressão logística indicaram que evocação tardia do teste de memória de figuras foi o melhor preditor para rastreio, com sensibilidade e especificidade superiores a 80%. Conclusão A BCSB, especialmente evocação tardia, identificou pessoas com DA em ambulatório de geriatria de hospital público, em uma amostra incluindo idosos com doenças crônicas, fragilidade física e comprometimento cognitivo.

O estudo teve como objetivo investigar a validade convergente do Inventário de Empatia. A amostra foi composta por 230 indivíduos adultos, de ambos os sexos, que responderam ao Inventário de Empatia (IE) e à Escala Multidimensional de Reatividade Interpessoal (EMRI). Correlações positivas e significativas fortes ou moderadas foram obtidas entre os fatores cognitivos do IE (Tomada de Perspectiva e Flexibilidade Interpessoal) e o fator cognitivo da EMRI. No entanto, os fatores afetivos do IE (Altruísmo e Sensibilidade Afetiva) apresentaram corelações positivas e significativas, porém baixas, ou correlações não-significativas, com os fatores afetivos da Escala EMRI. Os achados fornecem, portanto, evidências iniciais acerca da validade convergente dos fatores cognitivos do IE e aprimoram suas qualidades psicométricas no que diz respeito à avaliação da habilidade empática.


The study aimed at investigating the convergent validity of the Empathy Inventory. The sample was composed by 230 adult individuals from both sexes who answered to the Empathy Inventory (EI) and to the Multidimensional Scale of Interpersonal Reactivity (MSIR). Strong or moderate positive and significant correlations were obtained between the EI's cognitive factors (Perspective Taking and Interpersonal Flexibility) and the MSIR's cognitive factor. Therefore, the EI's affective dimensions (Altruism and Affective Sensibility) showed low positive significant correlations or no correlations with the MSIR's affective factors. These findings provide initial evidences about the convergent validity of EI and refine its psychometric qualities as a measure of assessment of empathic skills.


El propósito del estudio fue investigar la validez convergente del Inventario de Empatía. La muestra fue compuesta por 230 individuos adultos de ambos sexos, que respondieron al Inventario de Empatía (IE) y a la Escala Multidimensional de Reactividad Interpersonal (EMRI). Correlaciones positivas y significativas fuertes o moderadas fueran obtenidas entre los factores cognitivos del IE (Toma de Perspectiva y Flexibilidad Interpersonal) y el factor cognitivo de la EMRI. No obstante, los factores afectivos del IE (Altruismo y Sensibilidad Afectiva) presentaron correlaciones positivas y significativas, pero bajas o no significativas con los factores afectivos de la EMRI. ABSTRACT

Cada vez mais, reabilitação neuropsicológica e psicoterapia cognitivo-comportamental veem apresentado características em comum. O presente trabalho apresenta um modelo de interface entre essas duas abordagens de intervenções terapêuticas. O modelo tem como base mecanismos de plasticidade neural e descreve estratégias relacionadas com avaliação neuropsicológica detalhada do funcionamento neurocognitivo-comportamental do paciente. Essa avaliação representa uma condição essencial para o desenvolvimento de estratégias eficazes de intervenção psicológica. O modelo também sugere uma orientação com os familiares e outros indivíduos que convivem diretamente com o paciente. Finalmente o modelo propõe a associações de técnicas de estimulação das funções neuropsicológicas comprometidas e de desenvolvimento de formas alternativas para o emprego de funções neuropsicológicas preservadas assim como técnicas cognitivo-comportamentais capazes de promover mudanças para um repertório psicossocial adaptado à nova condição do paciente(AU)


There is an increasingly common ground between neuropsychological rehabilitation and cognitive behavioral psychotherapy. This paper presents a model that emphasizes the interface between these two therapeutic intervention approaches. The model is based on neural plasticity mechanism and describes strategies related to comprehensive neuropsychological assessment of the patient neurocognitivefunctioning. This evaluation is an essential requirement for developing effective strategies for psychological intervention. The model also suggests an orientation with family members and other individuals who live directly with the patient. Finally, the model proposes the associations between stimulation techniques of neuropsychological functions impaired and development of alternative forms for the use of neuropsychological functions preserved with cognitive behavioral techniques which promotes psychosocial wellness of the patient(AU)

ABSTRACT

O objetivo deste trabalho foi verificar a relação entre déficits em habilidades sociais e níveis de depressão. Participaram da amostra 46 indivíduos de ambos os sexos. Dos participantes, 23 eram pacientes de psicoterapia (grupo clínico) que preencheram os critérios do DSM-IV-TR para o diagnóstico de depressão e apresentaram escores compatíveis com esse diagnóstico no inventário Beck de depressão (BDI). Os outros 23 indivíduos (grupo de comparação) apresentaram escores no self report questionnaire (SRQ) compatíveis com baixa probabilidade de desenvolver quadro psiquiátrico. Ambos os grupos responderam ainda ao inventário de habilidades sociais (IHS-Del Prette) e ao inventário de empatia (IE). Os terapeutas do grupo clínico forneceram informações sobre seus pacientes por meio de um questionário. A análise estatística mostrou diferenças significativas entre os grupos. Uma análise qualitativa indicou relação entre sintomas depressivos e períodos de estresse interpessoal. Os resultados confirmam estudos anteriores e sugerem que déficits em habilidades sociais podem constituir um fator de vulnerabilidade para a depressão.(AU)


Objective: To investigate the relationship between deficits in social skills and depression levels. Method: The sample consisted of 46 participants of both sexes were 23 patients in psychotherapy (clinical group) met the DSM-IV-TR for the diagnosis of depression, and had scores compatible with this diagnosis in the Beck depression (BDI). The other 23 subjects (comparison group) had scores in the self report questionnaire (SRQ) compatible with a low probability of developing psychiatric disorder. Yet responded to the Social Skills Inventory (IHS-Del Prette) and Empathy Inventory (IE). Therapists Clinical Group provided information on their patients through a questionnaire. Results: Statistical analysis showed significant differences between groups. A qualitative analysis indicated the relationship between depressive symptoms and interpersonal stress periods. Conclusion: The results confirm previous studies and suggest that deficits in social skills may be a vulnerability factor for depression.(AU)

O objetivo deste trabalho foi verificar a relação entre déficits em habilidades sociais e níveis de depressão. Participaram da amostra 46 indivíduos de ambos os sexos. Dos participantes, 23 eram pacientes de psicoterapia (grupo clínico) que preencheram os critérios do DSM-IV-TR para o diagnóstico de depressão e apresentaram escores compatíveis com esse diagnóstico no inventário Beck de depressão (BDI). Os outros 23 indivíduos (grupo de comparação) apresentaram escores no self report questionnaire (SRQ) compatíveis com baixa probabilidade de desenvolver quadro psiquiátrico. Ambos os grupos responderam ainda ao inventário de habilidades sociais (IHS-Del Prette) e ao inventário de empatia (IE). Os terapeutas do grupo clínico forneceram informações sobre seus pacientes por meio de um questionário. A análise estatística mostrou diferenças significativas entre os grupos. Uma análise qualitativa indicou relação entre sintomas depressivos e períodos de estresse interpessoal. Os resultados confirmam estudos anteriores e sugerem que déficits em habilidades sociais podem constituir um fator de vulnerabilidade para a depressão.


Objective: To investigate the relationship between deficits in social skills and depression levels. Method: The sample consisted of 46 participants of both sexes were 23 patients in psychotherapy (clinical group) met the DSM-IV-TR for the diagnosis of depression, and had scores compatible with this diagnosis in the Beck depression (BDI). The other 23 subjects (comparison group) had scores in the self report questionnaire (SRQ) compatible with a low probability of developing psychiatric disorder. Yet responded to the Social Skills Inventory (IHS-Del Prette) and Empathy Inventory (IE). Therapists Clinical Group provided information on their patients through a questionnaire. Results: Statistical analysis showed significant differences between groups. A qualitative analysis indicated the relationship between depressive symptoms and interpersonal stress periods. Conclusion: The results confirm previous studies and suggest that deficits in social skills may be a vulnerability factor for depression.
