ABSTRACT

OBJECTIVES: To assess the prevalence of and factors associated with the lifetime medical diagnosis of depression in Brazil. DESIGN: Population-based, cross-sectional study. SETTING: Analysis of data from the 2019 Brazilian National Health Survey. PARTICIPANTS: 90 846 individuals aged ≥15 years were included. OUTCOME MEASURE: The self-reported medical diagnosis of depression at some point in one's life was the main outcome. Prevalence ratios (PRs) with 95% CIs were calculated by Poisson regression with robust variance. The independent variables included the geographical area of residence, sociodemographic characteristics, current smoking status, alcohol abuse, daily screen time, and the presence of physical disorders and mental health comorbidities. RESULTS: The self-reported lifetime prevalence of medical diagnosis of depression was 9.9% (95% CI 9.5% to 10.2%). The probability of having received a medical diagnosis of depression was higher among urban residents (PR 1.23; 95% CI 1.12 to 1.35); females (2.75; 2.52 to 2.99); those aged 20-29 years (1.17; 0.91 to 1.51), 30-39 years (1.73; 1.36 to 2.19), 40-49 years (2.30; 1.81 to 2.91), 50-59 years (2.32; 1.84 to 2.93) and 60-69 years (2.27; 1.78 to 2.90) compared with those under 20 years; white-skinned people (0.69 (0.61 to 0.78) for black-skinned people and 0.74 (0.69 to 0.80) for indigenous, yellow and brown-skinned people compared with white-skinned people); those with fewer years of education (1.33(1.12 to 1.58) among those with 9-11 years, 1.14 (0.96 to 1.34) among those with 1-8 years and 1.29 (1.11 to 1.50) among those with 0 years compared with those with ≥12 years of education); those who were separated/divorced (1.43; 1.29 to 1.59), widowed (1.06; 0.95 to 1.19) and single (1.01; 0.93 to 1.10) compared with married people; smokers (1.26; 1.14 to 1.38); heavy screen users (1.31; 1.16 to 1.48) compared with those whose usage was <6 hours/day; those with a medical diagnosis of a physical disorder (1.80; 1.67 to 1.97); and individuals with a medical diagnosis of a mental health comorbidity (5.05; 4.68 to 5.46). CONCLUSION: This nationwide population-based study of self-reported lifetime medical diagnosis of depression in Brazil showed that the prevalence was almost 10%. Considering the current Brazilian population, this prevalence corresponds to more than 2 million people who have been diagnosed with depression at some point in their lives.

Abstract Objective: This study aimed to describe the clinical and laboratory findings of patients diagnosed with pleural tuberculosis at two hospitals in southern Brazil. Methods: Patients aged < 18 years were evaluated retrospectively. The patients' medical and epidemiological history, tuberculin skin test results, radiological and pathological findings, and pleural fluid analysis results were retrieved. Results: Ninety-two patients with pleural tuberculosis were identified. The mean age was 10.9 years old. Twenty-one percent were children aged six years or less. The most common symptoms were fever (88%), cough (72%), and chest pain (70%). Unilateral pleural effusion was observed in 96% of the cases. Lymphocyte predominance was found in 90% of the pleural fluid samples. The adenosine deaminase activity of the pleural fluid was greater than 40 U/L in 85% of patients. A diagnosis of community-acquired pneumonia with antibiotic prescriptions was observed in 76% of the study population. Conclusions: Tuberculosis etiology must be considered in unilateral pleural effusion in a child with contact with a case of tuberculosis. Pleural fluid biomarkers contribute to the diagnosis of pleural tuberculosis in children and adolescents.

BACKGROUND: The fungus Agaricus subrufescens is grown commercially in China, the USA, Brazil, Taiwan and Japan, among others. However, each country adopts a cultivation system that significantly influences the agronomical parameters and chemical composition of the harvested mushrooms. In this study, the influence of the cultivation process on the content of ergosterol and vitamin D2 was evaluated. RESULTS: Four commercial strains of A. subrufescens (ABL 04/49, ABL CS7, ABL 18/01 and ABL 19/01) and two environmental cultivation conditions (in the field and a controlled chamber with the absence of sunlight) were used. Infield cultivation, ABL CS7 and ABL 19/01 strains presented better agronomic parameters, whereas in a protected environment ABL 19/01, ABL 04/49 and ABL 18/01 demonstrated better performance, respectively. The highest biological efficiency value (64%) was provided by ABL 19/01 strain in a controlled environment. CONCLUSION: The highest content in ergosterol (990 mg kg-1 ) and vitamin D2 (36.8 mg kg-1 ) were observed in mushrooms obtained in the field from strain ABL 04/49, which presents reasonable agronomic parameters for cultivation. © 2021 Society of Chemical Industry.

OBJECTIVE: To evaluate the accuracy of determining the adenosine deaminase (ADA) level, the 2'-deoxyadenosine/ADA ratio, and the LDH/ADA ratio in pleural fluid for the diagnosis of pleural tuberculosis (PT) in children and adolescents. METHODS: This was a retrospective cross-sectional study conducted at a tertiary hospital in a high-tuberculosis-incidence area, between 2001 and 2018. All patients with ADA in pleural fluid and a confirmed diagnosis of PT (cPT) or parapneumonic effusion (PPE) were included. RESULTS: The cPT and PPE groups comprised 25 and 68 individuals, respectively. At a cutoff of 40 U/L, ADA measurement showed the following: sensitivity, 88%; specificity, 31%; positive predictive value (PPV), 32%; negative predictive value (NPV), 88%; and overall accuracy, 46%. The best cutoffs were an ADA level of 125 U/L, a 2'-deoxyadenosine/ADA ratio of 0.5, and an LDH/ADA ratio of 8.3, with AUC of 0.67, 0.75, and 0.82, respectively. The sensitivity, specificity, PPV, NPV, and overall accuracy of the 125 U/L ADA cutoff were 84%, 65%, 47%, 92%, and 70%, respectively, compared with 79%, 79%, 59%, 91%, and 79%, respectively, for the 8.3 LDH/ADA ratio cutoff. Changing the LDH/ADA ratio cutoff to 3.0 increased the specificity to 98%. CONCLUSIONS: The ADA level and the 2'-deoxyadenosine/ADA ratio are not good biomarkers for the diagnosis of PT in pediatric patients. Determination of the LDH/ADA ratio provides the best overall accuracy for the diagnosis of PT in such patients.

RESUMO Objetivo: Avaliar a acurácia da determinação do nível de adenosina desaminase (ADA), da relação 2'-desoxiadenosina/ADA e da relação LDH/ADA no líquido pleural para o diagnóstico de tuberculose pleural (TP) em crianças e adolescentes. Métodos: Estudo transversal retrospectivo realizado em um hospital terciário em uma área de alta incidência de tuberculose entre 2001 e 2018. Todos os pacientes com determinação de ADA no líquido pleural e com diagnóstico confirmado de TP (TPc) ou de derrame parapneumônico (DPP) foram incluídos. Resultados: Os grupos TPc e DPP foram compostos por 25 e 68 indivíduos, respectivamente. Num ponto de corte de 40 U/L, a medida de ADA mostrou o seguinte: sensibilidade, 88%; especificidade, 31%; valor preditivo positivo (VPP), 32%; valor preditivo negativo (VPN), 88%; e acurácia geral, 46%. Os melhores pontos de corte foram ADA de 125 U/L, relação 2'-desoxiadenosina/ADA de 0,5 e relação LDH/ADA de 8,3, com ASC de 0,67, 0,75 e 0,82, respectivamente. A sensibilidade, especificidade, VPP, VPN e acurácia geral do ponto de corte de 125 U/L para ADA foram de 84%, 65%, 47%, 92% e 70%, respectivamente, em comparação com 79%, 79%, 59%, 91% e 79%, respectivamente, para o ponto de corte de 8,3 para a relação LDH/ADA. Ao alterar o ponto de corte da relação LDH/ADA para 3,0 a especificidade aumentou para 98%. Conclusões: O nível de ADA e a relação 2'-desoxiadenosina/ADA não são bons biomarcadores para o diagnóstico de PT em pacientes pediátricos. A determinação da relação LDH/ADA fornece a melhor acurácia geral para o diagnóstico de PT nesses pacientes.

ABSTRACT

The objective of this study was to compare the positive detection rates obtained using the Oxoid IMAGEN® direct immunofluorescence assay (designated as IF) with those obtained using the CLART® PneumoVir multiplex RT-PCR DNA microarray assay (designated as RT-PCR) in the diagnosis of respiratory viruses in hospitalized children. This was a retrospective study of 62 individuals < 18 years old who had nasopharyngeal aspirates collected for virus identification in a tertiary university hospital in south Brazil between January 1st, 2014 and December 31st, 2014. All 62 nasopharingeal aspirates were analyzed using both assay methods. The main outcome to be measured was the difference in the proportion of test samples returning a positive virus detection result between the IF and the RT-PCR. The McNemar test was used for data analysis and the results showed that the RT-PCR and the IF methods produced 55 (88.7 %) and 17 (27.4 %) virus-positive samples, respectively (p < 0.001). The most prevalent virus was rhinovirus (45.5 % of the RT-PCR positive samples). The RT-PCR method increased the detection rates of human respiratory syncytial virus, influenza A virus and parainfluenza 3 virus. The RT-PCR and IF had concordant results in 19 samples (30.6 %) and discordant results in 43 samples (69.4 %). It is concluded that in comparison to the Oxoid IMAGEN® IF method, the CLART® PneumoVir multiplex RT-PCR method had a greater potential to contribute to the clinical management of hospitalized children due its greater ability in detecting respiratory viruses than the IF method.

The objectives were to conduct a genetic evaluation of residual feed intake (RFI) and residual feed intake adjusted for fat (RFIFat) and to analyse the effect of selection for these traits on growth, carcass and reproductive traits. Data from 945 Nellore bulls in seven feed efficiency tests in a feedlot were analysed. Genetic evaluation was performed using an animal model in which the feed efficiency test and age of the animal at the beginning of the test were considered as a systematic effect. Direct additive genetic and residual effects were considered as random effects. Correlations and genetic gains were estimated by two-trait analysis between feed efficiency measures (RFI and RFIFat) and other traits. Feed conversion showed low heritability (0.06), but dry matter intake (DMI), average daily gain, RFI, RFIFat, metabolic body weight and scrotal circumference measured at 450 days of age (SC450) showed moderate to high heritability (0.49, 0.28, 0.33, 0.36, 0.38 and 0.80, respectively). Similarly, ribeye area, backfat thickness, rump cap fat thickness, marbling score and subcutaneous fat thickness also had high heritability values (0.46, 0.37, 0.57, 0.51 and 0.47, respectively). Genetic correlations between RFI and SC450 were null, and between RFIFat and SC450 were strongly positive. Genetic and phenotypic correlations of RFI and RFIFat with carcass traits were not different from zero, as correlated responses for carcass traits were also not different from zero. The Nellore selection for feed efficiency by RFI or RFIFat allows the recognition of feed efficient animals, with DMI reduction and without significant changes in growth and carcass traits. However, because of the observed results between RFIFat and SC450, selection of animals should be analysed with caution and a preselection for reproductive traits is necessary to avoid reproductive impairments in the herd.

ABSTRACT

PURPOSE: To assess the impact of balloon laryngoplasty on clinical and surgical outcomes in pediatric patients with acute subglottic stenosis. METHODS: Two case series were included and compared. The first group included patients treated initially either with tracheostomy (if severe symptoms) or with close follow-up (if mild symptoms). Those children underwent re-evaluation and specific treatment of their stenosis with laser incisions or open surgeries some weeks later. The other group included children treated initially with balloon laryngoplasty, reflecting a shift in surgical practice after 2009. Data as success of the procedure, mean hospital stay, mean pediatric intensive care unit (PICU) stay, post-procedure fever, need of antibiotics, procedure-related complications, and deaths were assessed and compared between both cohorts. RESULTS: The sample comprised 38 pediatric patients aged 0-5 years. Fifteen children were treated before 2009, of who 10 (66.7%) required tracheostomy soon after the diagnosis. Ultimately, 13 (86.6%) underwent laryngotracheal reconstruction. Twenty-three children were treated after 2009 and the success rate in these patients treated primarily with balloon laryngoplasty was 82.6%. Of these, only 3 (13%) required tracheostomy and 1 (4.3%) required further open laryngotracheal reconstruction. Patients treated by balloon laryngoplasty underwent fewer procedures under general anesthesia and had a lower burden of treatment-related morbidity, as denoted by shorter PICU stay, less antibiotic use, earlier postoperative resumption of oral feeding, and a lower incidence of postoperative complications and fever. CONCLUSION: When used for management of acute laryngeal stenosis, balloon laryngoplasty is associated with a high success rate, presenting lower morbidity than open surgery.

ABSTRACT

Neospora caninum is an apicomplexan parasite strongly related to reproductive problems in cattle. The neosporosis control is not well established and several fronts are under development, predominantly based on immune protection, immunomodulation and chemotherapy. The use of anti-malarial drugs as therapeutic sources has, in theory, considerable potential for any apicomplexan. Drugs such as methylene blue (MB) and pyrimethamine (Pyr) represent therapeutic options for malaria; thus, their use for neosporosis should be assessed. In this work, we tested the effects of MB and Pyr on N. caninum proliferation and clearance, using LacZ-tagged tachyzoites. The drugs inhibited at nanomolar dosages and its combination demonstrated an antagonistic interaction in proliferation assays, according to the Chou and Talalay method for drug combination index. However, the drug combination significantly improved the parasite in vitro clearance. The repositioning of well-established drugs opens a short-term strategy to obtain low-cost therapeutics approaches against neosporosis.

ABSTRACT: The study of the genetic evaluation of residual feed intake adjusted for fat (RFIFat) is important for the appropriate use of feed efficiency in selection programs. The objective was to analyze the influence of selection for RFIF at on carcass and performance traits by estimating various genetic parameters. Data were analyzed from five tests of feed efficiency, which were conducted with 677 Nellore males. Genetic evaluation was performed by Bayesian inference using an animal model via single- and two-trait analyses. Variables analyzed were dry matter intake, average daily gain, RFIFat, rib eye area, back fat thickness, rump fat thickness, marbling score, and subcutaneous fat thickness. The posterior mean distributions estimated at each analysis were used to estimate heritability of the traits and to perform various correlations. The studied traits showed high heritability estimates, and they should respond well to selection. The RFIFat presented a phenotypic correlation with carcass traits (which was next to zero), and there was also a negative genetic correlation. Additive genetic variability for RFIFat showed that selection for this trait can promote genetic gains in future generations, resulting in animals that are efficient in terms of nutrient use, and according to the genetic and phenotypic correlations, with no significant negative changes to carcass traits.


RESUMO: O estudo da avaliação genética do consumo alimentar residual ajustado para a gordura (CARFat) é importante para o uso apropriado da eficiência alimentar em programas de seleção. Objetivou-se analisar a influência da seleção para CARFat sobre características de carcaça e de desempenho, frente à estimação dos parâmetros genéticos. Foram analisados os dados de cinco provas de eficiência alimentar, com 677 machos Nelore. A avaliação genética foi realizada por Inferência Bayesiana, com modelo animal, em análises uni e bicaracterística. Foram analisadas as variáveis: ingestão de matéria seca, ganho de peso diário, CARFat, área de olho de lombo, espessura de gordura, espessura de gordura subcutânea na picanha, marmoreio e acabamento. As médias das distribuições a posteriori, estimadas em cada uma das análises, foram usadas para a estimação das herdabilidades e das correlações. As características estudadas apresentaram altas estimativas de herdabilidades, devendo responder bem à seleção. O CARFat apresentou correlações fenotípicas próximas a zero com as características de carcaça, e correlações genéticas negativas. A variabilidade genética aditiva observada para CARFat demonstra que a seleção para essa característica poderá promover ganhos genéticos nas futuras gerações, obtendo animais eficientes na utilização de alimentos e, de acordo com as correlações genéticas e fenotípicas encontradas, sem mudanças negativas significativas às características da carcaça.

The objective of this study was to evaluate the intake and apparent digestibility of nutrients, as well as the feeding behavior of sheep fed an exclusively corn silage diet (0%); this diet incorporated wet corn gluten feed (WCGF) in increments of 30%, 50%, 60%, and 90%, together with the profile of the particles of these silages. Five crossbreed sheep were used with an average weight of 40 kg; they were housed in metabolic cages and were arranged in a Latin square 5×5 experimental design. There was a quadratic effect on dry matter (DM) intake, with a maximum consumption of 1.75 kg/day­1 to an inclusion of 39.09% of WCGF. The crude protein (CP) consumption exhibited an ascending linear effect with the inclusion of the co-product, and fiber consumption in neutral detergent fiber (NDF) and acid detergent fiber decreased linearly with the inclusion of WCGF. The apparent digestibility of DM, CP, and NDF increased linearly with the increasing inclusion of co-product in corn silage. Rumination time, total chewing, and idleness were influenced by increases in the inclusion of WCGF in corn silage, and there was a decreasing linear effect on the amount of time spent ruminating and total chewing, as well as an increasing linear effect for idleness duration. The chewing time for bolus, the number of chews for bolus, and the number of chews per day were influenced by the inclusion of WCGF. Through the obtained results, especially the equation of DM intake, which estimated a maximum point of 39.09%, as well as the presence of diarrhea at levels up to 50%, it was concluded that the addition of up to 40% of WCGF in corn silage can be recommended as it improves nutrient intake and features good digestibility without negatively altering the normal behavior of sheep.


Objetivou-se com este estudo avaliar o consumo, a digestibilidade aparente dos nutrientes, e o comportamento ingestivo de ovinos alimentados com dieta exclusiva de silagem de milho (0%), e acrescidas de 30, 50, 60 e 90% de farelo úmido de glúten de milho (FUGM), na matéria natural, juntamente com o perfil das partículas dessas silagens. Foram utilizadas cinco ovelhas sem raça definida com peso vivo médio de 40 kg, alojadas em gaiolas metabólicas, arranjadas no delineamento experimental em quadrado latino 5x5. Houve efeito quadrático para o consumo de matéria seca, com consumo máximo de 1,75kg/dia aos 39,09% de inclusão de FUGM. O consumo de proteína bruta sofreu efeito linear ascendente a com a inclusão do coproduto, e os consumos de fibra em detergente neutro (FDN) e fibra em detergente ácido (FDA) diminuíram linearmente com a inclusão de FUGM. A digestibilidade aparente da MS, PB e FDN aumentaram linearmente com o aumento na inclusão do coproduto na silagem de milho. Os tempos de ruminação, mastigação total, e ócio foram influenciados pelo aumento na inclusão de FUGM na silagem de milho, houve efeito linear decrescente para o tempo em ruminação, e mastigação total, e efeito linear crescente para o tempo em ócio. O tempo de mastigação por bolo, número de mastigações por bolo, e o número de mastigações por dia, sofreram influencia da inclusão de FUGM. Através dos resultados obtidos, especialmente da equação de consumo de matéria seca, que estimou ponto de máximo de 39,09% e pela presença de diarreia em animais que consumiam níveis de inclusão acima de 50%, conclui-se que adições de até 40% de FUGM na silagem de milho são recomendadas devido ao fato do mesmo melhorar o consumo de nutrientes, tendo boa digestibilidade sem alterar negativamente o comportamento normal dos ovinos.

ABSTRACT

Objetivou-se avaliar aspectos físico-químicos e microbiológicos do farelo úmido de glúten de milho (FUGM) em função do tempo de ensilagem. O FUGM foi avaliado em silos experimentais e, nos dias um, três, sete, 14, 21, 28 e 42, foram realizadas as avaliações físicas, químicas e microbiológicas da silagem. O delineamento experimental foi inteiramente casualizado com quatro repetições e obtidas equações de regressão em função do tempo. O tempo de ensilagem não afetou (P>0,05) o percentual de fibra em detergente neutro, fibra em detergente ácido, proteína bruta, matéria mineral, extrativos não-nitrogenados, nutrientes digestíveis totais e contagem de clostrídios. Os teores de matéria seca, extrato etéreo, perdas por efluentes, perdas por gases e população de bactérias láticas variaram entre 46,54 e 49,05%; 0,96 e 1,71%; 17,97 e 35,14g kg-1; 1,90 e 5,68% e 5,0 e 6,9log UFCg-1 de silagem, respectivamente, todos com equações com r2≤0,6.O pH (Y=4,12926-0,00894dias+0,00024126dias2,r2=0,75) e a temperatura (Y=19,82657+0,11734dias-0,00502dias2,r2=0,73) tiveram seus menores e maiores valores, respectivamente, em 18,5 e 11,7 dias. A contagem de bolores e leveduras sofreu redução em função do tempo de ensilagem (Y=6,8983-0,0363dias,r2=0,84). A ensilagem permite eficiente conservação dos aspectos químicos, microbiológicos e físicos do FUGM até 42 dias de ensilagem.


This study aimed to evaluate physicochemical and microbiological aspects of wet corn gluten feed (WCGF) according to time of ensilage. The WCGF was evaluated in experimental silos and after one, three, seven, 14, 21, 28 and 42 days, physical, chemical and microbiological evaluation of the silage was carried out. The results were analyzed as a completely randomized design with four replications and regression equations as a function of time. The number of days of ensiling did not affect neutral detergent fiber, acid detergent fiber, crude protein, mineral matter, non-nitrogenous extract and total digestive nutrients percentages, as well as clostridia count. Dry matter, ether extract, effluents losses and percentages of gas losses and lactic acid bacteria count with variations of 46.54 and 49.05%; 0.96 and 1.71%; 17.97 and 35.14g kg-1; 1.90 and 5.68% and 5.0 and 6.9log CFUg-1 of silage, respectively, all had low coefficient of determination (r2≤0.6).The pH (Y=4.12926-0.00894days+0.00024126days2,r2=0.75) and temperature (19.82657+0.11734days-0.00502days2,r2=0.73) had their minor and major values, respectively, in 18.5 and 11.7 days. Moulds and yeasts count decrease with time of ensilage (Y=6.8983-0.0363days,r2=0.84). The ensiling allows efficient conservation of the chemical, microbiological and physical aspects of WCGF until 42 days of ensilage.

OBJECTIVE: To determine the prevalence and the severity of wheezing in the first year of life for infants who live in Santo André, São Paulo, Brazil. METHODS: Cross sectional study with the administration of the Estudio Internacional de Sibilancias en Lactantes (EISL), which is a standardized and validated written questionnaire applied to parents and/or guardians of infants aged 12-24 months treated at primary health units, vaccination centers, day care centers, or kindergartens. The questionnaire consisted of questions regarding demographic characteristics, presence of wheezing, respiratory infections, and risk factors. Results were analyzed using the SPSS for Windows, 20.0 (SPSS Inc. - Chicago, Il, United States). Logistic regression was applied to verify variables associated to recurrent wheezing. RESULTS: Among the 1,028 infants studied, 48.5% had one or more episodes of wheezing during the first 12 months of life (wheezing once), and 23.9% had three or more episodes (recurrent wheezing). Nocturnal symptoms, severe breathing difficulty, and visits to the emergency room were observed in 67.3%, 42.4%, and 60.7% of infants, respectively. Among the studied infants, 19.4% were hospitalized, and 11.0% had a medical diagnosis of asthma in the first year of life. Use of ß2-agonists, inhaled corticosteroids, oral corticosteroids, and leukotriene receptor antagonists were observed in 88.8%, 21.0%, 54.9%, and 3.2% of children with wheezing, respectively. Use of oral corticosteroids, perception of breathlessness by parents, diagnosis of asthma, pneumonia, and hospitalization for pneumonia were more frequent among infants with recurrent wheezing (p<0.001). CONCLUSIONS: In the city of Santo André, approximately half of infants had at least one episode of wheezing in the first year of life, and almost 25% had recurrent wheezing. Wheezing disorders in Santo André have early onset and high morbidity.

Objetivo: Verificar a prevalência e a gravidade da sibilância no primeiro ano de vida em lactentes no município de Santo André. Métodos: Estudo transversal com aplicação de questionário escrito, padronizado e validado do estudo EISL (do espanhol: Estudio Internacional de Sibilancias en Lactantes) aos pais e/ou responsáveis por lactentes de 12 a 24 meses de idade, em unidades básicas de saúde, durante campanhas de vacinação em creches e escolas maternais. O questionário consiste de questões sobre características demográficas, presença de sibilância, infecções respiratórias e fatores de risco. Os dados obtidos foram analisados com o Statistical Package for the Social Sciences for Windows, versão 20.0 (SPSS inc., Chicago, Il, EUA). A regressão logística foi aplicada para analisar as variáveis associadas à presença de sibilos recorrentes. Resultados: Do total de 1.028 lactentes estudados, 48,5% apresentaram um ou mais episódios de sibilância nos primeiros 12 meses de vida (sibilância alguma vez) e 23,9%, três ou mais episódios (sibilância recorrente). Sintomas noturnos, dificuldade intensa para respirar e visitas à emergência estiveram presentes em 67,3%, 42,4% e 60,7% dos lactentes, respectivamente, sendo que 19,4% foram hospitalizados e 11,0% tiveram diagnóstico médico de asma. O uso de β2-agonistas, corticoides inalatórios, corticoides orais e antagonistas do receptor de leucotrienos foram observados em 88,8%, 21,0%, 54,9% e 3,2% das crianças com sibilância, respectivamente. Uso de corticoide oral, percepção de falta de ar pelos pais, diagnóstico de asma, pneumonia e hospitalização por pneumonia foram mais frequentes entre os sibilantes recorrentes (p<0,001). ABSTRACT

In this study, we aimed to investigate whether the distribution of the FcγRIIa and FcγRIIIb polymorphisms determines susceptibility to systemic lupus erythematosus (SLE) and acts as predictors of SLE clinical manifestations in the Brazilian patients. A total of 157 patients that fulfilled the American College of Rheumatology classification criteria for SLE and 160 healthy volunteers were included in this study. FCGR2A and FCGR3B genotypes were determined by polymerase chain reaction-based allotyping methods with allele-specific primers; the clinical features were obtained from the patients' official medical records. In the case of FcγRIIa polymorphism, it was observed association of the allele FCGR2A-R-131 (p = 0.02, odds ratio (OR)=1.44) and genotype RR-131 (p = 0.03, OR = 2.09) with SLE. These associations were higher with allele (p < 0.01, OR = 1.67) as well genotype (p = 0.01, OR = 2.85) when lupus nephritis was considered. In contrast, the allele FCGR2A-H-131 was associated with susceptibility to arthritis and anti-DNA antibodies (p = 0.05 for both). As for FcγRIIIb polymorphism, skewing did not differ significantly between patients and controls, however the genotype FCGR3B*02*02 was associated with susceptibility to arthritis (p = 0.02) and malar rash (p = 0.03), but no association with nephritis was found. The results demonstrate that FcγRIIa polymorphism is associated with susceptibility to SLE in Brazilian patients, whereas for FcγRIIIb polymorphism no association was found. However, notably, both polymorphisms present allelic variants that influence the clinical manifestations and may contribute to the pathogenesis of the disease. In addition, to our knowledge, this is the first study considering the frequency of FcγRIIIb polymorphism in Brazilian SLE patients.

BACKGROUND/AIMS: Immune responses mediated by complement receptors (CR) are impaired in patients with systemic lupus erythematosus (SLE). Regarding CR3 (CD11b/CD18), the CD11b subunit is encoded by the ITGAM gene and a single nucleotide polymorphism (G230A; rs1143679) in ITGAM changes an arginine to a histidine at position 77 (R77H). We assessed whether the variant R77H, rs1143679 within ITGAM, is associated with the risk to developing SLE and the clinical manifestations of Brazilian SLE patients. METHODS: The rs1143679 was genotyped by SSP-PCR in 157 patients with SLE and 147 healthy individuals. Clinical and laboratorial manifestations were obtained from the official medical records according the criteria of the American College of Rheumatology. RESULTS: The 77H variant was associated with susceptibility to SLE (OR=1.8); the frequencies of the minor allele A were 0.25 (SLE) and 0.15 (healthy) (p<0.01). In addition, the minor allele A was associated with lupus nephritis (p=0.02) and antiphospholipid antibodies (p=0.04). CONCLUSION: These results showed that the rs1143679 variant is also associated with the risk to SLE in our population and with the risk to specific clinical manifestations, as nephritis and presence of antiphospholipid antibodies. These results may have implications for discussing the association of this polymorphism with the IC deposition in SLE.

ABSTRACT

Investigou-se a influência da estação do ano sobre as características termorreguladoras e de pelame bem como os gradientes térmicos em novilhas da raça Nelore. Utilizaram-se quinze animais com idade inicial e final de 12 e 18 meses, respectivamente. Foram avaliadas as temperaturas retal (TR), da epiderme (TEP) e da superfície corporal (TSC), bem como as frequências cardíaca (FC) e respiratória (FR), taxa de sudação (TS), comprimento do pelo, peso e número dos pelos e os gradientes térmicos entre temperaturas retal e do ar (TR-TAR), temperaturas da superfície corporal e do ar (TSC-TAR) e temperaturas retal e da superfície corporal (TR-TSC). No inverno, as médias de TR, TEP e TSC foram de 39,6ºC; 36,4ºC e 35,9ºC, respectivamente, e no verão foram de 39,1ºC; 37,6ºC e 37,3ºC, respectivamente. A média da FR, FC e TS no inverno foram de 33,6 mpm, 68,9 bpm e 82 g.m-2h-1, respectivamente, e no verão foram de 31,4 mpm, 74,7 bpm e 210,7 g.m-2h-1, respectivamente. No inverno o número médio (601,13 pelos/cm2) e o comprimento (1,04 mm) de pelos foram maiores em relação ao verão que foram, respectivamente, 289,2 pelos/cm2 e 0,59 mm. No verão, o gradiente TSC-TAR (10,20) foi superior ao inverno (8,63) e o gradiente TR-TSC no verão (1,85) foi inferior ao inverno (3,74). Os parâmetros fisiológicos, gradientes térmicos e características do pelame se encontraram dentro da normalidade, ou seja, novilhas da raça Nelore não tiveram dificuldade em manter o equilíbrio térmico, o que confirma que são adaptadas ao ambiente tropical.


The influence of season on thermo regulators traits and hair coat characteristics was investigated, as well as the thermal gradients in Nellore heifers. Fifteen animals with initial and final age of 12 and 18 months, respectively, were used. The rectal (RT), skin (ST) and body surface (BST) temperatures, heart (HR), respiratory (RR) and sweating (SR) rates, the length, weight and number of hairs, the thermal gradients between rectal and air temperatures (RT-AT), body surface and air temperatures (BST-AT) and rectal and body surface temperatures (RT-BST) were evaluated. In winter, the RT, ST and BST averages were 39.6ºC, 36.4ºC and 35.9°C, respectively, and in summer were 39.1°C, 37.6°C and 37.3°C respectively. The RR, HR and SR averages in winter were 33.6 mpm, 68.9 bpm and 82 g.m-2h- 1, respectively, and in summer were 31.4 mpm, 74.7 bpm and 210.7 g.m-2/h-1, respectively. In winter the average number (601.13 hairs/cm2) and the hair length (1.04 mm) were higher than in summer, that were respectively 289.2 hairs/cm2 and 0.59 mm. In summer, the BST-AT gradient (10.20) was higher than in winter (8.63) and RT-BST gradient in summer (1.85) was lower than in winter (3.74). The physiological parameters, thermal gradients and hair coat characteristics are normal, in other words, the Nellore heifers had no difficulty in maintaining the thermal balance, which indicates that they are adapted to the tropical environment.
