ABSTRACT

Allium dictyoprasum C.A. Meyer ex Kunth (A. dictyoprasum) underwent comprehensive analysis, encompassing quantum chemical computations to assess its radical scavenging potential, chemical and elemental composition, total phenolic content, and antimicrobial activity. Experimental and theoretical investigations focused on elucidating the radical scavenging properties of polyhydroxy phenolic compounds present in the plant. Quantum chemical calculations were employed to evaluate the antioxidants employed to evaluate selected polyhydroxy phenolic molecules including flavonoids, hydrocinnamic acid derivatives, and hydroxybenzoic acid derivatives from natural sources. Thermochemical parameters of these compounds were calculated by the B3LYP/6-311 G++(d,p) level in both gas and solvent phases to elucidate the radical scavenging mechanism including hydrogen atom transfer (HAT), single electron transfer followed by proton transfer (SET-PT) and sequential proton loss electron transfer (SPLET). Analysis of A. dictyoprasum extracts obtained via various extraction methods revealed the presence of several major compounds, including dimethyl trisulfide, 3,5-Dihydroxy-6-methyl-2,3-dihydro-4 H-pyran-4-one, 2-Methoxy-4-vinylphenol, Dimethyl phthalate, Methyl palmitate, Methyl oleate, Methyl stearate, (9Z)-9-Octadecenamide. Notably, Malic acid and Quinic acid were identified as major compounds, with concentrations of 43.31 and 17.47 mg kg-1 extract, respectively, based on LC-MS/MS analysis. The total phenolic content of the extract was measured as 17.83 mg gallic acid/mL, while its free radical scavenging activity was 80.89% per mg/mL. Elemental analysis revealed significant levels of Mg, K, Na, Fe, and P, with minor concentrations of elements such as Ti, Tl, B, and Be. Furthermore, A. dictyoprasum exhibited notable antibacterial activity against various bacteria strains, surpassing the efficacy of some commercial antibiotics.


Quantum chemical calculations of radical scavenging analysis were performed.Thermochemical parameters were calculated by the B3LYP/6­311 G++(d,p) level.Radical scavenging mechanism was evaluated based on HAT, SET-PT and SPLET.Chemical composition of Allium dictyoprasum C.A. Meyer ex Kunth was determined.A. dictyoprasum has a greater antibacterial effects than some commercial antibiotics.

ABSTRACT

OBJECTIVE: Heart failure is a leading cause of death and the most common diagnosis leading to hospitalization. Its awareness is lower than that of other cardiovascular diseases, both in the general population and among patients with heart failure (HF). This study aimed to establish the current level of knowledge about HF in patients with HF with reduced ejection fraction (HFrEF) and mildly reduced ejection fraction (HFmrEF) in Türkiye. METHODS: This questionnaire-based survey study is multicenter, conducted across 34 centers from December 2021 to July 2022. We performed a survey consisting of two sets of questions focusing on individual characteristics of the patients and HF-related knowledge. RESULTS: The study included a total of 2,307 outpatient HF patients, comprising 70.5% males and 29.5% females with a mean age of 64.58 ± 13 (56-74) years and a mean body mass index value of 32.5 ± 10 kg/m2. HFrEF and HFmrEF were determined in 74.7% and 25.3% of patients, respectively. Thirty percent of the patients were unaware that they had HF. While 28.7% of the patients thought that they had sufficient information about HF, 71.3% believed they lacked adequate knowledge. In the study, 25.2% of the participants identified dyspnea, 22% identified tiredness, and 25.4% identified leg edema as the most common symptoms of HF. Only 27.4% of patients recognized all three typical symptoms of HF. CONCLUSION: We found that the study population's knowledge about HF symptoms and the nature of the disease was poor. Educational and awareness activities are necessary to optimize outcomes and benefits.

ABSTRACT

Following the great success of various deep learning methods in image and object classification, the biomedical image processing society is also overwhelmed with their applications to various automatic diagnosis cases. Unfortunately, most of the deep learning-based classification attempts in the literature solely focus on the aim of extreme accuracy scores, without considering interpretability, or patient-wise separation of training and test data. For example, most lung nodule classification papers using deep learning randomly shuffle data and split it into training, validation, and test sets, causing certain images from the Computed Tomography (CT) scan of a person to be in the training set, while other images of the same person to be in the validation or testing image sets. This can result in reporting misleading accuracy rates and the learning of irrelevant features, ultimately reducing the real-life usability of these models. When the deep neural networks trained on the traditional, unfair data shuffling method are challenged with new patient images, it is observed that the trained models perform poorly. In contrast, deep neural networks trained with strict patient-level separation maintain their accuracy rates even when new patient images are tested. Heat map visualizations of the activations of the deep neural networks trained with strict patient-level separation indicate a higher degree of focus on the relevant nodules. We argue that the research question posed in the title has a positive answer only if the deep neural networks are trained with images of patients that are strictly isolated from the validation and testing patient sets.

ABSTRACT

Eremurus spectabilis M. Bieb was extensively investigated experimentally and theoretically, including the antioxidant properties of compounds such as flavonoids, hydroxycinnamic acid derivatives, hydroxybenzoic acid derivatives, and organic acids. Antioxidant activity was investigated using the Density Functional Theory (DFT) method based on three known mechanisms: hydrogen atom transfer (HAT), single electron transfer followed by proton transfer (SET-PT), and sequential proton loss electron transfer (SPLET). Subcritical water extraction (SWE), soxhlet extraction (SE), and solvent extraction (SOE) techniques were applied in the extraction process. Malic acid was the major compound with an extract concentration of 38,532.84 ± 1849.58 µg analyte/kg, total phenolics, and free radical scavenging activity were 10.67 mg gallic acid/mL extract and 73.89% per mg/mL extract, respectively. P, Fe, Na, Mg, K, and Ca were the main elements. The antibacterial activity of E. spectabilis against seven bacteria was evaluated, and it was found to be higher than the commercial antibiotics P10 and AMC30.

ABSTRACT

This study, in which a hundred human teeth with single straight canal were used, focused on the evaluation of contrast solution penetration ability into the simulated lateral canals activated by XP-endo Finisher (XPF), EDDY, Nd:YAG, and Er:YAG laser systems with three different observation methods. The root canals were prepared up to X4 at working length using the ProTaper Next system. The teeth were decalcified and simulated lateral canals were created with #8 K-file at 2, 4, and 6 mm levels. Then the teeth were cleared using methyl salicylate and divided into 5 equal groups according to irrigation activation techniques (CSI, XPF, EDDY, Nd:YAG, and Er:YAG laser). After the contrast solution was activated, images of the simulated lateral canals were obtained by a dental microscope, digital radiography, and CBCT. The contrast solution penetration scores at the 2, 4, and 6 mm levels according to irrigation activation techniques and observation methods were analyzed statistically using Kruskal-Wallis non-parametric analysis of variance Bonferroni test post hoc comparisons. With the other 2 parameters are ignored, the highest and lowest contrast solution penetration were observed at 6 and 2 mm simulated lateral canal levels (p < 0.05), in Er:YAG irrigation activation and CSI technique (p < 0.05), and in direct visual and digital radiographic observation method (p < 0.05), respectively. Within the limits of this study, it was determined that the contrast solution penetration into the simulated lateral canals decreased from the coronal to the apical and achieved with the most effective Er:YAG laser activation technique. In addition, direct visual observation was found to be the best method for the assessment of contrast solution penetration.

ABSTRACT

Fungi can be used as a potent chemotherapeutic agent to treat various cancers. In current study acetone and methanol extracts of Terfezia claveryi, Terfezia boudieri, Terfezia olbiensis, Picoa lefebvrei, Picoa juniperi were used to assess total phenolic contents, antioxidant activity, ion-chelating impact, antimicrobial activity, the cytotoxic and protective effects. Both methanol and acetone extracts of T. boudieri had the highest FRAP and DPPH scavenging abilities. Dose-dependent increased ion-chelating impact of all tested truffles species was found. Extracts of T. boudieri, T. claveryi, and T. albiensis exhibited higher antimicrobial activities. T. claveryi and T. boudieri showed the highest protective effects against H2O2-induced genotoxicity (P < 0.05), in S. cerevisiae BY4741. The least protective effect was showed by the acetone extracts of T. olbiensis (144 ± 8); methanol extracts of P. lefebvrei (140 ± 8) and P. juniperi (140 ± 10). MCF 7 cells showed more sensitivity against to methanol extracts of T. boudieri at 10-100 µg/mL concentrations. HepG2 cells showed more sensitivity against the methanolic extracts of T. boudieri at both doses. Overall, P. lefebvrei and P. juniperi extracts had the least cytotoxic effects. The species of Terfezia exhibit significant protective effects against DNA damage and also have the potential of cytotoxicity effects.

ABSTRACT

This study investigated the effect of taper and apical preparation size on the fracture resistance of roots. 84 mandibular incisor teeth were selected. Groups (n = 12): uninstrumented (control group), 25/0.04, 25/0.06, 25/0.08, 30/0.04, 30/0.06 and 30/0.08. K3XF files (Kerr Endodontics, Orange, CA) were used for canal preparation. Fracture resistance was tested using a Universal test machine. For statistical analysis, the level of significance was P ≤ 0.05, and one-way anova and post hoc LSD tests were used. The control group showed the highest fracture resistance (P < 0.05). No significant difference was found between 25/0.04 and 25/0.06; 25/0.06 and 25/0.08; 30/0.04 and 30/0.06; 30/0.06 and 30/0.08; 25/0.04 and 30/0.04; 25/0.06 and 30/0.06; and 25/0.08 and 30/0.08 (P > 0.05). Significant differences were found between 25/0.04 and 25/0.08; 30/0.04 and 30/0.08; and 25/0.08 and 30/0.04 (P < 0.05). In conclusion, the clinical selection of larger tapers can cause a higher risk of fracture.

PURPOSE: To evaluate the effectiveness of cabergoline and the parameters affecting cabergoline response as add-on treatment to somatostatin analaogues (SSA) in patients with acromegaly uncontrolled with SSAs. MATERIAL AND METHOD: One hundred and twenty-nine acromegalic patients uncontrolled with SSA who had cabergoline added to their treatment were included in this retrospective study. Patients were divided into the SSAs + cabergoline-responsive (group 1) and non-responsive groups (group 2), and biochemical, pathologic, and radiologic parameters were assessed. RESULTS: IGF-1 normalization was achieved in 75 of 129 patients (58%) when cabergoline was added to the SSA treatment. Female patients were significantly higher in group 1 compared to group 2 (p=0.006). Group 1 had significantly smaller pre- and post-cabergoline tumor size (p=0.011, p=0.007 respectively), lower levels of IGF-1 in pre-and post-operative period (p=0.040, p=0.001), and lower levels of IGF-1 in pre- and post-cabergoline treatment (p<0.001). Cavernous invasion on sellar magnetic resonance imaging, dural invasion in pathologic examination were not significantly different between the groups. Sellar invasion in pathologic examination was significantly higher in group 1 (p=0.011). No significant difference was found in proliferation indices between two groups. The presence of fibrous bodies was significantly lower in group 1 (p=0.010). CONCLUSION: Cabergoline can be added to the treatment of acromegalic patients uncontrolled with SSAs due to its ease of use and low economic cost, especially in patients with acromegaly who have small adenomas and no fibrous bodies.

ABSTRACT

Objective: The aim of this study was to compare apical debris extrusion when neodymium-doped yttrium aluminium garnet (Nd:YAG) lasers, erbium-doped yttrium aluminium garnet (Er:YAG) or photon-induced photoacoustic streaming (PIPS) are used for irrigation activation.Materials and methods: A total of 60 extracted human mandibular premolar teeth have similar dimensions were included and the samples were split into four groups according to the irrigation technique (n = 15): conventional needle irrigation, PIPS, Er:YAG and Nd:YAG. ProTaper Universal system up to F4 was used for root canal instrumentation. Bidistilled water was used as an irrigation solution during instrumentation and irrigation activation. Apically extruded debris was collected into preweighed Eppendorf tubes during instrumentation and irrigation activation procedures. The tubes were then kept in an incubator at 70 °C for 5 days. The initial weight of the tube was subtracted from the final weight and the result was recorded as the weight of dry extruded debris. The data were evaluated statistically using a one-way ANOVA test followed by least significant difference post hoc test (p < .05).Results: Conventional needle irrigation caused significantly less debris extrusion than laser-assisted irrigation activation groups (p < .05). Laser-assisted irrigation activation groups caused statistically similar debris extrusion (p > .05).Conclusion: Laser-assisted irrigation activation techniques caused more debris extrusion when compared to conventional needle irrigation.

ABSTRACT

The quality of grape cultivars not only depends on the grape cultivar but also is influenced by the molecular concepts and agro-climatic factors. For this purpose, four different grape cultivars were collected from five different locations in Siirt province (Turkey). Totally twenty different grape cultivars were investigated. In the present study, the antioxidant activity (total phenolic, flavonoid, proanthocyanidin content, DPPH and FRAP activity) in seeds were indicated and phylogenetic analysis (cpDNA;trnL-F region) of twenty native grape cultivars were investigated to construct their phylogenetic tree. According to reported data on antioxidant activity and content of phytochemicals, all cultivars exhibited different values from each other, but Rutik and Gadüv cultivars were found as significantly higher in comparison to others. According to bioinformatics analysis, twenty grape cultivars were distributed into six different major groups. Rutik and Sevkeye cultivars exhibit significant distinction from other grape cultivars. The phylogenetic analysis was also associated and supported with the results of obtained data from bioactivity. The bioactivity and phylogenetic analysis were firstly identified and quantified in these grape cultivars, however, with regard to obtained data from the current study, the grape cultivars grown in Siirt province were indicated significant and valuable results and as a result, these cultivars have to be evaluated before extinction.

ABSTRACT

OBJECTIVE: More cardiovascular events are seen in nephrotic syndrome (NS) patients than in the normal population. Fragmented QRS (fQRS) includes various RSR' patterns with different morphologies of the QRS complexes with or without the Q wave on a resting 12-lead ECG. A relationship between the presence of fQRS and myocardial function impairment has been shown in several studies. The purpose of this study was to evaluate the presence of fQRS in this patient group and the relationship with myocardial functions. METHODS: Thirty-four NS patients were included in the cross-sectional study. Demographic data were recorded, and electrocardiograms (ECGs) were analyzed for the presence of fQRS following investigation of biochemical parameters and 24-h protein excretion. In addition to classic echocardiographic parameters, the myocardial performance (Tei index) was calculated as an indicator of cardiac function. For comparison of group's data Student's t-test and Mann-Whitney U test were used. Multiple linear regression analysis was performed for parameters affecting presence of fQRS. RESULTS: We identified fQRS in half of our patients group. Patients with fQRS had significantly greater proteinuria level and Tei index than those without (p<0.05 and p<0.001, respectively). Tei index was also affected presence of fQRS (p<0.05, ß=0.45, R2=0.32) and positively correlated with proteinuria levels (p<0.05 r=0.366). CONCLUSION: We demonstrated, for the first time in the literature, that the determination of fQRS in patients with NS in surface ECG, an easily accessible technique, can be used as a parameter in the prediction of myocardial functions.

BACKGROUND. Ganglioneuromas (GNs) are neural crest cell-derived tumors and rarely occur in the adrenal gland. They are usually asymptomatic and hormonally silent. The majority of cases are detected incidentally during work-up for unrelated conditions. Hormone-secreting pure adrenal GNs in adults are extremely rare. To date, only four cases have been reported in the English literature. CASE REPORT. We describe an adult case of endocrinologically active adrenal GN incidentally diagnosed in a 64-year-old male patient with history of uncontrolled hypertension. On physical examination, he had a blood pressure (BP) of 160/100 mmHg. Abdominal computed tomography and magnetic resonance imaging showed a large solid tumor (8.5 × 7.5 × 7 cm) in the right adrenal gland. Urinary levels of norepinephrine, normetanephrine, vanillylmandelic acid and dopamin were elevated, although urinary level of epinephrine was suppressed. Right adrenalectomy was performed for treatment purposes. The histological diagnosis of the resected tumor was adrenal GN. CONCLUSIONS. Hormone-secreting pure adrenal GN occurs very rarely in adults and preoperative diagnosis is difficult. Adrenal GN may present with hormonal activity such as increased secretion of catecholamines and their metabolites. There are no specific diagnostic signs and symptoms discriminating GN and pheochromocytoma. Therefore, histopathological examination need for a definitive diagnosis of adrenal GN. The prognosis after completed surgical resection without further therapy seems to be excellent. To our knowledge, the present case is the second report that describes hormone-secreting pure adrenal GN in an adult from Turkey in the English literature. We discuss this case and review the literature on this unusual entity.
