ABSTRACT

Objective: To investigate the effects of maxillary orthodontic expansion on the alveolar bone tissue in adult patients treated with aligners by using cone-beam computed tomography. Methods: Thirty patients (22 females and 8 males; mean age: 36.3 years) were treated with Invisalign® aligners. Cone-beam computed tomography and digital models were obtained before (T0) and after (T1) upper arch expansion. The bone thicknesses in the cervical, middle, and apical areas of the incisors, canines, premolar, and first molars were buccally and palatally measured, totaling 96 areas and 2,880 measurements. The buccolingual inclinations and transverse measurements of the teeth were obtained from digital models to correlate them with the bone changes. The statistical tests used were Student's t-test, analysis of variance, and Pearson's correlation tests (p < 0.05). Results: From the 96 areas evaluated, 84 revealed an increase or stability in the alveolar bone thickness and twelve displayed significant bone loss. Bone changes did not correlate with the tooth inclination and transverse measurements. Conclusions: Within the limitation of the present study, mild levels of upper arch expansion obtained with Invisalign® aligners in adult patients did not result in any clinically significant loss of alveolar bone thickness.

ABSTRACT

In this report, we demonstrate the effectiveness of the InvisalignⓇ system in the treatment of severe gingival recession and bone dehiscence through torque, translation, and intrusion movements in a young woman. Cone-beam computed tomography was used to assess bone parameters and check the teeth during treatment. The root of the mandibular right central incisor, which was buccally positioned and exhibited bone dehiscence of 9.4 mm, was moved toward the center of the alveolar process by using the InvisalignⓇ system and SmartForceⓇ features. The patient was monitored by a periodontist throughout the orthodontic treatment period. Her gingival recession reduced, while the bone dehiscence reduced from 9.40 mm to 3.14 mm. Thus, movement of the root into the alveolus promoted bone neoformation and treated the gingival recession. The findings from this case suggest that orthodontic treatment using the InvisalignⓇ system, along with periodontal monitoring, can aid in the treatment of gingival recession and alveolar defects.

ABSTRACT

Objective: Traumatic spinal cord injury is one of the leading causes of disability worldwide, mainly related to automobile accidents. It was decided to establish a clinical-epidemiological profile of spinal cord injury (SCI) in Palmas, Brazil. Method: Prospective study at the Public General Hospital of Palmas (HGPP), including all patients admitted in this hospital from January 2011 to February 2012. Results: They refer to 59 patients, of whom 81.4% were men and 47.4% of the victims had up to 40 years. The main mechanism of injury was motor vehicle accidents involving motorcycles. Clinically, the vertebral segment C4-C7 was the most damaged, and most patients were admitted with spinal injury and no neurological deficit (Frankel E). Conclusions: Primary involvement of young men in working age, trauma mainly due to car accidents that are usually associated with the use of alcoholic beverages. Patients admitted with more severe neurological deficits have evolved to milder impairments after conservative or surgical treatment.


Objetivo: A lesão traumática da medula espinhal é uma das causas mais relevantes de incapacidade no mundo, relacionada principalmente aos acidentes automobilísticos. Decidiu-se traçar o perfil clínico-epidemiológico do trauma raquimedular (TRM), em Palmas, Brasil. Método: Estudo prospectivo, realizado no Hospital Geral Público de Palmas (HGPP), incluindo todos os pacientes admitidos nesse hospital, no período de janeiro de 2011 a fevereiro de 2012. Resultados: São referentes a 59 pacientes, dos quais 81,4% eram homens e 47,4% das vítimas tinham até 40 anos. O principal mecanismo de trauma foram os acidentes automobilísticos envolvendo motocicletas. Clinicamente, o segmento vertebral de C4-C7 foi o mais lesado, sendo que a maioria dos pacientes foi admitida com lesão vertebral sem déficit neurológico (Frankel E). Conclusões: Acometimento primordial de homens jovens na faixa etária laboral, com trauma principalmente devido a acidentes automobilísticos que geralmente estão associados ao uso de bebidas alcoólicas. Pacientes admitidos com déficits neurológicos mais severos evoluíram para comprometimentos mais brandos após o tratamento conservador ou cirúrgico.

Se decidió establecer un perfil clínico-epidemiológico de las lesiones de la médula espinal (LME) en Palmas, Brazil. Método: Estudio prospectivo en el Hospital Público General de Palmas (HGPP), incluyendo todos los pacientes ingresados en este hospital desde enero de 2011 a febrero de 2012. Resultados: Se refieren a 59 pacientes, de los cuales el 81,4% eran hombres y el 47,4% de las víctimas tenían hasta 40 años. El principal mecanismo de trauma fueron los accidentes de tráfico con motocicletas. Clínicamente, el segmento vertebral C4-C7 fue el más dañado, y la mayoría de los pacientes con lesión de la médula fue admitida sin déficit neurológico (Frankel E). Conclusiones: Implicación primaria de hombres jóvenes en edad laboral, trauma debido principalmente a los accidentes de tráfico que se asocian generalmente con el uso de bebidas alcohólicas. Los pacientes ingresados con déficit neurológicos más graves han evolucionado para deterioros leves después del tratamiento conservador o quirúrgico.

ABSTRACT

INTRODUÇÃO: dentro da ciência Bioprogressiva, o arco utilidade, juntamente com o quadri-hélice, é o dispositivo mais conhecido e utilizado para o tratamento de inúmeras más oclusões. Com um desenho ao mesmo tempo simples e original, o arco utilidade tornou-se parte integrante do arsenal ortodôntico utilizado nos períodos das dentaduras mista e permanente. O nome do arco é uma referência à grande versatilidade que este dispositivo oferece, e devido aos vários efeitos que o mesmo pode promover durante o tratamento. OBJETIVO: este artigo destina-se a discutir a fabricação, a ativação e o uso clínico do arco utilidade de Ricketts no tratamento ortodôntico, além de descrever alguns dos princípios da técnica ortodôntica Bioprogressiva. CONCLUSÃO: pode-se concluir que o arco utilidade é um aparelho extremamente versátil, com inúmeras possibilidades clínicas, sendo capaz de intruir, extruir, avançar ou retrair os incisivos superiores e inferiores; manter, diminuir ou aumentar o espaço no arco inferior; além de ancorar os molares inferiores. Entretanto, uma das maiores vantagens de seu uso consiste na correção de más oclusões do tipo Classe II, com mordida profunda.


INTRODUCTION: The utility archwire, along with the quad-helix, is the most widely known and used device within the Bioprogressive technique for the treatment of several malocclusions. Featuring a simple yet original design, the utility archwire has become an integral part of the orthodontic arsenal, with applications in the periods of mixed and permanent dentition. The name of this archwire refers to its great versatility, and to the multiple effects it can have during treatment. OBJECTIVE: This article aims to discuss the production, application and clinical use of the Ricketts utility archwire in orthodontic treatments, as well as to describe some of the basics aspects of the Bioprogressive orthodontic technique. CONCLUSION: It can be concluded that the utility archwire is an extremely versatile appliance with innumerable clinical applications - able to intrude, extrude, advance or retract maxillary or mandibular incisors; maintain, diminish or increase space in the lower arch, in addition to anchoring mandibular molars. Nevertheless, one of the most significant advantages of its use is in the correction of Class II malocclusions, with overbite.
