ABSTRACT

Abstract Introduction: The potential for emerging infectious diseases is a major global health problem. Among the viruses that cause emerging diseases are coronaviruses. Epidemiological and etiological investigations detected in 2019 a new coronavirus, which dramatically increased the workload in health facilities. Aim: To identify constructs that trigger Burnout syndrome in nurses at the front line of COVID-19. Method: Consolidated integrative review using five health databases, using two search strategies: (Coronavirus OR "COVID- 19" OR "SARV-COV-2") AND ("Professional Exhaustion" OR "Professional Exhaustion") AND ( Nursing OR "Nurses") and ("Coronavirus Infections" OR "COVID- 19" OR coronavirus OR "SARV-COV-2") AND ("Burnout, Professional" OR "Occupational Burnout") AND (Nursing OR Nurse) for PUBMED , Web of Science and Scopus. Rayyan was used for screening and selection of studies. Results: Nineteen articles were included, with health professionals at the forefront of care for people with COVID-19, in 14 health institutions in different geographic locations. 10 constructs related to the three dimensions of burnout were identified in nurses in the front line in assisting COVID-19 There was a greater identification of constructs in the dimension of emotional exhaustion, with a predominance of emotional exhaustion, occupational stress, fear, anxiety and overload occupational. Conclusions: The constructs that trigger the burnout syndrome in nursing professionals working on the front line to COVID-19 are occupational stress, emotional exhaustion, fear, anxiety, anguish, occupational overload, post-traumatic stress, changes in personality, family problems and reduction professional satisfaction.


Resumo Introdução: O potencial para doenças infecciosas emergentes é um grande problema de saúde global. Entre os vírus que causam doenças emergentes estão os coronavírus. Investigações epidemiológicas e etiológicas detectaram em 2019 um novo coronavírus, que aumentou drasticamente a carga de trabalho nas unidades de saúde. Objetivo: Identificar construtos que desencadeiam a síndrome de Burnout em enfermeiros na linha de frente da COVID-19. Método: Revisão integrativa consolidada por meio de cinco bases de dados de saúde, utilizando duas estratégias de buscas: (Coronavirus OR "COVID- 19" OR "SARV-COV-2") AND ("Esgotamento Profissional" OR "Exaustão Profissional") AND (Enfermagem OR "Enfermeiros") e ("Coronavirus Infections" OR "COVID- 19" OR coronavirus OR "SARV-COV-2") AND ("Burnout, Professional" OR "Occupational Burnout") AND (Nursing OR Nurse). Utilizou-se o Rayyan para triagem e seleção dos estudos. Resultados: Foram incluídos 19 artigos, com profissionais de saúde na linha de frente de cuidados às pessoas com COVID-19, em 14 instituições de saúde de diferentes localizações geográficas. Identificou-se 10 construtos relacionados às três dimensões de burnout em enfermeiros na linha de frente na assistência à COVID-19. Houve maior identificação de construtos na dimensão de exaustão emocional, com predomínio de exaustão/esgotamento emocional, estresse ocupacional, medo, ansiedade e sobrecarga ocupacional. Conclusões: Os constructos desencadeadores da síndrome de burnout nos profissionais de enfermagem atuantes na linha de frente da COVID-19 são o estresse ocupacional, esgotamento emocional, medo, ansiedade, angústia, sobrecarga ocupacional, estresse pós-traumático, mudanças na personalidade, problemas familiares e redução da satisfação profissional.

Rayyan se utilizó para el cribado y la selección de estudios. Resultados: Se incluyeron 19 artículos, con profesionales de la salud a la vanguardia de la atención de personas con COVID-19, en 14 instituciones de salud de diferentes ubicaciones geográficas. Se identificaron 10 constructos relacionados con las tres dimensiones del burnout en enfermeras en primera línea de atención al COVID-19. Hubo una mayor identificación de constructos en la dimensión de agotamiento emocional, con predominio de agotamiento emocional, estrés ocupacional, miedo, ansiedad y sobrecarga ocupacional. Conclusiones: Los constructos que desencadenan el síndrome de burnout en los profesionales de enfermería que trabajan frente al COVID-19 son el estrés ocupacional, el agotamiento emocional, el miedo, la ansiedad, la angustia, la sobrecarga ocupacional, el estrés postraumático, los cambios de personalidad, los problemas familiares y la reducción de la satisfacción laboral.

ABSTRACT

OBJECTIVE: To present a low-cost prototype for a barrier enclosure device which can be used during open surgeries such as tracheotomy. METHODS: We provide detailed description of a novel device called COVID-Box, developed by The Surgical Airway Team for COVID-19 Pandemic, a temporary task force formed by Walter Cantídio University Hospital. Safety guidelines for performing tracheotomies in COVID-19 patients are also stated. RESULTS: Our prototype device provides greater hand mobility than previous barrier devices reported, making it more suitable for airway surgical procedures, such as tracheotomy. The curved shaped format and the customizable access ports provides ergonomics, without compromising safety. CONCLUSION: The COVID-Box appears to be an efficient, reproduceable, low-cost barrier enclosure device that can be used for open tracheotomies in ICU patients during the COVID-19 pandemic.

ABSTRACT

OBJECTIVE: To evaluate effects of an educative intervention over the self-monitoring of blood glucose (SMBG) at home for metabolic control. METHOD: A total of 91 people with diabetes participated, recruited from the home capillary blood glycemia self-monitoring program. Two groups of participants were formed: one group participated in the SMBG program at home and with usual care (control group), while the other group participated in the SMBG at home and with educative intervention (intervention group). In total there were 12 meetings, three for each conversation map in the control of diabetes, during four months in 2011 and 2012. For all the analysis, a significance statistical level of 5% (p ≤ 0.05) was adopted. RESULTS: Most part of participants were females, married, with an average age of 62.1 years old and schooling from four to seven years of study. In the intervention group, an improvement was observed in the following measures: body mass index, abdominal circumference, diastolic blood pressure, fasting plasma glucose, HDL cholesterol, LDL cholesterol and triglycerides. The control group showed improvement in measures of systolic and diastolic blood pressure, fasting plasma glucose, HDL cholesterol and LDL cholesterol. Moreover, an increase on the values of glycated hemoglobin was observed in both groups. CONCLUSION: It was observed that there was not statistical significant improvement of the metabolic control. However, it was possible to confirm that an educative intervention for SMBG at home presented a clinical significance, which in turn, resonates in a special way on the health of participants.
