ABSTRACT

In the article the results of clinical researches of efficiency of preparation of Cocarnit are resulted for patients after endoprosthesis of large joints. It is routine that for patients, receiving preparation of Cocarnit after the operation period there was a decline in the amount of complaints of patients on the total somatical state. Preparation of Ccocarnit was positively estimated outside patients, meaningful by-reactions, serving reason of abolition of preparation, was not marked. At the reception preparation of Cocarnit greater part of investigational laboratory indexes (table of contents of glucose, ß-lipoproteines, total chondroitisulfates, TBC-productes (malonic dyaldehyde), activity of aspartataminotransferase, alkaline phosphatase and ß-glutamyltranspeptidase), the indexes of clinical blood test and leucocytar indexes during a supervision did not have reliable differences from such as the persons of the control group, that confirms good bearab leness of the indicated preparation. Application preparation of Cocarnit for patients in composition the chart of treatment of patients after endoprosthesis of large joints brought maintenance over of cholesterol to the decline, glycoproteins, TBC-products (malonic dyaldehyde), activity of alaninaminotransferase, that specifies on normalizing influence of the indicated preparation in relation to the basic types of exchange of matters.

ABSTRACT

The risk factors of venous thromboembolism (VTE) in 1146 patients who were underwent operated for the diseases of digestive organs, for obliterating atherosclerosis of abdominal aorta and peripheral arteries, diseases of bones and joints were studied. The methods of VTE prevention in these patients were studied. To evaluate the risk of VTE we designed the special chart. Three levels of risk of VTE: low, moderate and high were distinguished. The specific prophylaxis of the VTE using low molecular weight heparin fraxiparine was given to the patients moderate (90.7%) and high (93.3%) risk degree.

ABSTRACT

The use of vertebrectomy in 9 patients with long-standing complicated dislocation fractures of vertebrae and defects of tissues of the spinal cord from 1 up to 12 cm has shown that the operation of removal of up to 3 vertebrae is technically quite realizable. Clinical results however are yet minimum, which requires further search for methods of stimulation of regenerative processes in the zone of connection of the spinal cord stumps. The use of the method for treatment of 4 patients with tumors of the vertebrae has revealed a number of problems requiring a somewhat different approach both to the performance of the operation and to the following fixation of the backbone. Possible variants to settle the problems are proposed. Good prospects of this trend are noted.

ABSTRACT

On the basis of experimental and clinical investigations the authors consider the removal of the vertebra body as the whole block to be the optimal variant. Shortening of the vertebral column is permissible for the approximation of ends of the spinal cord or its roots for suturing without intention. The use of vertebrectomy in 9 patients with long-standing dislocation fractures and defects of the spinal cord tissues from 1 to 12 cm and in 4 patient without tumors of the vertebrae has shown that the operation with removal of as many as 3 vertebrae is quite realizable technically. However, positive clinical results are minimal for the present which requires further researches by stimulation of regenerative processes.

ABSTRACT

The article includes the description of successful case of surgical treatment of a patient (53 years old) with VC plasmocytoma, complicated with upper paraparesis, tetanoid paraplegia and pelvic disturbances. The patient has been subjected to complete removal of VC in one stage of 2 accesses. The patient's spine has been fixed by means of implant, made of corundum porous ceramic, metal plate and lavsan band. A year after the operation--no tumor recurrence, the patient walks without additional support. In the process of discussion of the given observation the authors suggest that the technical difficulties of the operation are quite surmountable and its expediency is obvious.

ABSTRACT

In the experiment with 22 dogs the authors have studied the effect of the spine shortening on the undamaged and damaged spinal cord. Observation period ranged from 1 week to 6 months. Partial resection of the vertebral body without spine shortening, practically, have not affected the spinal cord of an animal. Spine shortening without the spine cord transection is accompanied by the spinal cord crumpling and partial damaging. Combination of this operation with submembrane transection of the spinal cord permits to avoid an obvious scar formation but results in considerable cyst-formation that prevents regenerative process in the spinal cord. Further studies should be conducted in terms of the cyst-formation process prevention.
