ABSTRACT

During a programmed atrial stimulation in 80% of the cases disturbances of the stimulus conduction could be produced distally to the atrioventricular node. In the horizontal plane of the vector cardiogram two different configurations appeared. In 45 examinations an anterior displacement and in 36 examinations a posterior displacement of the vector loop could be evoked. Issuing from this, in healthy persons in 44 cases vector cardiograms with an emphasized anterior orientation (AD) and 21 cases with an opposite configuration of an emphasized posterior orientation (PD) were compiled. The criteria for the two up to now little or not described disturbances of the stimulus conduction are presented. It is referred to the extension of the principles of the subdivision of the disturbances of the stimulus conduction.

ABSTRACT

A pronounced anterior displacement in the VCG which was not evoked by a posterior infarction or a right-ventricular hypertrophy has its cause in a circumscribed disturbance of the conduction of the septal or medial fascicle of the left Tawara-branch of the conduction system of the heart. On the basis of personal observations of this new defined disturbance of the conduction system the parameters for the recognition in the 12 conventional leads of the ECG are demonstrated. Such a pronounced anterior displacement may be supposed in Rv2 greater than 1 m V and Rv2/Sv2 greater than or equal to 1.2. The analysis of an additional 57 VCG with left anterior hemiblock yields in 6 the additional proof of an amphysized anterior orientation. This shows that the group of the left-anterior hemiblocks is not unique and pleads for the enlargement of Rosenbaum's conception of the conduction system in the sense of a functional division into three parts of the left Tawara-branch.

ABSTRACT

The usability of the continuous ultrasonic Doppler-method for the measurement of the blood flow in the haemodynamic animal experiment was tested. For this purpose measurements at the ascending aorta and the commune carotid artery were performed on different current conditions and compared with the electromagnetically registered blood flow. The causes for the non-conformity are discussed.

ABSTRACT

On the basis of an own observation the rare clinical picture of an external otitis with pareses of the brain nerves is casuistically described. Up to now it was observed only in elder persons with diabetes mellitus. The pathogenic agent is always Pseudomonas aeruginosa. The inflammatory process, issuing from the external auditory passage and under circumvention of the tympanum, spreads to the skull base and according to the localisation causes adequate pareses of the brain nerves. Early operation and aimed antibiotic therapy is the therapy of choice.

ABSTRACT

The comparative measuring of blood pressure on the arm and foot artery by means of the ultrasound technique after Doppler is a method for the diagnostics of the disturbance of the peripheral arterial blood supply relevant to practice. Its advantages are in the short time of examination, the simple manipulation, the little technical expenditure and the providing of clear measuring data. The course of examination used by us is explained, the normal and borderline values got are reported. Patients with haemodynamically effective disturbances of the arterial blood supply of the vessels of the leg have on the malleolar artery a by more than 10 Torr lower blood pressure than on the brachial artery. For recognition of early changes we propose a simple loading test in form of a position test. The comparative ultrasound measuring of the blood pressure after Doppler on the upper and lower extremity is suitable for the observation of the course and should stand in the first place of the functional diagnostics of peripheral angioorganopathies.

ABSTRACT

It is reported on experiences with the demonstration of twodimensial cut pictures of the heart by means of the ultrasound device "Vidoson". To receive temporarily exactly defined picture, a trigger mechanism for releasing the camera dependent on the R-lead of the ECG is described as supplementation for the device. The advantages and possibilities of such defined cut pictures of the heart are shown.

ABSTRACT

Functional cardiovascular disturbances are very often in daily practice. Making of the diagnosis is possible by the exclusion of organic heart diseases, the behaviour and the special personal structure of the patient. Functional cardiovascular disturbances may manifest themselves as disturbances of the cardiac rhythm, circulatory dysregulations and so-called heart-aches, in which cases most different variations are possible. They coincide isolately, but also additionally with organic heart diseases, which emphasizes the necessity of thorough examinations. In detail the author enters into heart-ache and its differentiation in concomitant unspecific changes of the final part in the ECG.

ABSTRACT

On the basis of 6 own cases, 5 of which were observed during one year, the clinical picture, the diagnostics and the therapy of the arteriovenous pulmonary fistula are described. Of decisive diagnostic importance is the angiography of the pulmonary artery which should always be used for the clarification of unclear pulmonary foci as well as clinically unclear cyanosis. For the removal of this shunt connection in the lesser circulatory system therapeutically only the operation comes into question. On account of the threatening complications and the inclination to the dilation of the fistula with growing age it is indicated also in cases with only slight haemodynamic effect. Dependent on the anatomical position of the fistula the ligature of the afferent artery is recommended.
