ABSTRACT

PIK3CA-related overgrowth spectrum (PROS) encompasses different clinical entities caused by somatic activating mutations in PIK3CA. Among PROS, CLOVES syndrome represents a severe phenotype with poor survival rate. We present the case of a 4-month-old girl with CLOVES syndrome successfully treated with alpelisib, a PIKC3A inhibitor.

ABSTRACT

Parotid fistulae are a very infrequent complication after ear reconstruction in microtia when the local conditions are favorable. We report 2 cases of salival fistulae after microtic ear reconstruction. Timing of the diagnosis is important to decide the treatment. We recommend conservative management by restricted citric diet in early postoperative salival fistulae and Botox injections in the case that it persists.

ABSTRACT

The use of a local island flap, designed with retroauricular skin, is an excellent, simple, and reproducible innovation to the classic microtia ear reconstruction, allowing adequate coverage of the cartilage framework, specifically of the conchal area, sparing tense skin closure, reducing skin suffering and necrosis, with a better aesthetic result achieving the naturally depressed concha aspect. The flap design and surgical technique is described. This flap was used in 13 consecutive microtia ear reconstructions in our unit, no complications were encountered, and aesthetic results were excellent.

ABSTRACT

El lupus eritematoso sistémico corresponde a una enfermedad autoinmune multisistémica, que puede ser exacerbada por el embarazo, por lo que la gestación en una mujer lúpica se considera de alto riesgo. El objetivo de este trabajo fue revisar la literatura nacional e internacional acerca de lupus eritematoso neonatal (LEN), para lo cual se realizó una búsqueda bibliográfica en las bases de datos Medline y Lilacs, siendo los resultados más relevantes de dicha búsqueda los siguientes: LEN corresponde a una enfermedad poco frecuente, modelo de autoinmunidad pasiva, que se presenta en aproximadamente un 5 por ciento de los hijos de madres lúpicas. Sus principales manifestaciones son la dermatitis lúpica y el bloqueo AV congénito, en relación con el traspaso a través de la placenta de los anticuerpos con componentes SSA/Ro-SSB/La. Se recomienda que en embarazadas lúpicas se realice una estrecha vivilancia de la unidad fetoplacentaria, con detección de niveles de anticuerpos plasmáticos, ecocardiografía fetal seriada para monitorizar función cardíaca y coordinación con neonatólogo para eventual instalación de marcapasos a recién nacido. No hay evidencias acerca de la contraindicación absoluta de lactancia materna en mujeres lúpicas
