ABSTRACT

OBJECTIVE: The study aimed to establish recommendations and quality criteria to enhance the healthcare process of PBC. PATIENTS AND METHODS: It was conducted using qualitative techniques, preceded by a literature review. A consensus conference involving five specialists in the field was held, followed by a Delphi process developed in two waves, in which 30 specialist physicians in family and community medicine, digestive system and internal medicine were invited to participate. RESULTS: Seven recommendations and 15 sets of quality criteria, indicators and standards were obtained. Those with the highest consensus were «Know the impact on the patient's quality of life. Consider their point of view and agree on recommendations and care¼ and «Evaluate possible fibrosis at the time of diagnosis and during PBC follow-up, assessing the evolution of factors associated with poor disease prognosis: noninvasive fibrosis (elastography > 2.1 kPa/year), GGT, ALP and bilirubin annually¼, respectively. CONCLUSIONS: The implementation of the consensus recommendations and criteria would provide better patient care. The need for multidisciplinary follow-up and an increased role of primary care is emphasized.

Introducción: La colaboración entre Atención Primaria (AP) y Gastroenterología en la enfermedad inflamatoria intestinal (EII) es crucial para ofrecer una atención médica de alta calidad. El objetivo del presente estudio es analizar, en el ámbito nacional, la relación entre AP y los gastroenterólogos con el objetivo de conocer los aspectos de mejora en el manejo de pacientes con EII y cómo poder implementarlos, para así desarrollar posteriormente propuestas y proyectos concretos entre SEMERGEN y GETECCU. Metodología: Estudio descriptivo, observacional y transversal, realizado mediante cuestionario online anónimo entre octubre de 2021 y marzo de 2022. Resultados: Se recopilaron un total de 157 encuestas de Gastroenterología y 222 de AP. El 43,8% y el 34,3% de los gastroenterólogos y médicos de familia, respectivamente, consideraron tener una buena relación. El grado de conocimiento de los médicos de familia sobre diferentes aspectos de la EII (puntuado sobre 10) fue: aspectos clínicos 6,67±1,48, diagnósticos 6,47±1,46, terapéuticos 5,63±1,51, de seguimiento 5,53±1,68 y complicaciones 6,05±1,54. La percepción del apoyo y soporte entre ambas unidades no superó el 4,5 en una escala de 0 a 10 en ninguna de las encuestas. Las propuestas de mejora más votadas fueron: mayor coordinación entre especialistas, implantación de unidades de EII y colaboración de enfermería. Conclusión: Existen deficiencias en la relación entre AP y Gastroenterología con especial dedicación a la EII que requiere de los esfuerzos de las sociedades científicas que los representan para una mayor coordinación, con elaboración de protocolos conjuntos y medios de comunicación ágiles, rápidos y efectivos. The level of knowledge from family practitioners regarding different aspects of IBD out of 10 was: clinical 6.67±1.48, diagnosis 6.47±1.46, treatment 5.63±1.51, follow-up 5.53±1.68 and complications 6.05±1.54. The perception of support between both units did not exceed 4.5 on a scale from 0 to 10 in either of the surveys. The most highly voted improvement proposals were better coordination between the units, implementation of IBD units, and nursing collaboration. Conclusion: There are deficiencies in the relationship between PC and Gastroenterology with special dedication to IBD that require the efforts of the scientific societies that represent them for greater coordination with the development of joint protocols and agile, fast, and effective communication channels. ABSTRACT

OBJECTIVE: To investigate long-term efficiency of an intervention protocol for chronic daily headache with medication overuse (CDHwMO) in the general population. METHODS: The 72 subjects meeting CDHwMO criteria coming from an epidemiological study in the general population (Neurology 2004; 62: 1338-42) were offered follow-up and treatment for 1 year and then discharged to their general practitioner with treatment recommendations. Four years later, they were interviewed again. They filled in a diary for 1 month and the SF-12 test. RESULTS: After 1 year, 46 (64%) did not fulfill MO criteria while 26 (36%) did. After 4 years, 68 subjects were contacted. Of those, 38 (58%) did not have CDHwMO, while 30 (44%) still had MO. Among those 38 subjects without MO criteria, 6 still met CDH criteria. Remission at year 1 was a significant predictor for sustained remission at year 4. Age, gender, civil status, socioeconomic situation, and CDH type were not different in the group with MO vs those without MO. Consumption of nonsteroidal anti-inflammatory drugs and/or triptans was significantly higher in subjects without CDH and MO, while the use of ergotics and/or opioids was significantly higher in those patients who still met CDHwMO criteria. Quality of life (QoL) was significantly better at 4 years for the whole group. CONCLUSIONS: After 4 years, almost 60% of subjects did not fulfill CDHwMO criteria and their QoL was also improved. This justifies public health interventions that should include recommendations on a judicious use of symptomatic medications together with an early use of preventatives.
