ABSTRACT

Diesel-fueled buses have been replaced by Compressed Natural Gas (CNG) to minimize the high level of emissions in urban areas. However, differences in indoor exposure levels to Benzene, Toluene, Ethylbenzene and Xylene (BTEX) in those vehicles have not been investigated so far. The primary aim of this study was to determine if passengers are exposed to different BTEX levels when using buses powered by CNG or by diesel, and further explore if indoor levels are influenced by external air quality. For this purpose, BTEX air concentrations were measured in bus cabins (CNG and diesel), parking stations and in a background urban area using passive air samplers. Results showed that BTEX concentrations inside vehicles were higher than outside, but no significant differences were found between buses powered by CNG or by diesel. In CNG vehicles, high and significant positive correlation was found between benzene and the number of journeys in the same route (rs = 0.786, p < 0.05), vehicle operating time (rs = 0.738, p < 0.05), exposure time (rs = 0.714, p < 0.05) and exposure index (rs = 0.738, p < 0.05), but this was not observed for diesel vehicles. Benzene in bus cabins was found to be significantly below reference value for human health protection. However, excepting p-xylene, all other aromatic pollutants have a mean concentration significantly above the lowest effect level (p ≤ 0.002 for all comparisons). Additionally, higher BTEX levels in cabin buses than in outdoor air suggest the presence of other emission sources in indoor cabins. These findings emphasize the need for further studies to fully characterize indoor emission sources in order to minimize the negative impact of BTEX exposure to human health.

ABSTRACT

The volatile organic compounds benzene, toluene, ethylbenzene, and xylene (BTEX) are emitted into the atmosphere at gas stations (GS) leading to chronic exposure of nearby residents, which raises public health concerns. This study aimes at determining the contribution of GS emissions to BTEX exposure in nearby residents. Three Control and Exposed areas to BTEX emissions from GS were defined in a medium-sized European city (Porto, Portugal). BTEX atmospheric levels were determined in Control and Exposed areas using passive samplers deployed outdoors (n = 48) and indoors (n = 36), and human exposure was estimated for 119 non-smoking residents using the first urine of the day. Results showed that median BTEX outdoor and indoor concentrations were significantly higher for Exposed than Control areas, with exception of ethylbenzene and xylene indoor concentrations, where no marked differences were found. Comparison of urinary concentrations between Exposed and Control residents demonstrated no significant differences for benzene and ethylbenzene, whereas levels of toluene and xylene were significantly higher in Exposed residents. No marked correlation was obtained between atmospheric BTEX concentrations and urinary concentrations. Data indicate the potential impact on air quality of BTEX emissions from GS, which confirms the importance of these findings in urban planning in order to minimize the impact on health and well-being of surrounding populations.

ABSTRACT

Air quality traffic-related measures have been implemented worldwide to control the pollution levels of urban areas. Although some of those measures are claiming environmental improvements, few studies have checked their real impact. In fact, quantitative estimates are often focused on reducing emissions, rather than on evaluating the actual measures' effect on air quality. Even when air quality studies are conducted, results are frequently unclear. In order to properly assess the real impact on air quality of traffic-related measures, a statistical method is proposed. The method compares the pollutant concentration levels observed after the implementation of a measure with the concentration values of the previous year. Short- and long-term impact is assessed considering not only their influence on the average pollutant concentration, but also on its maximum level. To control the effect of the main confounding factors, only the days with similar environmental conditions are analysed. The changeability of the key meteorological variables that affect the transport and dispersion of the pollutant studied are used to identify and group the days categorized as similar. Resemblance of the pollutants' concentration of the previous day is also taken into account. The impact of the road traffic measures on the air pollutants' concentration is then checked for those similar days using specific statistical functions. To evaluate the proposed method, the impact on PM2.5 concentrations of two air quality traffic-related measures (M1 and M2) implemented in the city of Beijing are taken into consideration: M1 was implemented in 2009, restricting the circulation of yellow-labelled vehicles, while M2 was implemented in 2014, restricting the circulation of heavy-duty vehicles. To compare the results of each measure, a time-period when these measures were not applied is used as case-control.

ABSTRACT

(AU)

Porphyria is a rare disease, consequence of enzymatic deficiencies associated with heme group biosynthesis. The aim of this report is to describe a case of acute intermittent porphyria crisis in a 15-year-old patient who progressed rapidly with tetraplegia and chronic terminal renal failure, as well as severe neuropsychiatric disorders. It represents an entity of difficult diagnosis due to the lack of specificity of the clinical findings. At admission, creatinine was 6.44 mg / dL and urea was 103 mg / dL. Significant neurological deficit and chronic renal failure are rare complications, arising from late diagnosis and treatment. A greater degree of clinical suspicion is fundamental that treatment be started immediately after diagnosis, due not only to short-term morbidity but also to possible deleterious permanent effects (AU)

ABSTRACT

The fast economic growth of China along the last two decades has created a strong impact on the environment. The occurrence of heavy haze pollution days is the most visible effect. Although many researchers have studied such problem, a high number of spatio-temporal limitations in the recent studies were identified. From our best knowledge the long trends of PM2.5 concentrations were not fully investigated in China, in particular the year-to-year trends and the seasonal and daily cycles. Therefore, in this work the PM2.5 concentrations collected from automatic monitors from five urban sites located in megacities with different climatic zones in China were analysed: Beijing (40°N), Chengdu (31°N), Guangzhou (23°N), Shanghai (31°N) and Shenyang (43°N). For an inter-comparison a meta-analysis was carried out. An evaluation conducted since 1999 demonstrates that PM2.5 concentrations have been reduced until 2008, period which match with the occurrence of the Olympic Games. However, a seasonal analysis highlight that such decrease occurs mostly during warmer seasons than cold seasons. During winter PM2.5 concentrations are typically 1.3 to 2.7 higher than in summer. The average daily cycle shows that the lowest and highest PM2.5 concentrations often occurs in the afternoon and evening hours respectively. Such daily variations are mostly driven by the daily variation of the boundary layer depth and emissions. Although the PM2.5 levels have showing signs of improvement, even during the warming season the values are still too high in comparison with the annual environmental standards of China (35 µg m-3). Moreover, during cold seasons the north regions have values twice higher than this limit. Thus, to fulfil these standards the governmental mitigation measures need to be strongly reinforced in order to optimize the daily living energy consumption, primarily in the north regions of China and during the winter periods.

ABSTRACT

INTRODUCTION: Renal involvement is a severe form of schistosomiasis and occurs in 10% to 15% of patients with the hepatosplenic form of the disease. Nephrotic syndrome is the most common clinical presentation. It is a complication caused by immune complexes (IC), it is rare to appear in the Brazilian context with a immunoglobulin A (IgA) deposits. When installed the renal injury by Schistosoma mansoni, classically presents as membranoproliferative glomerulonephritis (mesangiocapillary) with lobular accentuation. OBJECTIVE: To report a case of schistosomiasis nephropathy that appeared 7 years after treatment of hepatosplenic schistosomiasis with histologic pattern of mesangial proliferative glomerulonephritis with IgA deposits in mesangium. Clinically developed with progressive decrease of proteinuria with angiotensin receptor blocker (ARB). METHOD: It was reported a case of a 36 years old patient, brown, with classical sintoms of nephrotic syndrome (proteinuria > 3.5 g/24h, hypoalbuminemia and hypercholesterolemia), however with hepatosplenic schistosomiasis history 7 years ago and portal hypertension. Patient underwent renal biopsy which showed IgA deposits in mesangial, being more intense than immunoglobulin G (IgG), accompanied by C1q and C3, with 4/13 glomeruli sclerotic, standard light mesangial glomerulonephritis renal injury with IgA deposits. Patient began taking ARB with progressive improvement in proteinuria. CONCLUSION: Patients with glomerulonephritis by schistosoma don't show improvement of disease progression with antiparasitic treatment. However the anti-proteinuric treatment can slow the progression of end stage kidney disease.

ABSTRACT

INTRODUCTION: The antineutrophil cytoplasmatic antibody (ANCA) is usually associated with pauci-immune crescentic glomerulonephritis (CrGN). However, the literature show an incidence unexpected high of ANCA in immunocomplex CrNP. The crescent IgA nephropaty is one of CrGN that associated with ANCAObjective:To relate an IgA nephropaty ANCA positive and sign of worse prognosis that improved with immunossupression. METHOD: 38-year-old pacient with arterial hypertension, renal impairment (CKD-EPI 37 ml/min/1,73 m2), non-nephrotic proteinuria and hematuria. He related occasionally epistaxis, rhnosinusitis and one arthritis episode that spontaneously resolved. During de investigation, the ANCA titles were 1/160 and anti-RP3 was positive, however renal biopsy showed IgA nephropaty with 38% of crescents. Regarding IgA nephropaty ANCA positive as the main diagnosis, immunossupression therapy with corticoids (1g IV methilprednisone for 3 days followed by 1 mg/kg/day prednisone) for 6 months and cyclophosphamide (500 mg initially then raise the dose until reach 750 m2. The patient improved renal function and reduced the proteinuria and ANCA titulation. CONCLUSION: The role of the association between IgA nephropathy and ANCA is it aggressive manifestation characterized by the presence of crescents, tubular atrophy and renal dysfunction, which may regress with early onset of immunosuppression treatment.

ABSTRACT

ABSTRACT

The accuracy and precision of air quality models are usually associated with the emission inventories. Thus, in order to assess if there are any improvements on air quality regional simulations using detailed methodology of road traffic emission estimation, a regional air quality modelling system was applied. For this purpose, a combination of top-down and bottom-up approaches was used to build an emission inventory. To estimate the road traffic emissions, the bottom-up approach was applied using an instantaneous emission model (Vehicle Specific Power - VSP methodology), and an average emission model (CORINAIR methodology), while for the remaining activity sectors the top-down approach was used. Weather Research and Forecasting (WRF) and Comprehensive Air quality (CAMx) models were selected to assess two emission scenarios: (i) scenario 1, which includes the emissions from the top-down approach; and (ii) scenario 2, which includes the emissions resulting from integration of top-down and bottom-up approaches. The results show higher emission values for PM10, NOx and HC, for scenario 1, and an inverse behaviour to CO. The highest differences between these scenarios were observed for PM10 and HC, about 55% and 75% higher (respectively for each pollutant) than emissions provided by scenario 2. This scenario gives better results for PM10, CO and O3. For NO2 concentrations better results were obtained with scenario 1. Thus, the results obtained suggest that with the combination of the top-down and bottom-up approaches to emission estimation several improvements in the air quality results can be achieved, mainly for PM10, CO and O3.

ABSTRACT

Benzene is a carcinogenic and genotoxic pollutant which mainly affects the people health through the inhalation. Nevertheless, this pollutant is not frequently measured by air-quality networks. To solve this problem, some models have been published to estimate benzene concentrations in the atmosphere. However, the lack of measures makes difficult the application of complex models in order to get a detailed spatio-temporal analysis, namely in urban areas. In this work was developed a simple semi-empirical model to predict benzene concentrations based on the ratio of benzene and carbon monoxide concentrations in order to predict the concentrations of this pollutant in large areas and periods with lack of benzene measurements but with higher impact in the human health. The model was applied to an urban area, the Metropolitan Area of Oporto, for a period of 12 years (1995-2006). Monthly correlations between benzene and carbon monoxide concentrations at Custóias air-quality station are significant (p = 0.01) and higher in winter (r(s) > 0.7) than in summer (0.3 > r(s) > 0.7). Estimate of the monthly ratio of the concentration of these two pollutants range between 199 and 305. The methodology validation shows good results (r(s) = 0.81) which allow, assuming the availability of carbon monoxide data, the use of this tool for areas with low benzene recorded data. The application of this methodology in the study area shows an annual average trend decrease of benzene concentrations during the study period, which may be linked to a general trend decrease of benzene emissions in European urban areas, including the study domain.
