ABSTRACT

BACKGROUND AND AIMS: Magnetic resonance imaging (MRI) with gadoxetic acid is widely used in clinical practice in Spain for the diagnosis, treatment, and follow-up of patients with liver metastases, although its use varies. This paper aims to provide recommendations for the use of MRI with gadoxetic acid in the detection and diagnosis of liver metastases in clinical practice in Spain. MATERIAL AND METHODS: This project was undertaken by a group of nine experts who analyzed a series of recommendations about the use of gadoxetic acid extracted from international consensus documents. From this analysis, the experts decided to reject, adopt, contextualize, or adapt each of the recommendations. Once established, the final recommendations were voted on by the same group of experts. RESULTS: The experts reached a consensus about five recommendations related to the use of this imaging technique in the management of liver metastases in three clinical situations: (i) in the detection, (ii) in the diagnosis and preoperative characterization, and (iii) in the detection after a chemotherapy treatment. CONCLUSION: The results support a clinical benefit for MRI with gadoxetic acid in the detection of liver metastases, favoring preoperative planning, especially in metastases measuring less than 1 cm, thus facilitating early diagnosis of metastatic spread.

Introducción: el aumento de peso depende de múltiples factores mediadores modificables, incluido el fenotipo del comportamiento ingestivo. La Escala de Fenotipos de Comportamiento Alimentario (EFCA) es un cuestionario autoadministrado, diseñado como herramienta de uso clínico para caracterizar diferentes subfenotipos de comportamiento ingestivo: hedónico, compulsivo, emocional, desorganizado e hiperfágico. Objetivos: el objetivo de este estudio es validar las propiedades psicométricas de la Escala de Fenotipos de Comportamiento Alimentario (EFCA) y analizar la estabilidad del constructo y su validez externa. Materiales y métodos: trescientos participantes adultos completaron una encuesta autoadministrada, desarrollada para identificar fenotipos de conducta alimentaria (EFCA). Se realizó un análisis factorial confirmatorio, se evaluó la consistencia interna mediante el coeficiente alfa de Cronbach y se determinó la validez concurrente mediante el método de correlación de Pearson entre EFCA e IMC. Resultados: la escala EFCA y las subescalas mostraron una aceptable consistencia interna (α > 0,70). El análisis factorial confirmatorio mostró un buen ajuste de los datos a la estructura propuesta (SBχ922 = 155, p < 0,05; CFI = 0,97, TLI = 0,96, RMSEA = 0,05, SRMR = 0,04). Se encontró una correlación positiva y estadísticamente significativa entre el IMC y cada subescala y la puntuación total de la escala. Conclusiones: la EFCA y sus subescalas son un instrumento válido para evaluar fenotipos alimentarios en adultos. La estructura de cinco componentes muestra una alta estabilidad y resultados consistentes en relación a un estudio previo realizado con una muestra de pacientes con exceso de peso (AU)


Introduction: weight gain depends on multiple modifiable mediating factors, including ingestive behavior phenotype. The Eating Behavior Phenotypes Scale (EFCA) is a self-administered questionnaire designed as a tool for clinical use to characterize different sub-phenotypes of ingestive behavior: hedonic, compulsive, emotional grazing, disorganized and hyperphagic. Objectives: the aim of this study is to validate the psychometric properties of the Eating Behavior Phenotypes Scale (EFCA), to analyze the stability of the construct and its external validity. Materials and methods: three hundred adult participants completed a self-administered survey developed to identify eating behavior phenotypes (EFCA). A confirmatory factor analysis was performed, internal consistency was evaluated using Cronbach's alpha coefficient, and concurrent validity was assessed using Pearson's correlation method between EFCA and BMI. Results: the EFCA scale and the subscales showed an acceptable internal consistency (α > 0.70). The confirmatory factor analysis showed a good adjustment of the data to the proposed structure (SBχ922 = 155, p < 0.05; CFI = 0.97; TLI = 0.96; RMSEA = 0.05; SRMR = 0.04). A positive and statistically significant correlation was found between BMI and both each subscale and total scale scores.Conclusions: EFCA and its subscales are a valid instrument to assess eating phenotypes in adults. The five-component structure shows high stability and consistent results in relation to a previous study carried out with a sample of obese patients (AU)

INTRODUCTION: Acrylates are widely used low-molecular-weight substances, initially introduced in industry in the 1930s and subsequently applied also in medicine and the home. One of their main features is the ability to undergo polymerization. The most commonly used acrylic compounds are cyanoacrylates, methacrylates, and acrylates. OBJECTIVE: To confirm suspicion of occupational disease in a group of workers in an elevator factory. MATERIAL AND METHODS: We studied 8 patients with dermatitis of the hands and finger pads. In their work, the patients came into contact with acrylates. Patch testing was applied with an acrylate panel (BIAL-Aristegui, Bilbao, Spain). RESULTS: Seven of the patients (87. 5%) had a positive result with 1% ethylene glycol dimethacrylate. Positive RESULTS: were also observed for 2% hydroxyethyl methacrylate (5 patients, 62. 5%), 1% triethylene glycol dimethacrylate (4 patients, 50%), 10% ethyl methacrylate monomer (3 patients, 37. 5%), 10% methyl methacrylate monomer (2 patients, 25%), 1% ethyl acrylate (1 patient, 12. 5%), and 0. 1% acrylic acid (1 patient, 12. 5%). CONCLUSIONS: We highlight the strong sensitizing capacity of acrylates and the importance of taking all necessary preventive measures in industries where these substances are used. Such measures should include avoidance of contact with the product in cases where sensitization has been confirmed.

ABSTRACT

Se trata de un hombre de 73 años que ingresó en nuestro hospital para evaluación de debilidad generalizada, hipertensión arterial de reciente comienzo e hipocaliemia. Durante su estancia hospitalaria se demostró hipocaliemia intensa y refractaria, reacción leucoeritroblástica en sangre periférica y enfermedad pulmonar intersticial. La actividad de renina plasmática basal y aldosteronemia fueron normales y los valores de hormona adrenocorticotrópica (ACTH) y cortisol en orina mostraron criterios bioquímicos de síndrome de Cushing ACTH-dependiente. Por biopsia de médula ósea se demostró metástasis de carcinoma de células pequeñas de probable origen pulmonar, y tras ello se procedió a seguimiento paliativo. Se comenta la rareza del síndrome y las peculiaridades de la asociación de hipertensión arterial con hipocaliemia (AU)

ABSTRACT

Many works have dealt with the study of the allergenic relevance of profilin from allergenic extracts, mainly derived from pollens and vegetable foods. Olive pollen extracts also contain a profilin allergen (Ole e 2). This protein has been characterized in detail, so the amino-acid sequence of three isoforms and the structural model of one of them are already known. The prevalence of Ole e 2 for olive allergenic patients has been evaluated by different in vivo and in vitro methods, and the results compared with those obtained for another pollen profilins.
