ABSTRACT

The possibility of laser coagulation of the retina after implantation of posterior-chamber lenses was examined in 44 patients with diabetes mellitus. Three-speculum Goldman's lens could be used to examine the central zone in 40 patients, the median periphery in 36 patients, and the utmost periphery in 6 patients. The principal causes impeding the examination of the retina were (1) posterior capsule opacity, (2) inadequate mydriasis because of synechias between the iris and the intraocular lens. The number of patients in whom the fundus oculi could be examined was reducing with time. Mydriasis of 8 mm was found necessary for the examination of the total fundus oculi of the artiphakic eye.

A system for estimating labor consumption in, and quality of the therapeutic and diagnostic process, based on the medicosocial and economic analysis of the current ocular morbidity structure in Russia, is suggested. This estimation system may be useful in condition of cost accounting relations introduction in research institutions of ophthalmologic profile.

ABSTRACT

Trans-scleral cryopexy (TSC) was carried out in 81 eyes with recurrent hemophthalmos of diabetic genesis. 73 patients were followed up after surgery for a period of 3 months to 5 years. Hemophthalmos resolved in 80 percent of eyes (completely in 60, partially in 20 percent). The condition recurred in 10 percent of eyes, detachment of the retina developed in 13, neovascular glaucoma in 4 percent, and cataract progressed in 10 percent of eyes. Vision acuity improved in 36 percent of eyes; the best results were observed in type II diabetes mellitus. TSC proved to be an effective method for the management of hemophthalmos of diabetic nature.

ABSTRACT

Fluorescent angiography was used to examine changes in the retina and vascular coating in traumatic and posttraumatic conditions of the eyes in 55 patients aged 12-66 who had traumas 3 days--4 years before. 36 patients had blunt injuries and 16 penetrating wounds of the eyeball. The type of posttraumatic changes in the fundus oculi was examined, and classification of these changes with consideration of their clinical and angiographic signs developed. Fluorescent angiography yields additional data on the process severity and dissemination, its data are prognostically important, it helps define the indications for laser and drug therapy and surgery.

ABSTRACT

The authors describe the clinical and angiographic picture of idiopathic parafoveal telangiectasis in 6 patients (10 eyes). Three forms of the condition were distinguished: edematous, occlusive, and with subretinal shifts. A natural course of the process is traced, the criteria and terms of laser coagulation are defined.

ABSTRACT

The authors describe the clinical forms of complicated angioid streaks (AS) of the retina and the diagnostic symptoms of this condition that help differentiate it from other fundus oculi abnormalities. 52 patients (104 eyes) with retinal AS have been examined. Ophthalmoscopic and angiographic picture permits distinguish 4 clinical forms of complicated retinal AS: exudative hemorrhagic macular changes, non-exudative macular dystrophy, optic nerve atrophy, hemophthalmos. The underlying condition has been differentiated from central involutional retinal dystrophy, complicated myopia, central chorioretinitis, vasculitis. Noteworthy, that complete or partial atrophy of the optic nerve in the presenCe of profuse gastrointestinal hemorrhages is rarely responsible for vision deterioration in the patients with AS.

ABSTRACT

Nonspecific aortoarteritis is a systemic autoimmune disease eventuating in gradual stenosis of the aorta and the main vessels with ischemia of the respective organs. Ophthalmologic symptoms have been examined in 54 patients with nonspecific aortoarteritis. Subjective disorders of vision (short-term binocular blindness, metamorphopsia, pain behind the eye, amaurosis fugax) have been detected in 52% of the examinees. Organic lesions of the eye have been diagnosed in 60% of the patients: hypertensive angiopathy (22%), venous stasis retinopathy (17%), occlusion of the central retinal artery (1%), etc. Three possible mechanisms of the development of ocular symptoms have been established: (1) a result of symptomatic hypertension, (2) chronic ocular ischemia, (3) acute hemodynamic ocular circulation insufficiency.
