ABSTRACT

OBJECTIVE: The object of this trial was to appreciate the acceptability of Misoprostol home use in medical abortion. PATIENTS AND METHOD: From 1 January 2001 to 28 February 2002, 120 voluntary patients were included in the study of medical voluntary pregnancy interruption with Misoprostol at home, and 289 patients were using the protocol "Misoprostol at hospital". RESULTS: The method was well tolerated by all patients. DISCUSSION AND CONCLUSION: The protocol "Misoprostol at home" is accepted under certain conditions: strict criteria selection, strict rules of security, good counseling of patients, and finally the availability at the telephone of the gynecologist to respond to any eventuality.

ABSTRACT

After recalling the medical reluctance as well as the risks that there are in complete elimination of milk in infants, the author presents several clinical pictures and then a classification of the immunological types. Allergic shock of neonates, digestive and extra-digestive (skin and respiratory airways) symptoms finally the rare chronic gastro-enteritis to cow milk. Non-reaginic food allergies: Acute gastro-enteropathy to cow milk, with villous atrophy and Heiner's syndrome, delayed hypersensitivities are studied, of difficult diagnosis that may cover almost all pathologies. They may be found in the digestive system, respiratory, the kidneys and even in the organs of behaviour. Migrane of food origin must be remembered. Development in regressive rules is a function of the type of allergy and the suddenness of the symptoms. Diagnosis is above all by questioning and confirmation or not by skin and in vitro tests. Certainty can only be shown by tests of elimination and re-introduction. The diet, at the same time of both diagnostic and therapeutic value, is based on the replacement of cow milk by foods that contain the same amount of proteins. It is essential, especially in the very small, to have perfect match of food so as to avoid any risk of a dramatic hypoprotinemia, which may happen if the child does not like the suggested diet, or if the parents cannot buy the substitution products. In such conditions great care must be taken to avoid provoking a crisis. Care must be taken to decide: If the elimination of cow milk is always justified each time. If it is, always check that the substituted protein is properly made, the family may change the diet mistakenly. It is better, finally, to keep the eczema, rather than die with it healed.

ABSTRACT

Intrauterine insemination (IUI) has been used for treatment of infertility for many years with discouraging results (8.3% in the analysis of Dorangeon and Quereux). A new approach of IUI, using ovarian stimulation, ovulation monitoring and sperm preparation has significantly improved the results. The obtained pregnancy rate (15 to 25%) are now comparable to those reported with IVF or GIFT. Therefore IUI should be offered to patients with patent tubes prior to their referral for the more invasive procedure FIV or GIFT.

ABSTRACT

This work is in the purview of epidemiological studies that concern the environment of asthmatic children. Infantile asthma is found especially in children who live in the country, in an electrically-heated house and who sleep in a badly orientated room. The environment is composed of feather-bedding, soft-toy animals and animals from the countryside. The role of tobacco and insufficient aeration has been shown in this enquiry. School attendance is not disturbed by development of asthma. The majority of teachers do not understand infantile asthma. This study is to emphasize the importance of the environment in the development of allergic infantile asthma and the possibilities of its prevention. Information to the teachers, as well as the treating physicians, is also necessary for prevention.

ABSTRACT

At the end of this quick survey of asthma in children we can only emphasize: The less than diagnostic characteristics of this syndrome, often ignored and so not considered. This explain perhaps, in part the current increase in frequency. The often precocious characteristics of the first manifestations right in the middle of the development of the respiratory tree. Of the need to make a team by associating the parents, the family physician and the allergy specialist, a team to which the child should be added as soon as possible. Of the multiplicity of factors that cause asthma, which embrace syndromes that are very personal to each patient and require special consideration for each, without stereotype or a chain.

ABSTRACT

The aim of this study was to present briefly the different techniques for measurement of nasal resistance and to show the importance that we attribute to this measurement. Within the framework of the allergological investigation rhinamometry is a parameter that allows evaluation of the results of allergenic nasal provocation. Nasal provocation is in the framework as a complementary examination that is often indespensable to confirm with most possible certainty the involvement of an allergen in the ORL pathology. Positive etiological diagnosis is a fundamental basis for therapeutic advice.

ABSTRACT

Asthma from intolerance of sulphites has been the subject of clinical work by our team since 1984. In 1986, we advised that asthma from sulphite intolerance was more frequent in patients who presented with illness from aspirin or the Fernand Widal syndrome. So it has emerged that there are clear therapeutic problems since sulphites as preservatives are present in other forms in some anti-allergy drugs, anti-histamines used for ORL.

ABSTRACT

In assessing the importance of the "allergen eviction" factor in the treatment of infantile allergies, the authors begin an inventory of those elements in the environment that concern perennial rhinitis. From 200 observations of allergic children (all mixed diagnoses) they found 21 observations of rhinitis alone, predominantly boys. The environments of two groups (all allergics--rhinitis alone) were compared. No difference for: Home, country, town heating allergens found. Found significant differences for the type of house new-old those elements of domestic comfort (carpets, feather beds, aeration, soft toys) agreement between the results of tests and the environment domestic animals or permanent proximity. Found fundamental differences for psychological environmental factors (absent in rhinitis). It seems that perennial rhinitis probably arises from abundance of domestic antigens more than for the other allergic manifestations, as the nose is the first filter to receive foreign particles.

ABSTRACT

A school environmental study of about 20,000 children from the French department of MAINE and LOIRE (1/100 of French territory and 1/100 of the total population) was made during the summer of 1987 in all the schools of the region. The replies allowed evaluation of: -- The elements of the environment; . urban or rural . type of building . soils . domestic animals (types, lifestyle, large or small, relationship with the children). -- The proportion of allergic children per classroom, types of sickness, suggestions made by parents, eventual allergic accidents during class hours. In addition, school teachers were asked to comment on the subject. Their answers showed extreme variety of attitude towards allergy, there were those who refuted it, some fully accepted and others who asked for more information so that they could be more help to their students. A prime interest of the study lies in the dialogue between parents, teachers and physicians as the children spent most of the daytime at school and therefore the quality of the environment is a major factor in infantile allergy.

A case of asthma in an infant child from a family already showed inability to tolerate cow milk and which only responded when such food was discontinued. The study concerns: a. The diagnostic methods of the disease; b. The relationship between the IPLV and hypotrophy that was perceptible in the infant child, a hypotrophy that was revealed in the course of the evolution of the disease. Finally, a study of the documents on the pathogenic processes enabled us to locate the observation involved to half-way between IgE dependent cases and those other cases that produce delayed hypersensitivity.

ABSTRACT

A retrospective study of results of adenoidectomy, tonsillectomy, or their association in 80 of 35% cases of children with allergic affections showed that operation was successful in 2 out of 10 cases, resulted in complications in one out of 10 and was ineffective in 7 out of 10. This emphasizes the need for selection of children with allergy before deciding to operate, failures with complications being rarely observed when correct decisions have been made. Poor results can be avoided if contact of harmful allergens is prevented during healing. This involves the eviction of allergens, with isolation of the patient for one week and antidegranulation treatment of the ketotifen type, and accelerated densensitization during the isolation period. Antibiotics and polyvalent vaccination may be necessary to treat infection. Four-year follow up of 65 patients operated upon under these conditions demonstrated excellent results.
