ABSTRACT

ABSTRACT Objective: to assess the psychometric properties of the Brazilian version of Quality of Care Through the Patient's Eyes - HIV (QUOTE-HIV). Method: this methodological study consisted of verifying and assessing convergent and structural validity and estimating the reliability of the instrument's translated and culturally adapted version. It was conducted in an HIV/AIDS referral center between August and November 2016 and included 141 participants. Confirmatory factor analysis was performed to assess structural construct validity, while Cronbach's alpha and composite reliability were performed to assess the internal consistency of each domain and estimate the instrument's reliability. Convergent construct validity was verified using Spearman's coefficient correlation. Results: QUOTE-HIV is structured on two components, Importance and Performance; hence, two models were generated at the end of the factor analysis, one for each component. Both models were statistically validated and presented satisfactory reliability parameters. Regarding convergent validity, positive and significant correlations were found between the scores obtained by the QUOTE-HIV domains and the Newcastle Satisfaction with Nursing Scales (NSNS), except for the performance domain in the importance component. Conclusion: QUOTE-HIV Brazilian version presented evidence of reliability, adequate internal consistency, and validity according to the convergent and structural construct validity methods adopted.

Through the Patient's Eyes - HIV (QUOTE-HIV).

ABSTRACT

Surgical site infection (SSI) is a common complication that is associated with delayed recovery, prolonged length of hospital stay, exorbitant cost, and mortality. The present prospective longitudinal study aimed to evaluate the relationships between the microbial load of trocars used in laparoscopic gynecological surgery, microbiota in surgical sites, and SSI. The final sample consisted of 24 patients, including 68 swab samples and 48 trocars. Microorganisms were recovered in 100.0% of the swabs collected from the umbilicus and vaginal fornix and in 58.3% (14/24) of the swabs collected from skin at the left McBurney's point. Most of the samples collected from trocars (87.5%) did not exhibit bacterial growth, suggesting proper disinfection. In addition, antisepsis was effective for decolonization of the skin to create an aseptic surgical field.

ABSTRACT

OBJECTIVE:: to identify evidences from the literature on the application of nursing process in care developed by the nurse in mental health. METHOD:: integrative literature review between 1990 and 2013, in the PubMed, Scopus, CINAHL and LILCACS bases. Descriptors: nursing processes, mental health, nursing care. RESULTS:: 19 papers were identified. Limited and partial usage of the nursing process in care established by a therapeutic relationship that respects the patient's individuality. We observe care proposals systematized for patients that present pathological aspects in the limits between the physical and psychical, which might be a response to the influence of the practice based on evidences. CONCLUSION:: it was found an antagonistic movement between care based on the relationship and located in the standardization of diagnoses that respond to physical malaise. A lack of evidence was verified for the usage of the nursing process in mental health, and we point at the necessity for the creation of new possibilities for dialogue between relational and biological perspectives.

ABSTRACT

OBJECTIVES: Improper placement of nasogastric tube used for feeding may lead to serious complications, including death of the patient. There are several different methods used to determine the appropriate length of nasogastric tube for optimal placement in adults. This integrative review of the literature was designed to identify the most accurate method to determine the internal length of nasogastric feeding tube in adults. DESIGN: An integrative review of the research literature (1979-2015) using the population-intervention-comparison-outcomes strategy. DATA SOURCES: The literature search included the Cumulative Index to Nursing and Allied Health Literature (CINAHL), Cochrane Library, Joanna Briggs, PubMed (MEDLINE), SCOPUS, and Web of Science electronic databases. REVIEW METHODS: Two researchers evaluated the literature to determine if an article met inclusion and exclusion criteria. The quality of the evidence was assessed using the Johns Hopkins Strength of the Evidence critical appraisal tool. RESULTS: Twenty studies, published between 1979 and 2014, met inclusion criteria. Of these, nine articles were expert opinion, seven were original research, three were review articles, and one was a guideline. Despite seven original research papers being found, only five reports were about the methods to determine the internal length of nasogastric feeding tube in adults. The literature suggests that four different methods for measuring the tube length are likely to result in proper placement of the tip of the tube in the stomach and all side ports inside it: [nose-to-ear-to-xiphisternum - 50]cm/2+50cm]; [gender-weight and nose-umbilicus-flat]; [xiphisternum-to-ear-to-nose+10cm]; [earlobe to xiphisternum to umbilicus - tip of the nose to earlobe]. Four studies found nose-to-ear-to-xiphisternum was most likely to result in a tube that is positioned incorrectly, either ending in the esophagus, in the stomach but too close to the esophagus, or too far into the stomach or duodenum. CONCLUSIONS: The nose-to-ear-to-xiphisternum and Hanson method should no longer be taught in nursing programs or used in practice by the nurse. The [gender-weight and nose-umbilicus-flat] method has been shown to be safer.

ABSTRACT

Objective To design and validate a surgical checklist to improve patient safety and prevent surgical site infection. Methods This quantitative study was carried out to validate an instrument created and used for surgical safety. Seven experts validated the instrument. For agreement among experts, was used Kendall’s concordance coefficient; if their opinions differed significantly, the Cochran’s test was adopted. An instrument is validated when concordance among experts is achieved and its clarity is significant. Results In the first assessment of the instrument, Kendall’s concordance coefficients were 0.230 in terms of pertinence and 0.390 for clarity. These results cauded a reformulation in the checklist. After reformulation, an absolute concordance was achieved for pertinence and no significant difference was seen in terms of clarity. After instrument validation, was created an information system to input data collected. Conclusion The instrument was validated. It can help improve patient safety and prevent surgical site infection.

ABSTRACT

BACKGROUND: The correct placement of a nasogastric tube for enteral nutrition is subject of several investigations, demonstrating the controversy of the procedure. AIM: To establish an external measure that can correspond to the internal measurement which determines the insertion length of nasogastric feeding tube up to the stomach. METHODS: External measures were obtained between points: nose tip vs earlobe vs xiphoid appendix vs umbilicus and height correlated with the standard measures obtained from patients undergoing diagnostic esophagogastroduodenoscopy. RESULTS: It was found a significative statistical correlation between esophagogastric junction, identified during the esophagogastroduodenoscopy, with the distance measured between the anatomic points of the earlobe and xiphoid appendix (r= 0.75) and from this line with the orthostatic height (r=0.72). CONCLUSION: The distance between the earlobe to the xiphoid appendix (0.75) and the distance between the earlobe to the xiphoid appendix to the midpoint of the umbilicus, subtracting the distance from tip of nose to earlobe, were safe anatomical parameters to reach the esophagogastric junction. The height in the standing position (r= 0.72) also can be used as an indicator of the length necessary to insert the tube into the stomach. The height in the standing position (r= 0.72) also can be used as an indicator of the length necessary to insert the tube into the stomach.

ABSTRACT

ABSTRACT

ObjectiveThis study investigated the clinical and sociodemographic characteristics of a populationaged 50 years or more with human immunodeficiency virus/acquired immunodeficiencysyndrome.MethodsThis exploratory, quantitative, cross-sectional study used a tested and validatedinstrument for data collection. The variables were compared and tested for associationsby the Chi-square, Fisher’s exact, and Mann-Whitney tests and Kappa statistic.ResultsThe study interviewed 109 patients with a mean age of 55.78 years. Most were males(57.8%), Catholics (65.1%), single (65.1%), and heads of household (81.7%). Educationlevel and treatment adherence were positively associated. Their clinical condition wassatisfactory, considering the problems caused by human immunodeficiency virusinfection.ConclusionSexual human immunodeficiency virus transmission prevailed for both genders. Thepatients’ medical records differed significantly from their reported opportunisticinfections. Education level and treatment adherence were positively associated. Thepatients’ openness and honesty during the interviews were unexpected.
