ABSTRACT

BACKGROUND: Dry socket is one of the most studied complications in dentistry and several studies have sought an effective and safe method for its prevention and treatment. The aim of this study was to evaluate the effectiveness of intra-alveolar gel chlorhexidine in preventing dry socket after the surgical removal of third molars. MATERIAL AND METHODS: The sample involved the treatment of 40 patients who required extraction of third molars impacted, which were randomly assigned to research groups: experimental group (chlorhexidine gel 0.12%) and control group (placebo gel). Performed the extraction was administered 1 mL of chlorhexidine gel or 1 mL of placebo gel within the socket. The removal of suture was on the fifth postoperative day in which the presence or absence of dry socket was evaluated. RESULTS: No relationship between the appearance of dry socket after application of chlorhexidine gel or placebo gel (X2 test, p = 0.311) was found. However, significant differences (U Mann-Whitney test, p = 0.036) in the pain presented on the fifth postoperative day were evident (VAS). CONCLUSIONS: The administration of intra-alveolar chlorhexidine gel 0.12% could generate a better response to postoperative pain after the removal of third molars. ABSTRACT

El fibrosarcoma ameloblástico es un tumor odontogénico raro, compuesto por epitelio benigno y tejido mesenquimal maligno que semeja a un fibrosarcoma. Se considera que es la contraparte maligna del fibrosarcoma ameloblástico. Aun cuando los cambios histológicos son claramente malignos, este tumor rara vez produce metástasis, pero su crecimiento progresivo es infiltrantey destruye los tejidos adyacentes pudiendo llegar hasta la base del cráneo. En este trabajo sepresenta un caso de fibrosarcoma ameloblástico desarrollado en la mandíbula derecha de una mujer joven de 19 años de edad. Las características clínicas, radiográficas y sintomatología de la lesión, correspondían a un tumor odontogénico benigno e igualmente en el estudio histológico se podía identificar una capsula fibrosa bien definida. Sin embargo, el tejido dentro de la cápsulamostraba cambios mesenquimales francamente malignos. El tratamiento consistió en la extirpación de la hemimandíbula complementado con radiaciones. El seguimiento después de 5,3 años mostró que la paciente se encontraba libre de enfermedad.


Ameloblastic fibrosarcoma is a rare odontogenic tumor composed by a benign epithelium and a malignant mesenchyme tissue resembling a fibrosarcoma. It is regarded as the malignant counterpart of the ameloblastic fibroma. Dispite the clearly malignant histologic features thistumor rarely produce distant metastasis. This work reports a case of ameloblastic fibrosarcoma developed in the right mandible of a 19-year-old girl. The clinical, radiographic and symptomsresembled a benign odontogenic tumor, also in the histology a well defined capsule was identified. However, the tissue inside the capsule showed defined malignant mesenchymal changes. Treatment was surgical removal of the hemimandible following by radiation of the remaining tissues. The follow up after 5.3 year demonstrated a patient free of disease.

ABSTRACT

Se revisa la literatura y se discuten las características clínicas, histológicas, diagnóstico y evolución de un caso de úlcera eosinofílica (UE) de la lengua en un paciente de 53 años de edad cuya presentación clínica semejaba a un carcinoma epidermoide. Sobre la base de los hallazgos histológicos y la respuesta clínica al procedimiento quirúrgioco de la toma de biopsia, se plantea la posibilidad, que por lo menos en este caso, la UE haya resultado, primariamente de una lesión de la pared de los vasos sanguíneos que condujo a ulceración, hemorragia y formación de extensas áreas de fibrina, con la consecuente alteración del proceso de reparación.
