ABSTRACT

Spatio-temporal variability of surface geostrophic mesoscale currents in the Balearic Sea (western Mediterranean) is characterized from satellite altimetry in combination with in-situ velocity measurements collected, among others, by drifting buoys, gliders and high-frequency radar. Here, we explore the use of tracking data from living organisms in the Balearic Sea as an alternative way to acquire in-situ velocity measurements. Specifically, we use GPS-tracks of resting Scopoli's shearwaters Calonectris diomedea, that act as passive drifters, and compare them with satellite-derived velocity patterns. Results suggest that animal-borne GPS data can be used to identify rafting behaviour outside of the breeding colonies and, furthermore, as a proxy to describe local sea surface currents. Four rafting patterns were identified according to the prevailing driving forces responsible for the observed trajectories. We find that 76% of the bird trajectories are associated with the combined effects of slippage and Ekman drift and/or surface drag; 59% are directly driven by the sea surface currents. Shearwaters are therefore likely to be passively transported by these driving forces while resting. The tracks are generally consistent with the mesoscale features observed in satellite data and identified with eddy-tracking software.

ABSTRACT

Metformin is an antihyperglycemic agent commonly used in diabetic patients. It is very effective and is able to reduce the plasma glucose and HbA1C. However, in some patients, specially those with comorbidities, metformin can provoke severe lactic acidosis with high morbimortality. Treatment of the lactic acidosis induced by metformin is based on the use of supportive general measures; in severe cases, procedures of extrarrenal purification like hemodialysis or continuous hemodiafiltration have been successfully used.

La metformina es un fármaco ampliamente utilizado en sujetos con diabetes mellitus y su eficacia para descender la glucemia y la hemoglobina A1C (HbA1C) es notable. ABSTRACT

La aparición de cifras tensionales elevadas en las primeras décadas de la edad adulta constituye un reto diagnóstico y terapéutico para los clínicos. En este sentido, presentamos un caso de hipertensión arterial en una enferma joven detectado en Atención Primaria y que se acompañaba de soplos abdominales y proteinuria significativa, lo que sugería la presencia de un cuadro de mayor entidad al esperado en relación con enfermedad vascular renal. Se realizó una arteriografía que evidenció no sólo la presencia de una lesión oclusiva cercana al ostium de la arteria renal izquierda, sino también afectación a otros niveles del árbol vascular. La revascularización percutánea de la lesión renal permitió una normalización tanto de las cifras de tensión arterial como de la proteinuria. Planteamos la importancia de realizar en enfermos jóvenes con hipertensión arterial una evaluación sensata orientada a despistar entidades potencialmente tratables y, por tanto, reversibles


The appearance of elevated blood pressure values in the first decades of the adult age is a diagnostic and therapeutic challenge for the clinicians. In this sense, we present a case of arterial hypertension in a young female patient in Primary Health Care. This was accompanied by abdominal murmurs and significant proteinuria, which suggested the presence of a larger picture than that expected in relationship with renal vascular disease. An arteriography was conducted that not only showed the presence of an occlusive lesion close to the left renal artery ostium but also involvement on other levels of the vascular tree. Percutaneous revascularization of the renal lesion allowed for normalization of the blood pressure and proteinuria values. We suggest the importance of conducting a sensible evaluation in young patients with arterial hypertension oriented at screening for potentially treatable and therefore reversible diseases (AU)
