ABSTRACT

Background: Pediatric COVID-19 patients have lower rates of hospitalization and fatal outcomes compared to adults with COVID-19; however, children represent a challenge in the detection, diagnosis, and treatment of COVID-19. Our aim was to determine the risk factors for hospital admission, invasive mechanical ventilation, and mortality in pediatric COVID-19 patients in Mexico during the COVID-19 pandemic. Material and methods: A retrospective cohort of pediatric patients with COVID-19 from February 2020 to April 2021 was reported on the National Epidemiological Surveillance System for Viral Respiratory Disease (SISVER) platform. Results: Among the 104,133 patients included in our study, 6214 were hospitalized, and 621 patients underwent invasive mechanical ventilation. A total of 0.65 % died during hospitalization. Children aged <12 months (odds ratio [OR]: 17.1; 95 % confidence interval [CI]: 15.9-19.4, p < 0.001), 1-4 years (OR: 3.69; 95 % CI: 3.2-4.1, p < 0.001), 5-9 years (OR: 1.86; 95 % CI: 1.66-2.08, p < 0.001), and 10-14 years (OR: 1.23; 95 % CI: 1.11-1.37, p < 0.001), and those diagnosed with diabetes (OR: 2.32; 95 % CI 1.68-3.20, p < 0.001) and obesity (OR: 1.24; 95 % CI 1.04-1.48, p = 0.015) were associated with hospital admission. Renal disease (OR: 3.85; 95 % CI: 2.25-6.59, p < 0.001) was associated with invasive mechanical ventilation. Pneumonia (OR: 15.9; 95 % CI: 12.6-20.1, p < 0.001) and renal disease (OR: 3.85; 95 % CI: 2.25-6.59, p value < 0.001) were associated with death. Conclusion: Pneumonia increases the risk of death. The youngest age group has a higher risk of hospital admission. Comorbidities such as renal disease or immunosuppression increase the risk of death in all age groups.

On the other hand, EDS peaks demonstrated the presence of O and Cl after CHX treatment. A reduction of cell viability and adhesion was noted on titanium surfaces treated with CHX after 24 and 72h. In conclusion, the results indicate that the decontamination with Cvc and T4ol on Ti surfaces does not alter the surface proprieties and allows an adequate interaction with cells involved in the re-osseointegration process such as fibroblasts and osteoblasts.

ABSTRACT

BACKGROUND: About 10% of ankle fractures are located above the syndesmosis, which causes injury to the ligament structure of the syndesmosis. There is no consensus regarding when to allow weight-bearing on the operated limb of patients who suffered ankle fractures requiring intraoperative fixation. OBJECTIVE: The aim of this study is the evaluation of radiographic parameters of syndesmosis reduction and fixation in patients with ankle fracture who underwent immediate protected weight-bearing (use of walking-boot and crutches). PATIENT AND METHODS: Retrospective case series from January 2015 to December 2020. Evaluation of the tibiofibular clear space, tibiofibular overlap, and medial clear space in the preoperative x-ray, immediate postoperative, 3-, 6- and 12-month after syndesmosis fixation and rehabilitation with protected immediate weight-bearing with crutches and walking-boot. RESULTS: Out of 137 ankle fractures 39 were included. There was no significant difference in the tibiofibular clear space between the immediate x-ray and after 12 months (4.5 mm ± 1.1 vs. 4.6 mm ± 1.8; p > 0.999). Tibiofibular overlap hasn't shown significant difference between immediate and 12-month x-ray (6.2 mm ± 2.6 vs. 6.6 mm ± 2.6; p > 0.999). Medial clear space also has shown no worsening from immediate to 12-month x-ray (3.1 mm ± 0.9 vs. 2.8 mm ± 0.9; p > 0.999). There were no fixation failure nor postoperative infection. CONCLUSION: Immediate weight-bearing for patients with ankle fractures with syndesmosis injury and treated with intraoperative fixation with positioning screw proved to be safe with good maintenance of the reduction, no failures of the synthesis material observed and no reports of postoperative infection in the period. LEVEL 3 EVIDENCE: therapeutic retrospective case-series.

Resumo O presente estudo avaliou a influência do carvacrol, terpineno-4-ol e clorexidina nas propriedades físico-químicas de superfícies de titânio, viabilidade celular, proliferação, adesão e esplhamento de fibroblastos e osteoblastos in vitro. Superfícies de titânio (Ti) foram tratadas com Carvacrol (Cvc), Terpinen-4-ol (T4ol), Clorexidina (CHX), DMSO e água ultrapura (Grupo Controle). As modificações físico-químicas foram avaliadas pela molhabilidade da superfície, a energia livre de superfície (ELS) calculada a partir dos valores do ângulo de contato usando a equação de Owens-Wendt-Rabel-Kaeble (OWRK), microscopia eletrônica de varredura (MEV) e espectroscopia de raios X por energia dispersiva (EDS). As células foram semeadas em superfícies tratadas com Ti e incubadas por 24 h e 72 h, e avaliadas pelo ensaio Alamar blue e microscopia de fluorescência. As superfícies tratadas com Cvc e T4ol mostraram a presença de Na, O e Cl. Todas as superfícies apresentaram características hidrofílicas e valores de ELS entre 5,5 mN/m e 3,4 mN/m. Por outro lado, os picos de EDS demonstraram a presença de O e Cl após o tratamento com CHX. Uma redução da viabilidade celular e adesão foi observada em superfícies de titânio tratadas com CHX após 24 e 72h. Em conclusão, os resultados indicam que a descontaminação com Cvc e T4ol em superfícies de Ti não altera as propriedades da superfície e permite uma interação adequada com células envolvidas no processo de reosseointegração como fibroblastos e osteoblastos.

ABSTRACT

BACKGROUND: Public health officials are responding to an outbreak of fungal meningitis among patients who received procedures under epidural anesthesia at two clinics (River Side Surgical Center and Clinica K-3) in Matamoros, Mexico, during January 1-May 13, 2023. This report describes outbreak epidemiology and outlines interim diagnostic and treatment recommendations. METHODS: Interim recommendations for diagnosis and management were developed by the Mycoses Study Group Research Education and Consortium (MSGERC) based on the clinical experience of clinicians caring for patients during the current outbreak or during previous outbreaks of healthcare-associated fungal meningitis in Durango, Mexico, and the United States. RESULTS: As of July 7, 2023, the situation has evolved into a multistate and multinational fungal meningitis outbreak. A total of 185 residents in 22 U.S. states and jurisdictions have been identified who might be at risk of fungal meningitis because they received epidural anesthesia at the clinics of interest in 2023. Among these patients, 11 suspected, 10 probable, and 10 confirmed U.S. cases have been diagnosed, with severe vascular complications and eight deaths occurring. Fusarium solani species complex has been identified as the causative agent, with antifungal susceptibility testing of a single isolate demonstrating poor in vitro activity for most available antifungals. Currently, triple therapy with intravenous voriconazole, liposomal amphotericin B, and fosmanogepix is recommended. CONCLUSIONS: Efforts to understand the source of this outbreak and optimal treatment approaches are ongoing, but infectious diseases physicians should be aware of available treatment recommendations. New information will be available on CDC's website.

ABSTRACT

BACKGROUNDWith widespread transmission of the Omicron SARS-CoV-2 variant, reinfections have become increasingly common. Here, we explored the role hybrid immunity, primary infection severity, and variant predominance on the risk of reinfection and severe COVID-19 during periods of Omicron predominance in Mexico. METHODSWe analyzed reinfections in Mexico in individuals with [≥]90 days from a previous primary infection using a national surveillance registry of SARS-CoV-2 cases from March 3rd, 2020, until August 13th, 2022. Immunity-generating events included primary infection, partial or full vaccination and vaccine boosting. Reinfections were matched by age and sex with controls with primary SARS-CoV-2 infection and negative RT-PCR or antigen test [≥]90 days after infection to explore risk factors for reinfection and reinfection-associated severe COVID-19. We also explored the protective role of heterologous vs. homologous vaccine boosters against reinfection or severe COVID-19 in fully vaccinated individuals. RESULTSWe detected 231,202 SARS-CoV-2 reinfections in Mexico, with most occurring in unvaccinated individuals (41.55%). Over 207,623 reinfections occurred during periods of Omicron (89.8%), BA.1 (36.74%) and BA.5 (33.67%) subvariant predominance and a case-fatality rate of 0.22%. Vaccination protected against reinfection, without significant influence of the order of immunity-generating events and provided >90% protection against severe reinfections. Heterologous booster schedules were associated with [~]11% and [~]54% lower risk for reinfection and reinfection-associated severe COVID-19 respectively, modified by time-elapsed since the last immunity-generating event. CONCLUSIONSSARS-CoV-2 reinfections have increased during periods of Omicron predominance. Hybrid immunity provides protection against reinfection and reinfection-associated severe COVID-19, with potential benefit from heterologous booster schemes. RESEARCH IN CONTEXTO_ST_ABSEvidence before this studyC_ST_ABSWe searched PubMed for the terms "SARS-CoV-2" AND "reinfection" AND "hybrid immunity" until November 20th, 2022, and identified a few population studies previously conducted in Israel, Sweden, Qatar, United States and Canada which explored risk of reinfection and the protective role of hybrid immunity in individuals with one, two or three doses of COVID-19 vaccines, predominantly during periods of predominance of Omicron BA.1 and BA.2 subvariants. Notably, no studies were conducted in any Latin American country or reported on the benefit of heterologous booster schemes or the order of immunity-generating events. Added value of this studyWe report the results of nation-wide study in Mexico of over 230,000 SARS-CoV-2 reinfections, with [~]90% occurring during periods of Omicron predominance. We identified that vaccination provided additional benefit on reducing the risk of SARS-CoV-2 reinfection, with the highest benefit observed in individuals with complete vaccination and booster protocols prior to primary infection or with primary infection during periods of BA.1 and BA.2 subvariant predominance. Hybrid immunity also provides a substantial reduction in the risk of reinfection-associated severe COVID-19, with >90% reduction in risk compared to unvaccinated individuals with previous SARS-CoV-2 infection, regardless of the order of immunity-generating events. Finally, heterologous COVID-19 booster schedules were associated with [~]11% and [~]54% lower risk for reinfection and reinfection-associated severe COVID-19 respectively, modified by time-elapsed since the last immunity-generating event. Implications of all the available evidenceOur results support that COVID-19 vaccination and boosters provide additional benefit to protect against SARS-CoV-2 reinfection and reinfection-associated severe COVID-19. The use of heterologous boosters appears to provide additional protection in previously infected individuals and such schemes may prove beneficial to increase vaccination coverage as newer, more transmissible variants emerge.

ABSTRACT

Objective: To evaluate the effects of the self-management program PARQVE in patients with severe knee osteoarthritis (KOA). Methods: Prospective randomized controlled clinical trial with 65 grade IV Kelgren & Lawrence (K&L) KOA patients who were allocated into groups: Control (CG) and Intervention (IG). Both groups received usual care. IG also participated in two days of multi-professional interventions about OA (causes and treatment) and received the program's DVD and book. Standing X-rays were obtained at inclusion and Ahlback's classification was registered. Western Ontario and McMaster Universities Index (WOMAC), Numerical Rating Scale (NRS), Lequesne, weight, and body mass index (BMI) were obtained at inclusion, and after 6, 12 and 24 months. Results: Groups were similar at baseline, despite higher WOMAC stiffness scores and a greater number of Ahlback's grade 4 and 5 in the IG. Only the IG improved WOMAC and total functions (p<0.001) during the study period above 12%, but did not reach the minimal clinically important difference of 20%. Best results were in one year. Non-significant improvements were observed without changes in body composition (P>0.05). Conclusions: Patients with severe KOA have mild to moderate function and quality of life improvement due to self-management program (PARQVE). Objetivo: Avaliar os efeitos do programa de autocuidado PARQVE em pacientes com osteoartrite grave de joelho (OAJ). Métodos: Ensaio clínico prospectivo randomizado controlado com 65 pacientes Kelgren & Lawrence (K&L) grau IV que foram alocados nos grupos: Controle (GC) e Intervenção (GI). Ambos os grupos receberam cuidados habituais. O IG também participou de dois dias de intervenções multiprofissionais sobre OA (causas e tratamento) e seus membros receberam o DVD e o livro do programa. Raios-X em pé foram obtidos na inclusão e a classificação de Ahlback foi registrada. Western Ontario e McMaster Universities Index (WOMAC), Escala de classificação numérica (ECN), Lequesne, peso e índice de massa corporal (IMC) foram obtidos na inclusão, e aos 6, 12 e 24 meses. Resultados: Os grupos eram semelhantes no início do estudo, apesar de maiores escores de rigidez WOMAC e um número maior de pacientes de Ahlback grau 4 e 5 no GI. Apenas o GI melhorou em WOMAC e função total (p <0,001) acima de 12% durante o período de estudo. Os melhores resultados foram após um ano. Melhorias não significativas foram observadas na composição corporal (P> 0,05). Conclusões: Pacientes com OAJ grave apresentam melhora leve a moderada de função e qualidade de vida pelo programa de autogerenciamento (PARQVE). ABSTRACT

We described the spatial and temporal dynamics of aquatic macrophytes in a Neotropical coastal estuarine river, and identified the negative effects associated to the presence and dominance of the invasive tanner grass. We compared macrophyte beds along the Guaraguaçu River (South Brazil) over four years, using taxonomic and functional dimensions. Biodiversity descriptors were higher in the driest periods compared to the rainiest, although this difference seems to be decreasing over the studied years. Moreover, the spatial organization of biodiversity and community structure slightly changed over time. Such spatial community stability was highlighted by the dominance of the highly invasive tanner grass. In beds dominated by the invasive species, the biodiversity was reduced. As aquatic macrophytes represent an important group in water bodies, the long-term loss of seasonal differences in community structuring is of concern, mainly regarding its potential to impact other groups and ecosystem functioning. By analysing data from standardized monitoring, we were able to identify a poorly discussed facet of biotic homogenisation-the seasonal homogenisation. We also discussed the impact of massive development of invasive species and its consequences for biodiversity in a Neotropical river of outmost importance for biological conservation. ABSTRACT

IntroductionGrowing data are demonstrating safety and immunogenicity of heterologous vaccination schemes against severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection. This strategy opens up the possibility of a shorter path towards the end of the pandemic. ObjectiveTo compare the homologous prime-boost vaccination scheme of Gam-COVID-Vac (Sputnik V, SpV) to its heterologous combination with mRNA-1273 (Moderna, Mod) vaccine. MethodsSARS-CoV-2 anti-spike (S)-receptor binding domain (RBD) IgG concentration was assessed three to seven weeks after complete vaccination. Reactogenicity was evaluated by declared side events and medical assistance required until day 7 post-boost. ResultsOf 190 participants enrolled, 105 received homologous SpV/SpV and the remaining heterologous SpV/Mod vaccination scheme, respectively. Median (interquartile range, IQR) age was 54 (37-63) years, 132 (69.5%) were female and 46 (24.2%) individuals had a prior confirmed COVID-19. Anti-S-RBD IgG median (IQR) titers were significantly higher for SpV/Mod [2511 (1476-3992) BAU/mL] than for SpV/SpV [582 (209-1609) BAU/mL, p<0.001] vaccination scheme. In a linear model adjusted for age, gender, time to the serological assay and time between doses, SpV/Mod [4.154 (6.585-615.554), p<0.001] and prior COVID [3.732 (8.641-202.010), p<0.001] were independently associated with higher anti-S-RBD IgG values. A higher frequency of mild-moderate adverse effects was associated with the heterologous scheme, although it was well tolerated by all individuals and no medical assistance was required. ConclusionThe heterologous SpV/Mod combination against SARS-CoV-2 is well tolerated and significantly increases humoral immune response as compared to the homologous SpV/SpV immunization.

ABSTRACT

BACKGROUNDVaccination has been effective in ameliorating the impact of COVID-19. However, estimation of vaccine effectiveness (VE) is still unavailable for some widely used vaccines and underrepresented groups. Here, we report on the effectiveness of a nation-wide COVID-19 vaccination program in Mexico. METHODSWe used a test-negative design within a national COVID-19 surveillance system to assess VE of the BNT162b2, mRNA-12732, Gam-COVID-Vac, Ad5-nCoV, Ad26.COV2.S, ChAdOx1 and CoronaVac vaccines, against SARS-CoV-2 infection, COVID-19 related hospitalization and death for adults [≥]18 years in Mexico. VE was estimated using Cox proportional hazard models considering time-varying vaccination status in partial and fully vaccinated individuals compared to unvaccinated adults, adjusted by age, sex, comorbidities and municipality. We also estimated VE for adults [≥]60 years, for cases with diabetes and comparing periods with predominance of variants B.1.1.519 and B.1.617.2. RESULTSWe assessed 793,487 vaccinated compared to 4,792,338 unvaccinated adults between December 24th, 2020, and September 27th, 2021. VE against SARS-CoV-2 infection was highest for fully vaccinated individuals with mRNA-12732 (91.5%, 95%CI 90.3-92.4) and Ad26.COV2.S (82.2%, 95%CI 81.4-82.9), whereas for COVID-19 related hospitalization were BNT162b2 (84.3%, 95%CI 83.6-84.9) and Gam-COVID-Vac (81.4% 95%CI 79.5-83.1) and for mortality BNT162b2 (89.8%, 95%CI 89.2-90.2) and mRNA-12732 (93.5%, 95%CI 86.0-97.0). VE for all evaluated vaccines was reduced for adults [≥]60 years, people with diabetes, and in periods of Delta variant predominance. CONCLUSIONSAll evaluated vaccines were effective against SARS-CoV-2 infection and COVID-19 related hospitalization and death. Mass vaccination campaigns with multiple vaccine products are feasible and effective to maximize vaccination coverage.

Reconstructing the life histories of extinct hominins remains one of the main foci of paleoanthropological inquiry, as an extended juvenile period impacts the social and cognitive development of species. However, the paucity of hominin remains, the lack of comparative hominoid data, and the destructive nature of many life history approaches have limited our understanding of the relationship between dental development (eruption) and weaning in primates. Alternatively, the rate of dental wear in early-forming teeth has been suggested a good proxy for the timing of weaning. Here we test this hypothesis on an ontogenetic series of Gorilla gorilla gorilla and Pan troglodytes troglodytes, using geographic information systems-based shape descriptors of M1s in relation to the nitrogen (δ15N) and carbon (δ13C) isotope composition of their associated hair. Results show that Gorilla g. gorilla are fully weaned considerably later than Pan t. troglodytes, that is, after M1s had been in full functional occlusion for some time. Yet, throughout ontogeny, gorilla dental wear rates are greater than they are in chimpanzees. This refutes the hypothesis that the rates of wear of early-forming teeth inform the time of weaning (i.e., nutritional independence). Instead, dietary breadth and seasonal variation in resource availability are implicated. This finding has implications for interpreting the hominin fossil record and raises questions about the triggers for, and the mechanisms of, life history change in hominin evolution. As a case in point, commonalities in life history patterns between early hominins and Western lowland gorillas seem to be a means to mitigate the effects of recurrent (i.e., seasonal) resource limitations and-conceivably-to prevent high infant mortality rates. Taken further, difference between hominid life histories are likely to be of degree, not kind.

Western Ontario and McMaster Universities Index (WOMAC), Numerical Rating Scale (NRS), Lequesne, weight, and body mass index (BMI) were obtained at inclusion, and after 6, 12 and 24 months. Results: Groups were similar at baseline, despite higher WOMAC stiffness scores and a greater number of Ahlback's grade 4 and 5 in the IG. Only the IG improved WOMAC and total functions (p<0.001) during the study period above 12%, but did not reach the minimal clinically important difference of 20%. Best results were in one year. Non-significant improvements were observed without changes in body composition (P>0.05). Conclusions: Patients with severe KOA have mild to moderate function and quality of life improvement due to self-management program (PARQVE). RESUMO Objetivo: Avaliar os efeitos do programa de autocuidado PARQVE em pacientes com osteoartrite grave de joelho (OAJ). Métodos: Ensaio clínico prospectivo randomizado controlado com 65 pacientes Kelgren & Lawrence (K&L) grau IV que foram alocados nos grupos: Controle (GC) e Intervenção (GI). Ambos os grupos receberam cuidados habituais. O IG também participou de dois dias de intervenções multiprofissionais sobre OA (causas e tratamento) e seus membros receberam o DVD e o livro do programa. Raios-X em pé foram obtidos na inclusão e a classificação de Ahlback foi registrada. Western Ontario e McMaster Universities Index (WOMAC), Escala de classificação numérica (ECN), Lequesne, peso e índice de massa corporal (IMC) foram obtidos na inclusão, e aos 6, 12 e 24 meses. Resultados: Os grupos eram semelhantes no início do estudo, apesar de maiores escores de rigidez WOMAC e um número maior de pacientes de Ahlback grau 4 e 5 no GI. Apenas o GI melhorou em WOMAC e função total (p <0,001) acima de 12% durante o período de estudo. Os melhores resultados foram após um ano. Melhorias não significativas foram observadas na composição corporal (P> 0,05). Conclusões: Pacientes com OAJ grave apresentam melhora leve a moderada de função e qualidade de vida pelo programa de autogerenciamento (PARQVE). ABSTRACT Introduction With the increasing number of cases related to Diabetes Mellitus (DM), glycemic control through laboratory methods or rapid tests is essential. Objective To analyze the correlation of three glucose determination methodologies (Glucometer, laboratory analysis and with point of care artificial intelligence equipment). Method Blood samples from the digital pulp and venous blood from the antecubital fossa were collected from 20 volunteers of different ages and sex. Blood glucose measurements were determined by the 3 methodologies mentioned above. Result Spearmans correlation analysis carried out between all types of tests shows that there is a strong and statistically significant positive correlation, indicating the compatibility of results regardless of the method applied. Conclusion The methodologies are correlated, however, the average values?? obtained by artificial intelligence were 40% higher, which can impact the clinical interpretation of results.


RESUMO Introdução Com o número crescente de casos relacionados a Diabetes Mellitus (DM), é indispensável o controle glicêmico através de métodos laboratoriais ou testes rápidos. Objetivo Analisar a correlação de três metodologias de determinação de glicose (Glicosímetro, análise laboratorial e com o equipamento com inteligência artificial de point of care). Método Foram coletadas amostras de sangue da polpa digital e sangue venoso da fossa antecubital de 20 voluntários de diferentes idades e sexo. Dosagens de glicose sanguínea foram determinadas pelas 3 metodologias acima citadas. Resultado A análise de correlação de Spearman realizada entre todos os tipos de testes mostra que há uma correlação positiva forte e estatisticamente significante, indicando a compatibilidade de resultados independentemente do método aplicado. Conclusão As metodologias apresentam correlação, no entanto, os valores médios obtidos pela inteligência artificial mostraram-se 40% mais elevada que pode impactar na interpretação clínica dos resultados.

Objetivo: avaliar a capacidade funcional dos idosos longevos da zona urbana de um município do Estado do Amazonas.Método: Estudo descritivo, transversal e quantitativo, realizado com 135 idosos longevos cadastrados nas Unidades Básicas de Saúde em um município do Amazonas, no período de dezembro de 2019 a fevereiro de 2020. Resultados: Os resultados demonstraram predominância do sexo feminino (61,5%) e a faixa etária entre 80 a 89 (79,3%). Com relação as Atividades Básicas, verificou-se um maior frequência de independência nas eliminações intestinais (88, 1 %) e menor para dependência para escada (38,5%). Quanto às atividades instrumentais, houve uma alta independência por parte dos idosos na administração do consumo de remédios (51, 1 %), enquanto a dependência total foi maior para lavar e passar roupas (44,4%). Conclusão: avaliar a capacidade funcional dos idosos permite um conhecimento sobre seus impasses na realização de suas necessidades básicas e mostra as dificuldades enfrentadas por eles.(AU)


Objective: to evaluate the functional capacity of older elderly people in the urban area of a municipality in theState of Amazonas. Method: Descriptive, cross-sectional and quantitative study, conducted with 135 long-lived elderly registered in Basic Health Units in a municipality in Amazonas, from December 2019 to February 2020. Results: The results showed a predominance of females (61.5 %) and the age group between 80 and 89 (79.3%). Regarding the Basic Activities, there was a higher frequency of independence in intestinal eliminations (88.1 %) and a lower frequency for dependence on stairs (38.5%). As for instrumental activities, there was a high degree of independence on the part of the elderly in administering medication consumption (51.1 %), while total dependence was greater for washing and ironing clothes (44.4%). Conclusion: evaluating the functional capacity of the elderly allows a better understanding of their impasses in meeting their basic needs and shows the difficulties faced them.(AU)

ABSTRACT

Patients with cancer are at higher risk of severe coronavirus infectious disease 2019 (COVID-19), but the mechanisms underlying virus-host interactions during cancer therapies remain elusive. When comparing nasopharyngeal swabs from cancer and non-cancer patients for RT-qPCR cycle thresholds measuring acute respiratory syndrome coronavirus-2 (SARS-CoV-2) in 1063 patients (58% with cancer, 89% COVID-19+), we found that malignant disease favors the magnitude and duration of viral RNA shedding concomitant with prolonged serum elevations of type 1 IFN that anticorrelated with anti-RBD IgG antibodies. Chronic viral RNA carriers exhibited the typical immunopathology of severe COVID-19 at the early phase of infection including circulation of immature neutrophils, depletion of non-conventional monocytes and a general lymphopenia that, however, was accompanied by a rise in plasmablasts, activated follicular T helper cells, and non-naive Granzyme B+ FasL+, EomehighTCF-1high, PD-1+CD8+ Tc1 cells. Virus-induced lymphopenia worsened cancer-associated lymphocyte loss, and low lymphocyte counts correlated with chronic SARS-CoV-2 RNA shedding, COVID-19 severity and a higher risk of cancer-related death in the first and second surge of the pandemic. Lymphocyte loss correlated with significant changes in metabolites from the polyamine and biliary salt pathways as well as increased blood DNA from Enterobacteriaceae and Micrococcaceae gut family members in long term viral carriers. We surmise that cancer therapies may exacerbate the paradoxical association between lymphopenia and COVID-19-related immunopathology, and that the prevention of COVID-19-induced lymphocyte loss may reduce cancer-associated death.

ABSTRACT

BackgroundReports analyzing drug overdose (OD) mortality data during the COVID-19 pandemic are limited. Outcomes across states are heterogenous, necessitating assessments of associations between COVID-19 and OD deaths on a state-by-state level. This report aims to analyze trends in OD deaths in Massachusetts during COVID-19. MethodsAnalyzing 3,924 death records, we characterize opioid-, cocaine-, and amphetamine-involved OD mortality and substance co-presence trends from March 24-November 8 in 2020 as compared to 2018 and 2019. ResultsOD deaths involving amphetamines increased by 85% from 2019 to 2020 (61 vs. 113; P<0.001) but were steady from 2018 to 2019. Heroins presence continued to decrease (341 in 2018, 247 in 2019, 157 in 2020; P<0.001); however, fentanyl was present in more than 85% of all OD deaths across all periods. Among OD deaths, alcohol involvement consistently increased, present in 250 deaths in 2018, 299 in 2019 (P=0.02), and 350 in 2020 (P=0.04). In 2019, 78% of OD decedents were White and 7% were Black, versus 73% and 10% in 2020 (P=0.02). ConclusionIncreased deaths involving stimulants, alcohol, and fentanyl reflect concerning trends in the era of COVID-19. Rising OD death rates among Black residents underscore that interventions focused on racial equity are necessary.

ABSTRACT

ObjectiveThe spike protein of SARS-CoV-2 has become the main target for antiviral and vaccine development. Despite its relevance, there is scarce information about its evolutionary traces. The aim of this study was to investigate the diversification patterns of the spike for each clade of SARS-CoV-2 through different approaches. MethodsTwo thousand and one hundred sequences representing the seven clades of the SARS-CoV-2 were included. Patterns of genetic diversifications and nucleotide evolutionary rate were estimated for the spike genomic region. ResultsThe haplotype networks showed a star shape, where multiple haplotypes with few nucleotide differences diverge from a common ancestor. Four hundred seventy nine different haplotypes were defined in the seven analyzed clades. The main haplotype, named Hap-1, was the most frequent for clades G (54%), GH (54%), and GR (56%) and a different haplotype (named Hap-252) was the most important for clades L (63.3%), O (39.7%), S (51.7%), and V (70%). The evolutionary rate for the spike protein was estimated as 1.08 x 10-3 nucleotide substitutions/site/year. Moreover, the nucleotide evolutionary rate after nine months of pandemic was similar for each clade. ConclusionsIn conclusion, the present evolutionary analysis is relevant since the spike protein of SARS-CoV-2 is the target for most therapeutic candidates; besides, changes in this protein could have consequences on viral transmission, response to antivirals and efficacy of vaccines. Moreover, the evolutionary characterization of clades improves knowledge of SARS-CoV-2 and deserves to be assessed in more detail since re-infection by different phylogenetic clades has been reported.

Justificativa e Objetivos: os hospitais são locais propícios para a instalação e propagação de insetos, especialmente formigas. Essas, além da sua capacidade adaptativa, vivem em mutualismo com outros animais, como fungos e bactérias, o que confere risco elevado para infecções nosocomiais. O presente estudo teve como objetivo identificar a microbiota bacteriana associada com formigas intra-hospitalares na cidade de Anápolis, Goiás, e discutir o papel de tais agentes no desenvolvimento de infecções hospitalares e o consequente risco para indivíduos hospitalizados. Métodos: foram montadas armadilhas para formigas em dois hospitais da cidade de Anápolis a fim de capturá-las nos setores de enfermaria, unidade de terapia intensiva/semi-intensiva e nutrição. As armadilhas eram deixadas por um período pré-determinado nos respectivos setores e depois eram levadas ao Laboratório de Microbiologia da UniEvangélica para cultivo, semeadura e identificação bacteriana. Resultados: foram realizadas três coletas em cada um dos setores de cada instituição hospitalar. Foi possível isolar os seguintes microrganismos: Staphylococcus spp., bacilos Gram-positivos, Klebsiella ozaenae, K. rhinoscleromatis, Escherichia coli e Yersinia pseudotuberculosis. Conclusão: pode-se concluir que as formigas podem atuar como veículos para microrganismos. Esse fato sugere que podem favorecer o processo de infecção em usuários de assistência hospitalar. Entretanto, permanece incerto a relação entre população de formigas e incidência de infecções nos hospitais, sendo necessário realizar estudos para associar tais variáveis.(AU)


Background and Objectives: hospitals are prone environments for the establishment and spread of insects, especially ants. In addition to their adaptive capacity, ants live in mutualism with other living beings such as fungi and bacteria, which increases the risk of nosocomial infections. This study aimed to identify the bacterial microbiota associated with intrahospital ants in the city of Anápolis, Goiás, and to discuss the role of such agents in the development of nosocomial infections and consequent risk for hospitalized individuals. Methods: ant traps were set up in two hospitals in Anápolis to capture them in the ward sectors as well as the intensive or semi-intensive care units and the nutrition sectors. The traps were left for a predetermined period in the respective locations and were then taken to the UniEvangélica Microbiology Laboratory for culture, sowing and bacterial identification. Results: three collections were performed in each of the hospital sectors of each hospital institution. The following microorganisms could be isolated: Staphylococcus spp., Gram-positive bacilli, Klebsiella ozaenae, K. rhinoscleromatis, Escherichia coli and Yersinia pseudotuberculosis. Conclusion: we can conclude that ants can act as vessels for microorganisms. This fact suggests that ants may favor infections in the hospitals. However, the relationship between ant population and incidence of infections in hospitals remains uncertain, and studies are necessary to associate these variables.(AU)

Además de su capacidad de adaptación, estos animales viven en mutualismo con otros, como los hongos y las bacterias, lo que confiere un alto riesgo de infecciones nosocomiales en los humanos. El presente estudio tuvo como objetivo identificar la microbiota bacteriana asociada con hormigas intrahospitalarias en la ciudad de Anápolis, Goiás, y analizar el papel de dichos agentes en el desarrollo de infecciones hospitalarias y el riesgo para las personas hospitalizadas. Métodos: se instalaron trampas para hormigas en dos hospitales de la ciudad de Anápolis para capturarlas en los sectores de enfermería, unidades de cuidados intensivos, de cuidados semiintensivos y nutrición. Las trampas se dejaron durante un período predeterminado en los sectores respectivos y después se las llevaron al Laboratorio de Microbiología UniEvangélica para su cultivo, siembra e identificación bacteriana. Resultados: se realizaron tres colectas en cada uno de los sectores hospitalarios de cada institución hospitalaria. Se pudieron aislar los siguientes microorganismos: Staphylococcus spp., bacilos Gram positivos, Klebsiella ozaenae, K. rhinoscleromatis, Escherichia coli y Yersinia pseudotuberculosis. Conclusiones: se concluye que las hormigas pueden servir como vehículos de microorganismos. Esto sugiere que pueden favorecer el proceso de infección a los usuarios de atención hospitalaria. Sin embargo, la relación entre la población de hormigas y la incidencia de infecciones en los hospitales sigue siendo incierta, y se necesitan más estudios para asociar estas variables.(AU)

ABSTRACT

The SARS-COV-2 pandemic has put pressure on Intensive Care Units, and made the identification of early predictors of disease severity a priority. We collected clinical, biological, chest CT scan data, and radiology reports from 1,003 coronavirus-infected patients from two French hospitals. Among 58 variables measured at admission, 11 clinical and 3 radiological variables were associated with severity. Next, using 506,341 chest CT images, we trained and evaluated deep learning models to segment the scans and reproduce radiologists annotations. We also built CT image-based deep learning models that predicted severity better than models based on the radiologists reports. Finally, we showed that adding CT scan information--either through radiologist lesion quantification or through deep learning--to clinical and biological data, improves prediction of severity. These findings show that CT scans contain novel and unique prognostic information, which we included in a 6-variable ScanCov severity score.
