ABSTRACT

The literature concerning the problem of "white coat" hypertension is reviewed. The diagnosis of such hypertension is important for valid treatment policy in hypertensive patients. This diagnosis is based primarily on the data of 24-h blood pressure monitoring.

ABSTRACT

AIM: To assess effectiveness of ACE inhibitor quinapril in patients with moderate cardiac failure of ischemic etiology. MATERIAL AND METHODS: 20 patients received quinapril in a daily dose 20 mg for 6 months. Clinical condition, exercise tolerance, functional and geometric parameters of the left ventricle, Holter monitoring, heart rhythm variability, late potentials were studied before and after the treatment. RESULTS: The treatment relieved symptoms significantly, exercise tolerance increased, positive changes occurred in systotic, diastotic functions and left ventricular geometry. Ischemic episodes, supraventricular extrasystoles reduced in number and duration. CONCLUSION: Quinapril is an effective drug for treatment of moderate ischemic heart failure.

ABSTRACT

Effectiveness of a prolonged form of diltiazem was studied in 20 patients with stable effort angina and chronic cardiac failure (NYHA functional class II). Dilcem-retard was given in a dose 180 mg/day for 6 months. Clinical condition, exercise tolerance, left ventricular functional and geometric parameters, heart rhythm variability, late potentials, Holter monitoring evidence were assessed before and after the treatment. Dilcem-retard improved the patients' condition, exercise tolerance, promoted normalization of left ventricular diastolic function, had no negative influence on cardiac output, reduced occurrence of sympathicotonia and late potentials.

ABSTRACT

Results of treatment of 129 patients with dysphagia recurrency after operative treatment of cardiospasm and achalasia of cardia for the 1970-2000 yrs period were analyzed. There were operated 52 (40.3%) of patients, 77--were treated conservatively. The mainly applied methods of operation were those elaborated in the clinic: reoesophagocardiomyotomy with incomplete fundoplication (in 28 observations) and extirpation of the eosophagus with plasty using gastric tube (in 18). Positive result, noted in majority of observations, witnessed the correctness of substantiation of indications and choice of operative intervention in these patients.

ABSTRACT

123 patients with diffuse esophagospasm were observed, including 56 (45.5%) with primary and 67 (55.5%) with secondary esophagospasm. Since 1986 due to unsatisfactory treatment results an original therapeutic complex including spasmolytics, neuroleptics, vitamins B, was used. In secondary esophagospasm therapy was complemented by pneumocardiodilatation, not more 5 sessions per course. Good and satisfactory clinical results were achieved in 30 (66.7%) patients with primary diffuse esophagospasm and in 45 (67.2%) patients with secondary esophagospasm.

ABSTRACT

Glycosylated hemoglobin levels were measured in dogs with experimental diabetes mellitus over two months after hyperglycemia onset. Diabetes was induced by pancreas resection combined with intraarterial alloxan infusion. Modified isoelectrofocusing in PAAG plates was used for glycosylated hemoglobin measurements. Its concentration was found to increase by 55% 4 weeks after diabetes development and to persist growing, making up 183% of its initial level in 2 months. The suggested method of isoelectric focussing in PAAG plates may be used to monitor glycemia in diabetes.

ABSTRACT

The levels of HbAIc, malonic dialdehyde (MDA), sugar and insulin were measured in the blood of dogs with experimental diabetes mellitus. HbAIc increased from postoperative week 4 to week 8 of the observation period. MDA levels stood high for postoperative week 1, returned to the baseline, rerose on week 4 and remained high till week 8. Hyperglycemia was observed through the whole experiment.

ABSTRACT

A study was made of the indices of blood sugar on an empty stomach, GTT, blood insulin, body mass and the general status in 17 mongrels during one month after various types of resection of the pancreas: the 1st group--resection (80%), the 2nd group--resection (68%), the 3rd group--resection combined with intraarterial administration of alloxan. Insulin was determined using a standard kit RIO-INS-PG-125I by a radioimmunoassay. Alloxan was injected during operation at a dose of 250 mg (5% solution) in the upper pancreatoduodenal artery. Stable hyperglycemia, diabetogenic disturbances in GTT, a decrease in the blood level of insulin, and a clinical picture of diabetes mellitus were observed after operation in the 1st and 3rd groups. A new model of experimental diabetes mellitus in dogs (a combination of pancreas resection with intraarterial administration of alloxan) was recommended.

ABSTRACT

The work presents results of the investigation of the content of sulfhydryl groups in the blood of patients with fractures of long tubular bones. The content of sulfhydryl groups of blood was studied in 168 patients with open fractures. The author comes to a conclusion that the investigation of sulfhydryl groups of blood allows not only facilitating the evaluation of severity of patients in the pre- and postoperative period and detecting complications at early terms but also enables a timely correction of the disorders to be fulfilled.
