ABSTRACT

Introduction: Dysregulated eating (emotional eating, cue-elicited eating, and dietary restraint and restriction) has been linked to being overweight or obese. The present investigation used a random controlled trial (RCT) to test the differential efficacy of remotely delivered Mindfulness-Based Eating Awareness Training (MB-EAT) and Behavioral Weight Loss (BWL) counseling. Methods: The sample was recruited through advertisements that offered help to people "with problems controlling their eating" or "interested in improving their relationship with food" (n = 135). Results: Retention was low in both groups (42%), but not dissimilar to retention rates reported in related clinical trials delivered "in person." Among the participants who completed treatment, we found no between-group differences in any of the treatment outcomes, but participants in both groups experienced significant increases in eating-related mindfulness [Mindful Eating Questionnaire (MEQ) and awareness [Multidimensional Assessment of Interoceptive Awareness (MAIA), and significant decreases in unhealthy eating patterns [Dutch Eating Behavior Questionnaire (DEBQ); Binge Eating Scale (BES), and weight over the course of treatment. Participants in both groups also experienced increases in self-reported depression and anxiety symptoms [Hospital Anxiety and Depression Scale (HADS)], although these increases likely reflected normative changes observed in the population at large during COVID-19. Discussion: Overall, the results suggest that dysregulated eating and weight loss intervention delivered remotely via teleconference can be effective.

ABSTRACT

BACKGROUND: Glioblastoma (GBM) is the most common and most aggressive primary malignant brain tumour. The standard of care is surgical resection, followed by radiotherapy with concurrent and adjuvant temozolomide. In Latin America, there is scarcity of information about the incidence of GBM and even less data regarding outcomes. In this study, we describe the clinicopathologic features, management and outcomes of GBM patients. METHODS: We describe a single-centre multidisciplinary team experience in managing GBM patients over an 11-year period (Jan 2005 to Dec 2016). Pathology was reviewed by the pathology collaborator and retrospective chart review performed for treatment and clinical outcomes. RESULTS: We identified 74 patients (50 males) with diagnosis of GBM. Median age at diagnosis was 58 years (range 24-79 years), and median Karnofsky performance status was 80%. Forty-three (58.1%) went to gross total resection, 20 (27%) partial resection and 11 (14.9%) biopsy. Sixty-four (87%) patients received Stupp regimen. The median overall survival (OS) was 13.9 months (standard error (SE) 1.71; 95% confidence interval (CI), 10.56-17.23). In patients treated according to Stupp regimen, the progression-free survival (PFS) was 10 months (SE 1.8; 95% CI, 6.481-13.519), the selfcare survival was 11.8 months (SE 1.61; 95% CI, 8.632-14.968) and the OS was 16.1 months (SE 1.53; 95% CI, 13.01-19.099). CONCLUSIONS: This study reports the most complete analysis of epidemiology, clinical management and outcomes of patients with diagnosis of GBM in Chile treated with Stupp regimen. The PFS and OS are consistent with reports of US and Europe.

Las intervenciones en mindful eating están siendo cada vez más utilizadas para tratar problemáticas relacionadas con la alimentación. En población con sobrepeso y obesidad esta herramienta busca disminuir los síntomas de atracón, mejorar la regulación emocional y conciencia de los estados internos de hambre y saciedad. Objetivo: conocer la valoración y aprendizajes atribuidos al taller Mindful Eating en un grupo de mujeres en tratamiento de reducción de peso en un centro de salud privado. Método: enfoque metodológico cualitativo, de alcance exploratorio y descriptivo. Para recolectar los datos se utilizaron entrevistas en profundidad semi- estructuradas a 7 mujeres entre 22-54 años, analizadas desde el modelo de la Grounded Theory. Resultados: a partir del taller hubo cambios en el modo de relacionarse con la comida y el sí mismas. Antes del taller las participantes tenían una relación inconsciente con la comida; comer en piloto automático, comer emocional, atracones y modo de relacionarse consigo mismas desde la autocrítica y autocastigo. Posterior al taller, se describen cambios favorables en una relación consciente con la comida, destacándose mayor conexión interna, autocuidado y modo de relacionarse sí mismas autocompasivo. Discusión: las participantes tuvieron una valoración positiva del taller mindful eating, apreciando el rol contendor del grupo, ser una herramienta de utilidad y uso a largo plazo que posibilitaría un cambio hacia una relación más saludable con la comida y si mimas. Sería útil integrar esta herramienta en los programas de control de peso, monitoreando y reforzando continuamente los aprendizajes que los participantes hayan obtenido en el taller. Esta integración podría ayudar a los participantes a continuar utilizando herramienta y, al hacerlo, ayudarlos a integrar hábitos alimenticios más saludables a largo plazo.


Summary Mindful eating interventions are being increasingly used to treat eating behavior problems. In overweight and obese population this tool seeks to reduce symptoms of binge eating, improve emotional regulation, and increase awareness of hunger and satiety.Aim of the study: to investigate how a group of women in weight reduction treatment assessed a mindful eating workshop and what were the main learnings they had from the program. Method: qualitative approach with an exploratory and descriptive scope. Data were collected through semi-structured in-depth interviews. Participants were 7 women between 22-54 years old.  The data analysis was carried out using Grounded Theory model. Results: participants in the workshop reported changes in the way they relate to the food and themselves. Before the workshop, the participants reported not being aware of their relationship to the food, eating on autopilot, eating emotionally, bingeing, and being self-critical and punishing themselves. After the workshop, participants reported changes, experiencing increased awareness of their relationship with food, greater connection to the inner-self, and increased self-care and self-compassion. It would be useful to integrate this tool in weight control treatments, monitoring and reinforcing the learnings participants have from the workshop. Having this integration might help participants to continuing using the tool and, by doing that, helping them to integrate healthier eating habits in the long term.

ABSTRACT

The 2016 WHO Classification of Tumours of the Central Nervous System incorporates a new diagnostic entity: the mutant diffuse midline glioma H3K27, a tumor with a characteristic location and special molecular biology. We report the case of a 51-year-old male patient with progressive diplopia. The imaging study showed a mesencephalic tumor; the stereotacic biopsy disclosed an Anaplastic Astrocytoma Isocitrate dehydrogenase (IDH) wild type. The molecular study concludes H3K27 mutation. The patient was treated with radiotherapy with concurrent and adjuvant chemotherapy (temozolomide) with partial recovery of the diplopia.

ABSTRACT

Resumen: La imagen corporal es un tópico relevante en el desarrollo de la identidad durante la adolescencia, donde los factores sociales son decisivos en la determinación de las expectativas individuales. Este estudio describe y analiza la percepción que mujeres adolescentes tienen acerca de la influencia de los aspectos socioculturales en su imagen corporal. El enfoque de investigación fue cualitativo, exploratorio y descriptivo, basado en la teoría fundamentada. Se entrevistó a cinco mujeres adolescentes de entre 16 y 18 años de edad. Los resultados muestran que existe una experiencia de la imagen corporal caracterizada por sentimientos ambivalentes y diferentes grados de insatisfacción corporal. En general, las participantes sobrevaloran la importancia del aspecto físico y, específicamente, la delgadez es considerada signo de atractivo, éxito y perfección; prototipo estético que favorece la insatisfacción corporal. Los parámetros socioculturales percibidos como más influyentes fueron los medios masivos de comunicación, especialmente las redes sociales digitales, que superan a los pares y a la familia. Se concluye que el fortalecimiento de la capacidad de juicio crítico para discernir entre los ideales corporales y los cuerpos reales podría favorecer la prevención de problemas alimentarios en las mujeres adolescentes.


Abstract: Body image is a relevant topic in the development of identity during the adolescence, where social factors are significant in determining individual expectations. This study aims to describe and analyze the perception that female adolescents have about the influence of sociocultural factors on their own body image. This research has a qualitative, exploratory and descriptive approach based on the Grounded Theory. Five adolescent girls aged between 16 and 18 years were interviewed. Results show that body image is characterized by ambivalent feelings and different degrees of body dissatisfaction. In general, participants overvalue physical aspects, for instance, thinness is believed as a sign of attractiveness, success and perfection, prototype linked to body dissatisfaction. The more influential sociocultural parameters were mass media, especially online social networks, peers and family. It is concluded that strengthening critical judgment skills to distinguish between the ideal body and the real body could enhance the prevention of eating disturbances in adolescent women.

ABSTRACT

PURPOSE: The objective of this study was to determine whether using mesenchymal stem cells (MSC) seeded in a collagen type I scaffold would be sufficient to regenerate the torn anterior cruciate ligament (ACL). METHODS: Anterior cruciate ligament transection was performed on both knees in 10 New Zealand rabbits and then repaired with as follows: suture alone (suture-treated group, n = 6), suture associated with collagen type I scaffold (collagen type I scaffold-treated group, n = 8) or suture associated with autologous MSC seeded on collagen type I scaffold (MSC/collagen type I scaffold-treated group, n = 6). At 12-week post-intervention, the animals were killed and the ACLs were characterised macroscopically and histologically. Data of the 3 groups were against normal ACL (normal group, n = 10). RESULTS: Macroscopic observation found that in MSC/collagen type I scaffold group, 33% of specimens showed a complete ACL regeneration, with a tissue similar to the normal ACL. Regeneration was not observed in the group treated with suture alone or associated with collagen type I scaffold without cells. In the latter, only a reparative attempt at the ends was observed. Histological analysis of the regenerated ACL showed a tissue with organised collagen and peripheric vessels. CONCLUSIONS: These results provide evidence that the use of MSC seeded in a collagen type I scaffold in the treatment of ACL injuries is associated with an enhancement of ligament regeneration. This MSC-based technique is a potentially attractive tool for improving the treatment of ACL ruptures.

ABSTRACT

OBJECTIVE: The aim of this study was to evaluate the contribution to hyaline cartilage regeneration of dexamethasone intraarticular administration after autologous mesenchymal stem cells (MSCs) implantation into a preestablished knee full-thickness chondral defect. DESIGN: Full-thickness chondral defects of 4.5 × 4.5 mm(2) were surgically made in both medial femoral condyles of adult male New Zealand rabbits. Two weeks later, autologous ex vivo expanded bone marrow-derived MSCs were embedded in hyaluronic acid and implanted into the chondral defects. Immediately and every week after the intervention, dexamethasone 0.25 mg/kg was intraarticularly administered (MSC/dexa-treated group). Six weeks after MSC transplantation, the animals were euthanized and condyles were characterized molecularly according to aggrecan, collagen type II, and collagen type I gene expression (quantitative reverse transcriptase-polymerase chain reaction) and histologically (hematoxylin-eosin staining). Data of MSC/dexa-treated condyles were compared with untreated, dexa-treated, MSC-treated, or normal unlesioned condyles. RESULTS: The ratio between collagen type II expression versus collagen type I expression in MSC/dexa-treated condyles was higher than one, even though the group mean value was not statistically different from that of untreated defects. Histological changes were observed between MSC/dexa-treated and untreated defects mainly in surface regularity and in hyaline matrix abundance. However, International Cartilage Repair Society score analysis did not support robust differences between those groups. CONCLUSION: Intraarticular administration of dexamethasone after autologous MSC implantation into a preestablished full-thickness chondral defect does not contribute significantly to the regeneration of a tissue with molecular and histological characteristics identical to hyaline cartilage.

OBJECTIVE: The aim of this study was to evaluate the contribution to hyaline cartilage regeneration of the microfracture (MFx) technique plus intraarticular betamethasone (BMS) or platelet-rich plasma (PRP). DESIGN: Full-thickness chondral defects of 3 × 6 mm(2) were surgically performed in both femoral condyles of each knee in 13 New Zealand rabbits and then treated with MFx associated with intraarticular BMS or PRP. At 12 weeks postimplantation, the animals were killed and the condyles were characterized macroscopically, molecularly according to collagen type II and I gene expression (quantitative reverse transcriptase-polymerase chain reaction), and histologically (hematoxylin-eosin staining). For the latter, samples were scored using the International Cartilage Repair Society visual histological scale. Data of MFx/BMS-treated and MFx/PRP-treated condyles were compared against untreated, MFx-treated, or normal condyles without lesions. RESULTS: Our macroscopic findings showed that in MFx/BMS-treated and MFx/PRP-treated groups, the defects were filled with an irregular, partially rough tissue similar to the MFx-treated group. No differences in the ratio between collagen type II versus collagen type I expression were observed among groups. Histological changes were observed between MFx/BMS-treated and MFx/PRP-treated groups versus untreated defects mainly in surface regularity and cell distribution. However, International Cartilage Repair Society score analysis did not support statistical differences between MFx/BMS-treated and MFx/PRP-treated groups versus MFx-treated group. CONCLUSIONS: These results provide evidence that the use of intraarticular BMS or PRP as coadjuvants to the microfracture technique in the treatment of acute chondral lesions is not associated with a significant improvement of hyaline cartilage regeneration.

We report a 47-year-old woman presenting with dysphagia. A chest CAT scan and barium swallow showed an endoluminal mass that extended to four vertebrae. An endoluminal sonography localized the pediculum of the mass, that was excised endoscopically. The pathological study disclosed a fibrovascular polyp. After 18 months of follow up, the patient is asymptomatic.

ABSTRACT

Despite well-established histopathological features and the development of immunostaining of human neoplasms, there are a number of cases in which surgical pathologists cannot assure the origin of synchronous and metachronous tumors. In many cases, the classification of these lesions as either two separate primary tumors or as a single primary tumor with a metastasis has significant implications with respect to patient prognosis and recommendations for therapy. To establish the origin of tumors, we assessed tumor cell clonality using PCR-based microsatellite analysis on microdissected archival tissues for loss of heterozygosity (LOH) and microsatellite instability (MSI) in a series of 19 paired synchronous and metachronous tumors from several organs. As a control group, 15 autopsy cases with an unequivocally recognizable primary tumor and associated metastases were also examined. Based on LOH and MSI findings, and using a panel of 4 to 12 (median 7) microsatellite markers, we were able to establish the clonal pattern of microsatellite changes in 17 out of 19 (89%) biopsy cases and thus determine if they were either double primary tumors (41%) or metastases (59%). Of interest, identical or similar pattern of microsatellite abnormalities were detected in 15 primary tumors and corresponding metastasis from autopsies. Our results indicate that microsatellite analysis for LOH and MSI, as an expression of clonality, provides a useful tool to distinguish double primary neoplasms and metastases in synchronous and metachronous tumors.
