ABSTRACT

Over the last quarter century, increasing honey bee colony losses motivated standardized large-scale surveys of managed honey bees (Apis mellifera), particularly in Europe and the United States. Here we present the first large-scale standardized survey of colony losses of managed honey bees and stingless bees across Latin America. Overall, 1736 beekeepers and 165 meliponiculturists participated in the 2-year survey (2016-2017 and 2017-2018). On average, 30.4% of honey bee colonies and 39.6% of stingless bee colonies were lost per year across the region. Summer losses were higher than winter losses in stingless bees (30.9% and 22.2%, respectively) but not in honey bees (18.8% and 20.6%, respectively). Colony loss increased with operation size during the summer in both honey bees and stingless bees and decreased with operation size during the winter in stingless bees. Furthermore, losses differed significantly between countries and across years for both beekeepers and meliponiculturists. Overall, winter losses of honey bee colonies in Latin America (20.6%) position this region between Europe (12.5%) and the United States (40.4%). These results highlight the magnitude of bee colony losses occurring in the region and suggest difficulties in maintaining overall colony health and economic survival for beekeepers and meliponiculturists.

ABSTRACT

PURPOSE: The study aimed to explore the role of parenthood at first episode of psychosis (FEP) on recovery, with a focus on potential sex differences. METHODS: Sociodemographic, clinical, and neurocognitive information was considered on 610 FEP patients form the PAFIP cohort (Spain). Baseline and three-year follow-up comparisons were carried out. Chi-square tests and ANCOVA analysis were performed controlling for the effect of age and years of education. RESULTS: Men comprised 57.54% of the sample, with only 5.41% having offspring when compared to 36.29% of women. Parenthood was related to shorter duration of untreated illness (DUI) in women with children (12.08 months mothers vs. 27.61 months no mothers), showing mothers better premorbid adjustment as well. Childless men presented the worst premorbid adjustment and the highest cannabis and tobacco consumption rates. Mothers presented better global cognitive function, particularly in attention, motor dexterity and executive function at three-year follow-up. CONCLUSIONS: Diminished parental rates among FEP men could be suggested as a consequence of a younger age of illness onset. Sex roles in caregiving may explain the potential role of parenthood on premorbid phase, with a better and heathier profile, and a more favorable long-term outcome in women. These characteristics may be relevant when adjusting treatment specific needs in men and women with and without offspring.

BACKGROUND: The intelligence quotient (IQ) of patients with first-episode psychosis (FEP) and their unaffected relatives may be related to the genetic burden of schizophrenia (SCZ). The polygenic score approach can be useful for testing this question. AIM: To assess the contribution of the polygenic risk scores for SCZ (PGS-SCZ) and polygenic scores for IQ (PGS-IQ) to the individual IQ and its difference from the mean IQ of the family (named family-IQ) through a family-based design in an FEP sample. METHODS: The PAFIP-FAMILIES sample (Spain) consists of 122 FEP patients, 131 parents, 94 siblings, and 176 controls. They all completed the WAIS Vocabulary subtest for IQ estimation and provided a DNA sample. We calculated PGS-SCZ and PGS-IQ using the continuous shrinkage method. To account for relatedness in our sample, we performed linear mixed models. We controlled for covariates potentially related to IQ, including age, years of education, sex, and ancestry principal components. RESULTS: FEP patients significantly deviated from their family-IQ. FEP patients had higher PGS-SCZ than other groups, whereas the relatives had intermediate scores between patients and controls. PGS-IQ did not differ between groups. PGS-SCZ significantly predicted the deviation from family-IQ, whereas PGS-IQ significantly predicted individual IQ. CONCLUSIONS: PGS-SCZ discriminated between different levels of genetic risk for the disorder and was specifically related to patients' lower IQ in relation to family-IQ. The genetic background of the disorder may affect neurocognition through complex pathological processes interacting with environmental factors that prevent the individual from reaching their familial cognitive potential.

ABSTRACT

OBJECTIVE: The processing speed (PS) is highly impacted in individuals experiencing their first episode of psychosis (FEP). Conducting family studies can help to determine whether PS can serve as an endophenotype of schizophrenia spectrum disorders (SSDs), offering valuable insights into the prevention and diagnosis of SSDs. METHOD: A comprehensive cognitive battery, encompassing tests for PS, verbal memory, visual memory, working memory, executive functions, motor dexterity, and attention, was administered to a sample consisting of 133 FEP patients, 146 parents, 98 siblings, and 202 healthy controls (HCs). Univariate analyses (analysis of covariance [ANCOVA]) were conducted to compare the different cognitive domains between groups, utilizing sex, age, and years of education as covariates and Bonferroni corrections. Effect sizes (ESs) were calculated for estimating the magnitude of differences between groups. RESULTS: Group comparisons revealed significant differences in all cognitive domains. PS was the most impaired function in patients. Parents and siblings had intermediate PS performance between FEP patients and HC. Large ES were observed in PS between FEP versus siblings, FEP versus controls, parents versus controls, and parents versus siblings. CONCLUSIONS: Despite not meeting all the necessary criteria, the PS observed in FEP patients and their first-degree relatives suggests its potential as a promising endophenotype of SSDs. ABSTRACT

Objetivo: describir el perfil clínico y sociodemográfico de mujeres con cáncer de mama (CM) en un hospital de tercer nivel del Bajío, México. Métodos: estudio descriptivo transversal con recogida de datos retrospectivos. Se revisaron 262 expedientes clínicos de mujeres con CM, seleccionados mediante muestreo probabilístico. Variables de estudio: edad, lugar de residencia, nivel de estudios, antecedentes familiares y personales de cáncer, comorbilidades, tabaquismo, alcoholismo, tiempo transcurrido desde el diagnóstico, variables clínicas y de tratamiento del CM. Resultados: la media de edad fue de 58,07 años. El 45,8% no tenía antecedentes de cáncer, pero en quienes sí lo tenían, madre y hermana eran los más frecuentes (10,7% y 10,3% respectivamente). El 42,4% de las pacientes presentaba obesidad y el 23,3% tenía más de dos enfermedades crónico-degenerativas. El 87% de las mujeres no presentó metástasis. Un 19,8% se detectó en la etapa IIA. El grado G0 de invasión a ganglios linfáticos fue el más frecuente (35%). La combinación de dos o más tratamientos fue la forma de abordaje terapéutico más común (95,8%). La mayoría de los pacientes fueron dadas de alta por presentar mejoría (71%). Conclusiones: conocer las características sociodemográficas y clínicas de las pacientes con CM puede ayudar a establecer estrategias específicas para la prevención y el manejo en este grupo de pacientes, brindando una atención holística.(AU)


Objective: to describe the clinical and sociodemographic profile of women with breast cancer (BC) in a tertiary hospital from the Bajío area, Mexico. Methods: a descriptive cross-sectional study with retrospective data collection. There was a review of 262 clinical records of women with BC, selected through probability sample. The study variables were age, place of residence, level of studies, family and personal history of cancer, comorbidities, smoking, use of alcohol, time since diagnosis, clinical and BC treatment variables. Results: the mean age was 58.07 years; 45.8% had no history of cancer, but in those who had it, the most frequent were mother and sister (10.7% and 10.3% respectively). 42.4% of the patients presented obesity, and 23.3% had over two chronic degenerative conditions. 87% of women did not present metastasis; 19.8% were detected in the IIA stage; G0 was the most frequent level of invasion of lymph nodes (35%). The most common treatment approach was the combination of two or more treatments (95.8%). The majority of patients were discharged due to improvement (71%). Conclusions: knowing the sociodemographic and clinical characteristics of BC patients can help to establish specific strategies for prevention and management in this group, thus offering holistic care.(AU)

ABSTRACT

BACKGROUND AND HYPOTHESIS: There is uncertainty about the relationship between the family intelligence quotient (IQ) deviation and the risk for schizophrenia spectrum disorders (SSD). This study tested the hypothesis that IQ is familial in first episode psychosis (FEP) patients and that their degree of familial resemblance is associated with different profiles. STUDY DESIGN: The participants of the PAFIP-FAMILIAS project (129 FEP patients, 143 parents, and 97 siblings) completed the same neuropsychological battery. IQ-familiality was estimated through the Intraclass Correlation Coefficient (ICC). For each family, the intra-family resemblance score (IRS) was calculated as an index of familial similarity. The FEP patients were subgrouped and compared according to their IRS and IQ. STUDY RESULTS: IQ-familiality was low-moderate (ICC = 0.259). A total of 44.9% of the FEP patients had a low IRS, indicating discordancy with their family-IQ. Of these patients, those with low IQ had more schizophrenia diagnosis and a trend towards poorer premorbid adjustment in childhood and early adolescence. Whereas FEP patients with low IQ closely resembling their family-IQ were characterized by having the lowest performance in executive functions. CONCLUSIONS: The deviation from the familial cognitive performance may be related to a particular pathological process in SSD. Individuals with low IQ who do not reach their cognitive familial potential show difficulties in adjustment since childhood, probably influenced by environmental factors. Instead, FEP patients with high phenotypic family resemblance might have a more significant genetic burden for the disorder.

ABSTRACT

The objective of this study was to describe the epidemiology of Toxoplasma gondii and Neospora caninum infection by estimating seroprevalence and its association with certain risk factors in goats from Aguascalientes, Mexico. A total of 150 blood samples was taken from 10 farms and serum samples were subjected to enzyme-linked immunosorbent assay indirect test to detect T. gondii and N. caninum antibodies; the association between seroprevalence and some potential risk factors was estimated through logistic regression analysis. The general seroprevalence for T. gondii was 12.6%, observed in the farms in a range of 6.6 to 60%, finding seropositive animals in 80% of them; for N. caninum the seroprevalence was 3.3% and in farms a range of 6.6 to 13.3% was identified and 30% of them had at least 1 seropositive animal. The coinfection was 0.66%. The risk analysis for T. gondii identified a history of abortions (odds ratio 9.25) as a factor associated with seroprevalence; for N. caninum, no risk factor was identified.

Introducción: La menopausia es una etapa, si bien normal, de profundos cambios en la vida de la mujer; la transición hacia la menopausia impone un reto al profesional de enfermería, encaminado a promover la salud, facilitar la transición y apoyar la búsqueda de solución a las necesidades que se presentan. Objetivo: El presente estudio está orientado a comprender los significados de la menopausia en mujeres de Armenia, Quindío, Colombia. Metodología: Se utilizó un enfoque cualitativo, fenomenológico e interpretativo desde la teoría de las transiciones de Meléis. Se efectuaron entrevistas a profundidad a seis mujeres de Armenia, previo consentimiento informado, con muestreo intencional, grabación y diario de campo; la información se analizó con apoyo de la base de datos ATLAS.ti, mediante codificación, categorización inductiva, triangulación y contestación con la literatura. Resultados: Se encontró una percepción negativa de la menopausia por sensación de fogajes, desasosiego, depresión y disfunción sexual; para los síntomas recurren a terapias complementarias y medicación, al respecto, algunas refieren una experiencia tranquilizadora y agradable. Urgen redes de acompañamiento en salud para promoción, prevención y apoyo familiar. Las participantes solicitan programas educativos específicos desde edades tempranas. Discusión: Los resultados coinciden con la literatura en cuanto a la experiencia de las participantes y la necesidad de apoyo y orientación. Desde la teoría de Meléis, la transición es de desarrollo, organizacional, con patrones múltiples, simultáneos y relacionados. La experiencia fue sentida y vivida negativamente, se necesitan espacios de análisis, reflexión y acompañamiento desde edades tempranas, para deconstruir el concepto de menopausia y climaterio como problema de salud, y abordarlo como evento vital y de renacimiento, como apertura a una nueva forma de vivir, con estilos de vida saludables. Conclusiones: Se concluye que la menopausia es percibida y experimentada como una etapa de ansiedad, desasosiego, tristeza y desesperanza, por el poco acompañamiento, deficiente educación y preparación de la mujer; las mujeres adultas "menopaúsicas" son poco reconocidas, rechazadas, y muchas veces solo toleradas por personas cercanas. Urgen redes de acompañamiento en salud.


Introduction: Menopause is a stage, although normal, of profound changes in a woman's life. The transition to menopause imposes a challenge on the nursing professional, aimed at promoting health, facilitating the transition and supporting the search for a solution to the needs that arise. Objective: The present study is aimed at understanding the meanings of menopause in women from Armenia, Quindio Department, Colombia. Metodology: A qualitative, phenomenological and interpretative approach was used, based on Meleis's theory of transitions. In-depth interviews were carried out on six women from Armenia, with prior informed consent, with intentional sampling, recording, and field diary; the information was analyzed with support of the ATLAS.ti database, through coding, inductive categorization, triangulation, and comparison with the literature. Results: A negative perception of menopause was found due to hot flashes, restlessness, depression and sexual dysfunction; for the symptoms they resort to complementary therapies and medication, as a result, some report a calming and pleasant experience; health accompaniment networks are urgently needed for promotion and prevention, and family support. They request specific educational programs from an early age. Discussion. The results coincide with the literature regarding the experience of the participants and the need for support and guidance. From Meleis's theory, the transition is developmental, organizational, with multiple, simultaneous and related patterns. The experience was felt and lived negatively, spaces for analysis, reflection and accompaniment are needed from an early age, to deconstruct the concept of menopause and climacteric as a health problem, and address it as a vital and rebirth event, as an opening to a new way of living, with healthy lifestyles. Conclusions: It is concluded that the menopause is perceived and experienced as a stage of anxiety, restlessness, sadness and hopelessness, due to the lack of accompaniment, poor education and preparation of women; "menopausal" adult women are little recognized, rejected and, many times, only tolerated by close people. Health support networks are urgently needed.

ABSTRACT

Objetivo: determinar las características clínicas y metabólicas en adultos con diabetes tipo 2 según la presencia de criterios diagnósticos de síndrome metabólico (SM).Método: estudio descriptivo transversal con muestreo probabilístico aleatorio simple que incluyó a 49 pacientes con diabetes tipo 2. Se midió composición corporal, presión arterial y parámetros bioquímicos. Para determinar el diagnóstico de SM se aplicaron los criterios de la Federación Internacional de Diabetes. Se realizó análisis bivariante entre quienes presentaron criterios diagnósticos de SM y quienes no, utilizándose la prueba de contraste de hipótesis U de Mann Whitney.Resultados: la edad media de la muestra estudiada fue de 59,7 (DE: 11,5 años). La mayoría de los participantes fue mujer (65,3%) y con nivel socioeconómico medio (75,5%). La prevalencia de SM fue del 40% y la hiperglicemia fue la alteración más frecuente (93,3%). La mayoría de los pacientes con SM mostró, de manera estadísticamente significativa (p< 0,05), sobrepeso y obesidad, así como cifras más altas de índice de masa corporal, circunferencia de cintura, colesterol total, triglicéridos, glucosa, índices aterogénicos y tensión arterial.Conclusiones: los pacientes con DMT2 mostraron una alta prevalencia de SM. En los pacientes con SM, la hiperglicemia fue la alteración más frecuente, seguida de obesidad central medida por la circunferencia de cintura, y de acuerdo con su IMC, la mayoría presentó sobrepeso y obesidad. La media de triglicéridos, tensión arterial e índices aterogénicos fueron mayores en comparación con quienes no cumplían criterios diagnósticos de SM. (AU)


Objective: to determine the clinical and metabolic characteristics in adults with Type 2 diabetes according to the presence of diagnostic criteria for metabolic syndrome (MS).Method: a descriptive cross-sectional study with simple randomized probability sampling, including patients with Type 2 diabetes. There was measurement of body composition, blood pressure, and biochemical parameters. In order to determine the metabolic syndrome diagnosis, criteria by the International Diabetes Federation were applied. There was bivariate analysis among those who presented diagnostic criteria for SM and those who did not, using the Mann Whitney’s U test for hypothesis contrast.Results: the mean age of the sample studied was 59.7% (SD: 11.5 years). Most participants were female (65.3%) and with a middle socioeconomic level (75.5%). The prevalence of metabolic syndrome was 40%, and hyperglycaemia was the most frequent alteration (93.3%). The majority of patients with MS showed statistically significant overweight and obesity (p< 0.05), as well as higher figures of body mass index, waist circumference, total cholesterol, triglycerides, glucose, atherogenic indexes and blood pressure.Conclusions: patients with T2D showed a high prevalence of MS. In patients with MS, hyperglycaemia was the most frequent alteration, followed by central obesity measured by waist circumference; and according to their BMI, the majority presented overweight and obesity. The mean triglycerides, blood pressure and atherogenic indexes of these patients were higher vs. those who did not meet diagnostic criteria for MS. (AU)

ABSTRACT

Arsenic exposure is a significant public health issue, with harmful effects caused by its use in commercial products such as car batteries, pesticides, and herbicides. Arsenic has three main compounds: inorganic, organic, and arsine gas. Inorganic arsenic compounds in water are highly toxic. The daily intake of arsenic from food and beverages is between 20 and 300 mcg/day. Arsenic is known for its carcinogenic properties and is classified as a human carcinogen by different institutions. Exposure can lead to oxidative stress, DNA damage, and epigenetic deregulation, which can cause endocrine disorders, altered signal transduction pathways, and cell proliferation. In addition, arsenic can easily cross the placenta, making it a critical concern for maternal and fetal health. Exposure can lead to complications such as gestational diabetes, anemia, low birth weight, miscarriage, and congenital anomalies. Female babies are particularly vulnerable to the negative impact of arsenic exposure, with a higher risk of low weight for gestational age and congenital cardiac anomalies. Therefore, it is crucial to monitor and regulate the levels of arsenic in drinking water and food sources to prevent these adverse health outcomes. Further research is necessary to fully understand the impact of arsenic exposure on human health, especially during pregnancy and infancy, by implementing preventative measures and monitoring the levels of arsenic in the environment.

ABSTRACT

Hypertrophic cardiomyopathy (HCM) is a hereditary cardiac condition characterized by unexplained left ventricular hypertrophy without a hemodynamic cause. This condition is prevalent in the United States, resulting in various clinical manifestations, including diastolic dysfunction, left ventricular outflow obstruction, cardiac ischemia, and atrial fibrillation. HCM is associated with several genetic mutations, with sarcomeric mutations being the most common and contributing to a more complex disease course. Early diagnosis of HCM is essential for effective management, as late diagnosis often requires invasive treatments and creates a substantial financial burden. Disparities in HCM diagnosis and treatment exist between high-income and low-income countries. High-income countries have more resources to investigate and implement advanced diagnostic and treatment modalities. In contrast, low-income countries face challenges in accessing diagnostic equipment, trained personnel, and affordable medications, leading to a lower quality of life and life expectancy for affected individuals. Diagnostic tools for HCM include imaging studies such as 2D echocardiography, cardiovascular magnetic resonance (CMR), and electrocardiograms (ECGs). CMR is considered the gold standard but remains inaccessible to a significant portion of the world's population, especially in low-income countries. Genetics plays a crucial role in HCM, with numerous mutations identified in various genes. Genetic counseling is essential but often limited in low-income countries due to resource constraints. Disparities in healthcare access and adherence to treatment recommendations exist between high-income and low-income countries, leading to differences in patient outcomes. Addressing these disparities is essential to improve the overall management of HCM on a global scale. In conclusion, this review highlights the complex nature of HCM, emphasizing the importance of early diagnosis, genetic counseling, and access to appropriate diagnostic and therapeutic interventions. Addressing healthcare disparities is crucial to ensure that all individuals with HCM receive timely and effective care, regardless of their geographic location or socioeconomic status.

INTRODUCTION: The proxies used to compose cognitive reserve (CR) for patients of a first episode of psychosis (FEP) have varied in the literature. The development of FEP is linked to peripheral pathways of the central nervous system, yet despite this knowledge, no research has considered the introduction of biomarkers as proxies for CR. Meanwhile, schizophrenia has been linked to the metabolic system, indicating that alterations in the levels of biological parameters, in particular high-density lipoproteins (HDL), cause worse global functioning and cognitive impairment. For these reasons, the present study aimed to create a quantifiable and objective CR index that adjusts for the multifactorial nature of FEP. MATERIALS AND METHODS: We included 668 FEP patients and 217 healthy controls. Participants were assessed for sociodemographic information, years of education, employment status, premorbid IQ and biological parameters: waist circumference, hypertension, and levels of HDL, triglycerides, and glucose. RESULTS: The findings suggest that the years of education proxy showed correlational and higher relationship with HDL levels for both FEP patients (r=0.23, b=0.185) and controls (r=0.31, b=0.342). We found that the CR index composed of years of education and HDL levels showed a higher explanatory power for the phenomenon than the classic CR index composed of years of education, employment status and premorbid IQ. CONCLUSIONS: This article proposes an objective and quantifiable method to measure CR that is more the multifactorial nature of FEP.

ABSTRACT

Novel materials using biowaste as adsorbents in wastewater treatment have been allocated considerable interest. Herein, we present the synthesis of different hydrogels of crosslinked polyacrylamide in presence of hemicelluloses with/ without bentonite, using a soft reaction condition. The structure of new hydrogels was characterized by spectroscopic, thermal and microscopic experiments. The semi-interpenetrated network with hemicelluloses: 10 %; acrylamide 79 %; bentonite 10 %; N,N,N',N'-tetramethylethylenediamine: 1 % allows reducing 20 % the use of non-renewable acrylamide, without changing its decomposition temperatures and keeping its water absorption capacity. This hydrogel was applied to dye removals, such as rhodamine B, methylene red and methylene blue in aqueous solutions. In the case of methylene blue, highest removal is observed with maximum adsorption of qmax = 140.66 mg/g, compared to material without hemicelluloses that only a qmax = 88.495 mg/g. The adsorption kinetics and equilibrium adsorption isotherms are in accordance with the pseudo-second-order kinetic model and Langmuir isotherm model, respectively. The developed hydrogel from hemicelluloses represents a potential alternative adsorbent for a sustainable system of sewage treatment.

Introducción: Las enfermeras buscan encontrar el equilibrio entre la fragmentación y despersonalización de los sistemas de salud y el desarrollo de un cuidado humano. Objetivo: Conocer la evaluación del cuidado humano brindado por enfermeras en un hospital materno infantil del centro de México. Métodos: Estudio descriptivo, prospectivo, transversal. Las conductas del cuidado enfermero fueron evaluadas por mujeres hospitalizadas que contestaron la versión en español del Caring Behaviors Assesment basado en los procesos caritas de Jean Watson. Resultados: Sesenta y ocho mujeres evaluaron la humanización del cuidado recibido por las enfermeras con puntaje promedio de 4.29±0.5. Refirieron que “casi siempre o siempre”: saben usar los equipos, dan tratamientos/medicamentos a tiempo, dejan la habitación limpia y ordenada, explican las precauciones de seguridad. En contraste, “casi nunca o algunas veces”: preguntaron cómo le gusta que se hagan las cosas, las tocaron cuando necesitaban consuelo o ánimo, no se alejaron y abandonaron si el paciente está difícil, dijeron lo que ocurrirá con el paciente diariamente, consideraron sus necesidades espirituales, permitieron que la familia lo visitara por más tiempo. Conclusiones: En general las mujeres evaluaron positivamente las conductas del cuidado humano de las enfermeras que las cuidaron. Las conductas de cuidado humano con menor puntaje están relacionadas con los procesos caritas 4 “Desarrollo de la relación de cuidado humano, de ayuda y confianza” y 5 “Promoción y aceptación de la expresión de sentimientos positivos y negativos”. Desarrollar las conductas de cuidado humano es una tarea, un reto, y una responsabilidad vigente de la enfermera profesional. (AU)


Introduction: Nurses seek to find a balance between the fragmentation and depersonalization of health systems and the development of humanized care. Objective: To know the evaluation of the human care provided by nurses in a maternal and child hospital in central Mexico. Methods: Descriptive, prospective, cross-sectional study. Nursing care behaviors were evaluated by hospitalized women who answered the Spanish version of the Caring Behaviors Assessment based on Jean Watson’s caritas processes. Results: Sixty-eight women evaluated the humanization of the care received by the nurses with an average score of 4.29 ± 0.5. They stated that “almost always or always”: they know how to use the equipment, they give treatments / medicines on time, they leave the room clean and tidy, they explain the safety precautions. In contrast, “almost never or sometimes”: they asked how they like things to be done, they touched them when they needed comfort or encouragement, they did not walk away and abandon if the patient is difficult, they said what will happen to the patient daily, they considered their spiritual needs, allowed the family to visit them longer. Conclusions: In general, the women positively evaluated the human care behaviors of the nurses who cared for them. The human care behaviors with the lowest scores are related to the caritas processes 4 “Development of the relationship of human care, help and trust” and 5 “Promotion and acceptance of the expression of positive and negative feelings”. Developing human care behaviors is a task, a challenge, and a current responsibility of the professional nurse. (AU)

ABSTRACT

Schizophrenia spectrum disorders (SSD) often show cognitive deficits (CD) impacting daily life. Family support has been shown to be protective against CD, yet the relationship between these in psychotic patients remains complex and not fully understood. This study investigated the association between a subdomain of family support, namely, family involvement (estimated through a proxy measure), cognitive functioning, and sex in first-episode psychosis (FEP) patients. The sample included 308 patients enrolled in the Program for Early Phases of Psychosis (PAFIP), divided into 4 groups based on their estimated family involvement (eFI) level and sex, and compared on various variables. Women presented lower rates of eFI than men (37.1% and 48.8%). Higher eFI was associated with better cognitive functioning, particularly in verbal memory. This association was stronger in women. The findings suggest that eFI may be an important factor in FEP patients' cognitive functioning. This highlights the importance of including families in treatment plans for psychotic patients to prevent CD. Further research is needed to better understand the complex interplay between family support, sex, and cognitive functioning in psychotic patients and develop effective interventions that target these factors.

BACKGROUND: Cognitive reserve (CR) has been associated with the development and prognosis of psychosis. Different proxies have been used to estimate CR among individuals. A composite score of these proxies could elucidate the role of CR at illness onset on the variability of clinical and neurocognitive outcomes. METHODS: Premorbid intelligence quotient (IQ), years of education and premorbid adjustment were explored as proxies of CR in a large sample (N = 424) of first-episode psychosis (FEP) non-affective patients. Clusters of patients were identified and compared based on premorbid, clinical and neurocognitive variables at baseline. Additionally, the clusters were compared at 3-year (N = 362) and 10-year (N = 150) follow-ups. RESULTS: The FEP patients were grouped into five CR clusters: C1 (low premorbid IQ, low education and poor premorbid) 14%; C2 (low premorbid IQ, low education and good premorbid adjustment) 29%; C3 (normal premorbid IQ, low education and poor premorbid adjustment) 17%; C4 (normal premorbid IQ, medium education and good premorbid adjustment) 25%; and C5 (normal premorbid IQ, higher education and good premorbid adjustment) 15%. In general, positive and negative symptoms were more severe in the FEP patients with the lowest CR at baseline and follow-up assessments, while those with high CR presented and maintained higher levels of cognitive functioning. CONCLUSIONS: CR could be considered a key factor at illness onset and a moderator of outcomes in FEP patients. A high CR could function as a protective factor against cognitive impairment and severe symptomatology. Clinical interventions focused on increasing CR and documenting long-term benefits are interesting and desirable.

Objetivo: evaluar el estrés percibido, la ansiedad y la depresión en pacientes con enfermedad renal crónica (ERC) en protocolo de trasplante renal de una unidad de tercer nivel durante la pandemia por COVID-19.Método: estudio descriptivo transversal en pacientes con ERC que se encuentran en lista de espera para trasplante renal. Una muestra de 73 sujetos fue elegida mediante muestreo simple aleatorio de quienes se encontraban en la lista de espera. Se estudiaron variables sociodemográficas y clínicas, además de aplicarse los cuestionarios validados: Escala de Estrés Percibido (Perceived Stress Scale, PSS) y el Inventario de Ansiedad y Depresión de Beck. Como pruebas de contraste de hipótesis fueron utilizadas la t de Student y la Chi cuadrado.Resultados: la edad media de la muestra estudiada fue de 35,3 (DE: 11,9 años), con una mayoría de género masculino (72,9%). En la escala de estrés percibido se obtuvo una puntuación media de 20 (DE: 10). La prevalencia de ansiedad y depresión fue de 12,5% y 10,4%, respectivamente. Los pacientes se encontraron con ansiedad mínima (58,3%) y depresión mínima (75%).Conclusiones: las personas con ERC en protocolo de trasplante renal durante la pandemia por COVID-19, presentan estrés, ansiedad y depresión mínima. Es necesario intervenir de manera oportuna para contribuir a mejorar su calidad de vida en las personas que aún no han recibido un trasplante renal.(AU)


Objective: to evaluate the stress perceived, the anxiety and depression in patients with chronic renal disease (CRD) under protocol for renal transplant at a third level unit during the COVID-19 pandemic.Method: a descriptive cross-sectional study in CRD patients on the waiting list for renal transplant. A sample of 73 individuals was selected through simple random sampling of those on the waiting list. Sociodemographic and clinical variables were studied, and the following validated questionnaires were applied: Perceived Stress Scale (PSS) and the Beck Anxiety and Depression Inventory. Student’s t-test and Square chi-test were used for hypothesis testing.Results: the mean age of the sample studied was 35.3 (SD: 11.9 years), with a majority of male gender (72.9%). A mean score of 20 (SD: 10) was obtained in the perceived stress scale. The prevalence of anxiety and depression was 12.5% and 10.4%, respectively. Patients presented minimal anxiety (58.3%) and minimal depression (75%).Conclusions: patients with CRD under protocol for renal transplant during the COVID-19 pandemic presented minimal stress, anxiety and depression. An appropriate intervention is needed in order to contribute to an improvement in their quality of life when they have not yet received a renal transplant.(AU)

ABSTRACT

OBJECTIVES: The work sough to know the adaptation strategies of students from Universidad del Quindío with motor functional diversity. METHODS: Descriptive qualitative study with a phenomenological approach. Data were collected through an in-depth interview with nine undergraduate students with moderate motor functional diversity, in face-to-face class attendance modality during the period 2022-2 at Universidad del Quindío (Colombia) with age ≥ 18 years and having scored from 20 - 40 in the Barthel index. The definition of the number of participants was conducted through theoretical saturation. RESULTS: Seven categories emerged from the descriptive analysis of the interviews: 1) support; 2) affection; 3) life project; 4) personal growth; 5) spirituality; 6) autonomy, and 7) education. Together, they reveal important aspects on the way students have adapted to the university campus and how interpersonal relations can contribute to promoting resilience processes. CONCLUSIONS: Support and affection provided by the social setting play a fundamental role in the adaptation of students with motor functional diversity, improving their mental health, generating resilience, and increasing their self-esteem. Noting that in spite of lifestyle changes after the acquisition of the diversity, the students set novel goals and develop new abilities that contribute to complying with their life project; likewise, they have set into practice and can recognize their coping mechanisms, acquiring qualities, like resilience and autonomy.

OBJECTIVE: This study aimed to analyse whether intelligence quotient (IQ) improves, declines, or remains stable over 10 years among FEP patients and healthy subjects. METHODS: A group of FEP patients enrolled in a Program of First Episode Psychosis in Spain called PAFIP, and a sample of Healthy Controls (HC) completed the same neuropsychological battery at baseline and approximately 10 years later, which included the WAIS vocabulary subtest to estimate premorbid IQ and 10-year IQ. Cluster analysis was performed separately in the patient group and the HC group to determine their profiles of intellectual change. RESULTS: One hundred and thirty-seven FEP patients were grouped into five clusters: "Improved low IQ" (9.49 % of patients), "Improved average IQ" (14.6 %), "Preserved low IQ" (17.52 %), "Preserved average IQ" (43.06 %), and "Preserved high IQ" (15.33 %). Ninety HC were grouped into three clusters: "Preserved low IQ" (32.22 % of the HC), "Preserved average IQ" (44.44 %), and "Preserved high IQ" (23.33 %). The first two clusters of FEP patients, characterized by a low IQ, earlier age at illness onset, and lower educational attainment, showed a substantial cognitive improvement. The remaining clusters demonstrated cognitive stability. CONCLUSIONS: The FEP patients showed intellectual improvement or stability, but no decline post-onset of psychosis. However, their profiles of intellectual change are more heterogeneous than that of HC over 10 years. Particularly, there is a subgroup of FEP patients with a significant potential for long-term cognitive enhancement.

ABSTRACT

The COVID-19 pandemic can seize the opportunity to explore the hypothesis of prenatal exposure to viral infections increases the risk for neurodevelopmental disorders. Advancing our knowledge in this regard would improve primary prevention of mental disorders in children. For this pilot study, six-week-old infants born to mothers exposed (n = 21) or unexposed (n = 21) to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) were assessed in Santander-Cantabria (Spain) using the Neonatal Behavioral Assessment Scale (NBAS). Groups comparisons were performed to explore the effects that infection and timing of exposure (in terms of the three trimesters of pregnancy). The infants' competencies and performances on the NBAS were generally similar in the exposed and unexposed to SARS-CoV-2 groups. The most significant difference found was a less optimally response to cuddliness (item on the state regulation domain) particularly in infants born to mothers exposed in the third trimester of pregnancy, and in pull-to-sit (item on the motor system domain). Although our interpretations must be careful, these preliminary results highlight the possible association between prenatal SARS-CoV-2 exposure and poorer development in motor skills and infant interactive behavior. Further longitudinal studies are needed to explore these relationships and disentangle the biological mechanisms implicated.
