ABSTRACT

Antecedentes y objetivo: Los inhibidores de la fosfodiesterasa 5 son los fármacos más utilizados para tratar a los pacientes con disfunción eréctil (DE). Los métodos para diagnosticar y evaluar la DE así como la respuesta al tratamiento son básicamente clínicos, es decir, la percepción del paciente sobre la calidad de su erección y la realización de cuestionarios validados. Actualmente aun es necesario identificar marcadores que puedan predecir la respuesta al tratamiento farmacológico. Este estudio pretende evaluar si la toma diaria de tadalafilo 5mg es capaz de inducir cambios en la hemodinámica peniana (midiendo la velocidad del pico sistólico (VPS) y la aceleración sistólica (AS) mediante ecografía doppler, sin utilizar alprostadilo) y estudiar si estos cambios tienen una correlación clínica. El objetivo es identificar un factor predictivo hemodinámico de respuesta al tratamiento farmacológico.Materiales y métodos: Se realizó un estudio prospectivo a 40 pacientes con DE a los que se inició tratamiento con tadalafilo 5mg/día. Estos respondieron a los cuestionarios IIEF-6, EHS, SEP-2 y SEP-3, y se les realizó una ecografía doppler peniana en 3 momentos distintos: antes de comenzar el tratamiento, a los 3 y a los 6 meses (estando previamente 30 días sin medicación).Resultados y conclusiones: El tratamiento durante 3 meses no modificó de forma significativa la VPS y la AS. Sí hubo una mejoría clínica tras 3 meses de tratamiento de acuerdo a los cuestionarios, que no se mantuvo a los 6 meses. No se pudo establecer una correlación clínico-radiológica ni encontramos un valor hemodinámico predictor de respuesta al tratamiento. (AU)

It is necessary to identify predictive markers to choose the best drug and the best dose for each individual patient. This study seeks to evaluate the effect of tadalafil 5mg daily treatment on penile haemodynamics (penis in flaccidity, using doppler ultrasound, without using alprostadil) and to study if these changes are clinically associated.Material and methods: 40 patients who started treatment with tadalafil 5mg daily were consecutively selected in a prospective study. Each patient underwent a penile doppler ultrasound and completed questionnaires (IIEF-6, EHS, SEP-2, SEP-3) at three different times: before starting the treatment, at 3 months and at 6 months (within the last 30 days without treatment).Results and conclusions: Tadalafil 5mg daily treatment for 3 months did not result in significant modifications to VPS and AS. There was clinical improvement according to the questionnaires after three months of treatment, which was not maintained at 6 months. Neither a clinical-radiological correlation nor a haemodynamic value predicting response to treatment could be established. (AU)

INTRODUCTION: Bladder reconstruction without using the intestine remains a challenge to this day despite the development of new biomaterials and cell cultures. Human bladder engineering is merely anecdotic, and mostly in vitro and animal studies have been conducted. MATERIAL AND METHODS: In our study using a porcine model, we performed a bladder augmentation using an autologous parietal peritoneum graft (peritoneocystoplasty) and determined whether the attachment of an acellular collagen-elastin matrix (Group 1) or lack of (Group 2) had better histologic and functional results. Thus far, peritoneocystoplasty has rarely been performed or combined with a biomaterial. RESULTS: After 6 weeks, we observed different degrees of retraction of the new bladder wall in both groups, although the retraction was lower and the histological analysis showed more signs of regeneration (neoangiogenesis and less fibrosis) in Group 1 than when compared with Group 2. No transitional cells were found in the new bladder wall in any of the groups, and no differences were observed in the functional test results. CONCLUSIONS: Performing a peritoneocystoplasty is an easy and safe procedure. The data supports the benefit of an acellular collagen-elastin matrix to reinforce bladder regeneration. However, in our study we observed too much retraction of the new wall and the histologic results were not acceptable to consider it an appropriate cystoplasty technique.

ABSTRACT

OBJECTIVE: The aim of this study was to evaluate hormonal recovery after cessation of androgen deprivation therapy (ADT) in a group of elderly prostate cancer patients. MATERIALS AND METHODS: Forty patients with locally advanced or metastatic prostate cancer, with a mean age of 71.5 years [95% confidence interval (CI) 69.1-73.9], were treated with ADT for a mean duration of 74.6 months (95% CI 59.7-89.5 months). Mean follow-up time after ADT cessation was 36.5 months (95% CI 30.6-42.3 months). Serum testosterone and luteinizing hormone (LH) were determined at 6 month intervals after ADT cessation. RESULTS: After 18 months of follow-up, all patients had recovered normal LH levels, while 38% of patients still presented castration levels of testosterone (< 50 ng/dl). A multivariate analysis was performed to find factors related to testosterone recovery (testosterone >50 ng/dl). Neither age at start of ADT nor clinical stage reached statistical significance. Only time under ADT was correlated with testosterone recovery (p = .031). Kaplan-Meier curves were obtained. Mean time for testosterone recovery was 14.5 months (95% CI 6.5-22.6 months) in patients treated with ADT for less than 60 months compared to 29.3 months (95% CI 19.6-39.1 months) in patients treated with ADT for more than 60 months (log-rank p = .029). CONCLUSIONS: Age did not correlate with testosterone recovery in a group of elderly prostate cancer patients in whom ADT was stopped. Testosterone recovery after ADT cessation was significantly correlated with time under ADT treatment. Significant implications related to economic aspects of the dosage schedule may be considered.
