ABSTRACT

Desmoplastic fibroma of the jaws is a rare and locally aggressive tumor that has a high rate of recurrence. The cellularity, extent of the tumor, and completeness of the local excision may be factors in its tendency to recur. We reviewed the literature in an attempt to determine if there was any correlation between the histologic features of a desmoplastic fibroma, or the surgical procedure utilized, and the tumor's recurrence. From the information gathered, it was found that a more cellular desmoplastic fibroma or inadequate surgical procedure may be factors that contribute to the recurrence of the tumor. A case report of desmoplastic fibroma of the mandible in a 9-year-old child is presented. The tumor, which displayed areas of moderate cellularity, was resected via a partial hemimandibulectomy. There has been no recurrence to date (46 months).

ABSTRACT

A retrospective comparison was made of tomographic change in the temporomandibular joint (TMJ) of patients who had been treated by either discoplasty or discectomy with polytetrafluoroethylene (PTFE) disc replacement. Fifty-five PTFE- and 18 discoplasty-treated joints were evaluated. More than 60% of the PTFE-treated joints showed severe, destructive osseous change, whereas none of the discoplasty joints showed such change. Histologic examination of the tissues surrounding removed PTFE implants showed a foreign body giant cell reaction. The severity of radiographic change raises questions about the propriety of PTFE as a disc replacement material.

ABSTRACT

Elastic bonds used for orthodontic tooth movement without benefit of attached mechanisms have been shown to migrate apically, causing severe periodontal damage and eventual tooth mobility and tooth loss. The literature indicates high mortality rates for such involved teeth. A case report is presented, illustrating initial successful retention of teeth following surgical removal of a displaced elastic band. Cooperation between oral surgeon, periodontist, and orthodontist, in conjunction with high standards of oral hygiene by the patient, may lead to successful retention of otherwise condemned teeth. It is also suggested that elastic band therapy be carefully supervised when used to move teeth without controlling attachment mechanisms.

ABSTRACT

A 4-month-old patient presented with a melanotic neuroectodermal tumor. The tumor continued to progress and recur following each surgical intervention and the patient died at the age of 38 months, 36 months following discovery of the tumor. This article describes the clinical and pathologic features of the disease. At autopsy, the tumor resembled that of a neuroblastoma.
