ABSTRACT

AIM: To study functional myocardial reserve in patients with nonspecific aortic arteritis (NAA) and aortic arch syndrome (AAS). MATERIAL AND METHODS: Myocardial functional reserve was studied in 42 patients with NAA using ECG, spiroveloergometry, section and doppler echocardiography, radionuclide ventriculography. Healthy nonsportswomen served control. RESULTS: ECG revealed left ventricular hypertrophy in most of the patients, spiroveloergometry showed low exercise tolerance in many NAA patients. Echocardiography detected differences in patients vs controls by diastolic thickness of the interventricular septum, left ventricular myocardial mass and aortic diameter. As shown by radionuclide ventriculography, 75% of NAA patients with predominant affection of the aortic arch branches had disturbed ventricular function. CONCLUSION: 84% of NAA patients with AAS at veloergometry exhibited signs of transitory myocardial ischemia. Hemodynamic changes at exercise demonstrated low coronary reserve. Silent myocardial ischemia is typical for NAA patients.

ABSTRACT

Transcranial Doppler echocardiography (TCECG) was used to study a response of enhanced blood flow after 5% CO2 inhalation in 30 patients with carotid atherosclerotic lesions. The technique significantly distinguishes critical stenosis and occlusion of the internal carotid artery from normal and stenotic carotid artery. It is concluded that the values of cerebrovascular CO2 reactivity can be an indicator of differences available between hemodynamically significant carotid lesions and the degree of collateralization in the carotid, tree.

ABSTRACT

The authors provide a scheme of a comprehensive diagnosis of the aortic arch syndrome in nonspecific aortoarteritis. It includes the clinical and laboratory data together with the data on the anamnesis, angiography and ultrasound dopplerography. The derangement of the aortic arch was diagnosed in 65 (41%) out of 128 patients. There were 53 (81.5%) women and 12 (18.5%) men. The derangement of the branches of the aortic arch was most frequently (in 33.8% patients) associated with vasorenal hypertension syndrome. The use of modern research methods in combination with the clinical data and findings of objective examinations turned out fairly effective in the diagnosis of the derangement of the aortic arch in nonspecific aortoarteritis.

ABSTRACT

The features of the blood flow in the vertebral artery were studied by ultrasonic dopplerography with functional test for reactive hyperemia in 52 patients. The authors claim this to be an objective screening method allowing the sequelae of occlusion of the vertebrosubclavian segment to be studied and providing information for establishing the diagnosis and appraising the efficacy of reconstructive operations.
