ABSTRACT

The article deals with clinical experience in early diagnosis of pancreatonecrosis in which enzymological methods of examination acquire importance. The most informative test in the diagnosis of fatty pancreatonecrosis is the increase of the pancreatic lipase content in blood serum, particularly in stimulation of pancreatic secretion by pancreozymin. A new Soviet-produced peptide agent--dalargin was included in the complex drug treatment of fatty pancreatonecrosis in 74 patients. With the use of dalargin as an inhibitor of the activity of pancreatic enzymes and kinins in the treatment of fatty pancreatonecrosis a marked positive effect was produced. The mortality in fatty pancreatonecrosis was 5.5%.

ABSTRACT

Experience in treating 351 patients with persistent wounds and trophic ulcers by low-intensity laser radiation in out-patient clinics is generalized. Complete epithelialization of the wound or ulcer surface occurred in 236 patients. There were 246 patients aged 60, all were treated without leave from work. Thus, the inclusion of laser therapy in the complex of measures for persistent wounds and trophic ulcers is a highly effective method which may be applied in preoperative management and which will reduce considerably the term of in-patient treatment.

ABSTRACT

The article discusses treatment of 72 patients suffering from duodenal ulcer complicated by bleeding with the Soviet-made preparation Dalargin which is a synthetic analogue of leucine-enkephalin. The preparation was infused intravenously in a dose of 3 mg in physiological solution once daily. No other additional antiulcerative agents were give. The diagnosis was established on the basis of endoscopy. Dalargin led to cicatrization of the ulcers in 20.3 days in 60% of patient, obvious positive dynamics was noted in 32% of patients; in 8% of patients Dalargin proved ineffective. Basal hydrochloric acid secretion reduced by 56% during treatment.

ABSTRACT

The authors have used gravitation plasmapheresis in the treatment of 26 patients with diabetic angiopathies of lower extremities. It was established that continuous gravitation plasmapheresis facilitated the immediate correction of most of the altered biochemical and coagulation parameters of blood, in patients with diabetes mellitus. The use of rheopolyglucinum is followed by stable central hemodynamics and stabilization of the general state of the patients during continuous plasmapheresis which allows to recommend rheopolyglucinum as the main plasma-substituting solution for fractionation of blood in patients with diabetic angiopathies. The gravitation plasmapheresis used in the treatment of critical degrees of ischemia in patients with diabetic angiopathies of lower extremities is thought by the authors to allow to preserve the extremity or to make the level of its amputation lower in most patients.

ABSTRACT

The epidural-sacral anesthesia was used in 242 patients operated upon for different forms of acute paraproctitis. No complications resulting from the anesthesia were noted. The incidence of failures in performing epidural-sacral anesthesia was 3.6%. The epidural-sacral anesthesia has advantages over other kinds of anesthesia, is safe and not technically difficult and practically has no contraindications.

ABSTRACT

The experiments on 24 dogs with experimental hemorrhagic pancreonecrosis showed that contrykal and 5-fluorouracil insignificantly decrease the area of the damaged parenchyma, fail to cease progression of the pancreas destruction. The peptide drug dalargin in these conditions increased the antitryptic activity and normalized BAEE esterase activity. Dalargin abolished erythrostasis and microthromboses in the vessels of the microcirculation, restored the blood supply throughout the area of the exocrine parenchyma, limited necrotic foci and suspended the development of pancreonecrosis.

ABSTRACT

The authors made operations on 113 patients with various forms of acute periproctitis by their original method. One of its main elements was to open the intermuscular space of the rectum by dissection of the internal sphincter and part of the circular muscular layer. The proposed method of treatment of acute periproctitis is thought by the authors to be radical since the operation was not followed by recidivations which shortened staying of the patients at the hospital.

ABSTRACT

The paper is concerned with the inhibitory effect of dalargia, a new analogue of leu-enkephalin synthesized at the Laboratory of Peptides, All-Union Cardiology Research Center, Academy of Medical Sciences of the USSR. Acute experiments on 22 dogs were performed to examine the effect of parenteral administration of dalargin in the total dose 3 mg on morphological and functional changes in the endocrine tissue of the pancreas. It was shown that intravenous injection of dalargin reduces the basal secretion and inhibits the synthesis of pancreatic enzymes in animals with the intact pancreas. In progressive hemorrhagic pancreonecrosis, intravenous drip of dalargin improves the microhemocirculation and recovers the blood supply of the pancreatic parenchyma, restricts the foci of necrosis, reduces the involvement into the pathological process of unaffected exocrine cells. Comparative analysis of changes in enzymological characteristics in the course of pancreonecrosis development before and after parenteral administration of dalargin has revealed a reduction in the proteolytic and kininogenase activity, normalization of the antitryptic activity of the pancreas.
