ABSTRACT

BACKGROUND: With the growing prevalence of obesity in the global population, alternative measures for weight loss and treatment of comorbidities must be considered due to the increasing difficulty of conservative management alone. Here we discuss the benefits of bariatric surgery on weight loss as well comorbidities that are present in a majority of obese patients.  Methods: In this review, we discuss the current practice and evidence of bariatric surgery as it pertains to weight loss and the beneficial effect on comorbidities commonly present in obesity. RESULTS:  Our review found that bariatric surgery with either the roux-en-y gastric bypass or laparoscopic sleeve gastrectomy can result in weight loss of up to 80% of excess weight. We also found that bariatric surgery has a profound effect on multiple comorbidities such as type 2 diabetes mellitus, hypertension, and hyperlipidemia through remission of the disease. CONCLUSION:  Bariatric surgery serves as an efficacious alternative for treatment of obesity and comorbidities.

ABSTRACT

Percutaneous endoscopic gastrostomy (PEG) tube placement is a widespread method of delivering sustained nutrition to individuals requiring long-term support. Multiple techniques exist to achieve this, and adverse events can arise if not done properly including but not limited to pneumoperitoneum and bowel perforation. Safeguard tactics exist to prevent these complications but they are not always successful. Herein, we explore a case of PEG tube misplacement through the transverse colon.  A 69-year-old male with a history of advanced dementia, cerebrovascular accident (CVA), and seizure disorder presented for a replacement of his malfunctioning PEG tube at a different site. On postoperative day one, the patient developed abdominal pain and shortness of breath. His subsequent imaging workup revealed pneumoperitoneum, and the patient ultimately underwent an exploratory laparotomy to repair the damage, washout his abdomen, and reinsert the PEG tube. Postoperatively, the patient had a lengthy hospital stay, which was complicated and prolonged by sepsis and mechanical ventilation.  The PEG tube placement procedure is not without its difficulties in all stages, pre- intra- and post-operatively, especially in patients with neurocognitive compromise, therefore, it is important to continue exploring methods to optimize the operation.

ABSTRACT

Background The most common soft tissue tumor of the hand is the dorsal ganglion cyst and often is an indication for surgical excision. The differential diagnoses for dorsal hand masses include but are not limited to tenosynovitis, epidermoid cyst, abscess, lipoma, xanthoma, rheumatoid nodule, tophus, carpal boss, myositis ossificans, foreign body granuloma, neoplasm, and anomalous muscles. Case Description Our case report reflects the latter, a 33-year-old female who presented with a 3-year history of a symptomatic dorsal ganglion cyst of her left wrist. She is a female in her 30s, with progressive, worsening pain on wrist extension. Intraoperatively, it was determined that an anomalous extensor digitorum brevis manus (EDBM) was present and may have contributed to her pain and was excised. Literature Review The literature is sparse with the surgical management of EDBM. This case study aims to explore algorithms for managing simultaneous EDBM with a dorsal wrist ganglion, which is clinically relevant for any hand surgeon, when faced with a similar intra-operative management situation.

ABSTRACT

Appendicitis is a common cause of right lower quadrant pain. However, appropriate diagnostic evaluation and a high clinical suspicion can reveal alternative etiologies that are not so commonly encountered. In this report, we present a rare case of an appendiceal collision tumor involving two distinct neoplasms as the source of the patient's pain and describe how thoughtful clinical maneuvering led to its diagnosis and treatment.

ABSTRACT

Automatic washing machine injuries are more commonly associated with minor injuries in the pediatric population but may cause life and limb-threatening adult injuries in rare instances. This case describes a severe upper extremity injury after a schizophrenic patient placed her arm into a running machine. Herein, we describe the management, complex reconstruction, and repair of radial, ulnar, and metacarpal fractures in addition to transected tendons and vasculature. The patient had an excellent functional outcome with minor restrictions in motion and complete recovery of sensation.

INTRODUCTION: A true brachial artery aneurysm is a rare pathology with an incidence of 0.17% of all peripheral artery aneurysms. This pathology can manifest devastating complications if overlooked, however, a high index of suspicion coupled with a thorough history and physical allows easy diagnosis. We present a rare case of the oldest documented patient with a true brachial artery aneurysm with idiopathic etiology. PRESENTATION OF CASE: An 83-year-old gentleman presented with left upper extremity pain, erythema, and swelling for 1 week. He denied trauma to the area. Examination revealed a pulsatile mass of the antecubital fossa and decreased distal pulses. Imaging illustrated a 9mm aneurysm of the brachial artery with stenosis of the radial artery and non-enhancement of the origin of the ulnar artery. The patient underwent a brachial aneurysm excision, radial and ulnar embolectomy, and brachial to ulnar and radial artery bypass. Postoperatively, palpable pulses were appreciated in the radial and ulnar arteries. Pathology demonstrated a true aneurysm. The patient's postoperative course was uneventful and follow-up 6 months later revealed normal perfusion. DISCUSSION: This case highlights the importance of maintaining a high index of suspicion coupled with a thorough history and physical examination when encountering neurovascular complaints of the upper extremities. Operative intervention even in asymptomatic patients is warranted due to a high complication rate of 33%. CONCLUSION: More research into the pathophysiology of this rare pathology is needed to further understand, prevent, or mitigate its complications.

ABSTRACT

Microvascular injury is associated with accelerated kidney transplant dysfunction and allograft failure. Molecular pathology can identify new mechanisms of microvascular injury while improving on the diagnostic and prognostic capabilities of traditional histology. We conducted a case-control study of archived kidney biopsy specimens stored up to 10 years with microvascular injury (n = 50) compared with biopsy specimens without histologic injury (n = 45) from patients of similar age, race, and sex. We measured WNT gene expression with a multiplex quantification platform by using digital barcoding, given the importance of WNT reactivation to the response to wounding in the kidney microvasculature and other compartments. Of 210 genes from a commercial WNT panel, 71 were associated with microvascular injury and 79 were associated with allograft failure, with considerable overlap of genes between each set. Molecular pathology identified 46 biopsy specimens with molecular evidence of microvascular injury; 18 (39%) were either C4d negative, donor-specific antibody negative, or had no microvascular injury by histology. The majority of cases with molecular evidence of microvascular injury had poor long-term outcomes. We identified novel WNT pathway genes associated with microvascular injury and allograft failure in residual clinical biopsy specimens obtained up to 10 years earlier. Further mechanistic studies may identify the WNT pathway as a new diagnostic and therapeutic target.
