ABSTRACT

Public communication on water availability is pivotal in highlighting water conservation needs as droughts impact water resources for critical use, such as drinking water quality and accessibility. This paper presents the results of research into public communication on water availability and the implementation of water conservation measures in the Republic of Ireland. The paper analyses social media (Twitter and Facebook) communication and newspaper publications from 2018 to 2020 on water conservation and drought events, in addition, to undertaking six key stakeholder interviews made up of journalists (n = 4), political representatives (n = 1), and a water and communication expert (n = 1). Our analysis indicates that Irish newspapers' coverage of drought and water availability was greater in 2018 compared to 2020. Uncertainty and risk was also identified as the prevalent frame, used by newspapers to cover drought events. Although the sentiments in communications on drought by the national utility, Irish Water, were scored as positive (63%), its engagement with the public on social media was considerably limited. Accessible information platforms that provides data and information on water resources were also found; nevertheless, no comprehensive national drought information management system nor national drought plan have been developed. Based on our findings, we demonstrate the need for public engagement and collaborative efforts to communicate drought and water conservation measures led by An Fóram Uisce|The Water Forum. Recommendations made in this study also aim to influence decision-making and awareness among stakeholders regarding drought communication on water conservation and resources availability.

ABSTRACT

The unprecedented scale and impact of COVID-19 pandemic, and the accompanying lockdown implemented across many countries, has exacerbated water scarcity and security globally. Many European governments have introduced policy interventions to mitigate and protect their economies. Yet, water resources, which are a potential enabler in revitalising Europe's economy, have received few of such policy interventions since the World Health Organization declared the COVID-19 outbreak as a public health emergency only in January 2020. Our scoping review of preliminary government responses of 27 European countries revealed that only 11 (40.7%) of these countries implemented at least one policy intervention that considered the water sector. These interventions were typically short-term measures involving either full cost absorption or deferment of water bills. Much attention on water governance and management processes, policies and financial investments required to augment the resilience of the water sector amid a growing scarcity of freshwater, triggered by extreme climate variation and also by COVID-19 pandemic should therefore be central to post COVID-19 recovery efforts in Europe. This paper also proposes future research directions, including a study that will harmonise water demand and consumption trends during the pandemic in Europe and an assessment on how the water sector can withstand possible external shocks in future.

ABSTRACT

PURPOSE: To compare stereoscopic digital mammography (DM) with standard DM for the rate of patient recall and the detection of cancer in a screening population at elevated risk for breast cancer. MATERIALS AND METHODS: Starting in September 2004 and ending in December 2007, this prospective HIPAA-compliant, institutional review board-approved screening trial, with written informed consent, recruited female patients at elevated risk for breast cancer (eg, personal history of breast cancer or breast cancer in a close relative). A total of 1298 examinations from 779 patients (mean age, 58.6 years; range, 32-91 years) comprised the analyzable data set. A paired study design was used, with each enrolled patient serving as her own control. Patients underwent both DM and stereoscopic DM examinations in a single visit, findings of which were interpreted independently by two experienced radiologists, each using a Breast Imaging Reporting and Data System (BI-RADS) assessment (BI-RADS category 0, 1, or 2). All patients determined to have one or more findings with either or both modalities were recalled for standard diagnostic evaluation. The results of 1-year follow-up or biopsy were used to determine case truth. RESULTS: Compared with DM, stereoscopic DM showed significantly higher specificity (91.2% [1167 of 1279] vs 87.8% [1123 of 1279]; P = .0024) and accuracy (90.9% [1180 of 1298] vs 87.4% [1135 of 1298]; P = .0023) for detection of cancer. Sensitivity for detection of cancer was not significantly different for stereoscopic DM (68.4% [13 of 19]) compared with DM (63.2% [12 of 19], P .99). The recall rate for stereoscopic DM was 9.6% (125 of 1298) and that for DM was 12.9% (168 of 1298) (P = .0018). CONCLUSION: Compared with DM, stereoscopic DM significantly improved specificity for detection of cancer, while maintaining comparable sensitivity. ABSTRACT

UNLABELLED: The technology revolution in image acquisition, instrumentation, and methods has resulted in vast data sets that far outstrip the human observers' ability to view, digest, and interpret modern medical images by using traditional methods. This may require a paradigm shift in the radiologic interpretation process. As human observers, radiologists must search for, detect, and interpret targets. Potential interventions should be based on an understanding of human perceptual and attentional abilities and limitations. New technologies and tools already in use in other fields can be adapted to the health care environment to improve medical image analysis, visualization, and navigation through large data sets. This historical psychophysical and technical review touches on a broad range of disciplines but focuses mainly on the analysis, visualization, and navigation of image data performed during the interpretive process. Advanced postprocessing, including three-dimensional image display, multimodality image fusion, quantitative measures, and incorporation of innovative human-machine interfaces, will likely be the future. Successful new paradigms will integrate image and nonimage data, incorporate workflow considerations, and be informed by evidence-based practices. This overview is meant to heighten the awareness of the complexities and limitations of how radiologists interact with images, particularly the large image sets generated today. Also addressed is how human-machine interface and informatics technologies could combine to transform the interpretation process in the future to achieve safer and better quality care for patients and a more efficient and effective work environment for radiologists. ABSTRACT

PURPOSE: To assess the diagnostic performance of digital radiography using charge-coupled device (CCD) technology in the detection of rib fracture in infant abuse. MATERIALS AND METHODS: Four fractured posterior rib arcs and eight normal ribs removed at autopsy from a 10-month-old abused infant were radiographed using a CCD prototype, four clinical film-screen systems, and direct-exposure film. Each rib was viewed with these six systems in nine different projections. The resultant 648 images were assessed for probability of fracture (0-100%) by four pediatric radiologists. The calculated area under the resultant ROC curves (A(z)) for the CCD was compared with those obtained with direct-exposure, high-detail, medium and fast film-screen radiographic systems. RESULTS: The mean A(z) for the CCD (0.937) fell within the range of the high-detail systems (0.934-0.940) and was significantly higher (p < 0.05) than the medium and fast system (0.861 and 0.858, respectively). Despite a lower spatial resolution than direct-exposure film (7 line pairs per millimeter vs. > 20 line pairs per millimeter) the CCD performance was comparable (A(z) = 0.944 vs. 0.937). The similar performance can be explained by the higher contrast resolution of the digital technology. CONCLUSION: This study indicates that in the ex vivo setting, digital radiology can perform comparably to high-detail film-screen imaging. The findings suggest that digital radiography has the potential to replace film-screen imaging in the evaluation of inflicted skeletal injury in infants.

ABSTRACT

RATIONALE AND OBJECTIVES: The purpose of this study was to measure and to clarify the diagnostic contributions of image-based features in differentiating benign from malignant and hepatocyte-containing from non-hepatocyte-containing liver lesions. MATERIALS AND METHODS: Six experienced abdominal radiologists each read images from 146 cases (including a contrast material-enhanced computed tomographic [CT] scan and contrast-enhanced and unenhanced magnetic resonance [MR] images) following a checklist-questionnaire requiring them to rate quantitatively each of as many as 131 image features and then reported on each of the two differentiations. The diagnostic value of each feature was assessed, and linear discriminant analysis was used to develop statistical prediction rules (SPRs) for merging feature data into computerized "second opinions." For the two differentiations, accuracy (area under the receiver operating characteristic curve [Az]) was then determined for the radiologists' readings by themselves and for each of three SPRs. RESULTS: Thirty-seven candidate features had diagnostic value for each of the two differentiations (a slightly different feature set for each). Radiologists' performance at both differentiations was excellent (Az = 0.929 [benign vs malignant] and 0.926 [hepatocyte-containing vs non-hepatocyte-containing]). Performance of the SPR that operated on the features from all modalities together was better than that of radiologists (Az = 0.936 [benign vs malignant] and 0.951 [hepatocyte-containing vs non-hepatocyte-containing]), but this difference was of marginal statistical significance (P = .11). Contrast-enhanced MR imaging and contrast-enhanced CT each made significant adjunctive contributions to accuracy compared with unenhanced MR imaging alone. CONCLUSION: Many CT- and MR imaging-based features have diagnostic value in differentiating benign from malignant and hepatocyte-containing from non-hepatocyte-containing liver lesions. Radiologists could also benefit from the fully informed SPR's "second opinions."
