ABSTRACT

The article presents the results of the study aimed to the evaluation the efficacy of combination therapy with alpha1-blocker (tamsulosin) and phosphodiesterase type 5 inhibitor (sildenafil) in patients with urination disorders and erectile dysfunction (ED). A pilot observational study involving 60 men aged from 50 and 80 years divided into 3 groups of 20 people was performed. Group 1 of patients received sildenafil 25 mg daily (dynamico), Group 2--tamsulosin 0.4 mg daily (Omnic-Ocas), and Group 3--tamsulosin 0.4 mg (Omnic-Ocas) and sildenafil 25 mg (dynamico) daily. The visits were carried out at the stage of screening, further--every 10 days (a total number--4 visits). Combination therapy of urination disorders and ED contributed to the significant improvement in uroflowmetry, the stopping of complaints according to the IPSS and IIEF-15 questionnaires, and improving the quality of life (according to the QoL questionnaire). Combination therapy significantly decreased obstructive and irritative symptoms, increased the maximum urine flow rate within the period of observation, as well as significantly decreased the residual urine volume, more pronounced when compared with monotherapy. Significant positive effect on erectile component and all components of the overall satisfaction in the sexual sphere were registered, that as a consequence led to the positive impact on the quality of life in patients treated with PDE5 inhibitor. Given the high prevalence of urinary disorders and erectile dysfunction, combined therapy with alpha1-blockers and PDE5 inhibitors in this case should be a promising area for drug therapy.

ABSTRACT

The study included 320 men with suspected prostate cancer (PC). The mean age was 59 years. The detection rate of prostate cancer depending on PSA level was evaluated. According to the results of histological examination, prostate cancer was detected in 32.8% (105 patients) of cases. The prostate cancer most frequently was diagnosed in patients with PSA levels above 20 ng/ml. Thus, the probability of detection of prostate cancer is directly dependent on the level of PSA. All groups showed signs of prostatic intraepithelial neoplasia (PIN), both low and high degrees, but the direct relationship of identification of PIN depending on PSA level was not established.

ABSTRACT

On the old (18-20 months) male rats of Wistar population the influence of the 10 days evening melatonin injections in physiological rang doses on morphological state pineal gland, was studied. It has been shown, that the course of melatonin injections to old rats brings appearance of histological pattern of pineal gland activation: increasing of area of pinealocytes nuclear and density of nuclear and cytoplasm with stein by hallocyanine on total nucleic acid that means stimulation to material both indole, and peptide nature production. Using of melatonin in dose 0.05 mg/kg mass of the body is sparing for pinealocytes of the old rats and slows the apoptosis processes on background of polyploidization. Using of melatonin in dose 0,5 mg/kg mass of the body causes cell's overstrain and induces the forced apoptosis. It's possible to suppose that the geroprotective effect of the evening injections of melatonin is increased due to stimulation of the biosynthesis of neuropeptides in pineal gland.

ABSTRACT

The genetic determination of insulin-dependent (IDDM) and non-insulin-dependent (NIDDM) diabetes mellitus and thyroid gland diseases (TGD) was carried out using clinical-genealogical data on 229 patients with IDDM, 275 patient with NIDDM, 247 patients with TGD and their relatives. Results of component decomposition showed the role of genetic factors in the determination of these diseases and the existence of interloci interaction in genetic control of IDDM and NIDDM. The genetic independence of these diseases and genetic homogeneity of NIDDM were revealed using models of Ch. Smith and T. Reich.
