ABSTRACT

Objective In the present study, we investigated the intra and interobserver agreement of the new Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association (AO/OTA) classification for fractures of the proximal extremity of the femur. Methods One hundred hip radiographs were selected from patients who suffered fractures of the trochanteric region or femoral neck. Four orthopedists, fellowship trained hip surgeons, and four orthopedic residents evaluated and classified fractures according to the new AO/OTA system on two separate occasions. The kappa (k) coefficient was used to evaluate intra and interobserver agreement in the different steps of the classification, namely: type , group , subgroup , and qualifier. Results Hip surgery experts obtained almost perfect intraobserver agreement of type , substantial for group and, only moderate, for subgroup and qualifiers. The residents had lower performance, with substantial agreement for type, moderate for group , and reasonable for subgroup and qualifier. In the specialists' interobserver evaluation, there was also a gradual decrease in the agreement between type (almost perfect) and group (moderate), which was even lower for subgroup and qualifiers. Residents had a substantial interobserver agreement for type , moderate for group , and reasonable in the other branches. Conclusion The new AO/OTA classification for fractures of the trochanteric region and femoral neck showed intra and interobserver agreements considered appropriate for type and group , with a drop in the subsequent branches, that is, for subgroup and qualifier. Still, in relation to the old AO/OTA classification, there was an improvement in the agreements for subgroup.

Objective The present study aimed to determine the average hip anthropometry of a regional Brazilian population using measurements based on computerized axial tomography (CAT). Methods Retrospective, descriptive analysis of hip measurements from 200 abdominal CATs from patients visiting a medical center. The tests were selected at random to determine 30 previously defined anthropometric measurements. The data were statistically analyzed and compared according to gender and age. Results The prevalence of hip dysplasia was 6%. Signs suggesting femoroacetabular impingement were seen in 26% of cases. Patients over 50 years old presented significantly greater measures of horizontal acetabulum sectors, center-edge angle, and acetabular arch, as well as lower extrusion index, cervical-diaphyseal angle and vertical offset. Some measurements were significantly different according to gender: the lateral center-edge angle (µ = 35.5°) and the acetabular arch (µ = 68.7°) were higher in females. Males presented increased extrusion index (µ = 16%), lateral offset (µ = 38.3 mm), depth (µ = 19.5 mm), and neck diameter (µ = 26.4 mm). Conclusion The present study characterized the hip anthropometry of a regional Brazilian population. It also demonstrated significant morphological differences per age group and gender.

Resumo Objetivo Determinar a antropometria média do quadril de uma população regional brasileira através de medidas obtidas pela tomografia axial computadorizada (TAC). Método Análise analítico-descritiva, retrospectiva, de medidas coxofemorais de 200 TACs do abdômen de pacientes atendidos em um centro médico. Foram selecionados aleatoriamente exames que permitissem a aferição de 30 medidas antropométricas previamente definidas. Os dados foram estatisticamente analisados e comparados quanto a sexo e idade. Resultados A prevalência de displasia do quadril foi de 6%. Sinais sugestivos de impacto fêmoro-acetabular foramvistosem26% doscasos.A análise dos resultadosnogrupoacima de 50 anos demonstrou medidas significativamente maiores dos: setores horizontais do acetábulo,doângulocentro-bordaedoarcoacetabular,acompanhadosdemenoríndicede extrusão, ângulo cérvico-diafisário e offset vertical. Algumas medidas foram significativamente diferentes em função do sexo: o ângulo centro-borda lateral (µ» 35.5°) e o arco acetabular(µ» 68.7°)semostraram maioresnosexo feminino.Nogrupomasculino,foram maiores o índice de extrusão (µ» 16%), o offset lateral (µ» 38,3mm), a profundidade (µ» 19,5mm) e o diâmetro do colo (µ» 26,4mm). Conclusão O presente estudo caracterizou a antropometria do quadril de uma população brasileira. Demonstrou ainda diferenças morfológicas significativas do quadril entre diferentes faixas etárias e sexos.

Resumo Objetivo Neste estudo, investigamos a concordância intra e interobservador da nova classificação Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association (AO/OTA) para fraturas da extremidade proximal do fêmur. Métodos Foram selecionadas 100 radiografias do quadril de pacientes que sofreram fraturas da região trocantérica ou do colo do fêmur. Quatro ortopedistas cirurgiões de quadril e quatro residentes de ortopedia e traumatologia avaliaram e classificaram as fraturas segundo o novo sistema AO/OTA em duas ocasiões distintas. O coeficiente de kappa (k) foi utilizado para avaliar a concordância intra e interobservadores nos diferentes passos da classificação, a saber: tipo, grupo, subgrupo e qualificador. Resultados Especialistas em cirurgia do quadril obtiveram concordância intraobservador quase perfeita de tipo, substancial para grupo e, apenas moderada para subgrupo e qualificadores. Os residentes tiveram desempenho inferior, com concordância substancial para o tipo, moderada para o grupo, e razoável para o subgrupo e qualificador. Na avaliação interobservadores dos especialistas, também se observou queda gradual da concordância entre tipo (quase perfeita) e grupo (moderada), que se mostrou ainda menor parasubgrupo e qualificadores.Residentestiveramumaconcordânciainterobservadoressubstancialparatipo, moderada para grupo e razoável nas demais ramificações. Conclusão A Nova Classificação AO/OTA para fraturas da região trocantérica e do colo do fêmur mostrou concordâncias intra e interobservadores consideradas adequadas para tipo e grupo com queda nas ramificações subsequentes ou seja para subgrupo e qualificador. Ainda assim em relação à classificação AO/OTA antiga houve melhora nas concordâncias para subgrupo.

ABSTRACT

OBJECTIVES: Femoroacetabular impingement (FAI) has been recently related to several pathologies, besides chondral injury and hip arthritis. We aim to investigate the prevalence of FAI morphology in an elderly cohort hospitalized due to a proximal femur fracture and compare these findings to a control group. We hypothesize that limited medial rotation due to FAI's morphology could increase stresses to the proximal femur, acting as a facilitating mechanism for fractures in this region. Therefore, a higher prevalence of FAI morphology would be present in the study group. METHODS: A retrospective cross-sectional study was performed based on the analysis of radiographic images in AP and lateral views of the fractured hip. Firstly, we have set to measure FAI prevalence in an elderly cohort victimized by fractures of the proximal by measures of the alpha, Tönnis, and lateral center edge angles of a hundred consecutive patients hospitalized for proximal femur fractures. Secondly, we have analyzed the possible relationship between the FAI subtypes and the type of fracture. Finally, we have compared this sample's data with that of a similar control cohort not affected by fracture. RESULTS: The cohort in this study displayed a higher prevalence of pathological changes in the Tönnis, center-edge, and alpha angles with odds ratios of 3.41, 2.56, and 4.80, respectively (with statistical significance). There was also a significant relationship between cam-type FAI and intertrochanteric fractures, corroborating our initial hypotheses. CONCLUSIONS: This study demonstrated that a cohort of older patients affected by fractures of the proximal femur had an increased prevalence of radiographic signs of femoroacetabular impingement. Furthermore, this is the first study demonstrating a statistically significant relationship of cam-type FAI with intertrochanteric fractures, suggesting a possible cause and effect relationship.

ABSTRACT

CASE: We report the case of a patient who suffered traumatic avulsion of the gluteus medius and minimus tendons associated with posterior fracture-dislocation of the femoral head. The patient was treated with open reduction and internal fixation, followed by gluteus tenodesis with intraosseous anchors. CONCLUSION: Although in isolation, gluteus medius or gluteus minimus tendons tear, hip dislocation, and head fractures are relatively common injuries; the concurrence of these lesions is noteworthy for its rarity. This patient had an uneventful postoperative evolution, with a complete functional recovery, healing of the femoral head fracture and the tendons, and absence of signs of avascular necrosis.

Objective To determine if the dimensions of selected national and imported implants used in total hip arthroplasty surgeries are adequate regarding the anthropometric profile of the Brazilian population. Methods A retrospective study of patients submitted to primary total hip arthroplasty. Patients with femoral or acetabular morphological deformities that could influence the choice of implant size, including dysplasia and previous fractures, were excluded from the study. Two national implants and six imported implants were included in the study. Every patient was treated by the same group, with one of the four senior authors or under their strict supervision, following the same surgical technique. The data were analyzed regarding gender, age, fixation type and implant model. Results The analysis of 682 hips submitted to total hip arthroplasty demonstrated that 2 models of cemented femoral stems and 1 of non-cemented stem did not seem to be perfectly adequate for the femoral morphology of the studied population, since these 3 implants were mostly used in the smallest size available, resulting in a non-Gaussian curve. The mean diameter of the native acetabulum was 54 mm in men and 52 mm in women. Conclusion Out of the eight models studied, five were deemed adequate for the studied population. The other three models available in our market (two national models and one imported model) apparently require more appropriate sizing. We emphasize that anthropometric studies of the Brazilian hip are necessary to give scientific subsidies to the ideal implant design for our market.

Resumo Objetivo Definir se as dimensões de alguns implantes de fabricação nacional e estrangeira utilizados nas cirurgias de artroplastia do quadril estão adequadas ao perfil antropométrico da população brasileira. Métodos Estudo retrospectivo de pacientes submetidos a cirurgia primária de substituição artroplástica total do quadril. Foram excluídos deste estudo pacientes comalterações morfológicas femorais ou acetabulares que pudessem influenciar na escolha do tamanho dos implantes, tais comosequelas de displasia, trauma, entre outras. Foramincluídos neste estudo dois implantes de fabricação nacional e seis modelos importados. Todos os pacientes foram operados pela mesma equipe, por um dos quatro autores seniores ou sob sua supervisão direta, seguindo a mesma técnica cirúrgica. Os dados foram estatisticamente analisados em relação a gênero, idade, tipo de fixação e modelo dos implantes. Resultados A análise estatística de 682 quadris submetidos a artroplastia total demonstrou que 2 modelos de hastes femorais cimentadas e 1 de haste não cimentada não estão perfeitamente adequados à morfologia femoral da população estudada, pois, na maioria dos casos, nestes 3 implantes, foi utilizado o menor tamanho disponível, resultando em uma curva não gaussiana. O diâmetro médio do acetábulo nativo foi de 54mm nos homens e de 52mm nas mulheres. Conclusão Dos oitomodelos estudados, cinco semostraram adequados à população estudada. Outros três modelos disponíveis em nosso mercado (dois nacionais e um importado) parecem necessitar de um escalonamento mais adequado. Destacamos que são necessários estudos antropométricos do quadril da população brasileira para dar subsídios científicos ao desenho ideal dos implantes para o nosso mercado.
