ABSTRACT

Nevus lipomatosus superficialis is a rare disorder characterized by a nevoid fatty growth within the papillary and reticular dermis. Lesions more often occur in the sacral, abdominal, or pelvic regions. A 36-year-old Brazilian female with giant nevus lipomatosus is reported. Our case seems to be the biggest reported in the literature.

ABSTRACT

BACKGROUND: The treatment of chronic lesions in hidradenitis suppurativa remains a challenge. For some clinical types surgical management is an excellent alternative. OBJECTIVE: This study evaluates an alternative surgical approach for the treatment of hidradenitis suppurativa of specific bridging lesions by subcutaneous resection of the tubular fibrotic tissue. METHODS: Periorificial fusiform skin incisions were made around the orifices parallel to the axillary or inguinal folds and the subcutaneous tubular fibrotic tissue was removed en bloc before skin suturing. RESULTS: The outcome was evaluated as satisfactory due to lower morbidity, minimizing the excised skin areas, prevention of bridles or adherences, shorter incisions, no healing difficulties, and less dehiscence or wound exposure. CONCLUSION: The subcutaneous fistulectomy is a surgical option in bridging hidradenitis suppurativa.

ABSTRACT

A bilobed island flap with an extended subcutaneous pedicle including part of the transverse portion of the elevator muscle of the nasal ala is described. This flap was employed for nasal ala reconstruction in 21 patients following excision of malignancy. The series includes 12 females and 9 males ranging in age from 25 to 83 years. In 19 patients, the reconstruction spared the mucosa layer and in two a skin graft was used under the flap to reconstruct the lining. The flap was viable in all cases and the aesthetic outcome satisfactory. The rotation fold dog ear is eliminated. In planning the BIF the length of its lobes (proximal and distal), should be equal to the defect; the width of the proximal lobe, compared to the defect, and the distal lobe compared to the proximal lobe should be progressively narrower.

ABSTRACT

A hidradenite supurativa e uma doenca cutanea de evolucao cronica e localizacao peculiar, intimamente relacionada a presenca de glandulas sudoriparas do tipo apocrino. Resulta de obstrucao dos foliculos glandulares com subsequente rotura e infeccao local. Apresenta progressiva evolucao se nao tratada adequadamente, incorrendo em acentuada morbidade. Devido a escassez de relatos semelhantes, o tratamento da hidradenite permanece assunto controverso, nao havendo ate o momento, normatizacao das condutas a serem empregadas em tais casos. O presente estudo visou a analise da eficacia das medidas terapeuticas empregadas em 32 casos de pacientes que necessitaram de internacao no HCFMUSP, no periodo de 1985 a 1990. Com relacao a localizacao, 75 por cento dos pacientes possuiam lesoes perianais, seguida por incidencia similar de lesoes axilares e inguinoperineais em cerca de 40 por cento dos casos. Utilizando-se os indices de recidiva e evolucao morbida da doenca, pode-se verificar a eficiencia dos metodos terapeuticos. Tendo em vista a necessidade de intervencao com resultados mais imediatos e eficazes, o tratamento cirurgico constitui-se na melhor opcao.

ABSTRACT

Hidradenitis suppurativa is a cutaneous disease of peculiar localization, related to the presence of apocrine glands. It results from the obstruction of the glandular follicle with subsequent rupture and local infection. The evolution uses to be prolonged and follows progressive course if not adequately treated. The therapeutic options have not been standardized yet. This study aims to analyze the therapeutic procedures applied in 32 patients of the University Hospital of São Paulo. Concerning to localization, 75% of the lesions were perianal, followed by a similar incidence at axillary and inguino-perineal regions. Considering the recurrence index and evolution of the disease we came to the conclusion that the surgical treatment produced the best results.

Embora descrita ha mais de um seculo a hidradenite supurativa apresenta varios aspectos ainda desconhecidos, principalmente quanto a sua etiopatogenia. Em relacao ao tratamento das lesoes em fase aguda ou em fase cronica, porem limitadas, existe pouca discordancia. As controversias surgem na terapeutica das lesoes cronicas extensas. O presente artigo analisa as abordagens atuais frente a esta situacao e propoe conduta cirurgica menos agressiva, porem tao satisfatoria quanto aquelas mais radicais
