ABSTRACT

l-asparaginases play a crucial role in the treatment of acute lymphoblastic leukemia (ALL), a type of cancer that mostly affects children and teenagers. However, it is common for these molecules to cause adverse reactions during treatment. These downsides ignite the search for novel asparaginases to mitigate these problems. Thus, this work aimed to produce and characterize a recombinant asparaginase from Phaseolus vulgaris (Asp-P). In this study, Asp-P was expressed in Escherichia coli with high yields and optimum activity at 40 °C, pH 9.0. The enzyme Km and Vmax values were 7.05 mM and 1027 U/mg, respectively. Asp-P is specific for l-asparagine, showing no activity against l-glutamine and other amino acids. The enzyme showed a higher cytotoxic effect against Raji than K562 cell lines, but only at high concentrations. In silico analysis indicated that Asp-P has lower immunogenicity than a commercial enzyme. Asp-P induced biofilm formation by Candida sp. due to sublethal dose, showing an underexplored potential of asparaginases. The absence of glutaminase activity, lower immunogenicity and optimal activity similar to physiological temperature conditions are characteristics that indicate Asp-P as a potential new commercial enzyme in the treatment of ALL and its underexplored application in the treatment of other diseases.

ABSTRACT

INTRODUCTION: 3-dimensional printing has enabled the development of unique and affordable additive manufacturing, including the prototyping and production of surgical forceps. Objective: demonstrate the development, 3D printing and mechanical-functional validation of a laparoscopic grasping forceps. METHODS: the clamp was designed using a computer program and printed in 3 dimensions with polylactic acid (PLA) filament and added 5 screws for better leverage. Size and weight measurements were carried out, as well as mechanicalfunctional grip and rotation tests in the laboratory with a validated simulator. RESULTS: Called "Easylap", the clamp weighed 48 grams, measured 43cm and was printed in 8 pieces, taking an average of 12 hours to produce. It allowed the simulation of the functional characteristics of laparoscopic pressure forceps, in addition to the rotation and rack locking mechanism. However, its strength is reduced due to the material used. CONCLUSION: It is possible to develop plastic laparoscopic grasping forceps through 3-dimensional printing.

ABSTRACT

This article raises considerations about the use of Cardiovascular Risk (CVR) stratification tools in Primary Health Care (PHC), focusing on the behavior of non-laboratory models as an alternative to laboratory predictions. This reflection is based on empirical research with a cross-sectional and exploratory methodology, focused on the behavior of two scales in an adult population (aged 40 to 74), with comorbidities (Hypertension and/or Diabetes) and without recorded cardiovascular events, in a medium-sized city in Minas Gerais. In the work, titled the "CardioRisk" project, the degree of agreement between CVR stratification performed by the Framingham Global Risk Score, using cholesterol information, and the non-laboratory version of the HEARTS calculator, which uses Body Mass Index instead of serum data, was evaluated. In this paper, the overall results of the research are analyzed, in which, for the constituted sample, a minimal agreement was found between the stratifiers. Recommendations are also presented regarding the management of CVR in the context of PHC, highlighting the importance of a comprehensive assessment of patients at notable risk, such as people with Diabetes.


Este artigo levanta considerações sobre o uso de ferramentas de estratificação do Risco Cardiovascular (RCV) na Atenção Primária à Saúde (APS), com foco no comportamento de modelos não-laboratoriais como alternativa às predições laboratoriais. Reflete-se sobre tal objeto a partir de pesquisa empírica de metodologia transversal e exploratória, voltada para o comportamento de duas escalas em uma população adulta (entre 40 e 74 anos), com comorbidades (Hipertensão e/ou Diabetes) e sem agravos cardiovasculares registrados, em uma cidade de médio porte de Minas Gerais. No trabalho, intitulado projeto "CardioRisco", avaliou-se o grau de concordância entre a estratificação de RCV realizada pelo Escore de Risco Global de Framingham, a partir de informação sobre colesterol, e pela versão não-laboratorial da calculadora HEARTS, que utiliza o Índice de Massa Corporal em substituição ao dado sérico. Neste artigo, analisa-se o resultado geral da pesquisa, no qual, para a amostra constituída, constatou-se uma concordância mínima entre os estratificadores, bem como apresenta-se recomendações em relação ao manejo do RCV no contexto da APS, com destaque para a importância da avaliação integral do paciente de notório risco, como pessoas com Diabetes.

ABSTRACT

Comparisons between simulated and experimental adsorption isotherms in MOFs are fraught with challenges. On the experimental side, there is significant variation between isotherms measured on the same system, with a significant percentage (∼20%) of published data being considered outliers. On the simulation side, force fields are often chosen "off-the-shelf" with little or no validation. The effect of this choice on the reliability of simulated adsorption predictions has not yet been rigorously quantified. In this work, we fill this gap by systematically quantifying the uncertainty arising from force field selection on adsorption isotherm predictions. We choose methane adsorption, where electrostatic interactions are negligible, to independently study the effect of the framework Lennard-Jones parameters on a series of prototypical materials that represent the most widely studied MOF "families". Using this information, we compute an adsorption "consensus isotherm" from simulations, including a quantification of uncertainty, and compare it against a manually curated set of experimental data from the literature. By considering many experimental isotherms measured by different groups and eliminating outliers in the data using statistical analysis, we conduct a rigorous comparison that avoids the pitfalls of the standard approach of comparing simulation predictions to a single experimental data set. Our results show that (1) the uncertainty in simulated isotherms can be as large as 15% and (2) standard force fields can provide reliable predictions for some systems but can fail dramatically for others, highlighting systematic shortcomings in those models. Based on this, we offer recommendations for future simulation studies of adsorption, including high-throughput computational screening of MOFs.

ABSTRACT

Purpose: To assess the effectiveness of topical and subconjunctival bevacizumab in suppressing vascularization in graft and host bed after high-risk corneal transplantation. Design: Secondary analysis of prospective, randomized, double-blind, placebo-controlled multicentric clinical trial. Participants: The study includes patients aged > 18 years who underwent high-risk penetrating keratoplasty, which was defined as corneal vascularization in ≥ 1 quadrants of the corneal graft and host bed, excluding the limbus. Methods: Patients were randomized to treatment and control groups. The patients in the treatment group received subconjunctival injection of bevacizumab (2.5 mg/0.1 ml) on the day of the procedure, followed by topical bevacizumab (10 mg/ml) 4 times per day for 4 weeks. The patients in control group received injection of vehicle (0.9% sodium chloride) on the day of procedure, followed by topical vehicle (carboxymethylcellulose sodium 1%) 4 times a day for 4 weeks. Main Outcome Measures: Vessel and invasion area of vessels in the corneal graft and host beds. Results: This study included 56 eyes of 56 patients who underwent high-risk corneal transplantation, with equal numbers in the bevacizumab and vehicle (control) treatment groups. The mean age of patients who received bevacizumab was 61.2 ± 15.9 years, and the mean age of those treated with vehicle was 60.0 ± 16.1 years. The vessel area at baseline was comparable in the bevacizumab (16.72% ± 3.19%) and control groups (15.48% ± 3.12%; P = 0.72). Similarly, the invasion areas were also similar in the treatment (35.60% ± 2.47%) and control (34.23% ± 2.64%; P = 0.9) groups at baseline. The reduction in vessel area was significantly higher in the bevacizumab-treated group (83.7%) over a period of 52 weeks compared with the control group (61.5%; P < 0.0001). In the bevacizumab-treated group, invasion area was reduced by 75.8% as compared with 46.5% in the control group. The vessel area was similar at 52 weeks postprocedure in cases of first (3.54% ± 1.21%) and repeat (3.80% ± 0.40%) corneal transplantation in patients who received bevacizumab treatment. In the vehicle-treated patients, the vessel area was significantly higher in repeat (9.76% ± 0.32%) compared with first (8.06% ± 1.02%; P < 0.0001) penetrating keratoplasty. In the bevacizumab treatment group, invasion areas at week 52 were comparable in first (11.70% ± 3.38%) and repeat (11.64% ± 1.74%) procedures, whereas invasion area was significantly higher in repeat (27.87% ± 2.57%) as compared with first (24.11% ± 2.17%) penetrating keratoplasty in vehicle-treated patients. Conclusions: In patients undergoing vascularized high-risk corneal transplantation, bevacizumab is efficacious in reducing vascularization of corneal graft and host bed, thereby reducing the risk of corneal graft rejection in vascularized host beds. ABSTRACT

The incorporation of nanostructures loaded with bioactive compounds into food matrices is a promising approach to develop new functional foods with improved nutritional, health profiles and good sensorial properties. The rheological and tribological properties of yogurt enriched with curcumin-loaded solid lipid nanoparticles (SLN) were evaluated. Also, the TCA solubility index, the bioaccessibility of curcumin and cell viability were assessed after dynamic in vitro digestion. The presence of SLN in yogurt did not affect its rheological properties; however, SLN addition increased the lubrication capability of yogurt. After in vitro digestion, yogurt with added SLN (yogurt_SLN) presented a lower TCA solubility index (22 %) than the plain yogurt (39 %). The bioaccessibility and stability of curcumin were statistically similar for yogurt_SLN (30 % and 42 %, respectively) and SLN alone (20 % and 39 %, respectively). Regarding cell viability results, the intestinal digesta filtrates of both controls (i.e., SLN alone and plain yogurt) did not affect significantly the cell viability, while the yogurt_SLN presented a possible cytotoxic effect at the concentrations tested. In general, the incorporation of SLN into yogurt seemed to promote the mouthfeel of the yogurt and did not adversely affect the bioaccessibility of curcumin. However, the interaction of SLN and yogurt matrix seemed to have a cytotoxic effect after in vitro digestion, which should be further investigated. Despite that, SLN has a high potential to be used as nanostructure in a functional food as a strategy to increase the bioactive compounds' bioaccessibility.

PURPOSE: To assess Meibomian gland dysfunction using meibography in patients with xeroderma pigmentosum and correlate with ocular surface changes. METHODS: This cross-sectional study evaluated patients with xeroderma pigmentosum. All patients underwent a comprehensive and standardized interview. The best-corrected visual acuity of each eye was determined. Detailed ophthalmic examination was conducted, including biomicroscopy examination of the ocular surface, Schirmer test type I, and meibography, and fundus examination was also performed when possible. Meibomian gland dysfunction was assessed by non-contact meibography using Oculus Keratograph® 5M (OCULUS Inc., Arlington, WA, USA). Saliva samples were collected using the Oragene DNA Self-collection kit (DNA Genotek Inc., Ottawa, Canada), and DNA was extracted as recommended by the manufacturer. Factors associated with abnormal meiboscores were assessed using generalized estimating equation models. RESULTS: A total of 42 participants were enrolled, and 27 patients underwent meibography. The meiboscore was abnormal in the upper eyelid in 8 (29.6%) patients and in the lower eyelid in 17 (62.9%). The likelihood of having abnormal meiboscores in the lower eyelid was 16.3 times greater than that in the upper eyelid. In the final multivariate model, age (p=0.001), mutation profile (p=0.006), and presence of ocular surface malignant tumor (OSMT) (p=0.014) remained significant for abnormal meiboscores. For a 1-year increase in age, the likelihood of abnormal meiboscores increased by 12%. Eyes with OSMT were 58.8 times more likely to have abnormal meiboscores than eyes without ocular surface malignant tumor. CONCLUSION: In the final model, age, xeroderma pigmentosum profile, previous cancer, and clinical alterations on the eyelid correlated with a meiboscore of ≥2. Meibomian gland dysfunction was common in patients with xeroderma pigmentosum, mainly in the lower eyelid. The severity of Meibomian gland dysfunction increases with age and is associated with severe eyelid changes.

ABSTRACT

Medical adhesives are emerging as an important clinical tool as adjuvants for sutures and staples in wound closure and healing and in the achievement of hemostasis. However, clinical adhesives combining cytocompatibility, as well as strong and stable adhesion in physiological conditions, are still in demand. Herein, a mussel-inspired strategy is explored to produce adhesive coacervates using tannic acid (TA) and methacrylate pullulan (PUL-MA). TA|PUL-MA coacervates mainly comprise van der Waals forces and hydrophobic interactions. The methacrylic groups in the PUL backbone increase the number of interactions in the adhesives matrix, resulting in enhanced cohesion and adhesion strength (72.7 Jm-2), compared to the non-methacrylated coacervate. The adhesive properties are kept in physiologic-mimetic solutions (72.8 Jm-2) for 72 h. The photopolymerization of TA|PUL-MA enables the on-demand detachment of the adhesive. The poor cytocompatibility associated with the use of phenolic groups is here circumvented by mixing reactive oxygen species-degrading enzyme in the adhesive coacervate. This addition does not hamper the adhesive character of the materials, nor their anti-microbial or hemostatic properties. This affordable and straightforward methodology, together with the tailorable adhesivity even in wet environments, high cytocompatibility, and anti-bacterial activity, enables foresee TA|PUL-MA as a promising ready-to-use bioadhesive for biomedical applications.

BACKGROUND: Hemorrhagic shock (HS), which causes insufficient tissue perfusion, can result in multiple organ failure (MOF) and death. This study aimed to evaluate whether doxycycline (DOX) protects cardiovascular, kidney, and liver tissue from damage in a rat model of HS. Immediately before the resuscitation, DOX (10 mg/kg; i.v.) was administered, and its protective effects were assessed 24 h later. Mean arterial pressure, renal blood flow, heart rate, vasoactive drug response, and blood markers such as urea, creatinine, AST, ALT, CPK, CPR, and NOx levels were determined. RESULTS: We showed that DOX has a significant effect on renal blood flow and on urea, creatinine, AST, ALT, CPK, and NOx. Morphologically, DOX reduced the inflammatory process in the liver tissue. CONCLUSIONS: We conclude that DOX protects the liver and kidney against injury and dysfunction in a HS model and could be a strategy to reduce organ damage associated with ischemia-and-reperfusion injury.

ABSTRACT

Colorectal cancer is the third most common neoplasm and the second most lethal worldwide. The most common histological type is adenocarcinoma, characterized by its glandular pattern. Medullary colon carcinoma is a rare histological variant of colorectal cancer, characterized by a predominantly solid architecture, poorly di?erentiated or undifferentiated morphology, often associated with an anomalous immunophenotype and microsatellite instability. The present study reports a case in an academic service of general surgery of a 74-year-old patient who presented with a tumor of the ascending colon, histologically with an exuberant lymphocytic in?ltrate, suggestive of large cell lymphoma, but which was revealed by subsequent immunohistochemistry to be medullary carcinoma of the colon with microsatellite instability.


O câncer colorretal é a terceira neoplasia mais comum e a segunda mais letal no mundo. O adenocarcinoma é o tipo histológico mais comum, caracterizado pelo seu padrão glandular. O carcinoma medular do cólon é uma variante histológica rara do câncer colorretal, caracterizada por uma arquitetura predominantemente sólida, morfologia pouco diferenciada ou indiferenciada, frequentemente associada a um imunofenótipo anômalo e instabilidade de microssatélites. O presente estudo relata um caso em um serviço acadêmico de cirurgia geral de um paciente de 74 anos que apresentou tumor de cólon ascendente, histologicamente com infiltrado linfocitário exuberante, sugestivo de linfoma de grandes células, mas que foi revelado através de exame subsequente imunohistoquímico como carcinoma medular do cólon com instabilidade de microssatélites.

OBJECTIVE: To present strategies for managing tumor mass formation and their corresponding postoperative outcomes. METHODS: We conducted a systematic literature review following the guidelines and protocol of Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). We searched the PubMed and EMBASE databases, screened titles and abstracts, and further evaluated full-text publications to select relevant studies. Additionally, a narrative review of other pertinent articles on PubMed was performed. Case reports, cohort studies, and clinical trials were included. Animal studies were excluded. RESULTS: Of 6 patients enrolled in this study, most had American Spinal Injury Association Impairment Scale grade A (66.7%) following intramedullary injury, and 1 patient had American Spinal Injury Association Impairment Scale grade D (16.65%). The discovery time of the intramedullary mass formation ranged from approximately 5 to 14 years. Surgical intervention was performed in most cases (66.7%), with improvement reported in 3 of the surgical cases (75%). The majority of cases (83.3%) involved cervical lesions, while only 1 case (16.7%) involved a thoracic lesion. CONCLUSIONS: Due to the scarcity of described cases, there is no specific treatment for this tumor. Although our patient remained stable after conservative treatment, other studies have shown improvement in symptoms after mass resection. It is essential that the management of this complication be researched further due to the variety of clinical characteristics presented.

ABSTRACT Purpose: To compare the differences between the apparent and actual chord μ. Methods: In this prospective, comparative, non-randomized, and non-interventional study, imaging examinations using Pentacam and the HD Analyzer were performed in the same room under the same scotopic conditions. The inclusion criteria were patients aged 21-71 years, able to provide informed consent, myopia up to 4D, and anterior topographic astigmatism up to 1D. Patients using contact lenses, those with previous eye diseases or surgeries, corneal opacities, corneal tomographic changes, or suspected keratoconus were excluded. Results: Altogether, 116 eyes of 58 patients were analyzed. The patients' mean age was 30.69 (±7.85) years. In the correlation analyses, Pearson's correlation coefficient of 0.647 indicates a moderate positive linear relationship between apparent and actual chord μ. The mean actual and apparent chord μ were 226.21 ± 128.53 and 278.66 ± 123.90 μm, respectively, with a mean difference of 52.45 μm (p=0.01). The analysis of mean pupillary diameter resulted in 5.76 mm using the HD Analyzer and 3.31 mm using the Pentacam. Conclusions: We found a correlation between the two measurement devices, and even though we found considerable differences, both can be used in daily practice. Given their differences, we should respect their peculiarities as well.


RESUMO Objetivo: Comparar as diferenças entre a chord aparente μ e o chord real μ. Métodos: Estudo prospectivo, comparativo, não randomizado e não intervencionista. Os exames de imagem (Pentacam e HD Analyzer) foram realizados na mesma sala e nas mesmas condições escotópicas. Os critérios de inclusão foram idade de 21 a 71 anos; compreensão do termo de consentimento; miopia até 4D e astigmatismo topográfico anterior até 1D. Os critérios de exclusão foram usuários de lentes de contato; pacientes com doenças oculares prévias ou cirurgias; opacidades da córnea; a presença de alterações tomográficas da córnea ou suspeita de ceratocone. Resultados: Em nosso estudo foram analisados 116 olhos de 58 pacientes. A média de idade foi de 30,69 anos (± 7,85). Análises de correlação foram desenvolvidas e o coeficiente de correlação de Pearson (0,647) indica uma relação linear positiva moderada entre as variáveis. A média do chord μ real foi 226,21 ± 128,53 μm e a média do chord μ média foi 278,66 ± 123,90 μm, com diferença média de 52,45 μm (p=0,01). A análise do diâmetro pupilar médio apresentou: 5,76mm no HD Analyzer e 3,31mm no Pentacam. Conclusões: Entendemos a existência de uma diferença significativa entre os métodos e assim a medida de ambos os dispositivos com base em princípios diferentes devemos respeitar suas peculiaridades. Como encontramos correlação entre as duas medidas, acreditamos que ambas podem ser utilizadas na prática diária.

Abstract Introduction Multiprofessional residency in health (MHS) is a professional training program focussed on Brazil's Unified Health System that aims to reorient techno-assistance logics and significantly contributes to the insertion and consolidation of physical therapy into primary health care (PHC). Objective To understand the perceptions of health professionals and managers about the contributions of the MRH to the insertion of physical therapy into PHC. Methods This qualitative study was based on hermeneutics and was conducted in the municipality of Aracati, Ceará, Brazil, with 15 professionals making up a convenience sample. Eligible were PHC professionals with higher education working in urban areas. The data were collected from March to June 2023 through semistructured interviews with the researchers. The data analysis was based on reading the material, organizing it into themes, and interpreting these in depth. Results The first contact with physical therapy occurs in scenarios that range from individual and shared consultations to health programs at school, whose actions are optimized by PHC from the MHS. There are numerous physical therapy care strategies, especially actions focussed on biopsychosocial aspects. The involvement of physical therapists in health promotion is revealed by the inclusion of professionals in health promotion groups. Health education from a multiprofessional perspective enhances care through promotion and prevention. Conclusion The MHS strengthens the need for physical therapy in PHC and enhances primary care.


Resumo Introdução A partir da Residência Multiprofissional em Saúde (RMS) ocorre uma capacitação profissional mais direcionada para o Sistema Único de Saúde, visando a reorientação das lógicas tecnoassistenciais e contribuindo de forma significativa para a inserção e consolidação da fisioterapia na Atenção Primária à Saúde (APS). Objetivo Compreender a percepção dos profissionais e gestores de saúde sobre as contribuições da RMS para a inserção da fisioterapia na APS. Métodos Trata-se de um estudo qualitativo fundamentado na hermenêutica, desenvolvido no município de Aracati, Ceará, Brasil, com 15 profissionais por conveniência. A elegibilidade dos participantes centrou-se em profissionais de nível superior da APS em área urbana. A coleta de dados aconteceu de março a junho de 2023, através de uma entrevista semiestruturada elaborada pelos pesquisadores. A análise de dados ocorreu com base na leitura do material, organização em temáticas e interpretação por profundidade. Resultados O primeiro contato com a fisioterapia acontece em cenários que envolvem desde a consulta individual e compartilhada a programas de saúde na escola, cujas ações são otimizadas pela APS a partir da residência. As estratégias de cuidado da fisioterapia são inúmeras, principalmente ações centradas nos aspectos biopsicossociais. A inserção da fisioterapia na promoção da saúde é revelada pela inclusão do profissional em grupos de promoção da saúde. A educação em saúde na perspectiva multiprofissional é potencializadora do cuidado pelas ações de promoção e prevenção. Conclusão A residência multiprofissional fortalece a inserção da fisioterapia na APS e potencializa os cuidados primários.

ABSTRACT Introduction: 3-dimensional printing has enabled the development of unique and affordable additive manufacturing, including the prototyping and production of surgical forceps. Objective: demonstrate the development, 3D printing and mechanical-functional validation of a laparoscopic grasping forceps. Methods: the clamp was designed using a computer program and printed in 3 dimensions with polylactic acid (PLA) filament and added 5 screws for better leverage. Size and weight measurements were carried out, as well as mechanicalfunctional grip and rotation tests in the laboratory with a validated simulator. Results: Called "Easylap", the clamp weighed 48 grams, measured 43cm and was printed in 8 pieces, taking an average of 12 hours to produce. It allowed the simulation of the functional characteristics of laparoscopic pressure forceps, in addition to the rotation and rack locking mechanism. However, its strength is reduced due to the material used. Conclusion: It is possible to develop plastic laparoscopic grasping forceps through 3-dimensional printing.


RESUMO Introdução: a impressão em 3 dimensões permitiu o desenvolvimento de manufaturas aditivas únicas e acessíveis, inclusive na prototipagem e produção de pinças cirúrgicas. Objetivo: Demonstrar o desenvolvimento, a impressão em 3D e a validação mecânico-funcional de pinça laparoscópica do tipo apreensão. Métodos: a pinça foi desenhada em programa de computador e impressa em 3 dimensões com filamento de ácido poliláctico (PLA) e acrescida de 5 parafusos para melhor efeito de alavanca. Foram realizadas aferições de tamanho e peso, bem como testes mecânicos-funcionais de preensão e rotação em laboratório com simulador validado. Resultados: denominada "Easylap", a pinça pesou 48 gramas, mediu 43 cm e foi impressa em 8 peças, levando em média 12 horas para sua produção. Ela permitiu a simulação das características funcionais de pinça laparoscópicas de apreensão, além de mecanismo de rotação e travamento por cremalheira. Porém sua força é reduzida devido ao material utilizado. Conclusão: é possível desenvolver pinça laparoscópica plástica de apreensão através de impressão em 3 dimensões.

ABSTRACT Purpose: To assess Meibomian gland dysfunction using meibography in patients with xeroderma pigmentosum and correlate with ocular surface changes. Methods: This cross-sectional study evaluated patients with xeroderma pigmentosum. All patients underwent a comprehensive and standardized interview. The best-corrected visual acuity of each eye was determined. Detailed ophthalmic examination was conducted, including biomicroscopy examination of the ocular surface, Schirmer test type I, and meibography, and fundus examination was also performed when possible. Meibomian gland dysfunction was assessed by non-contact meibography using Oculus Keratograph® 5M (OCULUS Inc., Arlington, WA, USA). Saliva samples were collected using the Oragene DNA Self-collection kit (DNA Genotek Inc., Ottawa, Canada), and DNA was extracted as recommended by the manufacturer. Factors associated with abnormal meiboscores were assessed using generalized estimating equation models. Results: A total of 42 participants were enrolled, and 27 patients underwent meibography. The meiboscore was abnormal in the upper eyelid in 8 (29.6%) patients and in the lower eyelid in 17 (62.9%). The likelihood of having abnormal meiboscores in the lower eyelid was 16.3 times greater than that in the upper eyelid. In the final multivariate model, age (p=0.001), mutation profile (p=0.006), and presence of ocular surface malignant tumor (OSMT) (p=0.014) remained significant for abnormal meiboscores. For a 1-year increase in age, the likelihood of abnormal meiboscores increased by 12%. Eyes with OSMT were 58.8 times more likely to have abnormal meiboscores than eyes without ocular surface malignant tumor. Conclusion: In the final model, age, xeroderma pigmentosum profile, previous cancer, and clinical alterations on the eyelid correlated with a meiboscore of ≥2. Meibomian gland dysfunction was common in patients with xeroderma pigmentosum, mainly in the lower eyelid. The severity of Meibomian gland dysfunction increases with age and is associated with severe eyelid changes.

BACKGROUND: Achalasia is an esophageal motility disorder, with clinical presentation of dysphagia and regurgitation. This is a chronic condition with no cure. Current treatment options aim to reduce lower esophageal sphincter tone by pharmacological, endoscopic or surgical means, with the aim of improving patients' symptoms. Peroral endoscopic myotomy (POEM) is an alternative endoscopic surgery to Heller cardiomyotomy, in which the procedure is performed orally, by endoscopy, offering efficacy comparable to surgical myotomy, with relative ease and minimal invasion, without external incisions. AIMS: To study the safety of POEM by analyzing its results, adverse events and perioperative complications and the main ways to overcome them, in addition to evaluating the effectiveness of the procedure and the short-term postoperative quality of life. METHODS: A qualitative and quantitative, observational and cross-sectional study that analyzed patients who underwent the POEM in a reference center, from December 2016 to December 2022, maintaining the technical standard of pre-, peri- and postoperative protocol. RESULTS: A total of 94 patients were included in the study, and only three had postoperative complications. The average early postoperative Eckardt score was 0.93 and the late 1.40, with a mean improvement of 7.1 in early results and 6.63 in late results (p<0.05). CONCLUSIONS: POEM can be reproduced with an excellent safety profile, significant relief of symptoms and improvement in esophageal emptying, and in quality of life.

ABSTRACT

Tissue inhibitor of metalloproteinase-2 (TIMP-2) and membrane-type 1-matrix metalloproteinase (MT1-MMP) are always expressed during the cancer process. The aim was to identify which regions of the colon mucosa MT1-MMP and TIMP-2 begin to express themselves, as well as to establish their expression in relation to cell proliferation and mucin production. After intraperitoneal injection of 15 mg/kg of azoxymethane (AOM) at 4, 12, and 20 weeks, histological sections of the middle segment of the rat colon mucosa were evaluated by immunohistochemistry for cell proliferation and expression of MT1-MMP and TIMP-2 and histochemistry for mucin. As a result, a single dose of AOM initially increased the intensity of MT1-MMP and TIMP-2 expression in the conjunctive cells and glands, concurrently with alterations in the distribution of the mucin produced in the gland of the large intestine mucosa and cell proliferation. As a result, at 4 and 12 weeks, a single dose of AOM initially stimulated the expression of MT1-MMP and TIMP-2 in the conjunctive cells and glands with greater intensity. Changes in the cell proliferation and distribution of the mucin produced in the large intestine mucosa gland were observed. We conclude that MT1-MMP and TIMP-2 were first and strongly expressed in all cells of the colon glands, concurrently with an increase in cell proliferation and a diffuse dispersion of mucin, indicating the onset of the dysplasia process following a single dosage of AOM.

AIMS: The rating of perceived exertion (RPE) provides correlations with physiological measurements of exercise intensity, including metabolic equivalent (MET), oxygen consumption (V˙O2), and heart rate (HR), in real (RS) and virtual (VS) sessions. To use RPE in patients with pathology, we aimed to examine the concurrent validity of RPE in type-1 diabetes mellitus (T1DM) patients while exergaming. METHODS: Ten T1DM patients performed two 30-min crossover sessions of moderate-intensity exercise (washout 72-196 h). The RS group performed running, and the VS group played the Kinect Adventures! video game. METs were measured by a direct gas analyzer during the sessions, and RPE was measured on the 6 - 20 point Borg scale after the sessions. RESULTS: RS and VS showed similar RPE (13.2 ± 2.7 vs. 14.2 ± 2.4) and MET (4.6 ± 1.1 vs. 4.0 ± 0.8) values (p > 0.05). RPE vs. MET correlation-coefficients were large in RS (r = 0.64; R2 = 41; p = 0.04) and were moderate in VS (r = 0.42; R2 = 18; p = 0.22). Additionally, RS secondary values (V˙O2 and HR vs. RPE) showed high coefficients (V˙O2-r = 0.62; average HRr = 0.62; maximal HRr = 0.50, p < 0.05). VS secondary values, on the other hand, showed low-moderate coefficients (V˙O2-r = 0.42; average HRr = 0.23; maximal HRr = 0.21, p > 0.05). CONCLUSION: The current validation showed that RPE may not be a valid and strong method for T1DM patients while exergaming. Healthcare professionals should cautiously use the 6 - 20 point RPE scale in pathological patients, specifically in T1DM while exergaming.

ABSTRACT

Objetivo: Relatar um caso clínico de um paciente com fratura zigomático orbitária vítima de tiro de bala de borracha e mostrar a importância de um planejamento adequado. Relato de caso: Paciente do sexo masculino, 19 anos de idade, deu entrada no Hospital de Urgências de Teresina, relatando ter sofrido uma agressão por bala de borracha durante uma manifestação. Foi observado no exame clínico: aumento de volume, equimose palpebral superior e dificuldade de abertura bucal. Na imagem foi observado, projétil de borracha na região zigomática esquerda, fraturas de zigoma com comunicação na região de pilar zigomático, fratura com deslocamento na região de margem Infraorbital e fratura na sutura fronto-zigomática. Foi proposto para o caso clínico, cirurgia sob anestesia geral com acesso intrabucal para fixação do pilar zigomático com placa do sistema 2.0mm, acesso superciliar para fixação da sutura fronto-zigomática com placa 1.5mm, e reconstrução do assoalho orbitário com malha de titânio. Conclusão: No pós-operatório o paciente não apresenta relato de enoftalmia, distopia ou diplopia, ausência de dor e déficits visuais.


Objective: To report a clinical case of a patient with zygomatic-orbital fracture, victim of rubber bullet shooting and show the importance of an adequate planning. Case Report: A 19-year-old male patient was admitted to the Teresina Emergency Hospital, reporting having suffered an aggression by rubber bullet during a demonstration. On clinical examination was observed: increased volume, upper eyelid ecchymosis and difficulty opening the mouth. In the image it was observed, rubber bullet in the left zygomatic region, zygoma fractures with communicationin the region of the zygomatic pillar, fracture with displacement in the region of infraorbital margin and fracture in the fronto-zygomatic suture. It was proposed for the clinical case, surgery under general anesthesia with intraoral access for fixation of the zygomatic pillar with a 2.0 mm plate, superciliary access for fixation of the fronto-zygomatic suture with a 1.5 mm plate, and reconstruction of the orbital floor with titanium mesh. Conclusion: Postoperatively, the patient did not report enophthalmia, dystopia or diplopia, absence of pain and visual deficits.


Objetivo: Informar de un caso clínico de un paciente confractura zigomática-orbital víctima de un disparo de bala de goma y mostrar la importancia de una planificación adecuada. Reporte de caso: Paciente masculino, de 19 años, fue admitido enel Hospital de Urgencias de Teresina, informando haber sufrido una agresión por bala de goma durante una manifestación. Se observó en elexamen clínico: aumento de volumen, equimosis del párpado superior y dificultad para abrir la boca. En la imagen se observó, bala de goma en la región cigomática izquierda, fractura del cigoma con comunicación en la región del pilar cigomático, fractura con desplazamiento en la región del margen infraorbitario y fractura en la sutura fronto-cigomática. Se propuso para el caso clínico, cirugía bajo anestesia general conacceso intraoral para fijacióndel pilar cigomáticocon sistema de placas de 2,0 mm, acceso superciliar para fijación de la sutura fronto-cigomática de placas de 1,5 mm, y reconstrucción del suelo orbitario con malla de titanio. Conclusión: En el post operatorio, la paciente no presentó informes de enoftalmia, distopía o diplopía, ausencia de dolor y déficit visual.

ABSTRACT

To study the process of neuronal differentiation, the human neuroblastoma (SH-SY5Y) and the murine neuroblastoma (Neuro2a) cell lines have proven to be effective models. For this approach, different protocols involving known neurotrophic factors and other molecules, such as retinoic acid (RA), have been assessed to better understand the neuronal differentiation process. Thus, the goal of this manuscript was to provide a brief overview of recent studies that have used protocols to promote neurodifferentiation in SH-SY5Y and Neuro2a cell lines and used acquired morphology and neuronal markers to validate whether differentiation was effective. The published results supply some guidance regarding the relationship between RA and neurotrophins for SH-SY5Y, as well a serum concentrations for both cell lines. Furthermore, they demonstrate the potential application of Neuro2a, which is critical for future research on neuronal differentiation.
