ABSTRACT

A five-step approach, starting from simple 1,5-disubstituted indoles, has been implemented for the synthesis of diversely substituted indole-pyrido-indene pentacyclic compounds up to 54 % yield via domino radical-mediated processes in the presence of the radical reagents DLP/TTMSS and AIBN/TTMSS. Reactions proceeded with diverse key starting radical cyano-precursors strategically synthesized which were subsequently transformed into the target pentacyclic compounds through an aryl/iminyl radical-mediated domino reactions sequence. In addition to the routine spectroscopic techniques, the structure of radical precursors, as well as, the target pentacyclic products were unequivocally established by single crystal X-ray diffraction, confirming the effectiveness of the proposed synthetic sequence.

ABSTRACT

Delirium is a common, serious, and often preventable neuropsychiatric emergency mostly characterized by a disturbance in attention and awareness. Systemic insult and inflammation causing blood-brain-barrier (BBB) damage and glial and neuronal activation leading to more inflammation and cell death is the most accepted theory behind delirium's pathophysiology. This study aims to evaluate the relationship between brain injury biomarkers on admission and delirium in acutely ill older patients. We performed a prospective cohort study which analyzed plasma S100B levels at admission in elderly patients. Our primary outcome was delirium diagnosis. Secondary outcomes were association between S100B, NSE and Tau protein and delirium diagnosis and patients' outcomes (admissions to intensive care, length of hospital stay, and in-hospital mortality). We analyzed 194 patients, and 46 (24%) developed delirium, 25 on admission and 21 during hospital stay. Median of S100B at admission in patients who developed delirium was 0.16 and median was 0.16 in patients who didn't develop delirium (p: 0.69). Levels S100B on admission did not predict delirium in acutely ill elderly patients.Trial registration: The study was approved by the local institutional review board (CAPPESq, no. 77169716.2.0000.0068, October 11, 2017) and registered in Brazilian Clinical Trials Registry (ReBEC, no. RBR-233bct).

ABSTRACT

OBJECTIVES: Culturable and unculturable microorganisms have been associated with periodontitis. Their differential proportions and composition have not been evaluated by their severity and complexity defined by stages in the 2018 AAP-EEP classification. METHODS: One hundred eighty subgingival biofilm samples were collected in Spain and Colombia from subjects categorized as health/gingivitis: periodontitis stages I/II periodontitis stages III/IV. Target culturable microorganisms (Porphyromonas gingivalis, Aggregatibacter actinomycetemcomitans, Tannerella forsythia, Treponema denticola, and Eubacterium nodatum) and target unculturable microorganisms (Filifactor alocis, Eubacterium saphenum, Eubacterium brachy, Desulfobulbus oralis) were evaluated by quantitative PCR analysis. In addition, their differences and association with periodontal status were analyzed by ANCOVA and logistic regression models once adjusted to age, current smoking, and country. RESULTS: P. gingivalis was significantly associated with periodontitis stages I/II, OR 2.44 (CI 95% 1.08-5.47) and stages III/V, OR 6.43 (CI 95% 2.43-16.9). T forsythia, OR 7.53 (CI 95% 2.07-27.4); D. oralis, OR 5.99 (CI 95% 2.71-13.23); F. alocis, OR 10.9 (CI 95% 4.56-23.2); E. brachy, 3.57 (CI 95% 1.40-9.11); and E. saphenum, 4.85 (CI 95% 1.99-11.7) were significantly associated only with stages III/IV periodontitis. P. gingivalis evidenced significant differences with the increase in the severity of the periodontal lesion: 2.97 colony forming unit (CFU)/µL (CI 95% 2.32-3.54) health/gingivitis, and 4.66 CFU/µL (CI 95% 4.03-5.30) and 5.90 CFU/µL (CI 95% 5.20-6.48) in stages I/II and III/IV respectively (p < 0.0001). Unculturable microorganisms only evidenced differences in concentration in stages III/IV compared with health-gingivitis (p ≤ 0.001). CONCLUSION: Culturable and unculturable are strongly associated with stages III/IV periodontitis. Classic culturable microorganisms are more sensitive to differentiate between stages of periodontitis in the quantitative analysis. CLINICAL RELEVANCE: Future interventional studies of periodontal disease should include Filifactor alocis, Eubacterium saphenum, Eubacterium brachy, and Desulfobulbus oralis as possible markers of therapy response and as indicators of progressive disease.

ABSTRACT

Objetivo: Determinar el nivel de incertidumbre del cuidador familiar (CF) de la persona hos-pitalizada en fase terminal y su asociación con carga de síntomas y funcionalidad del paciente. Materiales y método: Estudio observacional de tipo corte transversal, donde se identificó la prevalencia de incertidumbre en el CF de pacientes hospitalizados en fase terminal usando la escala de incertidumbre. Adicionalmente, se evaluó la carga de síntomas mediante escala Edmonton del paciente terminal hospitalizado en una institución de salud de alta complejidad. Resultados: La muestra incluyó información de 69 CF y pacientes terminales hospitalizados. El 85,51% de los cuidadores tuvo un nivel de incertidumbre medio. Se observó un nivel es-tadísticamente significativo de incertidumbre menor en el CF, a mayor intensidad de dolor del paciente (p=0,04), al igual que en náuseas (p=0,0004), depresión (p=0,03) y ansiedad (p=0,025) según carga de síntomas. Discusión: La vivencia del cuidador de un paciente terminal, refle-jada en la incertidumbre frente a la enfermedad de este, se relaciona principalmente con la de-pendencia funcional del enfermo, debido probablemente al impacto que su falta de suficiencia puede generar en las rutinas diarias del cuidador familiar, lo cual, puede alterar completamente la dinámica familiar y generar sobrecarga, estrés, ansiedad y trastornos en la comunicación. Conclusión: La mayoría de CF de pacientes terminales son mujeres en edad productiva, con carga familiar, y con nivel de incertidumbre medio alto frente a la enfermedad del paciente. La incertidumbre del cuidador familiar es diferente según los síntomas que presenta el paciente.


Objective: To determine the level of uncertainty of the family caregiver (FC) of the terminally hospitalised person and its association with symptom burden and patient functionality. Mate-rials and methods: Observational cross-sectional study, in which the prevalence of uncertain-ty in the FC of terminally ill hospitalised patients was identified using the uncertainty scale. In addition, the symptom burden was assessed using the Edmonton scale in terminally ill patients hospitalised in a highly complex health institution. Results: The sample included information from 69 CF and terminally ill hospitalised patients. 85.51% of caregivers had a medium level of uncertainty. A statistically significant lower level of uncertainty was observed in the CF, the greater the intensity of the patient's pain (p=0.04), as well as in nausea (p=0.0004), de-pression (p=0.03) and anxiety (p=0.025) according to symptom burden. Discussion: The ex-perience of the caregiver of a terminally ill patient, reflected in the uncertainty of the patient's illness, is mainly related to the functional dependence of the patient, probably due to the impact that their lack of sufficiency can have on the daily routines of the family caregiver, which can completely alter the family dynamics and generate overload, stress, anxiety and communica-tion disorders. Conclusion: The majority of FC of terminally ill patients are women of produc-tive age, with a family burden, and with a medium-high level of uncertainty about the patient's illness. The uncertainty of the family caregiver is different according to the patient's symptoms


Objetivo: Determinar o nível de incerteza do cuidador familiar (CF) da pessoa hospitalizada em estado terminal e sua associação com a carga de sintomas e a funcionalidade do paciente. Mate-riais e métodos: Estudo observacional de corte transversal, no qual a prevalência de incerteza no CF de pacientes terminais hospitalizados foi identificada usando a escala de incerteza. Além dis-so, a carga de sintomas de pacientes terminais hospitalizados em uma instituição de saúde de alta complexidade foi avaliada usando a escala de Edmonton. Resultados: A amostra incluiu infor-mações de 69 pacientes com FC e pacientes terminais hospitalizados. 85,51% dos cuidadores tin-ham um nível médio de incerteza. Foi observado um nível de incerteza estatisticamente significa-tivo menor na FC quanto maior a intensidade da dor do paciente (p=0,04), bem como na náusea (p=0,0004), depressão (p=0,03) e ansiedade (p=0,025) de acordo com a carga de sintomas. Dis-cussão: A experiência do cuidador de um paciente terminal, refletida na incerteza da doença do paciente, está relacionada principalmente à dependência funcional do paciente, provavelmente devido ao impacto que sua falta de suficiência pode ter nas rotinas diárias do cuidador familiar, o que pode alterar completamente a dinâmica familiar e gerar sobrecarga, estresse, ansiedade e distúrbios de comunicação. Conclusão: A maioria dos CF de pacientes terminais são mulheres em idade produtiva, com ônus familiar e com um nível médio-alto de incerteza sobre a doença do paciente. A incerteza do cuidador familiar é diferente de acordo com os sintomas do paciente.

Abstract Background: Monocytes are essential components in inflammatory signaling, and their recruitment is crucial in the signaling pathway, which directs and determines cell adhesion to the activated endothelium. A better understanding of the correlation between monocyte subsets and inflammatory signaling in patients with atherosclerotic disease in acute coronary syndrome (ACS) is essential for the development of more effective therapies for the prevention and treatment of cardiovascular diseases. Objective: To analyze differences between biomarkers and monocyte activation in the setting of ischemic heart disease. Methods: This was a case-control study comparing biomarkers and monocyte subsets between patients with ACS with and without ST-segment elevation and individuals without coronary stenosis. The nonparametric Kruskal-Wallis test was used to assess differences between groups, and Dunn's post hoc test was used to identify which groups were different. Cuzick's test for ordered group trends was used to assess falling or rising trends. Participants were classified into 3 groups: control (0); non-ST-elevation myocardial infarction (NSTEMI) (1); ST-elevation myocardial infarction (STEMI) D1 (2). Results: Forty-seven patients with ACS and 19 controls with no obstructive lesions on coronary angiography were recruited. Monocyte profile assessment was statistically different regarding time of symptom onset and the presence or absence of atherosclerotic disease (Kruskal-Wallis, p = 0.0009). Dunn's post hoc test showed a significant difference between the control group and the STEMI D1 (p = 0.0014), STEMI D3 (p = 0.0036), and STEMI D7 (p = 0.0195) groups, corresponding to a 2-fold increase in classical (p = 0.0022) and nonclassical (p = 0.0031) monocytes compared with controls. For classical monocytes, there was a difference between the control group and all STEMI groups and between the NSTEMI group and the STEMI D1, D3, and D7 groups. For nonclassical monocytes, there was a difference between the control group and the STEMI D7 group (p = 0.0056) and between the NSTEMI group and the STEMI D7 group (p = 0.0166). Conclusion: This study found that there was an increase in total and classical monocyte mobilization at the time of acute myocardial infarction in patients with ACS.

INTRODUCTION: Mini-tablets are considered a promising solid dosage form in the pharmaceutical industry due to advantages such as dosing accuracy, efficiency as a drug delivery system, and alleged improvement in mechanical properties. Nevertheless, only a few experimental studies are available in the literature regarding this topic and technical aspects, such as punch's shape and size effect on the stress and density distribution in the compact mini-tablets, are still not fully investigated. OBJECTIVES: In this paper, the influence of powder properties and process parameters, such as punch shape and size, on the evolution of mechanical properties during the tableting process and the potential occurrence of tablet defects are investigated using the mechanistic modeling approach, Finite Element Method (FEM). METHODS: The numerical simulation cases consist of four different die sizes, mini-tablets of 2 mm, and 3 mm, and conventionally sized tablets of 8 mm and 11.28 mm. Each tablet size is simulated using four distinctive excipients, Avicel® PH-102, Kollidon® VA64, Pearlitol® 100SD, and Supertab® 11SD, and two different punch geometries, a flat-face punch, and a bevel edge punch. RESULTS: The model predictions in terms of stress and density distribution at different stages of the compaction process indicate similar behavior in terms of density and stress distribution profiles between the conventionally sized tablets and mini-tablets for a particular excipient. CONCLUSIONS: Based on tablet size, small localized differences are noted (e.g., low-density regions, high shear bands, and heterogeneous density profiles), suggesting a possible risk of tableting defects for conventionally sized tablets compared to mini-tablets. Furthermore, it is observed that bevel-edged tablets could facilitate the formation of cracks, leading to possible capping failure.

ABSTRACT

The current dietary habits cause health problems due to foods' composition, with bread as an important example. Our aim was to formulate an optimum dough blend with flours from wheat, amaranth and orange sweet potato to obtain a physically good and highly nutritional bread. Bread was prepared with blends of wheat, amaranth and orange sweet potato flours, optimizing the technological properties of the doughs by the response surface methodology and analyzing their physical and nutritional properties. Amaranth provides protein and fiber, and sweet potatoes provide ß-carotenoids and high antioxidant activity. The prediction models were adjusted by mixing time (MT), peak dough resistance (PDR), setback (SB) and breakdown (BD). The interaction between wheat and amaranth significantly (p < 0.05) affected MT, PDR and SB, while the interaction between amaranth and sweet potato affected BD (p < 0.05); none of the components influenced PDR. The optimized blend (68.7% wheat, 22.7% amaranth and 8.6% sweet potato) produced a bread with the best crust and crumb appearance. This bread was comparable to that made with 100% wheat in specific volume and textural characteristics, but had better protein quality, higher content of fermentable fiber, pro-vitamin A, and bioactive compounds with good antioxidant capacity, and a lower glycemic index.

ABSTRACT

In this research the sequential generation and cyclization of N-alkoxyaminyl radicals to produce 1-azaspiro[4.4]nonane, a prominent scaffold in organic and medicinal chemistry, was studied. Competition experiments in benzene at 80 °C with brominated oxime ethers using Bu3SnH as chain transfer and AIBN as the initiator generated vinyl or aryl radicals which were captured by oxime ethers, allowing approximate 5-exo-trig cyclization constants at 4.6 × 108 s-1 and 9.9 × 108 s-1 respectively to be established. Similar results were obtained by kinetic studies using the transition state theory (TST) from ab initio calculations with density functional theory (DFT) using the M06-2X, B3LYP, mPW1PW91 and TPSSh functionals in combination with the 6-311+G(d, p) basis set. Additionally, it was found that the 5-exo-trig cyclization of the N-alkoxyaminyl radical onto CC double bonds is a reversible process whose constants were determined to be in the range of 6.2 × 100 s-1 and 3.5 × 106 s-1 at 80 °C, depending on the nature of the substituents. The calculation of the radical stabilization energy (RSE) shows that the N-alkoxyaminyl radical is a very stable species and its reactivity in the addition on alkenes is governed by its nucleophilic character and the stability of the carbon-centered radical formed after cyclization. The reduction constant of the N-alkoxyaminyl radical with Bu3SnH in the gas phase at 80 °C was also estimated to be 3.4 × 100 M-1 s-1 through computational calculations. This information facilitates the rational planning of cascades and other methodologies applied to the construction of carbocyclic and aza-heterocyclic compounds.

ABSTRACT

Resumen La hidrocefalia es un problema de salud frecuente en pediatría, en particular durante el primer mes de vida. La incidencia en América Latina es una de las más altas del mundo. En Colombia no existen datos representativos sobre la incidencia real de esta enfermedad. Recientes hallazgos relacionados con la dinámica del líquido cefalorraquídeo permitieron proponer nuevos modelos sobre la fisiopatología de la hidrocefalia que, junto con los hallazgos en la Resonancia Magnética, han llevado a tener una mejor comprensión de la enfermedad. El objetivo de este articulo es realizar una revisión de la información disponible en la literatura sobre los avances en la fisiopatología de la enfermedad y los hallazgos en neuroimágenes, además de realizar una breve revisión sobre el papel de estas en el diagnóstico y seguimiento de los pacientes. Se realizó una revisión bibliográfica con términos MeSH, en las bases de datos de PUBMED, OVID y SCOPUS con artículos publicados en los últimos 6 años, seleccionado un total de 30 artículos que abordaron el tema de forma integral. Los nuevos hallazgos descritos como lo son el sistema glinfático y el papel de las AQP4 y los avances en las neuroimágenes, sobre todo de la resonancia magnética, han ayudado a comprender mejor esta entidad, apoyando el desarrollo de un nuevo modelo de la dinámica del líquido cefalorraquídeo y a partir de él diferentes explicaciones sobre la fisiopatología. Abstract Hydrocephalus is a frequent health problem in pediatrics, particularly during the first month of life. The incidence in Latin America is one of the highest in the world. In Colombia there are no representative data. Recent findings related to the dynamics of cerebrospinal fluid allowed proposals of new models on the pathophysiology of hydrocephalus that, along with new findings on MRI, have led to a better understanding of the disease. The aim of this work is to review the information available in the literature about the progress in the pathophysiology of the disease and neuroimaging findings, in addition to conducting a brief review on the role of these in the diagnosis and follow-up of patients. A bibliographic review with MeSH terms was carried out in PUBMED, OVID and SCOPUS databases, with articles published in the last 6 years. 30 articles that dealt with the theme in a comprehensive way were included. New findings described as the glymphatic system and the role of AQP4, along with advances in neuroimaging, especially MRI, have helped to better understand hydrocephalus, supporting the development of a new model of cerebrospinal fluid dynamics, and based on it, different explanations regarding its pathophysiology. ABSTRACT

SARS-CoV-2 infection has a wide spectrum of presentations, from asymptomatic to pneumonia and sepsis. Risk scores have been used as triggers for protocols that combine several interventions for early management of sepsis. This study tested the accuracy of the score SIRS, qSOFA, and NEWS in predicting outcomes, including mortality and bacterial infection, in patients admitted to the emergency department (ED) during the COVID-19 pandemic. We described 2,473 cases of COVID-19 admitted to the ED of the largest referral hospital for severe COVID-19 in Brazil during the pandemic. SIRS, qSOFA and NEWS scores showed a poor performance as prognostic scores. However, NEWS score had a high sensitivity to predict in-hospital death (0.851), early bacterial infection (0.851), and ICU admission (0.868), suggesting that it may be a good screening tool for severe cases of COVID-19, despite its low specificity.

Abstract Introduction: Identifying the imaging features of renal tumors in pediatric population allows reaching more accurate diagnoses and implementing more appropriate treatments. Objective: To describe the imaging findings of renal tumors in children and to assess the association between imaging findings and histological diagnosis of Wilms tumors versus Non-Wilms tumors, and between imaging features and intraoperative rupture of Wilms tumors, as well as the level of agreement between radiological and histological diagnosis (Wilms vs. Non-Wilms tumor). Materials and methods: Cross-sectional study conducted in 47 children with a pathological diagnosis of kidney tumor and treated between 2012 and 2018 in a pediatric hospital in Bogotá D.C., Colombia. The patients' medical records, as well as their ultrasound, tomography and magnetic resonance studies were reviewed. Two univariate logistic regression analyses were performed to assess the association between imaging findings and histopathological diagnosis and between imaging features and intraoperative rupture of Wilms tumors, calculating the respective Odds Ratio (OR) with a 95% confidence interval. In addition, the level of agreement between radiological and histological diagnosis was determined using the Cohen's kappa coefficient. Results: A significant association was found between histological diagnosis of Wilms tumor and the presence of necrosis, tumor enhancement, pseudocapsule, rupture signs, tumor volume and tumor size (OR: 21.6, 15.17, 14.57, 8.21, 7.93, and 4.37, respectively; p<0.05). An association between having Wilms tumors and a lower frequency of metastases was also found (OR: 0.19; p<0.05). The kappa coefficient between radiological diagnosis of Wilms/non- Wilms tumors and histological diagnosis was 0.78 (CI95%: 0.59-0.96; p<0.05). Additionally, Wilms tumors volumen was significantly associated with the occurrence of rupture (OR: 3.08; p<0.05). Conclusions: There are imaging findings such as necrosis, tumor enhancement and tumor volume that can help predict the histological diagnosis of Wilms tumors, as well as perioperative rupture. In addition, a moderate to very good concordance between radiological diagnosis of Wilms/non-Wilms tumors and histological findings was found.


Resumen Introducción. Identificar las características por imagen de los tumores renales en la población pediátrica permite realizar diagnósticos más precisos e implementar tratamientos más apropiados. Objetivo. Describir los hallazgos de imagen de tumores renales en niños y evaluar la asociación entre hallazgos imagenológicos y el diagnóstico histopatológico de tumores de Wilms versus tumores no Wilms, y entre las características de imagen y ruptura quirúrgica de tumores Wilms, así como el grado de concordancia entre el diagnóstico radiológico e histológico. Materiales y métodos. Estudio transversal realizado en 47 niños con diagnóstico patológico de tumor renal atendidos entre 2012 y 2018 en un hospital pediátrico de Bogotá D.C., Colombia. Se revisaron las historias clínicas de los pacientes, así como sus estudios de ultrasonografía, tomografía y resonancia magnética. Se realizaron dos análisis de regresión logística univariados para evaluar la asociación entre hallazgos imagenológicos y diagnóstico histopatológico y entre las características imagenológicas de los tumores de Wilms y ruptura quirúrgica, calculando los respectivos odds ratio (OR) con un intervalo de confianza del 95%. Además, se determinó el grado concordancia entre el diagnóstico radiológico e histopatológico mediante el coeficiente de kappa de Cohen. Resultados. Se encontró una asociación significativa entre el diagnóstico histológico de tumor de Wilms y la presencia de necrosis, realce tumoral, pseudocápsula, signos de ruptura, volumen y tamaño del tumor (OR: 21.6, 15.17, 14.57, 8.21, 7.93 y 4.37, respectivamente; p<0.05). También se observó una asociación entre tener tumores de Wilms y menor frecuencia de metástasis (OR:0.19; p<0.05). El coeficiente de Kappa entre el diagnóstico radiológico de los tumores (Wilms/no-Wilms) y el diagnóstico histológico fue 0.78 (IC95%: 0.59-0.96; p<0.05). Además, el volumen de los tumores de Wilms se asoció significativamente con la ocurrencia de ruptura (OR: 3.08; p<0.05). Conclusiones. Hay hallazgos imagenológicos como la necrosis, el realce tumoral y el volumen tumoral que ayudan a predecir el diagnóstico histológico de tumores de Wilms, así como la ruptura perioperatoria. Abstract Introduction: Posterior fossa tumors are common in the pediatric population and require adequate characterization by means of structural magnetic resonance imaging (MRI) and advanced MRI techniques to achieve an appropriate therapeutic approach. Objectives: To determine the usefulness of apparent diffusion coefficient (ADC) values for the differential diagnosis of posterior fossa tumors in the pediatric population treated at a reference hospital in Bogotá D.C., Colombia. Materials and methods: Diagnostic accuracy study carried out in 28 pediatric patients diagnosed with posterior fossa tumor between 2017 and 2019 at the Fundación Hospital de la Misericordia, a quaternary care institution. ADC values were measured and compared with histopathological diagnosis as gold standard, obtaining sensitivity, specificity, and positive and negative predictive values. Differences between medians were determined using the Kruskall-Wallis test. The p value between the quantitative ADC value and the gold standard was calculated using Pearson's chi-squared test, with a significance level of p<0.05. Results: The mean age of the participants was 83.9 months (SD=57 months), and 64.28% of them were boys. Medulloblastoma was the most frequent tumor (39.29%). For the diagnosis of medulloblastoma, an ADC value of 0.6210x10-3mm2/s was established, obtaining a sensitivity and specificity of 81.82% and 76.47%, respectively. For the diagnosis of pilocytic astrocytoma, an ADC of 1.03x10-3mm2/s was determined, with a sensitivity of 66.67% and a specificity of 89.40%. Conclusions: ADC value is useful to classify and differentiate posterior fossa tumors in the Colombian pediatric population, showing an inversely proportional relationship with the tumor grade.


Resumen Introducción. Los tumores de la fosa posterior son frecuentes en población pediátrica y requieren de una adecuada caracterización mediante resonancia magnética (RM) estructural y técnicas avanzadas de RM para lograr un enfoque terapéutico apropiado. Objetivo. Determinar la utilidad de los valores del coeficiente de difusión aparente (ADC cuantitativo) en el diagnóstico diferencial de los tumores de la fosa posterior en población pediátrica de un hospital de referencia en Bogotá D.C., Colombia. Materiales y métodos. Estudio de validez de prueba diagnóstica realizado en 28 pacientes pediátricos diagnosticados con tumor de fosa posterior entre 2017 y 2019 en la Fundación Hospital Pediátrico de la Misericordia, hospital de IV nivel de complejidad. Se midieron los valores del ADC cuantitativo, los cuales fueron comparados con el diagnóstico histopatológico como estándar de oro, obteniendo datos de sensibilidad, especificidad, valores predictivos positivos y negativos. Las diferencias entre medianas fueron determinadas mediante la prueba de Kruskall-Wallis. El valor p entre el valor del ADC cuantitativo y el estándar de oro se calculó con la prueba X2 de Pearson, con un nivel de significancia de p<0.05. Resultados. La edad media fue 83.9 meses (DE=57 meses) y 64.28% fueron niños. El tumor más frecuente fue el meduloblastoma (39.29%). Para el diagnóstico de meduloblastoma se estableció un valor ADC cuantitativo de 0.6210x10-3mm2/s, obteniéndose una sensibilidad y especificidad de 81.82% y 76.47%, respectivamente, y para el diagnóstico de astrocitoma pilocítico, un ADC cuantitativo de 1.03x10-3mm2/s, con una sensibilidad de 66.67% y una especificidad de 89.40%. Conclusiones. El valor del ADC cuantitativo es útil para clasificar y diferenciar los tumores de la fosa posterior en población pediátrica colombiana, mostrando una relación inversamente proporcional con el grado tumoral.

ABSTRACT

Identification of the SARS-CoV-2 virus by RT-PCR from a nasopharyngeal swab sample is a common test for diagnosing COVID-19. However, some patients present clinical, laboratorial, and radiological evidence of COVID-19 infection with negative RT-PCR result(s). Thus, we assessed whether positive results were associated with intubation and mortality. This study was conducted in a Brazilian tertiary hospital from March to August of 2020. All patients had clinical, laboratory, and radiological diagnosis of COVID-19. They were divided into two groups: positive (+) RT-PCR group, with 2292 participants, and negative (-) RT-PCR group, with 706 participants. Patients with negative RT-PCR testing and an alternative most probable diagnosis were excluded from the study. The RT-PCR(+) group presented increased risk of intensive care unit (ICU) admission, mechanical ventilation, length of hospital stay, and 28-day mortality, when compared to the RT-PCR(-) group. A positive SARS-CoV-2 RT-PCR result was independently associated with intubation and 28 day in-hospital mortality. Accordingly, we concluded that patients with a COVID-19 diagnosis based on clinical data, despite a negative RT-PCR test from nasopharyngeal samples, presented more favorable outcomes than patients with positive RT-PCR test(s).

BACKGROUND: The decision to intubate COVID-19 patients receiving non-invasive respiratory support is challenging, requiring a fine balance between early intubation and risks of invasive mechanical ventilation versus the adverse effects of delaying intubation. This present study analyzes the association between intubation day and mortality in COVID-19 patients. METHODS: We performed a unicentric retrospective cohort study considering all COVID-19 patients consecutively admitted between March 2020 and August 2020 requiring invasive mechanical ventilation. The primary outcome was all-cause mortality within 28 days after intubation, and a Cox model was used to evaluate the effect of time from onset of symptoms to intubation in mortality. RESULTS: A total of 592 (20%) patients of 3020 admitted with COVID-19 were intubated during study period, and 310 patients who were intubated deceased 28 days after intubation. Each additional day between the onset of symptoms and intubation was significantly associated with higher in-hospital death (adjusted hazard ratio, 1.018; 95% CI, 1.005-1.03). CONCLUSION: Among patients infected with SARS-CoV-2 who were intubated and mechanically ventilated, delaying intubation in the course of symptoms may be associated with higher mortality. ABSTRACT

Systemic inflammatory response, as observed in sepsis and severe COVID-19, may lead to endothelial damage. Therefore, we aim to compare the extent of endothelial injury and its relationship to inflammation in both diseases. We included patients diagnosed with sepsis (SEPSIS group, n = 21), mild COVID-19 (MILD group, n = 31), and severe COVID-19 (SEVERE group, n = 24). Clinical and routine laboratory data were obtained, circulating cytokines (INF-γ, TNF-α, and IL-10) and endothelial injury markers (E-Selectin, Tissue Factor (TF) and von Willebrand factor (vWF)) were measured. Compared to the SEPSIS group, patients with severe COVID-19 present similar clinical and laboratory data, except for lower circulating IL-10 and E-Selectin levels. Compared to the MILD group, patients in the SEVERE group showed higher levels of TNF-α, IL-10, and TF. There was no clear relationship between cytokines and endothelial injury markers among the three studied groups; however, in SEVERE COVID-19 patients, there is a positive relationship between INF-γ with TF and a negative relationship between IL-10 and vWF. In conclusion, COVID-19 and septic patients have a similar pattern of cytokines and endothelial dysfunction markers. These findings highlight the importance of endothelium dysfunction in COVID-19 and suggest that endothelium should be better evaluated as a therapeutic target for the disease.

OBJECTIVES: Prior studies have examined sleep during the coronavirus disease 2019 (COVID-19) pandemic, but have few compared sleep measured both during and prior to COVID. We examined the impact of the COVID-19 pandemic on subjective sleep quality in general and separately by gender and age (<50 vs. ≥50 years). Further, we compared sleep quality between those who did and did not follow quarantine orders. METHODS: This sample is from the Baependi Heart Study, a family-based cohort of adults in South-eastern Brazil. Longitudinal data were from 417 individuals who completed the Pittsburgh Sleep Quality Index (PSQI) twice: between January 2010 and September 2014 (pre-COVID) and during the COVID-19 stay-at-home order March-June, 2020. Cross-sectional analysis included 800 participants. RESULTS: Mean (±SD) PSQI scores were significantly higher during than before COVID-19 (5.7 ± 3.8 vs. 5.0 ± 3.3, p < .01). This increase was significant among women and among adults ≥50 years but not in men or younger adults. The significant increase in PSQI was only observed in those who quarantined during COVID-19 (5.9 ±3.7 vs. 5.2 ±3.4, p < .01) and not those who did not quarantine (5.0 ± 3.7 vs. 4.5 ± 3, p = .12). In cross-sectional analyses, individuals who quarantined had higher PSQI scores than nonquarantined individuals (6.1 ± 3.9 vs. 5.0 ± 3.5, p < .01). The quarantine status-dependent differences were significant for women (6.4 ± 4 vs. 5.2 ± 3.7, p < .01) and older adults (6.6 ± 0.1 vs. 5.5 ± 3.3, p = .04). Differences by quarantine status were attenuated after adjusting for age and gender. CONCLUSIONS: Subjective sleep quality declined during the COVID-19 pandemic, particularly among women, older adults, and those compliant to quarantine orders.

ABSTRACT

The role of innate immunity in COVID-19 is not completely understood. Therefore, this study explored the impact of Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) infection on the expression of Pattern Recognition Receptors (PRRs) in peripheral blood cells and their correlated cytokines. Seventy-nine patients with severe COVID-19 on admission, according to World Health Organization (WHO) classification, were divided into two groups: patients who needed mechanical ventilation and/or deceased (SEVERE, n = 50) and patients who used supplementary oxygen but not mechanical ventilation and survived (MILD, n = 29); a control group (CONTROL, n = 17) was also enrolled. In the peripheral blood, gene expression (mRNA) of Toll-like receptors (TLRs) 3, 4, 7, 8, and 9, retinoic-acid inducible gene I (RIGI), NOD-like receptor family pyrin domain containing 3 (NLRP3), interferon alpha (IFN-α), interferon beta (IFN-ß), interferon gamma (IFN-γ), interferon lambda (IFN-λ), pro-interleukin(IL)-1ß (pro-IL-1ß), and IL-18 was determined on admission, between 5-9 days, and between 10-15 days. Circulating cytokines in plasma were also measured. When compared to the COVID-19 MILD group, the COVID-19 SEVERE group had lower expression of TLR3 and overexpression of TLR4.

ABSTRACT

We present a 54-year-old male garbage collector, who came to the ophthalmology department due to one-month history of bilateral blurred vision, palinopsia, headache and visual geometrical hallucinations. The examination showed a normal anterior segment, a homonymous left upper central scotoma and tilted segmental hypoplasia of the optic nerve of the left eye. Contrast-enhanced brain magnetic resonance imaging showed a mass lesion in the right occipital cortex. At biopsy, tuberculosis was found. Subsequently, anti-tuberculous treatment led to a good response with resolution of the palinopsia and visual hallucinations, and improved vision.
