ABSTRACT
BACKGROUND: Although the COVID-19 pandemic has increased the prevalence of cases with olfactory loss, other respiratory viruses can also cause this condition. We aimed to compare the prevalence of acute SARS-CoV-2 infection and other respiratory viruses in patients with sudden smell loss, and to assess the impact of SARS-CoV-2 viral load and co-infection on olfactory symptoms. METHODS: Patients with sudden smell loss were recruited in a multicenter prospective cohort study in 15 hospitals in Brazil. Clinical questionnaire, Connecticut Chemosensory Clinical Research Center (CCCRC) olfactory test and nasopharyngeal swab to perform a PCR-based respiratory viral panel were collected at first visit (day 0) and 30 and 60 days after recruitment. RESULTS: 188 of 213 patients presented positive test result for SARS-CoV-2, among which 65 were co-infected with other respiratory viruses (e.g., rhinovirus, enterovirus, and parainfluenza). 25 had negative test results for SARS-CoV-2. Patients in both SARSCoV-2 and non-SARS-CoV-2 groups had objective anosmia (less than 2 points according to the psychophysical olfactory CCCRC) at day 0, with no significant difference between them. Both groups had significant smell scores improvement after 30 and 60 days, with no difference between them. Co-infection with other respiratory viruses, and SARS-CoV-2 viral load did not impact olfactory scores. CONCLUSION: Patients with sudden smell loss associated with SARS-CoV-2 and other respiratory viruses had similar presentation, with most participants initiating with anosmia, and total or near total recovery after 60 days. SARS-CoV-2 viral load and co-infections with other respiratory viruses were not associated with poorer olfactory outcomes.

INTRODUÇÃO E/OU FUNDAMENTO: A acromegalia é uma doença endócrina rara causada por adenoma hipofisário secretor de Hormônio do Crescimento (GH), cursando com aumento dos níveis plasmáticos de GH e, consequentemente, do Fator de Crescimento Semelhante à Insulina-1 (IGF-1). Ambos os hormônios exercem importantes atividades regulatórias no sistema cardiovascular (CV) que é o principal responsável pelas complicações clínicas da síndrome. O relato de caso a seguir expõe diversas dessas complicações. MÉTODOS: Relatamos caso de paciente do sexo feminino, 47 anos, branca, tabagista, sem outras comorbidades, que procurou atendimento por cansaço progressivo, associado à dispneia aos esforços e edema de membros inferiores. RESULTADOS: O ecocardiograma transtorácico confirmou dupla lesão mitral grave, com predomínio de estenose, tratada cirurgicamente com implante de prótese mitral biológica. Cinco anos após, a paciente desenvolveu hipertensão arterial (HA) estágio 1, tratada com Hidroclorotiazida. Concomitantemente relatou cefaleia, perda visual bitemporal, separação dos dentes, aumento de extremidades (com necessidade de aumentar numeração dos sapatos) e língua. Com suspeita clínica de acromegalia, foi submetida a investigação que evidenciou elevação de níveis plasmáticos de GH e IGF1 e lesão expansiva intrasselar identificada por ressonância magnética nuclear. Iniciou-se tratamento com Bromocriptina. Nesse período a paciente interrompeu o uso da medicação anti-hipertensiva, mantendo controle adequado da pressão arterial. No entanto, três anos depois, foi constatado novo aumento dos níveis de IGF-1 para 785mcg/L (VR 2,5 segundos, sendo indicado e realizado implante de marcapasso definitivo. CONCLUSÕES: A doença CV é a comorbidade mais frequente e a principal causa de morte relacionada a acromegalia. O caso apresentado reúne e ilustra as principais manifestações CV da acromegalia: doença valvar; hipertrofia ventricular esquerda; bradiarritmias e HA, muito embora não tenha sido possível afastar doença reumática como etiologia da dupla lesão mitral. A prevalência de HA varia de 13 a 60% e contribui significativamente com o excesso de morbimortalidade observado. A alta prevalência de HA na população geral faz com que seja árduo discernir na prática clínica em que medida a HA é secundária a acromegalia, essencial ou mista. O controle pressórico obtido concomitantemente com a redução dos níveis plasmáticos de GH e IGF-1 indica uma associação direta destes hormônios com a HA em função do aumento da resistência insulínica, da reabsorção renal de sódio e da atividade simpática. Por outro lado, a persistência da HA após o controle hormonal obtido com o Octreotide sugere que nessa fase mais tardia, o remodelamento arteriolar tenha levado a instalação de HA primária.

INTRODUÇÃO E/OU FUNDAMENTO: A suplementação de vitamina D em indivíduos sem déficit aumentou significativamente nos últimos anos. Nos Estados Unidos a taxa de prescrição aumentou 30% ao longo de 10 anos. Contribuem para o fenômeno movimentos de opinião sustentando propriedades terapêuticas não comprovadas e a percepção geral de segurança e inocuidade. MÉTODOS: Relatamos um caso de intoxicação grave por vitamina D secundária a suplementação inadequada, inicialmente diagnosticada pela constatação de picos hipertensivos em paciente previamente normotensa. RESULTADOS: Paciente M.C.N., do sexo feminino, 60 anos, com história pregressa de fibromialgia, osteoartrite e depressão, mantinha acompanhamento com reumatologista e uso regular de trazodona, pregabalina e duloxetina. Após dosagem em consulta de rotina (vitamina D = 39ng/mL), recebeu prescrição de suplementação oral de colecalciferol (5000 UI/dia) por tempo indeterminado. Três semanas depois, a paciente, previamente normotensa, constatou picos hipertensivos e evoluiu com prurido generalizado, dor abdominal, náuseas e vômitos. Investigação inicial revelou hipercalcemia (cálcio total corrigido 13mg/ dL), injúria renal aguda (creatinina 2,3mg/dL) e hipertensão arterial (HA) confirmada em Monitorização Ambulatorial da Pressão Arterial (MAPA) de 24 horas. A tomografia de abdome não evidenciou alterações significativas e os sintomas digestivos foram atribuídos ao quadro metabólico. A dosagem de vitamina D confirmou a hipótese de intoxicação iatrogênica e Hipervitaminose D (vitamina D = incalculável). Após internação hospitalar, hidratação parenteral, suspensão da vitamina D e medidas para hipercalcemia, a paciente apresentou melhora progressiva da função renal e remissão dos sintomas. No entanto, mesmo após a resolução do quadro, a HA persiste até o momento, sendo necessárias para seu controle duas classes de anti-hipertensivos. CONCLUSÕES: Quase 500 anos atrás, o médico suíço Paracelso escreveu que "a diferença entre remédio e veneno está na dose". A incidência de intoxicação por vitamina D vem aumentando globalmente, seja por erro de manipulação, erro de prescrição ou uso suplementar em altas doses. Doses acima de 4000 UI/dia em períodos prolongados ou 10000 UI/ dia em períodos mais curtos foram associadas a elevação suprafisiológica do nível sérico (>150 ng/ml). O grau de intoxicação observado no caso apresentado parece desproporcional ao nível de suplementação e não podemos excluir erro de manipulação ou de uso. A associação entre Hipervitaminose D e HA é mediada tanto pela hipercalcemia como pela injuria renal. No entanto, neste caso a HA se manteve após resolução dos gatilhos iniciais, sugerindo que possam estar implicados outros mecanismos ainda não esclarecidos.

BACKGROUND: Although the risk of SARS-CoV-2 transmission within hospitals has been well recognized, there is a paucity of data on its occurrence. Our aim was to report the incidence of hospital-acquired (HA) COVID-19 at Brazilian hospitals. METHODS: We investigated the incidence of HA COVID-19 in Brazilian hospitals using data from a national surveillance system, from August 2020 through September 2021. Definitions of HA COVID-19 were: (1) symptom onset >14 days after hospital admission plus a positive SARS-CoV-2 RNA or antigen test; (2) symptom onset on days 8-14 after admission, plus a positive SARS-CoV-2 RNA or antigen test positive, plus documented high-risk exposure. We performed descriptive analyses and reported HA COVID-19 rates using pooled mean and percentile distribution. RESULTS: A total of 48,634 cases of HA COVID-19 were reported from 1428 hospitals. Incidence ranged from 0.16/1000 patient-days at neonatal intensive care units (ICUs) to 5.8/1000 patient-days at adult ICUs. The highest incidence of HA COVID-19 was during the months March to July 2021, similar to that which was observed for community-acquired COVID-19. CONCLUSIONS: This report provides a national view of the burden of HA COVID-19. The highest incidence of HA COVID-19 similar that which was observed for community-acquired COVID-19. We believe that this reflects the difficulty of implementing preventive measures. Further studies evaluating risk factors for the hospital transmission of SARS-Cov-2 should clarify strategies to minimize the risk of HA COVID-19 and may be applicable to other respiratory diseases. Furthermore, the implementation of a national system to evaluate HA COVID-19 has the potential to shine a light on this problem and lead to interventions in each hospital.

An important accessory anatomical variation, exclusively human, and related to the muscular ventres of the flexor pollicis longus and flexor digitorum profundus is frequently denominated Gantzer. These variations have close relations with the anterior interosseous nerve (AIN), which provides, for many authors, by direct compression, one of the rare neuropathic syndromes. In this work, thirty-four forearms were dissected from the collections of the Medical School of the Federal University of Minas Gerais and the Department of Basic Sciences of the Federal University of Juiz de Fora, with a prevalence of 50% of the 34 forearms studied for the Gantzer muscle. The muscle relationship was mainly with the flexor pollicis longus muscle and only one occurrence related to the flexor digitorum profundus muscle, described as a rare occurrence of unilateral double formation of Gantzer muscle. Bilaterality was observed in 88.23% of the findings and the dominant innervation for this muscle variation occurred in 82.35% by the anterior interosseous nerve (AIN). The type morphological in all forms found was the fusiform, with 10.5cm of total length and an average of 0.3cm in diameter and all related, as origin, in the medial aspect of the coronoid process of the ulna, next to the origin of the flexor digitorum superficialis muscle. Our work largely reflected the findings of most publications and, considering the controversy of the occurrence of a compressive neuropathy, the data were not sufficient, from a strictly anatomical point of view, to confirm or refute the hypothesis.

The objective of this study was to investigate the effects of this diet on dopamine receptors (drd1 and drd2), proopiomelanocortin (pomc), neuropeptideY (npy) genes expression, and preference food in adult rats. Wistar female rats were fed a hight-fat or control diet during pregnancy and lactation. The offspring were allocated into groups: Lactation Control (C) and High-fat (H). Post- weaning Control Control (CC), offspring of mothers C, fed a control diet after weaning; Control Hight-fat (CH), offspring of mothers C, fed a hight-fat diet after weaning; Hight-fat Control (HC), offspring of mothers H, fed with control diet after weaning; and Hight-fat Hight-fat (HH), offspring of mothers H, fed a H diet after weaning. The groups CH and HH presented greater expression of drd1 in comparison to the CC. The drd2 of CH and HC presented higher gene expression than did CC. HH presented higher pomc expression in comparison to the other groups. HC also presented greater expression in comparison to CH. The npy of HH presented greater expression in relation to CH and HC. HH and HC have had a higher preference for a high-fat diet at 102º lifes day. The high-fat diet altered the gene expression of the drd1, drd2, pomc and npy, and influencing the food preference for high-fat diet.

The monocyte chemoattractant protein 1 (MCP-1) belongs to the CC chemokine family and acts in the recruitment of C-C motif chemokine receptor 2 (CCR2)-positive immune cell types to inflammation sites. In testis, the MCP-1/CCR2 axis has been associated with the macrophage population's functional regulation, which presents significant functions supporting germ cell development. In this context, herein, we aimed to investigate the role of the chemokine receptor CCR2 in mice testicular environment and its impact on male sperm production. Using adult transgenic mice strain that had the CCR2 gene replaced by a red fluorescent protein gene, we showed a stage-dependent expression of CCR2 in type B spermatogonia and early primary spermatocytes. Several parameters related to sperm production were reduced in the absence of CCR2 protein, such as Sertoli cell efficiency, meiotic index, and overall yield of spermatogenesis. Daily sperm production decreased by almost 40%, and several damages in the seminiferous tubules were observed. Significant reduction in the expression of important genes related to the Sertoli cell function (Cnx43, Vim, Ocln, Spna2) and meiosis initiation (Stra8, Pcna, Prdm9, Msh5) occurred in comparison to controls. Also, the number of macrophages significantly decreased in the absence of CCR2 protein, along with a disturbance in Leydig cell steroidogenic activity. In summary, our results show that the non-activation of the MCP-1/CCR2 axis disturbs the testicular homeostasis, interfering in macrophage population, meiosis initiation, blood-testis barrier function, and androgen synthesis, leading to the malfunction of seminiferous tubules, decreased testosterone levels, defective sperm production, and lower fertility index.

Exposure to the hight-fat diet may alter the control of food intake promoting hyperphagia and obesity. The objective of this study was to investigate the effects of this diet on dopamine receptors (drd1 and drd2), proopiomelanocortin (pomc), neuropeptideY (npy) genes expression, and preference food in adult rats. Wistar female rats were fed a hight-fat or control diet during pregnancy and lactation. The offspring were allocated into groups: Lactation - Control (C) and High-fat (H). Post-weaning - Control Control (CC), offspring of mothers C, fed a control diet after weaning; Control Hight-fat (CH), offspring of mothers C, fed a hight-fat diet after weaning; Hight-fat Control (HC), offspring of mothers H, fed with control diet after weaning; and Hight-fat Hight-fat (HH), offspring of mothers H, fed a H diet after weaning. The groups CH and HH presented greater expression of drd1 in comparison to the CC. The drd2 of CH and HC presented higher gene expression than did CC. HH presented higher pomc expression in comparison to the other groups. HC also presented greater expression in comparison to CH. The npy of HH presented greater expression in relation to CH and HC. HH and HC have had a higher preference for a high-fat diet at 102º life's day. The high-fat diet altered the gene expression of the drd1, drd2, pomc and npy, and influencing the food preference for high-fat diet.

In this paper, we complement our previous study on the antiproliferative activity of Calea fruticosa (Asteraceae) by isolating the compounds apigenin-4',7-dimethyl ether (1), budlein A (2), quercetin (3), and cichoriin (4) from the plant's aerial parts. The antiproliferative activity of these compounds was evaluated by the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) method against human tumor cell lines. Compound 3 displayed moderate antiproliferative activity in three cell lines (HCT-116, PC-3, and SF-295, with cell growth inhibition values of 72.97, 74.55, and 68.94%) and high antiproliferative activity (90.86%) in the HL-60 cell line. The in vitro sun protection factor (SPF) of the extracts and compound 4, with and without sunscreen, was determined by a spectrophotometric method. The ethanol extract exhibited the highest SPF (9.67) at a concentration of 0.100 mg/mL, while compound 4, isolated from this extract, showed a SPF of 13.79 at the same concentration. A relative increased efficacy of SPF was observed for the extracts and compound 4 when sunscreen was also used. Compound 4 has not been reported previously from any species within the genus Calea. Compounds 1-4 were obtained from this species for the first time.

BACKGROUND: Pelvic floor muscles (PFM) and rectus abdominis muscles (RAM) of pregnant diabetic rats exhibit atrophy, co-localization of fast and slow fibers and an increased collagen type I/III ratio. However, the role of similar PFM or RAM hyperglycemic-related myopathy in women with gestational diabetes mellitus (GDM) remains poorly investigated. This study aims to assess the frequency of pelvic floor muscle disorders and pregnancy-specific urinary incontinence (PS-UI) 12 months after the Cesarean (C) section in women with GDM. Specifically, differences in PFM/RAM hyperglycemic myopathy will be evaluated. METHODS: The Diamater is an ongoing cohort study of four groups of 59 pregnant women each from the Perinatal Diabetes Research Centre (PDRC), Botucatu Medical School (FMB)-UNESP (São Paulo State University), Brazil. Diagnosis of GDM and PS-UI will be made at 24-26 weeks, with a follow-up at 34-38 weeks of gestation. Inclusion in the study will occur at the time of C-section, and patients will be followed at 24-48 h, 6 weeks and 6 and 12 months postpartum. Study groups will be classified as (1) GDM plus PS-UI; (2) GDM without PS-UI; (3) Non-GDM plus PS-UI; and (4) Non-GDM without PS-UI. We will analyze relationships between GDM, PS-UI and hyperglycemic myopathy at 12 months after C-section. The mediator variables to be evaluated include digital palpation, vaginal squeeze pressure, 3D pelvic floor ultrasound, and 3D RAM ultrasound. RAM samples obtained during C-section will be analyzed for ex-vivo contractility, morphological, molecular and OMICS profiles to further characterize the hyperglycemic myopathy. Additional variables to be evaluated include maternal age, socioeconomic status, educational level, ethnicity, body mass index, weight gain during pregnancy, quality of glycemic control and insulin therapy. DISCUSSION: To our knowledge, this will be the first study to provide data on the prevalence of PS-UI and RAM and PFM physical and biomolecular muscle profiles after C-section in mothers with GDM. The longitudinal design allows for the assessment of cause-effect relationships between GDM, PS-UI, and PFMs and RAMs myopathy. The findings may reveal previously undetermined consequences of GDM.

To date, there is no compilation of evidence-based information associating photobiomodulation effect and salivary glands. This systematic review aims to assess photobiomodulation effect of low intensity laser on salivary glands in the presence of systemic diseases. MEDLINE databases were searched in duplicate through December 2018. In vivo studies and clinical trials were included if photobiomodulation was performed in salivary glands of animal (rat or mice) or human in the presence of systemic disease. The methodological quality was assessed in duplicate using the modified Newcastle-Ottawa scale (NOS). Search strategy identified 483 potentially eligible articles, and 449 were included. The Boolean search naturally leads to a high amount of works the majority of which were excluded because the analysis of the title and abstract demonstrated it was not focusing on PBM. Only 34 studies were selected for the full-text analysis, of which 5 were excluded due to non-use of photobiomodulation, 4 due to lack of control group, 2 because they were studies of cell cultures and 1 because they did not have the total of animals used. Thus, 21 papers were included for the critical evaluation of the impact of photobiomodulation on the major salivary glands; the studies used rats (n = 10) and humans (n = 11). Although studies reported an increase in the salivary rate, decrease in pain, and increase in quality of life after the PBM, the lack of standards for the application of light and reporting of the parameters, make it hard to reproduce the results. This topic is still in need for further research.

Housing nursery pigs according to body weight is an observed common practice in production systems and, supposedly, improves growth performance and reduces body weight (BW) variation. This 42-day study evaluated the effects of housing nursery pigs according to BW on performance, onset of feed intake, and aggressive behavior. A total of 504 pigs were ranked by BW at weaning and categorized into three groups of 168 pigs each: light, medium, and heavy. Pigs were randomly distributed to unsorted pens (Unsorted) containing 6 pigs of each weight group and sorted pens with 18 pigs from just one group per pen (Sorted). From weaning to day 3, pigs were fed a diet containing 1% iron oxide dye and rectal swabs presenting red coloration were evaluated to assess feed intake onset. Eight pens were video recorded to evaluate aggressive behavior. Sorted-Heavy pigs delayed the onset of feed intake (P ≤ 0.011) and presented more aggressive behaviors than Sorted-Light and Sorted-Medium pigs (P ≤ 0.036). In Unsorted, onset of feed intake showed no differences between weight categories. Also, no differences were observed for aggressive behavior between Sorted and Unsorted. Final BW showed no differences between Sorted and Unsorted pigs. The within-pen weight coefficient variation (CV) was slightly different (P = 0.042) between Sorted and Unsorted pigs (13.3 and 15.6%, respectively), at the end of the study. Thus, sorting nursery pigs by BW did not improve growth performance and also, induce a lag of post-weaning feed intake onset and increased fights in Heavy pigs.

Polyacrylonitrile (PAN) was solubilized in N,N-dimethyl formamide (DMF) and the electrospinning process has been employed to obtain PAN nanofibers (PF). Multiwalled carbon nanotubes (MWCNT) were dispersed with the aid of Triton X-100 surfactant and subsequently centrifugated. Buckypapers (BP/PF) were prepared by vacuum filtration procedure of MWCNT suspension supernatant stacking four PF layers over a nylon membrane. The PF removal was carried out by immersing the BP/PF system in DMF and removal periods of 10 and 30 min were evaluated. Scanning electron microscopy (SEM) has not shown any PAN residue in the MWCNT network resulting in highly porous BP. However, by Fourier transform infrared spectroscopy (FT-IR) a PAN band was found around of 2243 cm-1 corresponding to nitrile group (C≡N). Besides, PAN leftover was evidenced by thermogravimetric analysis (TGA), high-resolution transmission electron microscopy (HR-TEM), electrical characterization through four-point probe, nitrogen adsorption at 77 K, and X-ray diffraction (XRD).
ABSTRACT
In this work we have assessed the decolorization of textile effluents throughout their treatment in a solid-state fermentation (SSF) system. SSF assays were conducted with peach-palm (Bactris gasipaes) residue using the white rot fungus Ganoderma lucidum EF 31. The influence of the dye concentration and of the amounts of peach-palm residue and liquid phase on both the discoloration efficiency and enzyme production was studied. According to our results, independently of experimental conditions employed, laccase was the main ligninolytic enzyme produced by G. lucidum. The highest laccase activity was obtained at very low effluent concentrations, suggesting the existence of an inhibitory effect of higher concentrations on fungal metabolism. The highest percentage of color removal was reached when 10 grams of peach palm residue was moistened with 60 mL of the final effluent. In control tests carried out with the synthetic dye Remazol Brilliant Blue R (RBBR) decolorization efficiencies about 20% higher than that achieved with the industrial effluent were achieved. The adsorption of RBBR on peach-palm residue was also investigated. Equilibrium tests showed that the adsorption of this dye followed both Langmuir and Freundlich isotherms. Hence, our experimental results indicate that peach-palm residue is suitable substrate for both laccase production and color removal in industrial effluents.(AU)
Neste trabalho, avaliamos a descoloração de efluentes têxteis durante seu tratamento em um sistema de fermentação em estado sólido (SSF). Os ensaios foram conduzidos com resíduo de pupunha (Bactris gasipaes) utilizando o fungo de podridão branca Ganoderma lucidum EF 31. A influência da concentração de corante, as quantidades de resíduo e da fase líquida foram estudadas tanto na eficiência de descoloração como na produção de enzima. De acordo com os resultados, independentemente das condições experimentais utilizadas, a lacase foi a principal enzima ligninolítica produzida por G. lucidum. A atividade de lacase mais elevada foi obtida em baixas concentrações de efluentes, sugerindo um efeito inibitório no metabolismo fúngico. A maior remoção de cor foi obtida com 10 gramas de resíduo da pupunha e 60 mL do efluente final. Nos ensaios de controle realizados com o corante sintético RBBR, foram atingidos cerca de 20% mais descoloração do que os obtidos com o efluente industrial. A adsorção de RBBR no resíduo de pupunha também foi investigada. Os testes de equilíbrio mostraram que a adsorção deste corante seguiu as isotermas de Langmuir e Freundlich. Assim, os resultados experimentais indicam que o resíduo de pupunha é um substrato adequado tanto para a produção de lacase quanto para a remoção de cor em efluentes industriais.(AU)
